Definitions
- the invention relates to a surgical device for testing nerves.
- HF surgery high-frequency surgery
- tissue structures are cut or coagulated by means of surgical instruments connected to surgical devices. If those tissue structures are crossed by nerves, the use of the surgical instrument can lead to damage or destruction of the nerves. If, for example, nerves on the face are injured, the mimic expression of the patient is impaired, which is perceived as unpleasant.
- US 2006/0184164 A1 discloses an HF surgical device with a surgical instrument which can be operated in various modes, namely those for cutting, coagulating tissue and stimulating.
- the electrosurgical unit includes a DC power supply that provides a DC current to stimulate nerves, and an RF generator that provides high frequency current for cutting or coagulating tissue.
- WO 00/13600 shows an HF surgical device to which a surgical instrument for treating motor nerves can be connected by means of high-frequency current.
- the energy for the treatment of the tissue is thereby provided by an RF generator, whereas the energy for the stimulation of the tissue is provided by a pulse generator.
- the HF surgical instrument of WO 00/13600 is relatively complicated and space-consuming.
- the invention has for its object to provide a surgical device which can be produced with less effort and space-saving. This object is achieved by a surgical device according to claim 1.
- the object is achieved by a surgical apparatus for testing nerves during surgery using an RF surgical generator and an electrosurgical instrument, the surgical apparatus comprising a transducer means for converting a high frequency treatment current from the RF surgical generator to a nerve stimulation current controllable switch for selectively supplying either the treatment current or the nerve stimulation current to the electrosurgical instrument.
- the converter device converts the high-frequency treatment current available in the surgical device from the HF surgical generator in such a way that it can also be utilized for stimulation of nerves. This eliminates the need for a separate DC or low-frequency current generator for nerve stimulation, with the result that the surgical device is designed not only space-saving than the devices known in the prior art, but also can be produced in a simplified manner. This can save costs.
- the converter means comprises a rectifier circuit for generating a direct current as a nerve stimulation current.
- the high-frequency treatment current of the HF surgical generator can be converted into a current suitable for the stimulation of nerves in a particularly simple manner.
- the converter device comprises a frequency divider for generating a low-frequency stimulation current, which also represents a particularly simple and structurally less complicated possibility for converting the high-frequency treatment current of the RF surgical generator into a current suitable for stimulating the nerves.
- the converter device preferably comprises a pulse modulation device for generating a time-defined pulse or a chronologically defined pulse following the nerve stimulation current. This allows suitable stimulation patterns to be produced.
- the converter device comprises a constant current source for generating a constant current intensity of the
- Nerve stimulation current This development is suitable for generating reproducible stimuli.
- the constant current source is adjustable to predetermined currents. This allows adaptation of the transducer device to the tissue varying within the human body or their specific electrical resistances.
- the transducer means includes a driver coupled to the RF surgical generator so as to adjust output current parameters and voltage parameters of the RF surgical generator to produce defined nerve stimulation current parameters. This is an easy way to set defined nerve stimulation current parameters.
- the converter device is preferably arranged in a separate housing or housing insert and is designed such that it can be connected to the surgical device via plug connections or the like. Such a form of training is particularly suitable for upgrading an existing surgical device with the transducer device.
- FIG. 1 is a schematic representation of a surgical device with connected electrosurgical instrument and a tissue section to be treated
- FIG. 2 shows an alternative embodiment of the surgical device deviating from FIG. 1 with the connected electrosurgical instrument and with the tissue to be treated from FIG. 1, FIG.
- Fig. 3 shows an alternative, deviating from Fig. 1 embodiment of
- FIGS. 1 and 2 shows a further alternative embodiment of the transducer device, together with the electrosurgical instrument and the tissue section from FIGS. 1 and 2.
- FIG. 1 shows a surgical device with a transducer device 20 and an HF surgical generator 10 and a connected electrosurgical instrument 4.
- a tissue section 2 is shown, which can be treated or stimulated with the electrosurgical instrument 4.
- the HF surgical generator 10 here comprises an RF oscillator 14 as well as three control elements 11a to 11c and two display elements 12a, 12b. Via a switch 13, the RF surgical generator 10 can be controlled.
- the switch 13 is arranged here outside of the RF surgical generator 10 and connected to this example, wired or wireless and z. B. formed as a footswitch.
- various parameters such as frequency, voltage, current, etc., which characterize the RF surgical generator 10, can be set.
- the display elements 12a, 12b display these adjustable and other parameters.
- a high-frequency treatment current from the RF surgical generator is passed via connecting lines 8 to converter device inputs 21 of the converter device 20.
- the high-frequency treatment current passes through the converter device 20 either substantially unchanged (as in the position of the changeover switch 22 in FIG. 1) or due to a detour via a converter circuit 23 in a modified form.
- a converter output 24a is connected via a connecting line 8 'to a neutral electrode 7 which is connected to a fabric 2.
- a converter output 24b is connected via a connecting line 8 "to the electrosurgical instrument 4, which comprises an active electrode 6.
- the electrosurgical instrument 4 has an operating element 5, which is connected via an information transmission device 3 with the switches 22, these controlling.
- This connection can also be wired or wireless, for example via an infrared or radio link, be made concrete.
- the transducer device 20 and the electrosurgical instrument 4 corresponding (not shown in figures) receiving and transmitting devices that receive or send the corresponding information.
- further information can also be transmitted via the information transmission devices 3.
- a malfunction of the converter device 20 is transmitted to a display element (not shown in FIGS.) Of the electrosurgical instrument 4.
- a fine adjustment of the transducer device 20, for example, with respect to various parameters such as frequency, voltage, current, etc., are made.
- the converter circuit 23 may comprise, for example, a rectifier circuit 25 (see FIG. 2) or else a frequency divider (not shown in figures).
- the converter circuit 23 comprises a rectifier circuit 25 with a diode 26. Furthermore, in FIG. 2, the operating element 5 of the electrosurgical instrument 4 via the information transmission device 3 not only with the switches 22 The driver also controls the RF surgical generator 10 such that the output current parameters and voltage parameters of the RF surgical generator 10 are adjustable to produce the desired nerve stimulation current.
- the overall arrangement of essentially two modules, namely the RF surgical generator 10 and the converter device 20, is also constructed in FIG. 2.
- the converter device 20 is an integral part of the RF surgical generator 10.
- the modular design has the advantage that it is easier to retrofit commercially available RF surgical equipment.
- FIG. 3 shows a development of the converter circuit 23 from FIG. 2.
- the rectifier circuit 25 is formed here by the diode 26 and a capacitor 27, while a constant current source 30 in a known manner by a transistor 28 (in this case a pnp transistor) and resistors 29a to 29c is formed.
- a transistor 28 in this case a pnp transistor
- resistors 29a to 29c is formed.
- the information transmission device 3 can also connect the operating element 5 of the electrosurgical instrument 4 to the converter circuit 23 (analogous to FIG. 3), so that an adjustment of the converter circuit 23 can take place via the operating element 5 of the electrosurgical element 4. Also, information or data of the converter circuit 23 via the
- a pulse modulation device 31 is arranged between the electrosurgical instrument 4 and the converter circuit 23 and is connected to both the converter circuit 23 and the electrosurgical instrument 4 by the information transmission device 3.
- the pulse modulation device 31 serves to generate a time-defined pulse or chronologically defined pulse sequences of the nerve stimulation current.
- a single actuation of the operating element 5 leads to such a triggering of the pulse or the pulse sequence, so that the tissue is exposed to a stimulation current only over a specific (short) period of time and a renewed actuation of the operating element 5 for resetting a cutting or coagulation of the electrosurgical instrument 4, is not necessary.
