Description
본 발명은 작약 추출물을 유효성분으로 포함하는 악액질의 예방, 개선 또는 치료용 조성물에 관한 것이다.
악액질(cachexia)이란 암, 결핵, AIDS, 만성 폐색성 폐질환 등과 같은 만성질환에서 흔하게 동반되는 증후군으로 지속적인 식욕부진과 체중감소를 보이며 이로 인한 영양실조나 대사불균형, 근육이나 지방의 감소를 동반한 체내대사의 이화현상(catabolic state)을 의미하는데, 암 환자의 경우 다른 만성 질환자와는 달리 악액질뿐만 아니라 암의 치료에 이용되는 다양한 항암치료법의 부작용까지 동반되는 특성이 있다 (Fearon K. 등, Lancet Oncol 2011;12(5):489-495).
악액질은 소화기암 및 폐암환자의 50-80%에서 발생하며 악액질에 의한 사망률은 20-30%에 이른다. 암성 악액질은 다양한 사이토카인들이 일으키는 염증반응 및 대사변화에 따른 이화반응의 증가로 근육 손실을 초래하여 체중감소가 나타나는 것이 특징이다. 이러한 변화는 항암화학요법이나 방사선치료에 대한 반응율을 낮추고 효과적인 항암치료의 진행을 어렵게하며 환자의 삶의 질을 저하시키게된다.
악액질의 발생기전은 신경 내분비계의 활동도의 변화, 다양한 염증성 사이토카인(IL-1beta, IL-6, TNF-α)과 암특이 악액인자의 분비, 이들에 의한 음식섭취의 감소 및 대사의 변화로 알려져 있다. 이중 염증성 사이토카인은 종양에서 직접 분비되거나 종양에 반응해서 인체의 면역세포에서 생산되는 단백으로 면역제어에 중요한 역할을 하며 식욕부진, 체중감소를 일으켜 악액질 발생에 가장 중요한 역할을 한다 (Argiles JM 등, Curr. Opin. Clin. Nutr. Metab. Care 1998;1:245-251). 염증성 사이토카인들은 미각 변화나 위장관 기능의 변화를 초래하여 식욕을 감소시킨다고 알려져 있다 (Durham WJ 등, Curr Opin Clin Nutr Metab Care. 2009; 12:72-77).
근손실은 악액질의 가장 큰 특징 중의 하나이며, 다양한 사이토카인의 과활성화로 인한 단백질 이화작용의 증가와 단백질 생성의 감소 때문으로 알려져 있다. 악액질은 근손실(근감소증)을 포함하고 있는 증상으로 서로 겹쳐지는 부분이 많다. 악액질을 가지고 있는 환자의 대부분이 근손실(근감소증)을 가지고 있으나, 근손실을 보이는 모든 환자가 악액질 증상을 보이지는 않는다. 임상적으로 표현하면 근손실(근감소증)은 악액질의 전구증상으로 말할 수 있다. 악액질에 작용하는 염증성 사이토카인은 근육의 대사를 조절하는 인슐린과 테스토스테론에 영향을 미쳐 근육 단백질 합성에 이상을 일으킨다 (류승완, J. Clin. Nutr. 2017;9:2-6).
악액질의 치료에는 현재 프로게스테론 제제(megestrol acetate; megace) 및 스테로이드(dexamethasone, prednisone) 등이 사용되고 있으나, 스테로이드는 치료효과가 지속적이지 않고 장기 사용 시 수분저류, 인슐린저항성, 부신기능저하 등의 부작용이 있어서 단기처방만 가능하다. 프로게스테론 제제(megace)는 미국 FDA에서 식욕부진이나 체중감소를 보이는 AIDS 환자에게 사용이 허가되었고, 현재 국내에서 암성 악액질의 치료목적으로 승인되어 있어서 본 발명의 비교 약물로 사용되었다. 그러나, 근육보다 지방조직을 증가시키고 발기부전, 자궁출혈, 혈전색전증, 부종, 고혈당증, 부신기능저하 등의 부작용이 있다.
악액질이 나타난 환자는 항암치료에 낮은 반응을 보이고 심각한 부작용을 경험한다. 암 환자를 대상으로 한 항암치료 방법에는 크게 외과 수술(Operation), 화학 요법(Chemotherapy), 방사선 요법(Radiotherapy) 등이 있다. 외과 수술은 병소(즉, 암)를 제거하는 치료 방법으로서 초기 암의 경우에는 외과 수술만으로도 완치가 가능하지만, 중기 이후 암의 경우에는 외과 수술 만으로는 치료가 불가능하다. 화학 요법이나 방사선 요법 또는 이의 병행 요법은 공통적으로 유리 라디칼을 생성시켜 표적 암세포를 제거하는 것이지만, 이들은 특이성이 낮기 때문에 조혈 세포나 면역 세포 등 빠르게 분열 및 증식하는 특성을 가진 정상 세포도 손상시킨다. 따라서 구토증세, 식욕감퇴, 구내염, 설사, 변비, 발열, 감염증, 백혈구감소증, 혈소판 감소증, 빈혈, 복통, 신장 독성, 간 독성, 심장 독성, 말초신경 독성, 중추신경 독성, 근육 통증, 뼈 통증, 골수 억제 등의 부작용을 유발한다. 암환자에서 높은 빈도로 발생하는 악액질 및 근손실은 항암치료에 대한 반응율을 낮추고 효과적인 항암치료의 진행을 어렵게하며 환자의 삶의 질을 저하시키게된다.
따라서, 암 및 기타 질환에서 발생하는 악액질 및 근손실을 치료하기 위하여 기존 치료제 보다 우수하고 인체에 안전한 천연소재의 개발이 요구되고 있다.
본 발명의 하나의 목적은 작약(Paeoniae Radix) 추출물을 유효성분으로 포함하는 악액질(cachexia) 및 근손실 예방 또는 치료용 약학적 조성물을 제공하는 것이다.
본 발명의 다른 목적은 작약 추출물을 유효성분으로 포함하는 악액질 및 근손실 예방 또는 개선용 식품 조성물을 제공하는 것이다.
본 발명의 또 다른 목적은 작약 추출물을 유효성분으로 포함하는 악액질 및 근손실 예방 또는 치료용 약학적 조성물을 포함하는 항암 보조제를 제공하는 것이다.

상기 목적을 달성하기 위하여, 본 발명의 일 양상은 작약 추출물을 유효성분으로 포함하는 악액질 및 근손실 예방 또는 치료용 약학적 조성물 제공한다.
본 명세서에 사용된 용어, '악액질'은 암, 결핵, 당뇨병, 후천성면역결핍증(Acquired immunodeficiency syndrome, AIDS) 등의 말기에서 볼 수 있는 고도의 전신쇠약증세를 말하며, 위암이나 식도암, 대장암 등 소화기암 환자와 폐암 환자에서 자주 나타난다. 식욕 감소, 체중 및 체력의 감소, 빈혈, 무기력, 소화 불량 등의 증상을 나타내며, 특히 식욕이 감소하여 정상적인 음식 섭취가 어려운 상태를 보이거나 또는 정상적으로 음식을 섭취하더라도 체중이 감소하는 상태를 나타낸다. 악액질이 발생하면 항암 화학요법이나 방사선 치료에 대해 낮은 반응을 보이므로 환자의 삶의 질이 저하되고, 기대 여명의 단축을 초래하며, 전체 암환자의 10 내지 20%에서 체중 감소로 인한 사망을 유발한다.
본 명세서에서 사용된 용어, '근손실(muscle loss)'은 체내 근육이 감소되는 것을 말하며, 예를 들어, 근육을 사용하지 않음으로써 발생하는 근육 조직의 감소, 근육 자체의 병으로 인한 근육 조직의 감소 또는 근육을 지배하는 신경의 손상으로 인한 근육 조직의 감소일 수 있고, 이와 같은 근손실은 예를 들어, 악액질 또는 근위축증(muscular atrophy)에 의하여 발생하는 것일 수 있다.
본 발명의 일 구체예에 따르면, 상기 악액질은 암에 의해 유발될 수 있으며, 한편, 암 치료에 이용되는 항암제에 의해 유발될 수 있다. 상기 항암제는 독소루비신(doxorubicin), 이리노테칸(irinotecan), 파클리탁셀 (paclitaxel), 다우노루비신(daunorubicin), 도세탁셀(docetaxel), 시스플라틴(cisplatin) 및 5-플루오로우라실(5-fluorouracil)로 이루어진 군에서 선택될 수 있다.
본 조성물의 유효성분으로 사용된 작약(Paeoniae Radix)는 대한민국약전 규격에 해당하며, 작약과(Paeoniaceae) 식물인 작약(Paeonia lactiflora Pallas)의 뿌리를 약재로 사용한다. 항균 효능이 있어 황색포도상구균, 이질균의 생장을 억제하고, 복통 및 설사에 효과가 있는 것으로 알려져 있으나, 식욕 감소 개선, 근육 손실 억제 등과 같은 악액질 개선 효과에 대해서는 알려진 바가 없다.
본 발명의 일 구체예에 따르면, 상기 작약 추출물은 당 업계에 공지된 통상의 방법에 따라 제조될 수 있다. 예를 들어, 작약을 분쇄한 후 추출에 통상적으로 사용되는 용매를 첨가하고, 적절한 온도와 압력으로 추출하여 작약 추출물을 제조할 수 있다.
본 발명의 일 구체예에 따르면, 상기 용매는 증류수, C1 내지 C4의 저급 알코올, 헥산, 에틸아세테이트, 클로로포름, 디에틸에테르, 디클로로메탄, 아세톤 및 이들의 혼합물로 이루어진 군에서 선택될 수 있다.
한편, 작약 추출물은 상술한 용매 추출법에 의한 추출물뿐만 아니라, 통상적인 정제 과정을 거친 추출물도 포함한다. 예컨대, 일정한 분자량 컷-오프 값을 갖는 한외 여과막을 이용한 분리, 다양한 크로마토그래피(크기, 전하, 소수성 또는 친화성에 따른 분리를 위해 제작된 것)에 의한 분리 등, 추가적으로 실시한 다양한 정제 방법을 통해 얻어진 분획도 본 발명의 작약 추출물에 포함될 수 있다. 또한, 상기 작약 추출물은 감압 증류 및 동결 건조 또는 분무 건조 등과 같은 추가적인 과정에 의해 분말 상태로 제조될 수도 있다.
본 발명의 일 구체예에 따르면, 상기 작약 추출물은 식욕 감소, 체중 감소, 피로도 증가, 염증 증가, 근육 손실 및 조혈 독성으로 이루어진 군에서 선택되는 증상을 개선할 수 있으므로 악액질 및 근손실의 개선, 예방 또는 치료 용도로 유용하게 사용될 수 있다.
상기 약학적 조성물은 투여를 위해서 상기 기재한 작약 추출물 이외에 추가로 약학적으로 허용 가능한 담체를 1종 이상 포함하여 약학적 조성물로 바람직하게 제제화할 수 있다.
상기 약학적 조성물의 제제 형태는 과립제, 산제, 정제, 피복정, 캡슐제, 좌제, 액제, 시럽, 즙, 현탁제, 유제, 점적제 또는 주사 가능한 액제 등이 될 수 있다. 예를 들어, 정제 또는 캡슐제의 형태로의 제제화를 위해, 유효 성분은 에탄올, 글리세롤, 물 등과 같은 경구, 무독성의 약학적으로 허용 가능한 불활성 담체와 결합될 수 있다. 또한, 원하거나 필요한 경우, 적합한 결합제, 윤활제, 붕해제 및 발색제 또한 혼합물로 포함될 수 있다. 적합한 결합제는 이에 제한되는 것은 아니나, 녹말, 젤라틴, 글루코스 또는 베타-락토오스와 같은 천연 당, 옥수수 감미제, 아카시아, 트래커캔스 또는 소듐올레이트와 같은 천연 및 합성 검, 소듐 스테아레이트, 마그네슘 스테아레이트, 소듐 벤조에이트, 소듐 아세테이트, 소듐 클로라이드 등을 포함한다. 붕해제는 이에 제한되는 것은 아니나, 녹말, 메틸 셀룰로스, 아가, 벤토니트, 잔탄 검 등을 포함한다.
액상 용액으로 제제화되는 조성물에 있어서 허용 가능한 약학적 담체로는, 멸균 및 생체에 적합한 것으로서, 식염수, 멸균수, 링거액, 완충 식염수, 알부민 주사용액, 덱스트로즈 용액, 말토 덱스트린 용액, 글리세롤, 에탄올 및 이들 성분 중 1 성분 이상을 혼합하여 사용할 수 있으며, 필요에 따라 항산화제, 완충액, 정균제 등 다른 통상의 첨가제를 첨가할 수 있다. 또한, 희석제, 분산제, 계면활성제, 결합제 및 윤활제를 부가적으로 첨가하여 수용액, 현탁액, 유탁액 등과 같은 주사용 제형, 환약, 캡슐, 과립 또는 정제로 제제화할 수 있다.
본 발명의 약학적 조성물은 경구 또는 비경구로 투여할 수 있고, 비경구 투여인 경우에는 정맥 내 주입, 피하 주입, 근육 주입, 복강 주입, 경피 투여 등으로 투여할 수 있다.
본 발명의 약학적 조성물의 적합한 투여량은 제제화 방법, 투여 방식, 환자의 연령, 체중, 성, 병적 상태, 음식, 투여 시간, 투여 경로, 배설 속도 및 반응 감응성과 같은 요인들에 의해 결정될 수 있다. 본 발명의 일 구체예에 따르면, 본 발명의 약학적 조성물의 1일 투여량은 0.001 내지 10 g/kg이다.
본 발명의 약학적 조성물은 당해 발명이 속하는 기술 분야에서 통상의 지식을 가진 자가 용이하게 실시할 수 있는 방법에 따라, 약학적으로 허용되는 담체 및/또는 부형제를 이용하여 제제화 함으로써 단위 용량 형태로 제조되거나 또는 다용량 용기 내에 내입시켜 제조될 수 있다.
본 발명의 다른 양상은 작약(Paeoniae Radix) 추출물을 유효성분으로 포함하는 악액질 및 근손실 예방 또는 개선용 식품 조성물을 제공한다.
본 발명에 따른 식품 조성물은 상기 약학적 조성물과 동일한 방식으로 제제화하여 기능성 식품으로 이용하거나, 각종 식품에 첨가할 수 있다. 본 발명의 조성물을 첨가할 수 있는 식품으로는 예를 들어, 음료류, 과자류, 다이어트바, 유제품, 육류, 초코렛, 피자, 라면, 기타 면류, 껌류, 아이스크림류, 비타민 복합제, 건강보조식품류 등이 있다.
본 발명의 식품 조성물은 유효성분으로서 작약 추출물을 포함하는 이외에 식품 제조시에 통상적으로 첨가되는 성분을 포함할 수 있으며, 예를 들어, 단백질, 탄수화물, 지방, 영양소, 조미제 및 향미제를 포함한다. 상술한 탄수화물의 예는 모노사카라이드, 예를 들어, 포도당, 과당 등; 디사카라이드, 예를 들어 말토스, 슈크로스, 올리고당 등; 및 폴리사카라이드, 예를 들어 덱스트린, 사이클로덱스트린 등과 같은 통상적인 당 및 자일리톨, 소르비톨, 에리트리톨 등의 당알콜이다. 향미제로서 천연 향미제 [타우마틴, 스테비아 추출물 (예를 들어 레바우디오시드 A, 글리시르히진 등)] 및 합성 향미제(사카린, 아스파르탐 등)를 사용할 수 있다. 예를 들어, 본 발명의 식품 조성물이 드링크제와 음료류로 제조되는 경우에는 본 발명의 작약 추출물 이외에 구연산, 액상과당, 설탕, 포도당, 초산, 사과산, 과즙, 및 각종 식물 추출액 등을 추가로 포함시킬 수 있다.
본 발명의 또 다른 양상은 상기 악액질 및 근손실 예방 또는 치료용 약학적 조성물을 유효성분으로 포함하는 항암 보조제를 제공한다.
본 명세서에 사용된 용어, '항암 보조제'는 항암 치료 중에 항암제의 부작용을 개선하는 효과를 나타내는 보조제를 의미하며, 상기 항암제의 부작용에는 조혈 독성, 식욕 감소, 체중 감소 등이 포함될 수 있다.
상기 항암 보조제는 유효성분으로 작약 추출물을 포함하는 이외에 동일 또는 유사한 기능을 나타내는 유효성분을 추가로 1종 이상 포함할 수 있다. 상기 항암 보조제는 임상 투여 시에 경구 또는 비경구로 투여가 가능하며, 비경구 투여 시 복강 내 주사, 직장 내 주사, 피하주사, 정맥주사, 근육 내 주사, 자궁 내 경막주사, 뇌혈관 내 주사 또는 흉부 내 주사에 의해 투여될 수 있고, 일반적인 의약품 제제의 형태로 사용될 수 있다.
상기 항암 보조제는 단독으로, 또는 수술, 방사선 치료, 호르몬 치료, 화학 치료 및 생물학적 반응 조절제를 사용하는 방법들과 병용하여 사용할 수 있다. 상기 항암 보조제의 일일 투여량은 약 0.0001 내지 1000 ㎎/㎏이고, 바람직하게는 1 내지 100 ㎎/㎏이며, 하루 1회 내지 수회 나누어 투여하는 것이 바람직하나 환자의 체중, 연령, 성별, 건강상태, 식이, 투여시간, 투여 방법, 배설률 및 질환의 중증도 등에 따라 그 범위가 다양하다. 본 발명의 항암 보조제는 실제 임상 투여 시에 비경구의 여러 가지 제형으로 투여될 수 있는데, 제제화할 경우에는 보통 사용하는 충진제, 증량제, 결합제, 습윤제, 붕해제, 계면활성제 등의 희석제 또는 부형제를 사용하여 조제된다. 비경구 투여를 위한 제제에는 멸균된 수용액, 비수성용제, 현탁제, 유제, 동결건조제제, 좌제가 포함된다. 비수성용제, 현탁용제로는 프로필렌글리콜(Propylene glycol), 폴리에틸렌 글리콜, 올리브 오일과 같은 식물성 기름, 에틸올레이트와 같은 주사 가능한 에스테르 등이 사용될 수 있다. 좌제의 기제로는 위텝솔(witepsol), 마크로골, 트윈(tween) 61, 카카오지, 라우린지, 글리세로제라틴 등이 사용될 수 있다.
본 발명의 또 다른 양상은 작약(Paeoniae Radix) 추출물을 유효성분으로 포함하는 조성물을 투여하여 악액질(cachexia) 및 근손실을 예방 또는 치료하는 방법을 제공하는 것이다.
본 발명의 작약 추출물을 유효성분으로 포함하는 조성물은 식욕 감소, 체중 감소, 근육 손실 및 피로도를 개선하고, 조혈 독성을 억제하므로 악액질 및 근손실의 예방 또는 치료에 유용하게 활용될 수 있다.
이하 하나 이상의 구체예를 실시예를 통하여 보다 상세하게 설명한다. 그러나, 이들 실시예는 하나 이상의 구체예를 예시적으로 설명하기 위한 것으로 본 발명의 범위가 이들 실시예에 한정되는 것은 아니다.
실시예 1. 작약 추출물 제조
작약(Paeoniae Radix) 250g과 30% 에탄올 2L를 3L 반응조에 넣고, 환류 추출기를 이용하여 65℃에서 4시간 동안 환류추출하였다. 1차 추출물을 여과하고, 잔사물에 다시 30% 에탄올 2L을 첨가하여 동일한 조건으로 2차 추출하였다. 이후 2차 추출물을 여과하여 1차 추출물과 혼합한 후 50℃ 이하에서 농축하여 에탄올을 제거하였다. 농축된 추출물을 동결건조시킨 다음 믹서기로 갈아 균질화하였다. 추출물 제조 결과, 건조 중량 43 g(수율 17.20%)의 작약 추출물을 수득할 수 있었다.
실시예 2. 종양에 의한 악액질 및 근손실 개선 효과 확인
2-1. 실험 동물 및 실험 방법
실험동물은 4주령의 수컷 C57BL/6 마우스(18 내지 20 g)로, 중앙실험동물(주)에서 공급받아 실험동물 사육장에서 1주일 동안 적응시킨 후 실험에 사용하였다. 실험 기간 중 고형사료와 물은 자유로이 섭취시켰으며 사육실의 온도(22±2℃), 상대습도(60±5%)와 명암은 12시간 주기를 유지하였다.
마우스를 각 그룹당 9마리씩 정상군(normal), 대조군(control, 종양유발군), 기존약물 투여군 및 작약 추출물 투여군으로 나누고, 정상군에는 PBS를 100 ㎕씩 피하주사하고 종양 유발군(대조군), 기존 약물 투여군 및 작약 추출물 투여군에는 MC38(1×106 cells/100 ㎕) 대장암 세포 또는 LLC(Lewis Lung Carcinoma)(1×106 cells/100 ㎕) 폐암 세포를 피하주사하였다. 2주간 종양을 유도한 후, 종양 부피(tumor volume)가 약 1000 ㎣이 되었을 때, 기존약물 투여군에는 현재 임상에서 가장 많이 사용되고 있는 암 악액질 치료제인 Megace(Megestrol acetate) 125 ㎎/㎏ (=625 mg[1일 인간용량] / 60kg[인간평균체중] x 12[인간 대비 마우스 체표면적대비 dose equivalent factor]) 을 14일 동안 경구투여하였고, 작약 추출물 투여군에는 실시예 1에서 제조한 작약 추출물을 Megace 용량보다 조금 적은 용량인 100 ㎎/㎏ 으로 14일 동안 경구투여하였다. 실험이 진행되는 동안 마우스의 식이량을 기록하였다. 또한, 마우스의 근력을 grip strengh test 기기(Bioseb, 미국)로 측정하였고, 최대 운동시간 600초에서, treadmill 기기(Columbus Instrument, 미국)를 사용하여 마우스의 지구력을 측정하였다.
실험 종료 후, 마우스를 희생하고 채혈한 다음, 종양 크기 및 뒷다리 근육 무게를 확인하고, ELISA 방법으로 혈청 및 근육내의 TNF-α, IL-6, IL-1 및 MCP-1을 정량하였다. 또한, LLC 폐암 마우스의 뒷다리 근육내 근육분해인자(MAFbx) 의 감소 효과를 당업계에 알려진 웨스턴 블롯(western blot) 및 면역 형광법(Immunofluorescence)을 사용하여 측정하였다.
2-2. 작약 추출물의 식욕 개선 효과
종양을 유도한 후, 14일 동안 작약 추출물을 경구 투여하면서 마우스의 식이량을 기록하였다. 그 결과, 하기 표 1 및 도 1A와 도 5A에 개시된 바와 같이, 종양 유발군(대조군) 대비 작약 추출물 투여군에서 식이량이 증가한 것을 확인할 수 있었다. 하기 표 1에서 * p<0.05; ** p<0.01; 및 *** p<0.001를 나타내며, 이하에 기재된 표 및 도면에서도 동일한 의미로 사용하였다.
한편, 도 6에서 확인할 수 있는 바와 같이, 작약 추출물을 50, 100 또는 200 ㎎/㎏ 투여한 LCC 마우스의 식이량(도 6A) 및 마우스의 체중(도 6B)의 경우, 작약 추출물 100 ㎎/㎏ 투여군에서 증가 효과가 가장 우수한 것으로 나타나, 최적의 작약 추출물 투여 농도는 100 ㎎/㎏임을 확인하였다.
2-3. 작약 추출물의 종양 크기 변화 여부 확인
종양을 유도한 후, 14일 동안 작약 추출물을 경구 투여한 다음, 마우스를 희생하여 종양 크기를 확인하였다. 그 결과, 하기 표 2 및 도 1B와 도 5B에 개시된 바와 같이, 종양 유발군(대조군) 대비 작약 추출물 투여군에서 유의적인 종양 크기의 변화는 나타나지 않은 것으로 확인되었다.
한편, 도 7A에서 확인할 수 있는 바와 같이, 작약 추출물을 50, 100 또는 200 ㎎/㎏ 투여한 LCC 마우스의 종양 무게의 경우, 각 군별로 유의적인 차이가 나타나지 않은 것으로 확인되었다.
즉, 작약 추출물의 악액질 및 근손실 개선 효과는 항암효과로부터 기인하는 것이 아님을 확인하였다.
2-4. 작약 추출물의 근육 무게 변화 여부 확인
종양을 유도한 후, 14일 동안 작약 추출물을 경구 투여한 다음, 마우스를 희생하여 뒷다리 근육 무게를 확인하였다. 그 결과, 하기 표 3 및 도 1C와 도 5C에 개시된 바와 같이, 종양 유발군(대조군) 대비 작약 추출물 투여군에서 유의적인 뒷다리 근육 무게가 증가한 것으로 확인되었다.
한편, 도 7B에서 확인할 수 있는 바와 같이, 작약 추출물을 50, 100 또는 200 ㎎/㎏ 투여한 LCC 마우스의 뒷다리 근육 무게의 경우, 작약 추출물 100 ㎎/㎏ 투여군에서 증가 효과가 가장 우수한 것으로 나타나, 최적의 작약 추출물 투여 농도는 100 ㎎/㎏임을 확인하였다.
2-5. 작약 추출물의 근손실 방지 효과
종양을 유도한 후, 14일 동안 작약 추출물을 경구 투여한 다음, 마우스를 희생하여 혈청 및 근육내 염증성 사이토카인의 농도를 확인하였다. 그 결과, 하기 표 4 및 도 2와 도 8에 개시된 바와 같이, 정상군에 비해 대조군(종양유발군)에서 종양에 의해 염증성 사이토카인이 증가하고, 대조군에 비해 작약 추출물 투여군에서 유의적으로 혈청 및 근육내 염증성 사이토카인이 감소한 것을 확인하여, 작약 추출물의 염증 및 근손실 개선 효과를 확인하였다.
2-6. 작약 추출물의 근력 및 지구력 개선 효과
종양을 유도한 마우스에 기존약물 또는 작약 추출물을 경구 투여한지 12일 차에 근력(grip strength, g) 및 지구력(treadmill, sec) 테스트로 마우스의 근력 및 지구력을 확인하였다. 측정 결과, 하기 표 5와 표 6 및 및 도 3과 도 9에 나타난 바와 같이 정상군과 비교하여 대조군에서는 근력 및 지구력이 현저하게 감소한 것을 확인하였다. 반면, 대조군과 비교하여 작약 추출물 투여군에서는 근력 및 지구력이 유의하게 증가한 것을 확인하여, 종양이 유도된 마우스의 근력 및 지구력이 개선된 것으로 확인되었다.
한편, 도 10에서 확인할 수 있는 바와 같이, 작약 추출물을 50, 100 또는 200 ㎎/㎏ 투여한 LCC 마우스의 근력(도 10A) 및 지구력(도 10B)의 경우, 작약 추출물 100 ㎎/㎏ 투여군에서 증가 효과가 가장 우수한 것으로 나타나, 최적의 작약 추출물 투여 농도는 100 ㎎/㎏임을 확인하였다.
2-7. 근육분해인자의 감소 효과
종양을 유도한 후, 14일 동안 작약 추출물을 경구 투여한 다음, 마우스를 희생하여 LLC 폐암 마우스의 뒷다리 근육내 근육분해인자(MAFbx) 의 감소 효과를 당업계에 알려진 웨스턴 블롯(western blot) 및 면역 형광법(Immunofluorescence)을 사용하여 측정하였다. 측정 결과, 하기 도 4에 나타난 바와 같이 정상군과 비교하여 대조군에서는 근육분해인자가 현저하게 증가한 것을 확인하였다. 반면, 대조군과 비교하여 작약 추출물 투여군에서는 근육분해인자가 유의하게 감소한 것을 확인하였다.
실시예 3. 작약 추출물의 항암제에 의한 악액질 개선 효과 확인
3-1. 실험 동물
실험동물은 4주령의 수컷 C57BL/6 마우스(18 내지 20 g)로, 중앙실험동물(주)에서 공급받아 실험동물 사육장에서 1주일 동안 적응시킨 후 실험에 사용하였다. 실험 기간 중 고형사료와 물은 자유로이 섭취시켰으며 사육실의 온도(22±2℃), 상대습도(60±5%)와 명암은 12시간 주기를 유지하였다.
마우스를 각 그룹당 9마리씩 정상군(normal), 대조군(control) 및 작약 추출물 투여군으로 나누고, 정상군을 제외한 나머지 그룹에는 소화기 계통의 암에 많이 사용되는 항암제인 5-플루오로우라실(5-fluorouracil; 이하, 5-FU로 기재함)을 200 ㎎/㎏의 농도로 복강 내에 1회 투여하여 항암 부작용을 유도하였다. 5-FU 투여 7일 후 각 그룹에 증류수(대조군) 또는 작약 추출물(500 ㎎/㎏)을 각각 7일 동안 경구 투여하고, 실험이 진행되는 동안 마우스의 체중과 식이량을 기록하였다.
3-2. 작약 추출물의 체중 및 식욕 개선 효과
5-FU 투여 후 7일 동안 작약 추출물(500 ㎎/kg)을 경구 투여하면서 마우스의 체중과 식이량을 기록하였다. 그 결과, 하기 표 7 및 도 11A와 도 11B에 개시된 바와 같이 정상군과 비교하여 대조군에서는 체중과 식이량이 유의하게 감소하나, 작약 추출물 투여군에서는 체중과 식이량이 회복하는 것을 확인할 수 있었다.
3-3. 작약 추출물의 근육 손실 방지 효과
실험이 종료된 후 마우스의 대퇴부에서 근육을 분리하고, 근육에 존재하는 글리코겐 성분(muscleglycogen)을 당업계에 알려진 방법에 따라 측정하였다.
측정 결과, 하기 표 8 및 도 11C에 기재된 바와 같이 정상군과 비교하여 대조군에서는 글리코겐이 현저하게 감소하나, 작약 추출물 투여에 의하여 글리코겐 농도가 회복되는 것을 확인할 수 있었다.
3-4. 작약 추출물의 피로도 개선 효과
항암 부작용을 유도한 마우스에 작약 추출물을 경구 투여한지 6일 차에 강제유영시험으로 마우스의 피로도를 확인하였으며, 마우스 체중의 5% 정도를 부하한 후 유영시간(swimming time)을 측정하였다.
측정 결과, 하기 표 9 및 도 11D에 나타난 바와 같이 정상군과 비교하여 대조군에서는 유영시간이 현저하게 감소하여 피로도가 현저한 것을 확인할 수 있었다. 반면, 대조군과 비교하여 작약 추출물 투여군에서는 유영시간이 유의하게 증가한 것을 확인하여 항암 부작용이 유도된 마우스의 피로도가 개선된 것을 알 수 있었다.
3-5. 작약 추출물의 조혈기능 개선 효과
작약 추출물을 7일 동안 경구투여한 후 마우스로부터 혈액을 분리하여 혈구분석기로 분석하였다. 분석 결과, 하기 표 10 및 도 12에 나타난 바와 같이 대조군(control)과 비교하여 작약 추출물 투여군에서는 백혈구(white blood cell, WBC) 및 림프구(lymphocyte)의 수가 유의하게 증가한 것을 확인할 수 있었다. 또한, 작약 추출물 투여군에서는 대조군과 비교하여 혈소판과 적혈구 수 및 헤모글로빈 수치가 증가하는 것을 알 수 있었다.
이제까지 본 발명에 대하여 그 바람직한 실시예들을 중심으로 살펴보았다. 본 발명이 속하는 기술 분야에서 통상의 지식을 가진 자는 본 발명이 본 발명의 본질적인 특성에서 벗어나지 않는 범위에서 변형된 형태로 구현될 수 있음을 이해할 수 있을 것이다. 그러므로 개시된 실시예들은 한정적인 관점이 아니라 설명적인 관점에서 고려되어야 한다. 본 발명의 범위는 전술한 설명이 아니라 특허청구범위에 나타나 있으며, 그와 동등한 범위 내에 있는 모든 차이점은 본 발명에 포함된 것으로 해석되어야 할 것이다.
Claims (7)
- 작약(Paeoniae Radix) 추출물을 유효성분으로 포함하는 악액질(cachexia) 및 근손실 예방 또는 치료용 약학적 조성물.
- 제1항에 있어서, 상기 작약 추출물은 증류수, C1 내지 C4의 저급 알코올, 헥산, 에틸아세테이트, 클로로포름, 디에틸에테르, 디클로로메탄, 아세톤 및 이들의 혼합물로 이루어진 군에서 선택되는 용매로 추출되는 것인 악액질 및 근손실 예방 또는 치료용 약학적 조성물.
- 제1항에 있어서, 상기 악액질은 암에 의해 유발되는 것인 악액질 및 근손실 예방 또는 치료용 약학적 조성물.
- 제1항에 있어서, 상기 악액질은 항암제에 의해 유발되는 것인 악액질 및 근손실 예방 또는 치료용 약학적 조성물.
- 제1항에 있어서, 상기 조성물은 식욕 감소, 체중 감소, 피로도 증가, 염증 증가, 근육 손실 및 조혈 독성으로 이루어진 군에서 선택되는 증상을 개선하는 것인 악액질 및 근손실 예방 또는 치료용 약학적 조성물.
- 작약(Paeoniae Radix) 추출물을 유효성분으로 포함하는 악액질 및 근손실 예방 또는 개선용 식품 조성물.
- 제1항의 조성물을 유효성분으로 포함하는 항암 보조제.
