现有技术中,医用吻合器包括医用吻合器本体、活动连接所述医用吻合器本体的操作把手以及与所述本体配合的钉头部。所述钉头部包括相对设置的钉仓和钉砧。在手术时,首先握持活动把手,通过闭合驱动机构拉动钉头部的闭合拉片向吻合器的近端侧运动,从而将钉仓和钉砧闭合后,再次握持操作把手,可以推动吻合钉向组织运动,通过钉仓中的吻合钉在钉砧组件处的成型,将组织吻合,同时切刀从吻合器的近端侧向远端侧运动,而切割组织。在本揭露中,远端侧和近端侧是相对于操作者来说的,距离操作者较近的一端为近端侧,距离操作者较远的一端,即更靠近手术位置的一端为远端侧。
现有的闭合驱动机构中,结构较为复杂,并且在击发吻合器的过程中,闭合拉片可能会有向吻合器远端侧方向的位移,而影响钉仓和钉砧之间的闭合效果,对手术效果有不利影响。此外,现有的闭合驱动机构中,在吻合器击发完成后,钉仓和 钉砧仍然保持闭合,医生在操作吻合器完成后,医生需要回拉推刀杆,使得切刀沿着切刀槽返回至初始位置,在回拉切刀的过程中,如果钉仓和钉砧仍保持闭合,切刀会受到钉仓和钉砧之间的组织的阻力而无法顺畅地返回至初始位置。
发明内容
针对现有技术中的问题,本揭露的目的在于提供一种闭合驱动机构及包括其的医用吻合器,通过保持件和第一滑块的嵌合对钉头部闭合时闭合驱动件的位置进行限定,并且在吻合器击发完成后,通过推块的抬升作用使得保持件与第一滑块脱离,闭合驱动件可以运动以打开钉头部。
本揭露实施例提供一种闭合驱动机构,用于医用吻合器,所述闭合驱动机构包括第一滑块、保持件、闭合驱动件和推块,所述第一滑块与所述闭合驱动件相连接,所述第一滑块包括第一嵌合部,所述保持件包括相连接的第二嵌合部和第一抬升部,所述推块包括位于所述第一抬升部下方的第二抬升部;
所述吻合器的钉头部闭合时,所述第一滑块的第一嵌合部位于所述保持件的第二嵌合部的下方,且所述第一嵌合部与所述第二嵌合部嵌合,此状态下向上抬起所述推块时,所述推块的第二抬升部向上抬起所述保持件的第一抬升部,所述第二嵌合部向上抬起而与所述第一嵌合部脱离。
在一些实施例中,初始状态下,所述推块的第二抬升部不与所述保持件的第一抬升部接触。
在一些实施例中,所述第一嵌合部的初始位置位于所述第二嵌合部的近端侧,所述闭合驱动机构还包括活动把手和按压件,所述按压件的初始位置位于所述保持件的近端侧;
在初始状态下握持所述活动把手时,所述第一滑块向吻合器的远端侧方向运 动,使得所述第一嵌合部运动至所述第二嵌合部的下方,所述按压件向所述吻合器的远端侧方向运动并向下按压所述保持件,使得所述第二嵌合部向下运动而与所述第一嵌合部嵌合,所述第一滑块带动所述闭合驱动件向所述吻合器的近端侧运动,以闭合所述钉头部。
在一些实施例中,所述闭合驱动机构还包括致动杆,所述按压件设置于所述致动杆的远端侧,在初始状态下握持所述活动把手时,所述活动把手驱动所述致动杆向所述吻合器的远端侧运动。
在一些实施例中,所述保持件包括第二滑块和第三滑块,所述第二滑块包括所述第二嵌合部和所述第一抬升部,在初始状态下握持所述活动把手时,所述按压件向下按压所述第三滑块,使得所述第三滑块带动所述第二滑块向下运动。
在一些实施例中,所述第二滑块与所述第三滑块之间设置有第一偏置件,所述第一偏置件给所述第二滑块一朝向下方的偏置力;
在所述第一嵌合部和所述第二嵌合部嵌合的状态下,向上抬起所述推块时,所述第二滑块相对于所述第三滑块向上运动,所述第一偏置件产生变形。
在一些实施例中,所述吻合器的钉头部闭合时,所述推块的第二抬升部不与所述第二滑块的第一抬升部接触;或,
所述吻合器的钉头部闭合时,所述推块的第二抬升部与所述保持件的第一抬升部接触,但所述第二抬升部给所述第二滑块的作用力不足以驱使所述第一偏置件产生变形。
在一些实施例中,所述第三滑块的内部设置有容置槽,所述第二滑块设置于所述容置槽中。
在一些实施例中,所述推块与所述吻合器的壳体之间设置有第二偏置件,所述 第二偏置件给所述推块一朝向下方的偏置力;
向上抬起所述推块时,所述第二偏置件发生变形。
在该实施例中,所述闭合驱动件的近端侧设置有第三偏置件,所述第三偏置件给所述闭合驱动件一朝向所述吻合器的远端侧方向的偏置力。
在一些实施例中,所述推块还包括操作部,所述推块的操作部的第一侧突出于所述吻合器的壳体外侧。
在一些实施例中,所述推块的第二抬升部为突出于所述操作部的第二侧的抬升臂。
在一些实施例中,所述保持件与所述吻合器的壳体之间设置有第四偏置件,所述第四偏置件给所述保持件一朝向上方的偏置力。
在一些实施例中,所述第一嵌合部为凸台,所述第二嵌合部为凹槽;或
所述第一嵌合部为凹槽,所述第二嵌合部为凸台。

现在将参考附图更全面地描述示例实施方式。然而,示例实施方式能够以多种形式实施,且不应被理解为限于在此阐述的实施方式;相反,提供这些实施方式使得本揭露将全面和完整,并将示例实施方式的构思全面地传达给本领域的技术人员。在图中相同的附图标记表示相同或类似的结构,因而将省略对它们的重复描述。
为了解决现有技术中的技术问题,本揭露提供了一种用于医用吻合器的闭合驱 动机构以及包括该钉仓的医用吻合器。所述吻合器包括钉头部和吻合器本体,所述吻合器本体中设置有用于闭合所述钉头部的闭合拉片。所述闭合驱动机构用于医用吻合器,所述闭合驱动机构包括第一滑块、保持件、闭合驱动件和推块,所述闭合驱动件与所述闭合拉片相连接,可以驱动所述钉头部打开和闭合。具体地,定义所述闭合驱动件具有闭合位置和打开位置,所述闭合驱动件在所述闭合位置时,所述钉头部闭合,所述闭合驱动件在所述打开位置时,所述钉头部打开。所述第一滑块与所述闭合驱动件相连接,所述第一滑块包括第一嵌合部,所述保持件包括相连接的第二嵌合部和第一抬升部,所述推块包括位于所述第一抬升部下方的第二抬升部。
所述吻合器的钉头部闭合时,所述第一滑块的第一嵌合部位于保持件的第二嵌合部的下方,且所述第一嵌合部与第二嵌合部嵌合,在所述保持件的作用下,所述闭合驱动件被保持在所述闭合位置,所述钉头部保持在闭合状态而不会意外张开。在所述吻合器击发完成后,需要打开钉头部时,操作者向上抬起推块,所述推块的第二抬升部向上抬起所述保持件的第一抬升部,所述第一抬升部带动与其相连接的所述第二嵌合部向上抬起而与所述第一嵌合部脱离,由此所述保持件不再对第一滑块的位置进行限定,也就不会限制与第一滑块相连接的闭合驱动件的位置,闭合驱动件失去了保持件的保持作用而可以移动到打开位置,从而打开所述钉头部,此时可以十分顺畅地拉回切刀。
下面结合图1~13具体介绍本揭露在具体实施方式中的闭合驱动机构和吻合器的结构。
如图1~5所示,在本揭露第一实施例中,所述吻合器包括吻合器本体1和位于吻合器本体1的远端侧的钉头部(图中未示出),吻合器本体1包括壳体13和 固定把手11,吻合器本体1的内部还设置有用于闭合钉头部和打开钉头部的闭合拉片12,闭合拉片12在初始状态下时,钉头部打开,闭合拉片12向吻合器近端侧运动时,钉头部闭合,闭合拉片12再向吻合器远端侧运动时,钉头部可以重新打开。所述闭合驱动机构包括第一滑块5、保持件、闭合驱动件83和推块4,所述第一滑块5与所述闭合驱动件83相连接,所述第一滑块5包括第一嵌合部,所述保持件包括相连接的第二嵌合部和第一抬升部64,所述推块4包括位于所述第一抬升部64下方的第二抬升部41。
在本揭露中,远端侧和近端侧是相对于操作者来说的,其中,远端侧指的是距离操作者较远的一端,近端侧指的是距离操作者较近的一端。例如,在图1和图2的视角中,吻合器本体1的远端侧为图中左边一端,吻合器本体1的右端侧为图中右边一端,致动杆2的远端侧为图中左边一端,致动杆2的近端侧为图中右边一端。此外,在本揭露中,上方和向上的方向指的是对于致动杆2来说,朝向背离所述齿条22的一侧,下方和向下的方向与之相反,例如,在图2的视角中,向上的方向是指图中上方一端。
在该实施例中,所述第一滑块5的所述第一嵌合部为第一凹槽51,所述保持件的第二嵌合部为凸台61。在其他可替代的实施方式中,也可以第一嵌合部为凸台,所述第二嵌合部为凹槽。下面以该实施例的第一嵌合部和第二嵌合部的结构为例进行说明。
如图3和图4所示,在该实施例中,所述保持件包括第二滑块6和第三滑块7,所述第三滑块7的内部设置有容置槽71,所述第二滑块6设置于所述容置槽71中。所述第二滑块6包括所述凸台61和所述第一抬升部64。所述第二滑块6与所述第三滑块7之间设置有第一偏置件,所述第一偏置件给所述第二滑块6一朝向下方的 偏置力。所述第三滑块7和所述吻合器的壳体直接设置有第四偏置件,所述第四偏置件给所述第三滑块7一朝向上方的偏置力。在该实施例中,所述第一偏置件为设置于所述容置槽71的内壁与所述第二滑块6之间的第一压簧74,所述第四偏置件为设置于所述第三滑块7的底部的第二压簧75。但本揭露不限于此,在其他可替代的实施方式中,第一偏置件和第四偏置件也可以分别采用拉簧、金属弹片等其他弹性结构。
如图5所示,所述推块4还包括操作部42,所述推块4的第二抬升部41为突出于所述操作部42的第二侧的抬升臂。所述推块4的操作部42的第一侧突出于所述吻合器的壳体外侧,医生可以直接在器械的外侧操作推块4的操作部42。所述推块4与所述吻合器的壳体之间设置有第二偏置件,所述第二偏置件给所述推块4一朝向下方的偏置力。在该实施例中,所述第二偏置件为设置于所述推块4上方的第三压簧43。但本揭露不限于此,在其他可替代的实施方式中,第二偏置件也可以采用压簧、金属弹片等其他弹性结构。如图10和图11所示,所述推块4设有第一导向部44,壳体13(壳体13示于图1中)与之适应地设有第二导向部(图未示),所述第一导向部44及所述第二导向部互为凸台和凹槽结构,且使得所述推块4沿所述第三压簧43的方向设置。即如图10所示,由于第一导向部44和第二导向部的导向作用,推块4可以沿图中A方向或与A方向相反的方向运动。所述推块4的底部即背离所述第三压簧43的一侧,抵持于所述壳体13,使得所述推块4不脱离所述壳体。
进一步地,所述闭合驱动件83的近端侧设置有第三偏置件,所述第三偏置件给所述闭合驱动件83一朝向所述吻合器的远端侧方向的偏置力。在该实施例中,所述第三偏置件为设置于所述闭合驱动件83的近端侧的第三压簧84,在其他可替 代的实施方式中,所述第三偏置件也可以采用拉簧、金属弹片等其他弹性结构。
所述第一滑块5通过一个转向组件与闭合驱动件83相连接。所述转向组件包括转向支撑梁82和拉绳81,所述转向支撑梁82固设于壳体13,优选地,所述转向支撑梁82设为滑轮,以减少所述拉绳81运动时的阻力。所述滑轮82位于所述第一滑块5的近端侧,所述拉绳81套设于所述滑轮82的外部,且所述拉绳81连接于所述第一滑块5和所述闭合驱动件83之间。优选地,所述拉绳81的两端分别设有固定端。第一滑块5的近端侧可以设置有第二凹槽52,所述拉绳81的一固定端可拆卸地安装于第二凹槽52中。所述拉绳81的另一固定端可拆卸地连接于所述闭合驱动件83。通过拉绳81和滑轮82的结构,可以将第一滑块5的运动转化为反向的驱动力传递给闭合驱动件83。
在该实施例中,所述闭合驱动机构还包括活动把手3和按压件,所述按压件为设置于致动杆2的远端侧的凸缘23。在初始状态下,所述第一滑块5的第一凹槽51的初始位置位于所述保持件的凸台61的近端侧,所述按压件的初始位置位于所述保持件的近端侧。
如图6~11所示,在初始状态下握持所述活动把手3时,所述活动把手3的旋转运动带动连杆32向吻合器的远端侧方向运动,进而使得所述连杆32推动所述第一滑块5向吻合器的远端侧方向运动,使得所述第一凹槽51运动至所述凸台61的下方。所述转向组件将所述第一滑块5向吻合器的远端侧方向的运动转化为闭合驱动件83向吻合器的近端侧方向的运动,使得所述闭合驱动件83从张开位置运动至闭合位置,进而拉动闭合拉片12向吻合器的近端侧运动,从而闭合所述吻合器的钉头部。如图6所示,此时,所述第三压簧84受到所述闭合驱动件83的作用力而产生压缩变形。
同时,在初始状态下活动把手3在被握持而旋转时,活动把手3上的卡爪31与致动杆2上的推进齿21接触,并向吻合器远端方向推动推进齿21,使得致动杆2也向吻合器远端侧方向运动,所述致动杆2带动凸缘23向所述吻合器的远端侧方向运动至与所述第三滑块7相接触后,向下按压所述第三滑块7,所述第三滑块7带动所述第二滑块6向下运动,所述第二滑块6的凸台61与所述第一滑块5的第一凹槽51嵌合,以阻碍所述第一滑块5向吻合器的近端侧的运动,将闭合驱动件83保持在闭合位置,保持钉头部闭合时的稳定性。如图6和图7所示,此时,由于所述凸缘23对所述第三滑块7的向下的压力,所述第三滑块7向下压缩所述第二压簧75,使得所述第二压簧75产生压缩变形。优选地,在初始状态下,所述第二滑块6的第一抬升部64与所述推块4的第二抬升部41之间设有一定距离,即第二滑块6的第一抬升部64与推块4的第二抬升部41不接触;如图10和图11所示,在握持所述活动把手3,使得钉头部保持闭合时状态时,所述第二滑块6的凸台61与所述第一滑块5的第一凹槽51嵌合,此时所述第二滑块6的第一抬升部64与所述推块4的第二抬升部41之间设有微量距离,即第一抬升部64与第二抬升部41不接触,但是需保证向上抬起推块4时,推块4在向上运动过程中会接触并抬升第一抬升部64,或者所述第二滑块6的第一抬升部64与所述推块4的第二抬升部41有轻微接触,但是第二抬升部41给第一抬升部64的作用力不足以压缩第一压簧74产生变形。如此设置,以使得在不施加推力至所述推块4时,所述第二抬升部41的设置不阻碍所述第二滑块6的凸台61与所述第一滑块5的第一凹槽51嵌合,保证稳定的闭合。
如图8所示,所述凸缘23的远端侧面为从其近端侧边向远端侧边向上倾斜的凸缘第一斜面24,所述第三滑块7的近端侧面为与所述凸缘第一斜面24相配合的 第三滑块第一斜面72。通过凸缘第一斜面24和第三滑块第一斜面72的相互引导作用,凸缘23在向吻合器远端侧运动时,可以更加顺畅地接触并压迫第三滑块7。
如图4和图9所示,所述第一滑块5的远端侧面为从其远端侧边向近端侧边向上倾斜的第一滑块第一斜面53,所述第一滑块第一斜面53为从其近端侧向远端侧向下倾斜的一斜面,所述第二滑块6的凸台61的近端侧面为与所述第一滑块第一斜面53配合的凸台斜面62。通过第一滑块第一斜面53和凸台斜面62的相互引导作用,可以更好地引导第二滑块6的凸台61进入到第一滑块5的第一凹槽51中。进一步地,所述第一滑块5的第一凹槽51具有与第一凸台61的形状相适应的形状,即第一凹槽51的近端侧面为从其远端侧边向近端侧边向上倾斜的第一滑块第二斜面54。
所述第一滑块5的第一凹槽51的远端侧面形成为第一止挡面55,与此对应地,第二滑块6的凸台61的远端侧面形成为第二止挡面63,第一止挡面55和第二止挡面63均为垂直面。在初始位置时,所述第一止挡面55与所述第二止挡面63间隔设置。通过第一止挡面55和第二止挡面63这两个垂直面的配合,在凸台61位于第一凹槽51中时,阻挡第一滑块5向近端侧运动。
在钉头部闭合之后,在图6中示出的吻合器的状态下,再次握持活动把手3时,活动把手3被握持旋转,其卡爪31与齿条22接触,并通过推动齿条22而继续推动致动杆2向吻合器的远端方向运动,致动杆2上设置有击发件25,击发件25在向吻合器远端方向的运动过程中可以击发吻合器,使吻合器吻合和切割组织。而在吻合器的击发过程中,在凸缘23的按压作用下,第二滑块6的凸台61一直保持在第一滑块5的第一凹槽51中,从而保持在吻合器击发过程中,钉头部闭合的稳定性。
在吻合器击发完成时,如果需要打开钉头部,使得钉仓和钉砧分离,则需要使得保持件与第一滑块5脱离,即在该实施例中,使得第二滑块6的凸台61与第一滑块5的第一凹槽51脱离。
在图10中示出的钉头部闭合时闭合驱动机构各个部件的配合状态下,医生沿图10中A箭头示出的方向抬起推块4的操作部42,使得推块4向上运动时,推块4的第二抬升部41向上抬起第二滑块6的第一抬升部64,使得第二滑块6向上运动。如图12和图13所示,在第二滑块6被推块4向上抬起时,由于第一滑块7仍在凸缘23的按压下高度保持不变,第二滑块6相对于第一滑块7具有向上运动的趋势而压缩第一压簧74产生变形,第二滑块6的凸台61向上运动而与第一滑块5的第一凹槽51分离。同时,在推块4的抬起过程中,推块4压缩第三压簧43产生压缩变形,第二滑块6被推块4向上抬起,以分离第二滑块6的凸台61和第一滑块5的第一凹槽51。如图13所示,在第二滑块6不再对第一滑块5的位置进行限定后,在第三压簧84朝向吻合器远端侧的变形恢复力的作用下,闭合驱动件83会拉动拉绳81向吻合器的远端侧方向运动,而在转向支撑梁82的转向作用下,闭合驱动件83的运动驱动所述第二滑块6向吻合器的近端侧运动。此时,闭合驱动件83驱动闭合拉片12向吻合器的远端侧运动,吻合器的钉头部可以打开,钉砧和钉仓分离,可以释放钉砧和钉仓之间的组织,更顺畅地将切刀拉回。
图1~13仅示出了本揭露一个具体实施例的闭合驱动机构的具体结构,而不作为本揭露的保护范围的限制。在其他可替代的实施方式中,其中的各个部件可以进行各种变形,均属于本揭露的保护范围之内。例如,第二滑块6和第三滑块7的位置关系不限于采用容置槽的方式,第三滑块7上可以开设导槽,第二滑块6可以嵌设于导槽中,并且导槽中可以设置弹性件,使得第一滑块7向下运动时带动第二滑 块6向下运动,而第一滑块7固定不动时,第二滑块6同时可以相对于第一滑块7有向上的运动趋势。推块4的第二抬升部的结构也不限于抬升臂,也可以采用凸块、圆台等其他结构。按压件也不限于致动杆2上的凸缘结构,也可以采用击发件25压迫第三滑块7,或增设其他的在活动把手3握持时向远端侧运动的凸台结构作为按压件等等。

  1. 一种闭合驱动机构,其特征在于,用于医用吻合器,所述闭合驱动机构包括第一滑块、保持件、闭合驱动件和推块,所述第一滑块与所述闭合驱动件相连接,所述第一滑块包括第一嵌合部,所述保持件包括相连接的第二嵌合部和第一抬升部,所述推块包括位于所述第一抬升部下方的第二抬升部;
    所述吻合器的钉头部闭合时,所述第一滑块的第一嵌合部位于所述保持件的第二嵌合部的下方,且所述第一嵌合部与所述第二嵌合部嵌合,此状态下向上抬起所述推块时,所述推块的第二抬升部向上抬起所述保持件的第一抬升部,所述第二嵌合部向上抬起而与所述第一嵌合部脱离。
  2. 根据权利要求1所述的闭合驱动机构,其特征在于,初始状态下,所述推块的第二抬升部不与所述保持件的第一抬升部接触。
  3. 根据权利要求1所述的闭合驱动机构,其特征在于,所述第一嵌合部的初始位置位于所述第二嵌合部的近端侧,所述闭合驱动机构还包括活动把手和按压件,所述按压件的初始位置位于所述保持件的近端侧;
    在初始状态下握持所述活动把手时,所述第一滑块向吻合器的远端侧方向运动,使得所述第一嵌合部运动至所述第二嵌合部的下方,所述按压件向所述吻合器的远端侧方向运动并向下按压所述保持件,使得所述第二嵌合部向下运动而与所述第一嵌合部嵌合,所述第一滑块带动所述闭合驱动件向所述吻合器的近端侧运动,以闭合所述钉头部。
  4. 根据权利要求3所述的闭合驱动机构,其特征在于,所述闭合驱动机构还包括致动杆,所述按压件设置于所述致动杆的远端侧,在初始状态下握持所述活动把手时,所述活动把手驱动所述致动杆向所述吻合器的远端侧运动。
  5. 根据权利要求3所述的闭合驱动机构,其特征在于,所述保持件包括第二滑块和第三滑块,所述第二滑块包括所述第二嵌合部和所述第一抬升部,在初始状态下握持所述活动把手时,所述按压件向下按压所述第三滑块,使得所述第三滑块带动所述第二滑块向下运动。
  6. 根据权利要求5所述的闭合驱动机构,其特征在于,所述第二滑块与所述第三滑块之间设置有第一偏置件,所述第一偏置件给所述第二滑块一朝向下方的偏置力;
    在所述第一嵌合部和所述第二嵌合部嵌合的状态下,向上抬起所述推块时,所述第二滑块相对于所述第三滑块向上运动,所述第一偏置件产生变形。
  7. 根据权利要求6所述的闭合驱动机构,其特征在于,所述吻合器的钉头部闭合时,所述推块的第二抬升部不与所述第二滑块的第一抬升部接触;或,
    所述吻合器的钉头部闭合时,所述推块的第二抬升部与所述保持件的第一抬升部接触,但所述第二抬升部给所述第二滑块的作用力不足以驱使所述第一偏置件产生变形。
  8. 根据权利要求5所述的闭合驱动机构,其特征在于,所述第三滑块的内部设置有容置槽,所述第二滑块设置于所述容置槽中。
  9. 根据权利要求3所述的闭合驱动机构,其特征在于,所述推块与所述吻合器的壳体之间设置有第二偏置件,所述第二偏置件给所述推块一朝向下方的偏置力;
    向上抬起所述推块时,所述第二偏置件发生变形。
  10. 根据权利要求3所述的闭合驱动机构,其特征在于,所述闭合驱动件的近端侧设置有第三偏置件,所述第三偏置件给所述闭合驱动件一朝向所述吻合器的远 端侧方向的偏置力。
  11. 根据权利要求1所述的闭合驱动机构,其特征在于,所述推块还包括操作部,所述推块的操作部的第一侧突出于所述吻合器的壳体外侧。
  12. 根据权利要求11所述的闭合驱动机构,其特征在于,所述推块的第二抬升部为突出于所述操作部的第二侧的抬升臂。
  13. 根据权利要求1所述的闭合驱动机构,其特征在于,所述保持件与所述吻合器的壳体之间设置有第四偏置件,所述第四偏置件给所述保持件一朝向上方的偏置力。
  14. 根据权利要求1所述的闭合驱动机构,其特征在于,所述第一嵌合部为凸台,所述第二嵌合部为凹槽;或
    所述第一嵌合部为凹槽,所述第二嵌合部为凸台。
  15. 一种医用吻合器,其特征在于,包括权利要求1至14中任一项所述的闭合驱动机构。
