Abstract
Banda protésica (1), en particular para la reparación de una válvula mitral, que comprende una banda alargada (2) de un material flexible para la fijación de cuerdas artificiales, que incluye una pluralidad de almohadillas (4) separables que están unidas a lo largo de la trayectoria de dicha banda alargada (2) y que sobresalen con respecto a ésta, cuyas almohadillas (4) están separadas entre sí y son susceptibles de recibir tales cuerdas artificiales. De este modo, se permite establecer la longitud adecuada de las cuerdas artificiales, las cuales pueden ser anudadas sin deslizamiento de los nudos con la finalidad de aplicarlas durante una operación de reparación del prolapso de la válvula mitral.

una válvula mitral
MEMORIA DESCRIPTIVA OBJETO DE LA INVENCIÓN
La presente solicitud de patente de invención tiene por objeto el registro de una banda protésica para la reparación de una válvula mitral que incorpora notables innovaciones y ventajas.
Más concretamente, la invención propone el desarrollo de una banda protésica destinada a facilitar la reparación de la insuficiencia mitral degenerativa la cual está provista de puntos de referencia para la disposición de cuerdas artificiales durante la operación quirúrgica.
ANTECEDENTES DE LA INVENCIÓN
En la actualidad la principal causa de afectación de la válvula mitral es la patología degenerativa que determina insuficiencia mitral.
La válvula mitral es una de las cuatro válvulas que tiene el corazón que presenta un estructura compleja con varios elementos que deben funcionar de forma coordinada para lograr que la sangre fluya en la dirección correcta. Si el cierre de la válvula no es adecuado tiene lugar la denominada insuficiencia mitral.
Básicamente, la válvula consta de dos velos (anterior y posterior) que se insertan en un anillo y que están anclados a los músculos papilares de la pared del ventrículo izquierdo por una gran cantidad de cuerdas tendinosas (véase las figuras 1 y 2), tal como puede verse en las figuras 1 y 2. El anillo mitral normal 10
(ver figura 4) tiene una zona posterior y una zona anterior situada entre los dos trígonos que son estructuras fibrosas que forman parte de la base del corazón.
En condiciones normales, durante la fase de contracción del corazón esos dos velos se juntan (zona de coaptación) y cierran la válvula evitando que aparezca insuficiencia. El cierre de la válvula se produce prácticamente a nivel del anillo y nunca por encima, puesto que las cuerdas tendinosas no dejan que los velos suban más allá de ese nivel.
En la insuficiencia mitral de causa degenerativa, el cierre de la válvula no es correcto porque alguno de los velos, o los dos, se mueven más allá del plano del anillo y penetran parcialmente en la aurícula izquierda, dando lugar a lo que se denomina prolapso de los velos. La causa principal del prolapso es la elongación o ruptura de una o varias de las cuerdas tendinosas.
El prolapso más frecuente es el del velo posterior, pero también son frecuentes los prolapsos del velo anterior o de ambos velos a la vez. Asociadamente, en la insuficiencia mitral degenerativa siempre existe una dilatación de grado variable del anillo mitral.
Cuando la insuficiencia mitral es severa, el paciente debe someterse a una intervención quirúrgica. Las posibilidades son el reemplazo de la válvula por una prótesis valvular artificial o bien la reparación déla propia válvula del paciente.
En la actualidad hay una clara evidencia de que la reparación es superior al reemplazo de la válvula en términos de
supervivencia, disminución de complicaciones y mejoría del estado funcional del paciente.
La reparación correcta de la válvula mitral tiene tres obj etivos :
- Restaurar la movilidad de los velos sin restricción ni exceso de movimiento;
Crear una adecuada superficie de coaptación o cierre;
Remodelar la forma y tamaño del anillo mitral dilatado. La reparación del prolapso aislado del velo posterior es sencilla y reproducible y se ha tratado clásicamente con resección parcial del velo. Sin embargo, cuando el prolapso es del velo anterior o de ambos velos, la dificultad aumenta y muchos cirujanos no pueden reparar la válvula.
Una técnica conocida y extendida consiste en el uso de cuerdas artificiales (denominadas de aquí en adelante neocuerdas), habitualmente fabricadas en PTFE, que sustituyen a las cuerdas tendinosas rotas o estiradas. Esas neocuerdas se cosen a los músculos papilares y van hasta el borde libre de los velos y mimetizan la función de las cuerdas naturales.
Las neocuerdas artificiales de PTFE se han impuesto como la técnica de elección para corregir el prolapso mitral y por ello su uso se ha extendido. Tales neocuerdas permiten reparar prolapsos de uno o ambos velos o de múltiples segmentos prolapsantes de un mismo velo, aplicando tantas neocuerdas como sea necesario.
Sin embargo, el uso de esas cuerdas ratifícales se asocia a dos problemas técnicos importantes. Uno de ellos consiste en la
dificultad en la medición de forma exacta de la longitud que debe tener las neocuerdas artificiales mientras que un segundo problema conocido es aquel relacionado la dificultad en anudar las cuerdas sobre el velo sin que los nudos resbalen hacia abajo (con lo cual la cuerda queda más corta de lo necesario y provoca sobre- corrección) por la naturaleza deslizante del material PTFE.
Para intentar resolver los problemas anteriormente mencionados se han realizado diferentes intentos.
DESCRIPCIÓN DE LA INVENCIÓN
La presente invención se ha desarrollado con el fin de proporcionar una invención que resuelva los inconvenientes anteriormente mencionados, aportando, además, otras ventajas adicionales que serán evidentes a partir de la descripción que se acompaña a continuación.
Es por lo tanto un objeto de la presente invención proporcionar una banda protésica que comprende una banda alargada de un material flexible para la fijación de cuerdas artificiales, y se caracteriza por el hecho de que se incluye una pluralidad de almohadillas (preferentemente hechas de PTFE) separables que están unidas a lo largo de la trayectoria de dicha banda alargada y que sobresalen con respecto a ésta, cuyas almohadillas están separadas entre sí y son susceptibles de recibir tales cuerdas artificiales.
Cabe mencionar que estas almohadillas actúan como punto de referencia para establecer la correcta longitud de las neocuerdas que se fijan a la banda alargada. Esto es gracias a la distancia
entre los músculos papilares y el anillo mistral permanece constante durante todo el ciclo cardiaco.
Gracias a estas características, se permite establecer la longitud adecuada de las neocuerdas y pueden ser anudadas sin deslizamiento de los nudos con la finalidad de aplicarlas durante una operación de reparación del prolapso de la válvula mitral . Además, se trata de una solución en la que no es necesario ninguna medición, ya sea del tamaño del anillo o banda o de la longitud de las neocuerdas implantadas, siendo dicha banda protésica de fácil utilización y manipulación por parte del cirujano.
Otra ventaja de la invención es que además no existe la posibilidad de deslizamiento de los nudos ya que se anuda sobre las almohadillas que están fijas a la banda alargada y no pueden desplazarse hacia abajo al anudar.
Según una realización particularmente preferida de la invención, cada una de las almohadilla está formada por una porción principal sensiblemente laminar y un tramo de unión de menor espesor fijable a la banda flexible, presentando además la porción principal de la almohadilla dos orificios pasantes que son atravesados por neocuerdas.
Otro aspecto no menos importante de la invención es que resulta útil para prolapsos de un solo velo (anterior o posterior) o de ambos velos a la vez y reduce el tiempo necesario de una operación con lo que se reduce el riesgo para el paciente.
De acuerdo con una realización de la invención, las almohadillas están unidas a la banda alargada por un material adhesivo .
En una alternativa de la invención, las almohadillas pueden estar unidas a la banda alargada por medio de cosido.
Otras características y ventajas de la banda protésica objeto de la presente invención resultarán evidentes a partir de la descripción de una realización preferida, pero no exclusiva, que se ilustra a modo de ejemplo no limitativo en los dibujos que se acompañan, en los cuales:

Figuras 5A-5C- Son tres vistas esquematizadas donde se muestran las etapas de aplicación de la banda protésica de la invención .
DESCRIPCION DE UNA REALIZACION PREFERENTE
Tal como se muestra en las figuras adjuntas, se aprecia una banda protésica de acuerdo con la invención destinada a la reparación de una válvula mitral (representada en las figuras 1 y 2) en técnicas de cirugía que emplean las neocuerdas que se han explicado anteriormente.
En la figura 2 se aprecia las diversas partes que forman parte del aparato mitral donde puede verse la pared auricular izquierda, el anillo mitral 10, múltiples cuerdas tendinosas 20, los velos mitrales 30 y músculos papilares 40.
Más en particular, dicha banda protésica 1 comprende una banda alargada 2 (con una longitud aproximada de 63 milímetros que resulta idónea para pacientes adultos) de un material flexible para la fijación de neocuerdas 3 (cuerdas artificiales), preferentemente conformada por un núcleo central de material de silicona una envoltura exterior de material de poliéster, con una sección transversal circular con un diámetro aproximado de 3 milímetros. Adicionalmente, se proporciona una pluralidad de almohadillas 4 separables unidas a la banda y que sobresalen con respecto a ésta, cuyas almohadillas 4 están separadas entre sí y son susceptibles de fijar tales neocuerdas 3. Todas las almohadillas 4 sobresalen en un mismo sentido cuando la banda alargada 2 está extendida de forma recta.
Como puede verse más claramente en la figura 3, cada una de las almohadillas 4 está formada por una porción principal 4a sensiblemente laminar y un tramo de unión 4b de menor espesor
fijable a la banda flexible que facilita su separación con respecto a la porción principal 4a, habiéndose provisto en dicha porción principal 4a de la almohadilla 4 un par de orificios pasantes 5 que pueden ser atravesados por neocuerdas 3.
Las almohadillas 4 pueden estar unidas a la banda alargada 2 por un material adhesivo, por medio de cosido o cualquier otro método de fabricación adecuado.
En la figura 4 puede verse la disposición de la banda protésica respecto al anillo mitral 10.
A continuación se detalla el uso de la banda protésica 1 a partir de las figuras 5A-5C donde se han representado de forma esquemática las almohadillas 4.
En la figura 5A se identifican los segmentos que prolapsan de ambos velos. Así, en primer lugar se cose uno de los extremos de la neocuerda 3 a la cabeza fibrosa 6 de los músculos papilares 40 dejándose el extremo opuesto libre. Seguidamente, el cirujano realiza una anuloplastia del anillo posterior con la banda alargada 2 de la banda protésica 1 anclando sus extremos en los trígonos 7.
Posteriormente se anuda el extremo libre de las neocuerdas a las almohadillas 4, tal como puede verse en la figura 5B.
Finalmente, se corta la almohadilla separándola de la banda alargada 2 y se traslada la almohadilla 4 en cuestión con su correspondiente neocuerda 3 suturada hasta el segmento prolapsante del velo (véase la figura 5C), fijándose a éste por cosido con una sutura de polipropileno.
Los detalles, las formas, las dimensiones y demás elementos accesorios, así como los materiales empleados en la fabricación de la banda protésica de la invención podrán ser convenientemente sustituidos por otros que sean técnicamente equivalentes y no se aparten de la esencialidad de la invención ni del ámbito definido por las reivindicaciones que se incluyen a continuación.
Claims
1. Banda protésica (1), en particular para la reparación de una válvula mitral, que comprende una banda alargada (2) de un material flexible para la fijación de cuerdas artificiales, caracterizada por el hecho de que se incluye una pluralidad de almohadillas (4) separables que están unidas a lo largo de la trayectoria de dicha banda alargada (2) y que sobresalen con respecto a ésta, cuyas almohadillas (4) están separadas entre sí y son susceptibles de recibir tales cuerdas artificiales.
2. Banda protésica (1) según la reivindicación 1, caracterizada por el hecho de que cada una de las almohadillas (4) está formada por una porción principal sensiblemente laminar y un tramo de unión de menor espesor fijable a la banda flexible.
3. Banda protésica (1) según la reivindicación 2, caracterizada por el hecho de que la porción principal de la almohadilla presenta al menos dos orificios pasantes (5) que son atravesados por cuerdas artificiales.
4. Banda protésica (1) según la reivindicación 1, caracterizada por el hecho de que todas las almohadillas (4) sobresalen en un mismo sentido cuando la banda alargada (2) está extendida de forma recta.
5. Banda protésica (1) según la reivindicación 1, caracterizado por el hecho de que las almohadillas (4) están unidas a la banda alargada por un material adhesivo.
6. Banda protésica (1) según la reivindicación 1, caracterizada por el hecho de que las almohadillas (4) están unidas a la banda alargada por medio de cosido.
7. Banda protésica (1) según la reivindicación 1, caracterizada por el hecho de que la banda alargada (2) presenta una longitud aproximadamente de 63 milímetros.
8. Banda protésica (1) según la reivindicación 1, caracterizada por el hecho de que la banda alargada (2) está conformada por un núcleo central de material de silicona una envoltura exterior de material de poliéster.
9. Banda protésica (1) según la reivindicación 1, caracterizada por el hecho de que las almohadillas (4) están hechas a partir de PTFE .
