Abstract

Bei der Herstellung einer transkutanen Verbindung zwischen einem Außenraum und einem Inneren eines menschlichen oder tierischen Körpers, mit einem einen Hautdurchtritt bewirkenden zylindrischen Hautdurchtrittsstück (5) mit einer die Hautdurchtrittsrichtung bestimmenden Längsachse, wird eine wundheilende Wirkung und eine zuverlässige Desinfektion dadurch bewirkt, dass an dem Hautdurchtrittsstück (5) eine ringförmige, flächige, eine in einem Winkel zur Längsachse stehende Anlagefläche aufweisende Elektrodenanordnung (7) befestigbar ist, die die flächige Elektrode (19) und ein die Elektrode (19) zur Hautoberfläche abschirmendes flächiges Dielektrikum (20) ausweist und zur Erzeugung eines dielektrisch behinderten Plasmas (16) zwischen dem Dielektrikum (20) und der Hautoberfläche als Gegenelektrode ausgebildet ist.

Description

Elektrodenanordnung für eine Plasmabehandlung und Vorrichtung zur Herstellung einer transkutanen Verbindung
Die Erfindung betrifft eine Elektrodenanordnung für eine dielektrisch behinderte keimtötende Plasmabehandlung einer Wundfläche einer Hautoberfläche, mit einer flächigen Elektrode und einem als flächiges Material ausgebildeten Dielektrikum, wobei die flächige Elektrode am Dielektrikum so befestigt ist, dass eine Schicht des Dielektrikums die Elektrode von der Hautoberfläche abschirmt. Es ist bekannt, dass die Haut eines menschlichen oder tierischen Körpers mit einem kalten Plasma behandelt werden kann, um verschiedene Effekte zu erzeugen. Ein wesentlicher Effekt besteht in der keimtötenden Wirkung des das Plasma bildenden ionisierten Gases. Die keimtötende Wirkung kann insbesondere die Wundheilung för- dem, da die Wundheilung durch Keime, insbesondere Streptokokken, behindert werden kann. Darüber hinaus erscheint es möglich, Hautinfektionen mit einem Plasma zu behandeln (Heinlin et al. "Plasmamedizin: Anwendungsmöglichkeiten in der Dermatologie" in JDDG 12- 2010 (Band 8) Seiten 968 bis 977).
Für die Erzeugung eines Plasmas zur Behandlung der Hautoberfläche sind verschiedenste Anordnungen bekannt geworden.
Eine häufig angewandte Ausführungsform sieht die Erzeugung eines sogenannten Plasma-Jets vor, bei dem das Plasma innerhalb eines stabförmigen Gehäuses erzeugt wird. Das Gehäuse bildet eine Applikationsspitze aus, aus der das ionisierte Gas als Plasmastrahl austritt und auf die Haut geschleudert wird. Das Gerät kann nach Art eines Kugelschreibers gehalten und geführt werden, um die zu behandeln- de Hautoberfläche nach und nach dem eher punktuell auftreffenden lonenstrahl auszusetzen.
Bestätigungskopie| Durch DE 10 2007 030 915 A1 ist es ferner bekannt, Hohlfasern aus einem dielektrischen Werkstoff auszubilden, die in ihrem Inneren mit einer metallisch leitenden Beschichtung versehen werden, sodass die Hohlfaser ein Dielektrikum mit einer inneren abgeschirmten Elektrode bildet, die zusammen mit der als Gegenelektrode dienenden Oberfläche eines leitenden Körpers beispielsweise der Hautoberfläche, ein Plasmafeld ausbilden kann. Dabei ist es auch vorgesehen, mit den Hohlfasern ein Gewebe auszubilden, das flächige auf eine unregelmäßige Oberfläche, insbesondere die Hautoberfläche eines mensch- liehen Körpers, auflegbar ist. Dadurch entsteht der Vorteil einer an die unregelmäßige Topologie der Hautoberfläche anpassbaren Elektrodenanordnung für die Durchführung einer Plasmabehandlung.
Durch WO 2011/076193 A1 ist ferner eine flexible flächige Elektro- denanordnung der eingangs erwähnten Art bekannt. Das Dielektrikum ist dabei durch ein flexibles flächiges Material gebildet, das eine flächige flexible Elektrode gegenüber der zu behandelnden Oberfläche abschirmt, wenn das Dielektrikum auf der zu behandelnden Oberfläche aufliegt. Das Dielektrikum ist auf der zur zu behandelnden Oberfläche zeigenden Seite mit einer Struktur versehen, um Luftführungsbereiche auszubilden, in denen das Plasma während der Behandlung, also unmittelbar auf der Hautoberfläche, entsteht. Die flächige Elektrode kann dabei in das Dielektrikum eingebetet sein und ragt nur mit einem elektrischen Hochspannungsanschluss aus dem Dielektrikum heraus. Durch die Einbettung wird die Abschirmung gegenüber der Hautoberfläche realisiert. Die Einbettung kann daher im Wesentlichen einseitig, aber auch allseitig erfolgen.
Für die Befestigung von Exoprothesen, also Prothesen, die ein ampu- tiertes Körperteil ersetzen, ist es üblich, einen Prothesenschaft auszubilden, der über den verbliebenen Amputationsstumpf des amputierten Körperteils greift und dort, beispielsweise mit Hilfe von Unterdruck, befestigt wird. Diese Art der Befestigung von Prothesen führt zu Problemen, wenn sich die Weichteilstruktur des Amputations- Stumpfs verändert. Es ist daher vorgeschlagen worden, eine Osseoin- tegration durchzuführen, also die Exoprothese am Knochen des Amputationsstumpfs zu befestigen. Hierzu muss ein Adapter, an dem die Prothese befestigbar ist, durch die Haut in den Knochen eingebracht und dort befestigt werden. Da der Adapter auf unbegrenzte Zeit implantiert werden soll und durch die Haut nach außen ragen muss, ist es wichtig, dass durch die Trennstelle zwischen Adapter und Haut bzw. Weichgewebe keine Keime in das Innere des Körpers eintreten. Es wird daher davon ausgegangen, dass der Adapter eine die An- Siedlung von Keimen erschwerende Oberfläche haben sollte, beispielsweise aus einem polierten Titan besteht. Dabei ist es erforderlich, den eingesetzten Adapter und den Hautbereich um den Adapter herum regelmäßig zu desinfizieren, um so das Entstehen von Infektionen durch Keime zu verhindern.
Zur Stabilisierung des Adapters in dem Weichgewebe ist ferner bekannt, den Adapter so in den Amputationsstumpf einzusetzen, dass ein flächiger Weichgewebeanker den Adapter umgibt und eine große poröse durchlöcherte Oberfläche bildet, die von Weichgewebe durch- drungen werden kann, sodass der Adapter mit seinem Weichgewebeanker in das Weichgewebe einwächst.
Es ist auch bereits untersucht worden, ob das Einwachsen des Adapters, ggf. mit einem Weichgewebeanker, durch eine bioaktive Be- Schichtung, beispielsweise mit Hydroxylapatit, beschleunigt und verbessert werden kann. Eine derartige Beschichtung kann auf längerer Sicht hilfreich sein, verhindert jedoch nicht die Probleme durch mögliche Infektionen. Ähnliche Probleme bestehen auch bei anderen transkutanen Vorrichtungen, wie beispielsweise Dauerkathetern, Dauerinfusionskanülen usw. Der vorliegenden Erfindung liegt daher die Aufgabe zugrunde, die Verwendung von transkutanen Vorrichtungen zu erleichtern, um eine Möglichkeit zur einfachen und dauerhaften Desinfektion des die am Körper befestigte transkutane Vorrichtung umgebenden Haut- und Gewebebereichs aufzuzeigen.
Diese Aufgabe wird erfindungsgemäß mit einer Elektrodenanordnung der eingangs erwähnten Art dadurch gelöst, dass die Elektrodenanordnung eine ringförmige, durch das Dielektrikum gebildete Anlage- fläche aufweist und zur Befestigung an einem einen Hautdurchtritt mit einer Längsachse bewirkenden Hautdurchtrittsstück derart vorgesehen ist, dass die Elektrodenanordnung das Hautdurchtrittsstück umgibt und die Anlagefläche in einem Winkel > 20° zur Längsachse steht.
Das Dielektrikum kann dabei auf seiner zur Hautoberfläche zeigenden Seite mit einer Struktur versehen sein, um Luftführungsbereiche auszubilden, wenn das Dielektrikum über die Struktur auf der Hautoberfläche aufliegt. Die Struktur wird dabei vorzugsweise durch vor- stehende Noppen gebildet, deren Oberflächen die Anlagefläche bilden.
Das Dielektrikum - und damit die flächige Elektrode - kann aus einem starren dielektrischen Material, beispielsweise Keramik, bestehen. Zum Anliegen an die Hautoberfläche kann es jedoch vorteilhaft sein, die flächige Elektrode flexibel auszubilden, indem sowohl die Elektrode selbst als auch das Dielektrikum aus einem flexiblen Material bestehen. Vorzugsweise ist das Dielektrikum ein flexibles Polymer oder Elastomer.
Zur Lösung der gestellten Aufgabe ist ferner erfindungsgemäß eine Vorrichtung der eingangs erwähnten Art dadurch gekennzeichnet, dass an dem Hautdurchtrittsstück eine ringförmige, flächige und eine in einem Winkel zur Längsachse stehende Anlagefläche aufweisende Elektrodenanordnung befestigbar ist, die die flächige Elektrode und ein die flächige Elektrode zur Hautoberfläche abschirmendes flächiges Dielektrikum ausweist und zur Erzeugung eines dielektrisch behinderten Plasmas zwischen dem Dielektrikum und der Hautoberfläche als Gegenelektrode ausgebildet ist.
Mit der vorliegenden Erfindung lässt sich somit eine Plasmaelektrode mit dem transkutanen Adapter kombinieren, um so ein geeignetes Plasmafeld generieren zu können, mit dem der Hautdurchtrittsbereich und seine Umgebung auf der Hautoberfläche zuverlässig desinfiziert werden können. Dabei lässt sich auch die Entstehung eines schädlichen Biofilms auf dem Adapter selbst im Hautdurchtrittsbereich verhindern. Somit kann auf die bisherige, ggf. schädliche Verwendung von Chemikalien, die Zugabe von Silberionen usw. verzichtet werden. Nach dem Einsetzen des Adapters besorgt die Plasmaelektrodenvor- richtung auch eine verbesserte Wundheilung für die Operationswunde. Das nach dem Einsetzen des Adapters üblicherweise austretende Exsudat wird durch die Plasmabehandlung sowohl vermindert als auch keimfrei gemacht, sodass eine verbesserte Wundheilung durch eine regelmäßige Plasmabehandlung ermöglicht wird. Die Elektro- denanordnung kann dabei permanent mit dem Adapter verbunden sein oder in einer bevorzugten Ausführungsform leicht lösbar mit dem Adapter verbindbar sein, um so für die Plasmabehandlung auf den Adapter aufgesteckt werden zu können. Die Elektrodenanordnung umgibt den Adapter ringförmig. Die Ringform muss sich dabei nicht über 360° erstrecken, sondern kann sich zur Erleichterung des nachträglichen Anbringens über einen Teilkreis, beispielsweise über einen Dreiviertelkreis oder einen Halbkreis, erstrecken. Die Ringform erstreckt sich vorzugsweise über einen Um- fangswinkel zwischen π und 2ττ.
Die erfindungsgemäße Vorrichtung, die einen Adapter zur Befestigung einer Exoprothese aufweist, kann alle Merkmale aufweisen, die sich für derartige Vorrichtungen bisher als zweckmäßig erwiesen ha- ben und oben erläutert worden sind, beispielsweise eine polierte Oberfläche, die die Ansiedlung von Keimen erschwert, eine mit einer bioaktiven Beschichtung gebildete Oberfläche, einen Weichgewebeanker, der ebenfalls mit einer bioaktiven Beschichtung versehen sein kann, usw.
Die erfindungsgemäße Vorrichtung kann auch eine der erwähnten anderen transkutanen Vorrichtung sein und mit der Plasmaelektrode den Eintritt von externen Keimen in das Körperinnere verhindern oder eine Wundheilung zu beschleunigen. Demgemäß kann die erfin- dungsgemäße Elektrodenanordnung auch in Verbindung mit transkutanen Kanülen und externen Fixateuren verwendet werden. Die Vorrichtung zur Herstellung einer transkutanen Verbindung kann somit auch eine Kanüle oder einen externen Fixateur umfassen. Eine elektrische Steuerung der erfindungsgemäßen Elektrodenanordnung bzw. der erfindungsgemäßen Vorrichtung kann unmittelbar an der Elektrodenanordnung angebracht sein. Es ist aber auch möglich, die Steuerung beispielsweise an der Exoprothese selbst vorzusehen und beispielsweise über eine Steckverbindung mit der Elektro- denanordnung zu verbinden, wenn die Elektrodenanordnung benötigt wird, beispielsweise für den Einsatz auf den Adapter aufgesteckt wird. Die mechanische Verbindung der Elektrodenanordnung mit dem Adapter kann dabei mittels einer geeigneten Befestigungsvorrichtung erfolgen. Besonders vorteilhaft ist es, wenn die Elektrodenvorrichtung flexibel ausgebildet ist und der Adapter mit einer Andruckvorrichtung zum Andrücken der flexiblen Elektrodenvorrichtung in Richtung des proximalen Endes des Adapters versehen ist, sodass die Elektrodenvorrichtung mit ihrer Andruckfläche gegen die Hautoberfläche gedrückt wird. Die Steuerung der Elektrode kann auch so ausgebildet sein, dass mit ihr eine Gaszufuhr auf die Hautoberfläche erfolgt, sodass das Plasma nicht in erster Linie mit der Umgebungsluft, sondern mit dem zugeführten Gas ausgebildet wird. Auf diese Weise kann mit der Plasmabehandlung eine unterstützende therapeutische Behandlung mittels des zugeführten Gases vorgenommen werden. Ferner ist es möglich, das Plasma als Niederdruck-Plasma auszubilden, wenn mittels einer Glockenanordnung mit der Hautoberfläche ein Unterdruckraum ausgebildet wird. Das dielektrisch behinderte Plasma ist dann in dem Unterdruckraum in einem Unterdruck erzeug- bar. Die Glockenanordnung kann hierfür aus einem geformten Kunststoff bestehen, der so stabil ist, dass er die Glockenform auch bei angelegtem Unterdruck beibehält. Der freie Rand der Glockenanordnung kann mit einem Dichtring zur Hautoberfläche abgedichtet sein. In einer besonders bevorzugten Anordnung wird die Glockenanordnung durch das Dielektrikum gebildet, an dem die Elektrode befestigt ist. Vorzugsweise ist die Elektrode in dem Dielektrikum eingebettet. Das die Glockenanordnung bildende Dielektrikum hat dabei sowohl die Funktion, die Plasmaentladung dielektrisch zu behindern als auch den Unterdruckraum mit der Hautoberfläche zu bilden.
Die Erfindung soll im Folgenden anhand von in der Zeichnung dargestellten Ausführungsbeispielen näher erläutert werden. Es zeigen: eine schematische Darstellung eines Oberschenkelam putationsstumpfes, an dessen Knochen eine Exoprothe se befestigt ist; eine schematische Darstellung des Aufbaus eines am Knochen befestigten Adapters, an dem die Exoprothese befestigbar ist;

Figur 1 lässt einen Amputationsstumpf 1 erkennen, der durch eine Beinamputation oberhalb des Knies entstanden ist. Der Amputationsstumpf besteht von innen nach außen aus einem Knochen 2, nämlich dem restlichen Oberschenkelknochen, Weichgewebe 3 und Haut 4.
Durch die Haut 4 ist am distalen Ende des Amputationsstumpfs 1 ein ein Hautdurchtrittsstück bildender Adapter 5 hindurchgeführt und als Implantat im Inneren des Knochens 2 befestigt. Der Adapter 5 trägt im Inneren des Amputationsstumpfs 1 einen Weichgewebeanker 6 und außerhalb des Amputationsstumpfs 1 , unmittelbar benachbart zu der den Adapter 5 umgebenden Haut 4, eine Elektrodenanordnung 7 zur Erzeugung eines kalten, dielektrisch behinderten Plasma zwischen der Haut 4 und der Elektrodenanordnung 7. An dem Adapter 5 ist eine Prothese 8 befestigt, die im dargestellten Ausführungsbeispiel ein Kniegelenk 9, ein Unterteilschenkelteil 10 und eine gelenkig am Unterschenkelteil 10 befestigte Fußprothese 11 aufweist. Der prinzipielle Aufbau derartiger Prothesen ist bekannt und für die vorliegende Erfindung nicht von Bedeutung, sodass auf eine nähere Beschreibung der Einzelheiten verzichtet wird.
Figur 1 lässt erkennen, dass eine Steuerung 12 mit einer Energieversorgung für die Elektrodenanordnung 7 am Unterschenkelteil 10 der Prothese 8 befestigt ist. Dieser Anordnung der Steuerung stellt jedoch nur eine Ausführungsmöglichkeit dar, da es zahlreiche weitere Möglichkeiten gibt, die Steuerung an der Prothese 8 oder am Adapter 5 zu befestigen oder in die Elektrodenanordnung 7 zu integrieren. Figur 2 zeigt eine Variante der Anordnung aus Figur 1. In der Darstellung der Figur 1 ist das den Knochen 2 umgebende Weichgewebe 3 dargestellt als Muskelgewebe 13 und Fettgewebe 14 oder Subkutis, das sich unmittelbar unterhalb der Haut 4 befindet. Der Adapter 5 wird durch ein zylindrisches Rohr mit einem kreisförmigen Querschnitt gebildet und trägt unterhalb der Haut 4 den Weichgewebeanker 6, der als flächiger Teller mit einer leichten, an die Form des Amputationsstumpfs 1 am distalen Ende angepassten Wölbung ausgebildet ist. Schematisch dargestellt sind Durchgangs- öcher 15 in dem flächigen Weichgewebeanker, durch den das Weichgewebe hindurch wachsen kann, um so das Einwachsen des Weichgewebeankers 15 in dem Weichgewebe 3 zu unterstützen.
Außerhalb des Amputationsstumpfs 1 befindet sich auf dem Adapter 5 die Elektrodenanordnung 7, die flächig und flexibel ausgebildet ist. Zwischen der Elektrodenanordnung 7 und der Haut 4 als Gegenelektrode entsteht ein Plasma 16, das in Figur 2 schematisch angedeutet ist. Von dem Adapter wird in diesem Ausführungsbeispiel die Steuerung 12 getragen, die über ein Kabel 13 mit der Elektrodenanordnung 7 verbunden ist. Der Adapter 5 ist im Bereich der Haut 4 und im Inneren des Amputationsstumpfs 1 mit einer bioaktiven Beschichtung versehen, die sich zumindest über dem größten Teil der Länge des Adapters 5 innerhalb des Amputationsstumpfs 1 erstreckt und das Einwachsen des Adapters 5 in das Knochengewebe bzw. das Weichgewebe 3 erleichtert. Figur 3 lässt den Aufbau der Elektrodenanordnung 7 gemäß einem Ausführungsbeispiel der Erfindung erkennen.
Der Adapter erstreckt sich im Inneren des Amputationsstumpfs 1 durch die Haut 4, das Fettgewebe 14, das Muskelgewebe 13 in den Knochen 2 hinein, wo er in einer für Implantate üblichen Weise befestigt wird. Der Weichgewebeanker 6 befindet sich auf der Innenseite der Haut 4, während sich auf der Außenseite der Haut 4 sofort die Elektrodenanordnung 7 anschließt, um zwischen der Elektrodenanordnung 7 und der Haut 4 das Plasma auszubilden.
Die Elektrodenanordnung 7 besteht aus einer flexiblen Elektrode 19, die beispielsweise durch einen Drahtgitter gebildet sein kann. Die Elektrode 19 ist in dem dargestellten Ausführungsbeispiel allseitig in ein flexibles Dielektrikum 20 eingegossen, sodass die Elektrode 19 zur Haut 4 hin durch das Dielektrikum 20 abgeschirmt ist. Beim Anlegen eines Hochspannungs-Wechselpotentials wird so ein steter Stromfluss zwischen der Elektrode 19 und der Haut 4 verhindert und es stellt sich eine dielektrisch behinderte Plasmaentladung ein, mit der das Plasma 16 ausgebildet wird.
Vorzugsweise ist das Dielektrikum 20 auf seiner zur Haut 4 zeigenden Oberfläche mit einer Struktur versehen, durch die sich zwischen der Haut 4 und dem Dielektrikum 20 Luft ansammeln und bewegen kann, in der sich beim Anlegen einer Hochspannung an der Elektrode 19 gegenüber der Haut 4 als Gegenelektrode das Plasma ausbilden kann. Die Struktur kann vorzugsweise aus einzelnen vorstehenden Noppen des Dielektrikums 20 bestehen, die im Wesentlichen gleich hoch ausgebildet sind, deren Stirnseite eine Anlagefläche für die Haut 4 am distalen Ende des Amputationsstumpfs 1 bilden.
Figur 3 lässt schematisch erkennen, dass der Adapter 5 an seinem distalen Ende mit einem Kopplungselement 21 versehen ist, mit dem der Adapter 5 mit der Prothese 8 - ggf. einstellbar - koppelbar ist. Derartige Kopplung einer Prothese mit einem rohrförmigen Element ist dem Fachmann geläufig und wird hier nicht näher erläutert.
Die Elektrodenanordnung 7 wird in diesem Ausführungsbeispiel durch ein in Richtung Haut 4 vorgespanntes flächiges Andruckelement 22 vervollständigt, mit dem das flexible Dielektrikum 20 und die flexible Elektrode 19 flächig in Richtung auf den Amputationsstumpf 1 vorgespannt werden, sodass die flexible Elektrodenanordnung 7 fest gegen das distale Ende des Amputationsstumpfs 1 gedrückt wird und sich dabei etwaigen Unregelmäßigkeiten des Amputationsstumpfs 1 anpassen kann, um eine definierte Ausbildung eines Plasmas 16 zu er- reichen.
Die Draufsicht auf die Elektrodenanordnung 7 in Figur 4 verdeutlicht, dass die Elektrodenanordnung 7 den Adapter 5 als flächige Elektrodenanordnung ringförmig umgibt. Das Dielektrikum 20 ist dabei als mit einem radialen Schlitz 23 versehene Ringanordnung ausgebildet. Der Schlitz 23 ermöglicht im Zusammenspiel mit der Flexibilität der Elektrodenanordnung 7, dass diese nachträglich durch Verformung um den Adapter 5 herum angeordnet wird. Die beispielsweise durch ein Drahtgeflecht gebildete flexible flächige Elektrode 19 wird allseitig von dem flexiblen Dielektrikum 20 umgeben und lediglich an einer geeigneten Stelle des Dielektrikums 20 isoliert aus diesem herausgeführt, um die Elektrode 19 mit einer für die Bildung des Plasmas 16 benötigten Hochspannung, vorzugsweise als Wechsel-Hochspannung, zu versorgen. Demgemäß ist auch die Elektrode 19 ring- förmig ausgebildet, weist jedoch eine geringere Breite als das Dielektrikum 20 auf. Darüber hinaus endet die Elektrode 19 mit Abstand vor dem Ende des Dielektrikums 20 am Schlitz 23, um die allseitige Einbettung der Elektrode 19 in das Dielektrium 20 zu gewähr- leisten.
Figur 4 zeigt ein mögliches Ausführungsbeispiel. Selbstverständlich kann der Schlitz 23 auch entfallen, sodass die Elektrodenanordnung 7 eine geschlossene ringförmige Anordnung darstellt und als solche auf dem Adapter 5 befestigt wird, wodurch sie vorzugsweise fest installiert auf dem Adapter 5 verbleibt.
Für eine lösbare Verbindung, die ein nachträglich oder zeitweises Befestigen der Elektrodenanordnung 7 auf dem Adapter 5 ermöglicht, kann der Schlitz 23 auch wesentlich größer ausgebildet sein, sodass die Elektrodenanordnung beispielsweise einen 3/4-, 2/3-Kreis o. ä. darstellt. Ferner ist es möglich, die Elektrodenanordnung mit zwei diametral gegenüberliegenden Schlitzen 23 ausbilden, um sie so in Form von zwei Halbringen nachträglich montierbar zu gestalten. In anderen Fällen kann es ausreichend sein, wenn die Elektrodenanordnung 7 lediglich auf einem Teilring den Adapter 5 umgibt. Gegebenenfalls kann die Elektrodenanordnung dabei drehbar an dem Adapter 5 befestigt werden, um so ein Plasma 16 nacheinander füreinander angrenzende Abschnitte der den Adapter 5 umgebenden Haut 4 zu erzeugen.
Als Dielektrikum eignen sich alle nichtleitenden flexiblen Kunststoffe, vorzugsweise solche, in denen die Elektrode 19 durch Eingießen, Einspritzen o. ä. eingebetet werden kann. Die Einbettung muss nicht zwangsläufig allseitig erfolgen, jedoch sicherstellen, dass eine Abschirmung der Elektrode 19 zur Haut hin erfolgt, um so einen direkten Stromfluss von der Elektrode 19 zur Haut 4 zu unterbinden. In den dargestellten Ausführungsbeispielen erstreckt sich die Elektrodenanordnung 7 im Wesentlichen rechtwinklig zur Längsachse des Adapters 5. Diese Anordnung ist bevorzugt, jedoch nicht zwingend. Insbesondere wenn die Elektrodenanordnung an einer Vorrichtung 5 befestigt ist, die nicht als Adapter für eine Prothese 8 fungiert, kann es sinnvoll sein, dass die Elektrodenanordnung 7 auch einen spitzen Winkel mit der Längsachse der Vorrichtung bildet, wobei der Winkel jedenfalls größer als 20° sein sollte. Der Winkel wird dabei zwischen der Längsachse und einer Ebene der Elektrodenanordnung 7 defi- niert. Bei einer gewölbten Elektrodenanordnung definiert sich dieser Winkel durch eine tangentiale Ebene der Elektrodenanordnung 7 im Schnittpunkt mit der Längsachse der Vorrichtung 5.
Figur 5 zeigt eine Variante des Aufbaus gemäß Figur 3, bei der eben- falls der Adapter 5 durch die Haut 4 und das Weichgewebe 3 in den Knochen 2 hinein erstreckt, wo er durch Einschrauben befestigt ist. Auf der Innenseite der Haut 4 befindet sich der Weichgewebeanker 6. Die Haut ist an der Außenseite durch eine Oberhaut abgeschlossen, die die Hautoberfläche 4a bildet.
Auf die Hautoberfläche 4a ist eine Glockenanordnung 24 aufgesetzt und mittels eines Dichtrings 25, beispielsweise aus Silikon, gegenüber der Hautoberfläche abgedichtet. Die Glockenanordnung 24 ist domartig gewölbt und umschließt im höchsten Punkt ihrer Wölbung den Adapter 5 auf der Außenseite der Haut 4. Ein zentraler Dichtring 26 stellt die Abdichtung zwischen der Glockenanordnung 24 und dem Adapter 5 her. Die Glockenanordnung 24 begrenzt zusammen mit der Hautoberfläche 4a einen Unterdruckraum 27, in dem sich das Plasma 16 ausbilden kann.
In dem dargestellten Ausführungsbeispiel ist die Glockenanordnung durch die Elektrodenanordnung 7 gebildet, indem das Dielektrikum 20 in die Glockenform gebracht ist und in das Dielektrikum 20 die Elektrode 19 eingebettet ist. Die Elektrode 19 besteht dabei vorzugsweise aus superelastischem Material, beispielsweise aus einer Nickel-Titan- Legierung, wie Nitinol.
In der Zeichnung ist schematisch dargestellt, dass durch den zentra- len Dichtring 26 ein Ein- und Auslassventil 28 hindurchführt, an das beispielsweise eine (manuelle oder elektrische) Vakuumpumpe anschließbar ist.
Zwischen dem Kopplungselement 21 und dem Adapter ist in dem dargestellten Ausführungsbeispiel ein Piezoelement 29 positioniert, das ggf. aus mechanischen Belastungen eine Versorgungsspannung für die Elektrode 19 generiert und daher mit einer am zentralen Dichtring 26 befestigten Spannungssteuerung 30 über eine Leitung 31 verbunden ist. Es ist möglich, das Plasma direkt durch die von dem Piezoelement 29 bei Belastung gelieferten Spannungsimpulse impulsartig zu generieren.
Die Ausbildung des Plasmas 16 als Niederdruck-Plasma in dem Unterdruckraum 27 hat den Vorteil, dass ein intensiver Kontakt zwischen der Oberfläche 4a der Haut und dem Plasma hergestellt wird, sodass die Desinfektion und Sterilisation durch das Plasma sicher an allen Stellen der Hautoberfläche und am Hautdurchtritt des Adapters 5 bewirkt wird.

Claims

Ansprüche
1. Elektrodenanordnung für eine dielektrisch behinderte keimtötende Plasmabehandlung einer Wundfläche einer Hautoberfläche, mit einer flächigen Elektrode (19) und einem als flächiges Mate- rial ausgebildeten Dielektrikum (20), wobei die flächige Elektrode (19) am Dielektrikum (20) so befestigt ist, dass eine Schicht des Dielektrikums (20) die Elektrode (19) von der Hautoberfläche abschirmt, dadurch gekennzeichnet, dass die Elektrodenanordnung (7) eine ringförmige, durch das Dielektrikum (20) ge- bildete Anlagefläche aufweist und zur Befestigung an einer einem Hautdurchtritt mit einer Längsachse bewirkenden Haut- durchtrittsstück (5) derart vorgesehen ist, dass die Elektrodenanordnung (7) das Hautdurchtrittsstück (5) umgibt und die Anlagefläche in einem Winkel > 20° zur Längsachse steht.
2. Elektrodenanordnung nach Anspruch 1 , dadurch gekennzeichnet, dass das Dielektrikum (20) auf seiner zur Hautoberfläche zeigenden Seite mit einer Struktur versehen ist, um Luftführungsbereiche auszubilden, wenn das Dielektrikum (20) über die Struktur auf der Hautoberfläche aufliegt.
3. Elektrodenanordnung nach Anspruch 2, dadurch gekennzeichnet, dass die Struktur durch vorstehende Noppen gebildet ist, deren Oberflächen die Anlagefläche zur Haut (4) bilden.
4. Elektrodenanordnung nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass die flächige Elektrode flexibel ausgebildet ist.
5. Vorrichtung zur Herstellung einer transkutanen Verbindung zwischen einem Außenraum und einem Inneren eines menschlichen oder tierischen Körpers, mit einem einen Hautdurchtritt bewirkenden zylindrischen Hautdurchtrittsstück (5) mit einer die Hautdurchtrittsrichtung bestimmenden Längsachse, dadurch gekennzeichnet, dass an dem Hautdurchtrittsstück (5) eine ringförmige, flächige, eine in einem Winkel zur Längsachse stehende Anlagefläche aufweisende Elektrodenanordnung (7) befestigbar ist, die eine flächige Elektrode (19) und ein die Elektrode (19) zur Hautoberfläche abschirmendes flächiges
Dielektrikum (20) ausweist und zur Erzeugung eines dielektrisch behinderten Plasmas (16) zwischen dem Dielektrikum (20) und der Hautoberfläche als Gegenelektrode ausgebildet ist.
6. Vorrichtung nach Anspruch 5, dadurch gekennzeichnet, dass das Hautdurchtrittsstück ein Adapter (5) zur Befestigung einer Exoprothese an einem Knochen (2) des Körpers und mit einem proximalen Ende mit dem Knochen (2) und mit einem distalen mit der Exoprothese (8) verbindbar ist.
7. Vorrichtung nach Anspruch 6, dadurch gekennzeichnet, dass der Adapter (5) einen Weichgewebeanker (6) aufweist.
8. Vorrichtung nach Anspruch 6 oder 7, dadurch gekennzeichnet, dass die Elektrodenanordnung (7) flexibel ausgebildet ist und der Adapter (5) mit einem Andruckelement (22) zum Andrücken der flexiblen Elektrodenvorrichtung (7) in Richtung des proximalen Endes des Adapters (5) versehen ist.
9. Vorrichtung nach einem der Ansprüche 6 bis 8, dadurch gekennzeichnet, dass der Adapter (5) aus poliertem Titan besteht.
10. Vorrichtung nach einem der Ansprüche 5 bis 9, dadurch gekennzeichnet, dass das Hautdurchtrittsstück (5) mit einer bioaktiven Beschichtung (18'), insbesondere Hydroxylapatit, versehen ist.
11. Vorrichtung nach einem der Ansprüche 5 bis 10, dadurch gekennzeichnet, dass die Elektrodenanordnung (7) lösbar an dem Hautdurchtrittsstück (5) befestigbar ist.
12. Vorrichtung nach einem der Ansprüche 5 bis 11 , dadurch gekennzeichnet, dass die Elektrode (19) in dem Dielektrikum (20) eingebettet ist.
Vorrichtung nach einem der Ansprüche 5 bis 12, dadurch gekennzeichnet, dass mittels einer Glockenanordnung (24) mit der Hautoberfläche (4a) ein Unterdruckraum (27) ausgebildet ist, in dem das dielektrisch behinderte Plasma (16) in einem Unterdruck erzeugbar ist.
Vorrichtung nach Anspruch 13, dadurch gekennzeichnet, dass die Glockenanordnung (24) aus einem geformten Kunststoff besteht und am freien Rand mit einem Dichtring (25) zur Hautoberfläche (4a) abgedichtet ist.
Vorrichtung nach Anspruch 13 oder 14, dadurch gekennzeichnet, dass die Glockenanordnung (24) durch das Dielektrikum (20) gebildet ist, das die Elektrode (19) trägt.
