Abstract
Description
[1]長手軸方向を有するシースであって、上記長手軸方向に延在する第1の内腔と第2の内腔とを有するシースと、上記第1の内腔に配置されている内視カメラと、上記第2の内腔に上記長手軸方向に移動可能に挿入されているシャフトと上記シャフトの遠位側に設けられたバルーンとを有するバルーンカテーテルと、を備える気管支鏡であって、
上記バルーンは、流体を注入して陽圧がかけられる加圧内腔を備え、
上記第2の内腔は、陰圧をかけられることにより上記シースの遠位端を気管支の内壁に吸着させることを特徴とする気管支鏡。
[2]上記加圧内腔に流体が注入された加圧状態の上記バルーンの上記シャフトの長手軸方向中心における外径(mm)は、上記シースの遠位端における上記シースの外径(mm)よりも大きい[1]に記載の気管支鏡。
[3]上記バルーンは直管部を備え、上記加圧内腔に流体が注入された加圧状態において、上記直管部の近位端から遠位端までの上記シャフトの長手軸方向の長さ(mm)は、上記バルーンの上記シャフトの長手軸方向中心における外径(mm)以上の長さである[1]または[2]に記載の気管支鏡。
[4]上記バルーンは球状部または長球状部を備え、上記球状部または上記長球状部の近位端から遠位端までの上記シャフトの長手軸方向の長さ(mm)は、上記バルーンの上記シャフトの長手軸方向中心における外径(mm)以上の長さである[1]または[2]に記載の気管支鏡。
[5]上記シースの長手軸方向に垂直な方向の断面において、上記第2の内腔の面積(mm2)は、収縮状態における上記バルーンの外周線に囲まれた領域の面積(mm2)の1.2倍以上である[1]~[4]のいずれかに記載の気管支鏡。
[6]上記第2の内腔の近位端側には、陰圧発生装置が連結されている[1]~[5]のいずれかに記載の気管支鏡。
[7]上記バルーンは、上記シャフトに固定されている固定部と、固定されていない非固定部とを有し、上記バルーンカテーテルは、上記非固定部の遠位端から遠位側に向かって延在する棒状部を備える[1]~[6]のいずれかに記載の気管支鏡。
[8]上記棒状部の長手軸方向の長さ(mm)は、上記加圧内腔に流体が注入された加圧状態の上記バルーンの上記シャフトの長手軸方向中心における外径(mm)の0.5倍以上の長さである[7]に記載の気管支鏡。
[9]上記棒状部の遠位端部は、長手軸方向の断面図において、曲率半径が0.1mm以上、10mm以下の曲線部を備えている[7]または[8]に記載の気管支鏡。
[10]上記バルーンはノンコンプライアントバルーン、またはセミコンプライアントバルーンである[1]~[9]のいずれかに記載の気管支鏡。
[11]上記第2の内腔に陰圧をかけることにより上記シースの遠位端を気管支の内壁に吸着させる工程を含むことを特徴とする[1]~[10]のいずれかに記載の気管支鏡の使用方法。
[12]上記シースの遠位端を気管支の内壁に吸着させた状態で、上記シースの遠位端を摺動させて、上記内視カメラの遠位端に付着した気管支内の夾雑物を除去する工程を含む[11]に記載の気管支鏡の使用方法。
[13]上記気管支の内壁に吸着させる工程は、上記バルーンの上記加圧内腔に陰圧をかけながら行う[11]または[12]に記載の気管支鏡の使用方法。
[14]上記バルーンを上記第2の内腔の遠位端から遠位側に押し出す工程、
上記バルーンに陽圧をかけて上記バルーンを拡張する工程、
上記バルーンに陰圧をかけて上記バルーンを収縮する工程、
上記バルーンを上記第2の内腔内に引き戻す工程、及び
上記内視カメラで観察する工程を含む[11]~[13]のいずれかに記載の気管支鏡の使用方法。
[15]上記バルーンを上記第2の内腔内に引き戻す工程後に、上記気管支鏡を遠位側に押し込む工程を含む[14]に記載の気管支鏡の使用方法。
[曲げ荷重の測定方法]
ステンレス製の長さ方向の長さが5cm以上、長さ方向に垂直な方向の幅が2cm以上、厚さが2cm以上である直方体の下側ブロックと、ステンレス製の長さ方向の長さが5cm以上、長さ方向に垂直な方向の幅が2cm以上、厚さが2cm以上である直方体の上側ブロックとを用意する。次にシャフト21の長手軸方向が下側ブロックと上側ブロックの長さ方向と平行になるようにバルーンカテーテル20を下側ブロックと上側ブロックの間に配置する。次に下側ブロックの遠位端と、上側ブロックの遠位端と、バルーン22の非固定部28の遠位端28Bとがシャフト21の長手軸方向に揃うように位置合わせし、バルーンカテーテル20を下側ブロックと上側ブロックで挟み込んで固定する。次に棒状部25の遠位端からシャフト21の長手軸方向に1.0mm離れた位置までの部分を、加圧面が矩形である加圧子によりシャフト21の長手軸方向に垂直な方向に1.0mm押し込んだ時の荷重(N)を測定する。

Claims (15)
- 長手軸方向を有するシースであって、前記長手軸方向に延在する第1の内腔と第2の内腔とを有するシースと、前記第1の内腔に配置されている内視カメラと、前記第2の内腔に前記長手軸方向に移動可能に挿入されているシャフトと前記シャフトの遠位側に設けられたバルーンとを有するバルーンカテーテルと、を備える気管支鏡であって、
前記バルーンは、流体を注入して陽圧がかけられる加圧内腔を備え、
前記第2の内腔は、陰圧をかけられることにより前記シースの遠位端を気管支の内壁に吸着させることを特徴とする気管支鏡。 - 前記加圧内腔に流体が注入された加圧状態の前記バルーンの前記シャフトの長手軸方向中心における外径(mm)は、前記シースの遠位端における前記シースの外径(mm)よりも大きい請求項1に記載の気管支鏡。
- 前記バルーンは直管部を備え、前記加圧内腔に流体が注入された加圧状態において、前記直管部の近位端から遠位端までの前記シャフトの長手軸方向の長さ(mm)は、前記バルーンの前記シャフトの長手軸方向中心における外径(mm)以上の長さである請求項1または2に記載の気管支鏡。
- 前記バルーンは球状部または長球状部を備え、前記球状部または前記長球状部の近位端から遠位端までの前記シャフトの長手軸方向の長さ(mm)は、前記バルーンの前記シャフトの長手軸方向中心における外径(mm)以上の長さである請求項1または2に記載の気管支鏡。
- 前記シースの長手軸方向に垂直な方向の断面において、前記第2の内腔の面積(mm2)は、収縮状態における前記バルーンの外周線に囲まれた領域の面積(mm2)の1.2倍以上である請求項1~4のいずれかに記載の気管支鏡。
- 前記第2の内腔の近位端側には、陰圧発生装置が連結されている請求項1~5のいずれかに記載の気管支鏡。
- 前記バルーンは、前記シャフトに固定されている固定部と、固定されていない非固定部とを有し、前記バルーンカテーテルは、前記非固定部の遠位端から遠位側に向かって延在する棒状部を備える請求項1~6のいずれかに記載の気管支鏡。
- 前記棒状部の長手軸方向の長さ(mm)は、前記加圧内腔に流体が注入された加圧状態の前記バルーンの前記シャフトの長手軸方向中心における外径(mm)の0.5倍以上の長さである請求項7に記載の気管支鏡。
- 前記棒状部の遠位端部は、長手軸方向の断面図において、曲率半径が0.1mm以上、10mm以下の曲線部を備えている請求項7または8に記載の気管支鏡。
- 前記バルーンはノンコンプライアントバルーン、またはセミコンプライアントバルーンである請求項1~9のいずれかに記載の気管支鏡。
- 前記第2の内腔に陰圧をかけることにより前記シースの遠位端を気管支の内壁に吸着させる工程を含むことを特徴とする請求項1~10のいずれかに記載の気管支鏡の使用方法。
- 前記シースの遠位端を気管支の内壁に吸着させた状態で、前記シースの遠位端を摺動させて、前記内視カメラの遠位端に付着した気管支内の夾雑物を除去する工程を含む請求項11に記載の気管支鏡の使用方法。
- 前記気管支の内壁に吸着させる工程は、前記バルーンの前記加圧内腔に陰圧をかけながら行う請求項11または12に記載の気管支鏡の使用方法。
- 前記バルーンを前記第2の内腔の遠位端から遠位側に押し出す工程、
前記バルーンに陽圧をかけて前記バルーンを拡張する工程、
前記バルーンに陰圧をかけて前記バルーンを収縮する工程、
前記バルーンを前記第2の内腔内に引き戻す工程、及び
前記内視カメラで観察する工程を含む請求項11~13のいずれかに記載の気管支鏡の使用方法。 - 前記バルーンを前記第2の内腔内に引き戻す工程後に、前記気管支鏡を遠位側に押し込む工程を含む請求項14に記載の気管支鏡の使用方法。
