Abstract
種々のβ-ラクタマーゼ産生菌に対して有効な医薬組成物を提供する。 本発明は、式(I)で示される化合物またはその製薬上許容される塩、および式(II)もしくは(III)で示される化合物またはそれらの製薬上許容される塩を組み合わせることを特徴とする医薬組成物を提供する。
Description
本発明は、細菌感染症治療または予防用の医薬に関する。さらに詳しくは、セフィデロコルおよびボロン酸構造を有するβ-ラクタマーゼ阻害剤を組み合わせることを特徴とする抗菌作用を有する医薬に関する。
これまで、さまざまなβ-ラクタム薬の開発がなされており、β-ラクタム薬は臨床上非常に重要な抗菌薬となっている。しかし、β‐ラクタム薬を分解するβ-ラクタマーゼを産生することによりβ-ラクタム薬に対して耐性を獲得した菌種が増加している。
アムブラー(Ambler)の分子分類法によると、β-ラクタマーゼは大きく4つのクラスに分類される。すなわち、クラスA(TEM型、SHV型、CTX-M型、KPC型など)、クラスB(NDM型、IMP型、VIM型、L-1型など)、クラスC(AmpC型、ADC型など)、クラスD(OXA型など)である。これらのうち、クラスA,C,D型はセリン-β-ラクタマーゼ、一方、クラスB型はメタロ-β-ラクタマーゼに大別され、それぞれ異なるメカニズムによってβ-ラクタム薬を加水分解することが知られている。
近年、基質域を拡張したクラスA型(ESBL)に加え、カルバペネム薬を分解するクラスA(KPC型など)やクラスD(OXA-48など)セリン-β-ラクタマーゼおよびクラスBメタロ-β-ラクタマーゼをカルバペネマーゼと呼び、これらの産生によりグラム陰性菌感染症治療に対し重要な位置付けであるカルバペネム系抗菌薬を含むほぼ全てのβ-ラクタム薬に高度耐性化したグラム陰性菌の存在が臨床上問題となっている。

セフィデロコル(Cefiderocol)はセファロスポリン系抗菌剤であり、式(I):
で示される化合物であり、S-649266、(6R,7R)-7-[(2Z)-2-(2-アミノ-1,3-チアゾール-4-イル)-2-{[(2-カルボキシプロパン-2-イル)オキシ]イミノ}アセトアミド]-3-({1-[2-(2-クロロ-3,4-ジヒドロキシベンズアミド)エチル]ピロリジン-1-イウム-1-イル}メチル)-8-オキソ-5-チア-1-アザビシクロ[4.2.0]オクタ-2-エン-2-カルボン酸とも称される。本出願人による特許文献1には、セフィデロコルがベタインの形態で開示されており、広範な抗菌スペクトルを有し、特にβ-ラクタマーゼ産生菌に対し強い抗菌活性を示すため、感染症の治療または予防剤として有用であることが記載されている。
セフィデロコルは、ESBLやカルバペネマーゼを産生するグラム陰性菌に対して単剤で抗菌活性を示すセファロスポリン系β-ラクタム抗菌薬であり、KPCやNDMを含む各種(クラスA,B,C,D)β-ラクタマーゼを産生する腸内細菌科細菌、緑膿菌やアシネトバクターを含む多くのグラム陰性菌に対して強い抗菌活性を有することが非特許文献1に報告されている。しかし、グローバルサーベイランスにおいて、それら菌株の中にセフィデロコル低感受性株も少なからず存在することが非特許文献2に報告されており、それら多剤耐性菌株による感染症を治療するためには、セフィデロコルの抗菌活性を改善する手段などの新たな治療薬が必要となる。

β-ラクタム抗菌薬(1つまたは複数のβ-ラクタム骨格を有する抗菌薬、例えば、ペニシリン系、カルバペネム系、セファロスポリン系等)を含む医薬組成物は、抗菌活性の改善を目的に、β-ラクタマーゼ阻害剤と共に投与することが可能であることが知られている。
セフィデロコルにおいても、特許文献2に活性増強を目的にβ-ラクタマーゼ阻害剤と併用することが可能であることが記載されている。また、特許文献3には、セフィデロコルとクラブラン酸、タゾバクタム、アビバクタム、またはジアザビシクロオクタン骨格を有する化合物のβ-ラクタマーゼ阻害剤との併用が記載されているが、これらの併用によって、セフィデロコル低感受性株に対して有効な抗菌活性を示したことについては記載されていない。
β-ラクタマーゼ阻害剤においても、これまでに数多くの研究がなされており、非特許文献3には、特徴の異なる数多くのβ-ラクタマーゼ阻害剤が報告されている。また、2019年6月20日~24日開催のASM Microbe 2019の学会で以下のQPX7728を含む種々のβ-ラクタマーゼ阻害剤が報告されている(非特許文献4)。
特許文献3~31には、ボロン酸構造を有するβ-ラクタマーゼ阻害剤が開示されており、当該β-ラクタマーゼ阻害剤が種々のβ-ラクタマーゼに対して阻害活性を有することが記載されている。しかし、これらの文献には本発明は記載されていない。
セフィデロコルにおいても、特許文献2に活性増強を目的にβ-ラクタマーゼ阻害剤と併用することが可能であることが記載されている。また、特許文献3には、セフィデロコルとクラブラン酸、タゾバクタム、アビバクタム、またはジアザビシクロオクタン骨格を有する化合物のβ-ラクタマーゼ阻害剤との併用が記載されているが、これらの併用によって、セフィデロコル低感受性株に対して有効な抗菌活性を示したことについては記載されていない。

本発明の目的は、種々のβ-ラクタマーゼを産生する多剤耐性菌株に対して有効な抗菌薬を提供することにある。好ましくは、多剤耐性菌を含む薬剤耐性菌によって引き起こされる細菌感染症の治療及び/又は予防に有用な医薬組成物を提供することにある。また、別の好ましい態様としては、クラスBのβ-ラクタマーゼに加えて、クラスA,C,および/またはDのβ-ラクタマーゼを複数産生した多剤耐性グラム陰性菌によって引き起こされる細菌感染症の治療及び/又は予防に有用な医薬組成物を提供する。

本発明の医薬組成物は、β-ラクタム薬である式(I)で示される化合物またはその製薬上許容される塩およびβ-ラクタマーゼ阻害剤である式(II)または(III)で示される化合物またはその製薬上許容される塩を併用または組み合わせて使用することで、式(I)で示される化合物またはその製薬上許容される塩を単独で使用する場合に比べて格別優れた抗菌作用を示すこと、さらにその効果は式(II)または(III)で示される化合物とは異なるβ-ラクタマーゼ阻害剤と併用または組み合わせて使用した場合に比べても格別優れた抗菌作用を発揮するという優れた効果を奏するものである。
また、本発明に係る医薬組成物は、少なくとも以下のいずれか一つの特徴を有する点で医薬品として有用である。
A)グラム陰性菌の種々の細菌に対して、良好な抗菌スペクトルを示す。
B)種々のβ-ラクタマーゼを産生する菌(特に、メタロ型β-ラクタマーゼおよびセリン型β-ラクタマーゼ(例:クラスA,C,D)の複数のβ-ラクタマーゼを産生する菌)に対して有効な抗菌作用を有する。
C)多剤耐性菌に対し強い抗菌活性を示す。
D)基質特異性拡張型β-ラクタマーゼ(ESBL)産生菌に対し強い抗菌活性を示す。
E)カルバペネム耐性菌に対し強い抗菌活性を示す。
F)市販薬に耐性のある腸内細菌科細菌に対し強い抗菌活性を示す。

「アルキニル」は、任意の位置に1又は2以上の三重結合を有する直鎖又は分岐状の炭化水素鎖を指す。アルキニル基は、2から20個の炭素原子を有してもよいが、本定義は、数値範囲が指定されていない「アルキニル」という用語にも及ぶ。アルキニル基はまた、2から9個の炭素原子を有する中間の大きさのアルキニルであってもよい。アルキニル基は、2から4個の炭素原子を有する低級アルキニルであることもできる。また、アルキニルの炭素原子数としては、例えば「C2-4アルキニル」の様に指定されてもよく、当該アルキニルは炭素原子数が2~4個である、任意の位置に1または2以上の三重結合を有する直鎖または分枝状の炭化水素鎖を意味する。すなわち、「C2-4アルキニル」は、エチニル、プロピン-1-イル、プロピン-2-イル、ブチン-1-イル、ブチン-3-イル、ブチン-4-イル、及び2-ブチニルからなる群から選択される基である。好ましいアルキニル基としては、決して限定されないが、エチニル、プロピニル、ブチニル、ペンチニル、及びヘキシニル等が挙げられる。
「芳香族」とは、共役π電子系を有する環または環系を示す。この用語は、単環式または縮合環多環式(すなわち、離接する原子対を共有する環)基を包含するが、全体の環系は芳香族であることを条件とする。
「芳香族炭素環式基」とは、単環または2環以上の環状芳香族炭化水素基を意味する。芳香族炭素環式基は、6~18個の炭素原子を有してもよいが、本定義は、数値範囲が指定されていない「芳香族炭素環式基」にも及ぶ。また、芳香族炭素環式基の炭素原子数としては、例えば「C6-10芳香族炭素環式基」の様に指定してもよく、当該基は炭素原子数が6~10個である単環または2環以上の環状芳香族炭素水素基を意味する。芳香族炭素環式基の例としては、特に限定されないが、フェニル、ナフチル、アントリル、フェナントリル等が挙げられる。
「芳香族炭素環」とは、上記「芳香族炭素環式基」から導かれる環を意味する。
「芳香族複素環式基」とは、O,SおよびNを含む、炭素以外の元素から任意に選択される同一または異なるヘテロ原子を環内に1または2以上有する、単環または2環以上の、芳香族環式基を意味する。2環以上の場合、系中のあらゆる環は全て芳香族である。2環以上の芳香族複素環式基は、単環または2環以上の芳香族複素環式基に、上記「芳香族炭素環式基」における環が縮合したものも包含する。芳香族複素環式基は5~18員(すなわち、炭素原子およびヘテロ原子を含めて、環骨格を構成する原子の数)を有していてもよいが、本定義は、数値範囲が指定されていない「芳香族複素環式基」にも及ぶ。好ましい実施態様としては、5~10員または5~7員である。芳香族複素環式基の例としては、特に限定されないが、フリル、チエニル、フタラジニル、ピロリル、オキサゾリル、チアゾリル、イミダゾリル、ピラゾリル、イソオキサゾリル、イソチアゾリル、トリアゾリル、チアジアゾリル、ピリジニル、ピリダジニル、ピリミジニル、ピラジニル、トリアジニル、キノリニル、イソキノリニル、ベンズイミダゾリル、ベンズオキサゾリル、ベンゾチアゾリル、インドリル、イソインドリル、及びベンゾチエニルが挙げられる。
「芳香族複素環」とは、上記「芳香族複素環式基」から導かれる環を意味する。
「非芳香族炭素環式基」とは、単環または2環以上の、環状飽和炭化水素基または環状非芳香族不飽和炭化水素基を意味する。2環以上の「非芳香族炭素環式基」は、単環または2環以上の非芳香族炭素環式基に、上記「芳香族炭素環式基」における環が縮合したものも包含する。なお、結合手はいずれの環から出ていてもよい。2環以上の環式基としては、縮合環、架橋又はスピロ結合様式で一緒に連結されている環式基も包含する。非芳香族炭素環式基は、環系中の少なくとも1つの環が芳香族でないことを条件として、任意の飽和度を有してもよい。したがって、炭素環式基は、シクロアルキル、シクロアルケニル、及びシクロアルキニルを包含する。非芳香族炭素環式基は、3から20個の炭素原子を有してもよいが、本定義は、数値範囲が指定されていない「非芳香族炭素環式基」にも及ぶ。非芳香族炭素環式基はまた、3から10個の炭素原子を有する中間の大きさの非芳香族炭素環式基であってもよい。非芳香族炭素環式基はまた、3から6個の炭素原子を有する非芳香族炭素環式基であることもできる。例えば、非芳香族炭素環式基の炭素原子数は、「C3~6カルボシクリル」の様に指定されてもよく、当該基は炭素数3~6個の単環または2環以上の、環状飽和炭化水素基または環状非芳香族不飽和炭化水素基を意味する。非芳香族炭素環式基の例としては、特に限定されないが、シクロプロピル、シクロブチル、シクロペンチル、シクロヘキシル、シクロヘキセニル、2,3-ジヒドロ-インデン、ビシクル[2.2.2]オクタニル、アダマンチル、及びスピロ[4.4]ノナニルが挙げられる。

「非芳香族複素環」とは、上記「非芳香族複素環式基」から導かれる環を意味する。
「アシル」とは、-C(=O)R(ここで、Rは本明細書で定義されるとおりの、水素、C1-6アルキル、C2-6アルケニル、C2-6アルキニル、C3-7非芳香族炭素環式基、芳香族炭素環式基、5~10員非芳香族複素環式基、及び5~10員芳香族複素環式基である)を指す。非限定的な例としては、ホルミル、アセチル、プロパノイル、ベンゾイル、及びアクリルが挙げられる。

本明細書で開示される化合物またはその製薬上許容される塩は、プロドラッグを形成する場合があり、本発明はそのような各種のプロドラッグも包含する。プロドラッグは、化学的又は代謝的に分解できる基を有する本発明化合物の誘導体であり、加溶媒分解により又は生理学的条件下でインビボにおいて薬学的に活性な本発明化合物となる化合物である。プロドラッグは、生体内における生理条件下で酵素的に酸化、還元、加水分解等を受けて本明細書で開示される化合物に変換される化合物、胃酸等により加水分解されて本明細書で開示される化合物に変換される化合物等を包含する。適当なプロドラッグ誘導体を選択する方法および製造する方法は、例えば “Design of Prodrugs, Elsevier, Amsterdam, 1985”に記載されている。プロドラッグは、それ自身が活性を有する場合がある。
本明細書で開示される化合物またはその製薬上許容される塩がヒドロキシル基を有する場合は、例えば、ヒドロキシル基を有する化合物と適当なアシルハライド、適当な酸無水物、適当なスルホニルクロライド、適当なスルホニルアンハイドライド及びミックスドアンハイドライドとを反応させることにより或いは縮合剤を用いて反応させることにより製造されるアシルオキシ誘導体やスルホニルオキシ誘導体のようなプロドラッグが例示される。例えば、CH3COO-、C2H5COO-、tert-BuCOO-、C15H31COO-、PhCOO-、(m-NaOOCPh)COO-、NaOOCCH2CH2COO-、CH3CH(NH2)COO-、CH2N(CH3)2COO-、CH3SO3-、CH3CH2SO3-、CF3SO3-、CH2FSO3-、CF3CH2SO3-、p-CH3O-PhSO3-、PhSO3-、p-CH3PhSO3-が挙げられる。
一部の実施形態において、ボロンエステルの容易な交換のために、本明細書で記載されるボロン化合物は、下式のとおり交互形態に変換するか、又はそれと平衡状態で存在してもよい。したがって、本明細書で記載されるボロン化合物は、いずれか一方の形態で示されている場合でもそれぞれの形態および各形態が組み合わせて存在する場合も包含する。例えば、以下に示されるように、本明細書で開示されるボロン化合物は、式(II)のような環状ボロネートモノエステルとして環状形態若しくは式(III)のようなボロン酸として非環状形態で存在してもよいか(Biochemistry、2000年、39、5312~21頁)、又は媒体に応じてこれら2つの形態の混合物として存在してもよい。

本明細書で開示される化合物の一つ以上の水素、炭素および/または他の原子は、それぞれ水素、炭素および/または他の原子の同位体で置換され得る。そのような同位体の例としては、それぞれ2H、3H、11C、13C、14C、15N、18O、17O、31P、32P、35S、18F、123Iおよび36Clのように、水素、炭素、窒素、酸素、リン、硫黄、フッ素、ヨウ素および塩素が包含される。式(I),(II),または(III)で示される化合物は、そのような同位体で置換された化合物も包含する。該同位体で置換された化合物は、医薬品としても有用であり、式(I),(II),または(III)で示される化合物の全ての放射性標識体を包含する。
式(I)、(II)、または(III)で示される化合物の放射性標識体は、当該技術分野で周知の方法で調製できる。例えば、式(I)、(II)、または(III)で示されるトリチウム標識化合物は、トリチウムを用いた触媒的脱ハロゲン化反応によって、式(I)、(II)、または(III)で示される特定の化合物にトリチウムを導入することで調製できる。この方法は、適切な触媒、例えばPd/Cの存在下、塩基の存在下または非存在下で式(I)、(II)、または(III)で示される化合物が適切にハロゲン置換された前駆体とトリチウムガスとを反応させることを包含する。トリチウム標識化合物を調製するための他の適切な方法は、“Isotopes in the Physical and Biomedical Sciences,Vol.1,Labeled Compounds (Part A),Chapter 6 (1987年)”を参照することができる。14C-標識化合物は、14C炭素を有する原料を用いることによって調製できる。
本明細書で使用される場合の「対象」は、哺乳類、すなわちヒト又は非ヒト哺乳動物、例えば、イヌ、ネコ、マウス、ラット、ウシ、ヒツジ、ブタ、ヤギ、非ヒト霊長類又は鳥、例えば、ニワトリのみならず、任意の他の脊椎動物又は非脊椎動物も意味する。
本明細書で使用される場合の「有効量」又は「治療有効量」は、疾患又は状態の症状の1つ又は複数の発症の可能性を、ある程度軽減する、又は低減する(疾患又は状態を治癒することを含む)のに有効である治療剤の量を指す。「治癒すること」は、疾患又は状態の症状が除去されることを意味するが、しかしながら、ある長期の又は永続的な作用が、治癒が得られた後でも存在することがある(広範囲にわたる組織の損傷など)。
本明細書で使用される場合の「処置する」、「処置」、又は「処置すること」は、予防及び/又は治療の目的のために医薬組成物を投与することを指す。「治療」とは、既に疾患又は状態に羅患している患者に処置を施すことを指す。「予防」とは、疾患又は状態の症状をまだ示していないが、特定の疾患又は状態に罹りやすい、又はそうでなければ、その危険性がある対象を処置することを指し、それにより、その処置は患者が疾患又は状態を発症させる可能性を低減する。
本明細書で使用される場合の「細菌感染症」とは、その生物が脊椎動物、無脊椎動物、魚、植物、鳥、又は哺乳動物であるかどうかに関わらず、病原性細菌が宿主生物へ侵入し、増殖することによって引き起こす様々な疾患を指す。特に限定されないが、例えば、気道感染症、尿路感染症、呼吸器感染症、腹腔内感染、敗血症、腎炎、胆嚢炎、口腔内感染症、心内膜炎、肺炎、骨髄膜炎、中耳炎、腸炎、蓄膿、創傷感染、日和見感染等が挙げられる。別の態様として、本明細書において提供される任意の医薬組成物は、複雑性尿路感染症(cUTI)または院内感染肺炎/人工呼吸器関連肺炎(HABP/VABP)の治療および/または予防のために使用することができる。当該病原性細菌は、特に限定されないが、好ましくはグラム陰性菌、より好ましくは腸内細菌科のグラム陰性菌(大腸菌、クレブシエラ、セラチア、エンテロバクター、シトロバクター、モルガネラ、プロビデンシア、プロテウス等)、呼吸器に定着するグラム陰性菌(ヘモフィルス、モラキセラ等)およびブドウ糖非発酵のグラム陰性菌(緑膿菌以外のシュードモナス、ステノトロフォモナス、バークホルデリア、アシネトバクター等)等が挙げられる。別の好ましい態様としては、好気性グラム陰性菌が挙げられる。別の好ましい態様としては、大腸菌(Escherichia coli),エンテロバクター・クロアカ(Enterobacter cloacae)、および肺炎桿菌(Klebsiella pneumoniae)からなる群から選択される細菌によって引き起こされる細菌感染症が挙げられる。また別の態様として、クラスBのβ-ラクタマーゼに加えて、クラスA,C,および/またはDのβ-ラクタマーゼを複数産生した多剤耐性グラム陰性菌によって引き起こされる細菌感染症が挙げられる。また別の好ましい態様としては、VIMもしくはNDMのクラスBのβ-ラクタマーゼ、ならびにCTX-M、TEM、SHV、VEB、ACC、MIR、DHA、CMY、およびOXAから選択される1または2つ以上のクラスA,C,Dのβ-ラクタマーゼを産生した多剤耐性グラム陰性菌によって引き起こされる細菌感染症が挙げられる。
本明細書における「併用」または「組み合わせて投与」とは、投与時期、投与形態には限定されず、2つ以上の薬剤を合わせて投与することを意味する。すなわち、2つ以上の薬剤は、投与対象に対し、同時に投与してもよいし、時間差をおいて別々に投与してもよい。また、2つ以上の薬剤を含む単一の製剤として投与されてもよいし、それぞれの活性成分を含む2種類以上の製剤として投与されてもよい。
さらに、本発明は、上記(a)成分である式(I)で示される化合物またはその製薬上許容される塩と(b)成分である式(II)または(III)で示される化合物またはそれらの製薬上許容される塩を組み合わせたものであれば特に限定はなく、本発明の効果を損なわない範囲で、抗菌作用の増強もしくは補強、または投与量の低減等を目的として、その他の薬剤(例えば、抗菌薬、β-ラクタマーゼ阻害剤等)と併用、すなわち組み合わせて用いることができる。これらの含有量は特に限定されない。
さらに、本発明は、上記(a)成分である式(I)で示される化合物またはその製薬上許容される塩と(b)成分である式(II)または(III)で示される化合物またはそれらの製薬上許容される塩を組み合わせたものであれば特に限定はなく、本発明の効果を損なわない範囲で、抗菌作用の増強もしくは補強、または投与量の低減等を目的として、その他の薬剤(例えば、抗菌薬、β-ラクタマーゼ阻害剤等)と併用、すなわち組み合わせて用いることができる。これらの含有量は特に限定されない。
また、その他の製剤原料として、賦形剤、結合剤、崩壊剤、滑沢剤、甘味剤、矯味剤、防腐剤、キレート剤、抗酸化剤、清涼化剤、コーティング剤、安定化剤、流動化剤、粘稠剤、溶解補助剤、増粘剤、緩衝剤、香料、着色剤、吸着剤、湿潤剤、防湿剤、帯電防止剤、可塑剤、消泡剤、界面活性剤、乳化剤等の添加剤を含有してもよい。具体的には、結合剤(例えば、トウモロコシでん粉等)、充填剤(例えば、ラクトース、微結晶性セルロース等)、崩壊剤(例えば、でん粉グリコール酸ナトリウム等)、滑沢剤(例えば、ステアリン酸マグネシウム等)等が挙げられる。これらの含有量は特に限定されない。
本明細書に開示される実施形態は、治療有効量の本明細書で開示される化合物および薬学的に許容される添加剤を含む、医薬組成物を包含する。
本発明の医薬組成物は、経口的、非経口的のいずれの方法でも投与することができる。非経口投与の方法としては、経皮、皮下、静脈内、動脈内、筋肉内、腹腔内、経粘膜、吸入、経鼻、点眼、点耳、膣内投与等が挙げられる。
経口投与による場合、本発明化合物は通常の製剤、例えば、錠剤、散剤、顆粒剤、カプセル剤等の固形剤、水剤、油性懸濁剤、又はシロップ剤もしくはエリキシル剤等の液剤のいずれかの剤形としても用いることができる。非経口投与による場合、本発明化合物は、水性又は油性懸濁注射剤、点鼻液として用いることができる。その調製に際しては、慣用の賦形剤、結合剤、滑沢剤、水性溶剤、油性溶剤、乳化剤、懸濁化剤、保存剤、安定剤等を任意に用いることができる。本発明の製剤は、治療有効量の本発明化合物を製薬上許容される担体又は希釈剤とともに組み合わせる(例えば混合する)ことによって製造される。
経口投与による場合、本発明化合物は通常の製剤、例えば、錠剤、散剤、顆粒剤、カプセル剤等の固形剤、水剤、油性懸濁剤、又はシロップ剤もしくはエリキシル剤等の液剤のいずれかの剤形としても用いることができる。非経口投与による場合、本発明化合物は、水性又は油性懸濁注射剤、点鼻液として用いることができる。その調製に際しては、慣用の賦形剤、結合剤、滑沢剤、水性溶剤、油性溶剤、乳化剤、懸濁化剤、保存剤、安定剤等を任意に用いることができる。本発明の製剤は、治療有効量の本発明化合物を製薬上許容される担体又は希釈剤とともに組み合わせる(例えば混合する)ことによって製造される。
本発明化合物は、注射剤、カプセル剤、錠剤、顆粒剤として非経口または経口的に投与できるが、好ましくは注射剤として投与される。投与量は、通常、患者または動物の体重1kg当たり、約0.1~100mg/日、好ましくは約0.5~50mg/日を、所望により1日2~4回に分割して投与すればよい。注射剤として用いられる場合の担体は、たとえば蒸留水、生理食塩水などであり、またpH調節のための塩基等を使用してもよい。カプセル剤、顆粒剤、錠剤として用いられる場合の担体は、公知の賦形剤(例:デンプン、乳糖、白糖、炭酸カルシウム、リン酸カルシウムなど)、結合剤(例:デンプン、アラビアゴム、カルボキシメチルセルロ-ス、ヒドロキシプロピルセルロ-ス、結晶セルロ-スなど)、滑沢剤(例:ステアリン酸マグネシウム、タルクなど)等である。
本発明化合物の有効量にその剤型に適した賦形剤、結合剤、崩壊剤、滑沢剤等の各種医薬用添加剤を必要に応じて混合し、医薬組成物とすることができる。さらに、該医薬組成物は、本発明化合物の有効量、剤型および/または各種医薬用添加剤を適宜変更することにより、小児用、高齢者用、重症患者用または手術用の医薬組成物とすることもできる。小児用医薬組成物は、12歳または15歳未満の患者に投与するのが好ましい。また、小児用医薬組成物は、出生後27日未満、出生後28日~23か月、2歳~11歳または12歳~17歳若しくは18歳の患者に投与されうる。高齢者用医薬組成物は、65歳以上の患者に投与するのが好ましい。
本発明化合物の投与量は、患者の年齢、体重、疾病の種類や程度、投与経路等を考慮した上で設定することが望ましいが、経口投与する場合、通常0.5~300mg/kg/日であり、好ましくは1~50mg/kg/日の範囲内である。非経口投与の場合には投与経路により大きく異なるが、それぞれの化合物に関して通常0.5~300mg/kg/日であり、好ましくは1~50mg/kg/日の範囲内である。なお、かかる投与量は、一度に投与しても分割して投与してもよい。
本発明における式(II)および(III)で示される化合物の、R1,R2,R3,R4,Y1,R8,R9,G,n,R6a,R6b,およびR6cの好ましい態様を以下に示すが、本発明の範囲は下記に記載されるものに限定されない。式(II)および(III)で示される化合物としては、以下に示される具体例の全ての組み合わせの態様が例示される。
R1は好ましくは水素原子である。
R3は好ましくは水素原子である。
R4は好ましくは水素原子である。
R2は好ましくは、-Y1-(CR8R9)n-Gである。
Y1は好ましくは-S-である。R8およびR9は好ましくは水素原子である。nは好ましくは0である。
Y1は好ましくは-S-である。R8およびR9は好ましくは水素原子である。nは好ましくは0である。
Gは好ましくは置換もしくは非置換の5~10員の芳香族複素環式基である。より好ましくは、置換もしくは非置換の5員の含窒素芳香族複素環式基である。さらに好ましくは置換もしくは非置換のチアヂアゾリルである。特に好ましくは下式:

以下に試験例を挙げて本発明をさらに詳しく説明するが、本発明はこれらにより限定されるものではない。
試験例1:併用効果(MIC)
細菌に対する式(I)で示される化合物および式(II)で示される化合物との併用効果を評価した。式(II)で示される化合物を併用した場合の式(I)で示される化合物の最小阻害濃度(MIC:μg/mL)を測定した。MICの測定は、CLSI(Clinical and Laboratory Standards Institute)法に従って行い、接種菌量は5×105cfu/mLとし、試験培地として陽イオンをキレート樹脂により除去後にCaイオン,MgイオンおよびZnイオンを添加したID-CAMHB(Iron-depleted Cation-adjusted Mueller Hinton broth)を使用した微量液体希釈法で試験を行った。式(II)で示される化合物は4μg/mLとなるように培地中に添加した。比較対象として式(I)で示される化合物のみを用いた場合、および式(II)で示される化合物のみを用いた場合のMICを測定した。測定方法は上記と同様の方法で実施した。
使用した菌株は以下の表のとおりである。

本発明は、種々のβ-ラクタマーゼを産生する多剤耐性菌に対して有効な阻害活性を有しており、細菌感染症、特に、多剤耐性菌を含む薬剤耐性菌によって引き起こされる細菌感染症の治療及び/又は予防に有用な医薬となり得る。

- (a)式(I):

R1、R3、およびR4はそれぞれ独立して水素原子、ハロゲン、シアノ、ヒドロキシ、置換もしくは非置換のC1-6アルキル、置換もしくは非置換のC1-6アルキルオキシ、置換もしくは非置換のC1-6ハロアルキルオキシ、置換もしくは非置換の(C1-6アルキルオキシ)C1-6アルキル、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~10員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、置換もしくは非置換の5~10員の芳香族複素環式基、置換もしくは非置換の(C3-7炭素環)C1-6アルキル、置換もしくは非置換の(3~10員の複素環)C1-6アルキル、置換もしくは非置換の(C6-10芳香族炭素環)C1-6アルキル、置換もしくは非置換の(C6-10芳香族炭素環)C1-6アルキルオキシ、置換もしくは非置換の(5~10員の複素環)C1-6アルキル、アシル、カルボキシ、-C(O)NRaRb、-N(Ra)C(O)Rb、-S(O)2NRaRb、-N(Ra)S(O)2Rb、-SRc、-C(O)(CH2)0-3SRc、-C(O)(CH2)1-3Rd、-NRfC(O)NRfRg、-NRfS(O)2NRfRg、-C(=NRe)Rc、-C(=NRe)NRfRg、-NRfCRc(NRe)、-NRfC(=NRe)NRfRg、-S(O)(CH2)1-3Rc、または-NRfS(O)2NRfORdであり;
RaおよびRbはそれぞれ独立して、水素原子、C1-6アルキル、C2-6アルケニル、C2-6アルキニル、C3-7非芳香族炭素環式基、C6-10芳香族炭素環式基、5~10員の芳香族複素環式基、または5~10員の非芳香族複素環式基であり;
Rc、Rd、Re、Rf、およびRgはそれぞれ独立して、水素原子、ハロゲン、置換もしくは非置換のC1-4アルキル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~10員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、または置換もしくは非置換の5~10員の芳香族複素環式基であり;
R2は、水素原子、ハロゲン、シアノ、ヒドロキシ、カルボキシ、アシル、-C(O)NRaRb、-N(Ra)C(O)Rb、-S(O)2NRaRb、-N(Ra)S(O)2Rb、または-Y1-(CR8R9)n-Gであり;
R8およびR9はそれぞれ独立して、水素原子、ヒドロキシ、置換もしくは非置換のC1-10アルキルオキシ、置換もしくは非置換のC1-10アルキル、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~8員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、または置換もしくは非置換の5~10員の芳香族複素環式基であり;
Y1は単結合、-S-、-S(O)-、-S(O)2-、-CRfRg-、または-NRg-であり;
nは0~3の整数であり;
Gは-NR11R12、-N3、-C(O)NR11R12、-S(O)2NR11R12、-SR13,-OR13、-CH2NR11C(O)R15、-C(=NOR13)-X、C(=NOR13)-Z、-C(O)OR13、-S(O)2R13、-C(O)NR11C(O)R15、-NR11(OR13)、-NR11C(O)R15、-NR11C(O)NR12R11a、-NR11C(O)OR13、-NR11S(O)2R13、-NR11S(O)2NR12R11a、-C(O)NR11NR12R11a、-S(O)2NR11NR12R11a、-C(=NR11)R15、-C(=NR11)NR12R11a、-NR11CR15(=NR12)、-NR11C(=NR12)NR11aR12a、置換もしくは非置換のC1-10アルキル、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の5~10員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、または置換もしくは非置換の5~10員の芳香族複素環式基であり;
R11,R12,R11a,およびR12aはそれぞれ独立して、水素原子、置換もしくは非置換のC1-10アルキル、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~8員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、または置換もしくは非置換の5~10員の芳香族複素環式基であり;
R13は水素原子、置換もしくは非置換のC1-10アルキル、置換もしくは非置換のC1-10アルキルカルボキシ、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~8員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、または置換もしくは非置換の5~10員の芳香族炭素環式基であり;
R15は水素原子、ヒドロキシ、置換もしくは非置換のC1-6アルキルオキシ、置換もしくは非置換のC1-10アルキル、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~8員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、または置換もしくは非置換の5~10員の芳香族複素環式基であり;
Xは水素または置換もしくは非置換のC1-6アルキルであり;
Zは、置換もしくは非置換のC3-8非芳香族炭素環式基、置換もしくは非置換の非芳香族複素環式基、置換もしくは非置換の芳香族炭素環式基、または置換もしくは非置換の芳香族複素環式基であり;
R2およびR3はそれぞれ結合している炭素原子と一緒になって、C3-7非芳香族炭素環、3~10員の非芳香族複素環、C6-10芳香族炭素環、および5~10員の芳香族複素環から選択される単環もしくは2環以上の環を形成していてもよく、ここで、該環はR5から選択される1以上の基で置換されていてもよく;R5はハロゲン、アミノ、シアノ、ヒドロキシ、置換もしくは非置換のC1-6アルキル、アシル、カルボキシ、-C(O)NRaRb、-N(Ra)C(O)Rb、-S(O)2NRaRb、-N(Ra)S(O)2Rb、-SRc、-C(O)(CH2)0-3SRc、-NRfC(=NRc)NRfRg,-S(O)(CH2)1-3Rc、および-NRfS(O)2NRfORdから選択される基であり;
R2およびR3は一緒になって単結合を形成してもよく;
R6a,R6b,およびR6cはそれぞれ独立して、水素原子、アミノ、ヒドロキシ、ハロゲン、シアノ、置換もしくは非置換のC1-6アルキル、置換もしくは非置換のC1-6ハロアルキル、置換もしくは非置換のC1-6アルキルオキシ、置換もしくは非置換のC1-6ハロアルキルオキシ、置換もしくは非置換の(C1-6アルキルオキシ)C1-6アルキル、置換もしくは非置換のC2-10アルケニル、置換もしくは非置換のC2-10アルキニル、置換もしくは非置換のC3-7非芳香族炭素環式基、置換もしくは非置換の3~10員の非芳香族複素環式基、置換もしくは非置換のC6-10芳香族炭素環式基、置換もしくは非置換の5~10員の芳香族複素環式基、置換もしくは非置換の(C3-7非芳香族炭素環)C1-6アルキル、置換もしくは非置換の(3~10員の非芳香族複素環)C1-6アルキル、置換もしくは非置換の(3~10員の非芳香族複素環)C1-6アルキル、置換もしくは非置換の(C6-10芳香族炭素環)C1-6アルキル、(C6-10芳香族炭素環)C1-6アルキルオキシ、置換もしくは非置換の(5~10員の非芳香族複素環)C1-6アルキル、アシル、カルボキシ、-C(O)NRaRb、-N(Ra)C(O)Rb、-S(O)2NRaRb、-N(Ra)S(O)2Rb、-SH、-C(O)(CH2)0-3SRc、-C(O)(CH2)1-3Rd、―NRfC(O)NRfRg、-NRfS(O)2NRfRg、―C(=NRe)Rc、-C(=NRe)NRfRg、-NRfCRc(=NRe)、-NRfC(=NRe)NRfRg、-S(O)(CH2)1-3Rc、-NRfS(O)2NRfORd、または-(CH2)p-Y1-(CH2)qKであり;
Kは-C(O)NRaRb、-N(Ra)C(O)Rb、-S(O)2NRaRb、-N(Ra)S(O)2Rb、-NRfC(O)NRfRg、置換基群(C1-4アルキルオキシ、アミノ、ハロゲン、および-C(O)NRfRg)から選択される1または2の基で置換されていてもよいC1-4アルキル、置換基群(C1-4アルキル、C1-4アルキルオキシ、アミノ、ハロゲン、および-C(O)NRfRg)から選択される1または2の基で置換されていてもよいC6-10芳香族炭素環式基、置換基群(C1-4アルキル、C1-4アルキルオキシ、アミノ、ハロゲン、-C(O)NRaRb、および-N(Ra)C(O)Rb)から選択される1または2の基で置換されていてもよいC3-7非芳香族炭素環式基、置換基群(C1-4アルキル、C1-4アルキルオキシ、アミノ、ハロゲン、-C(O)NRaRb、および-N(Ra)C(O)Rb)から選択される1または2の基で置換されていてもよい5~10員の芳香族複素環式基、または置換基群(C1-4アルキル、C1-4アルキルオキシ、アミノ、ハロゲン、-C(O)NRaRb、および-N(Ra)C(O)Rb)から選択される1または2の基で置換されていてもよい3~10員の非芳香族複素環式基であり;
pおよびqはそれぞれ独立して0または1の整数である)
で示される化合物またはその製薬上許容される塩を含有する医薬組成物。
