明細書 ス トーマ用袋及び処理法 技術分野
この発明は人工肛門等のストーマを設けた人の排泄物用の袋、 即ちバウチとそ の排泄物の処理方法に関する。 背景技術
従来人工肛門等に於いては底部が開放されているバウチを用いて、 常時はピン チで挟んだり、 第 9図 Aから D迄のように何度も折ってゴムバンドで固めたりし て便や尿が漏れないようにしておき、 ある程度溜まったら底部を開放し、 中身を 排出していた。 その際どちらも便排出後はバウチの端末を洗浄しなければならな かったし、 ピンチは簡単であつたが漏れやすく、 ゴムバンドで固める方法は面倒 で手間の掛かるものであった。 密封が難しいと、 装着中に便が衣類や袋の外側に 付着したり、 不完全な装着により使用中に漏れたりする事故も起きた。
底をシールして密封してあるバウチもあるが、 1日用とか短期間用なので、 こ の場合は装着する回数が増える問題が生じた。 装着する場合はその都度患部を清 潔に洗い、 乾燥しなければならず、 何時でも、 何処でもとは行かず、 又費用の点 でも大変であった。
さらに溜まった便を処理しようとバウチを開けると、 当然であるが非常に強い 便臭が漂うので、 本人も不快になるが、 介護する人も不快になった。 そしてその 介護人も含めて周囲の人々を不快にさせるであろうという心配が本人を非常に委 縮させ、 旅行に行ったり、 外出したりといった外の生活を出来なくさせてしまつ てもいた。 尿の場合も臭気こそ弱いが同様な問題があった。 発明の開示
バウチがポリェチレン等の熱溶着可能なフィルムを用いた袋であることを利用 して、 シール機により必要な部分をヒートシール、 ヒートカッ トして、 便や臭気 を外気に触れさせることなく、 溜まった排泄物を排除する方法を案出した。 又力 ッ 卜した縁をさらに密閉して漏れの可能性を完全に排除した。
そしてその方法に適した丈の長いバウチや、 バウチの粘着部から下部の、 適当 な長さの範囲でその幅全面に渡り、 片側の内壁全面にメジュ一ムや他のィンキで
5印刷を施した、 増設の出来るバウチを用意した。
この使用目的のために、 従来は生産現場で使用されていたィンパルスヒ一トシ —ラーを、 軽く手で持てて、 洗浄も出来、 握力の弱い人にも使えるものにして、 ヒーターもその断面で見て中央部から両端迄、 常に凸である曲線を描く形状とし た。

2 0発明を実施するための最良の形態
本発明を人工肛門用バウチに応用した例を第 1図の平面図で説明すると、 第 1 図 Aのバウチ 1に溜まつた便 2を袋の下部に集め、 その少し上を第 1図 Bのよう にイ ンパルスヒ一トシーラ一 3で挟み、 閉じて圧力を掛け、 1秒ほど通電して加 熱する。 2秒ほどそのまま冷却してシーラーを外し、 シール線の上下でバウチ 1
2 5を強く引っ^ると、 バウチ 1は第 1図 Cのようにシールで密封されたまま二つの 袋に分かれる。 この便 2の入った袋をゴミとして処理すれば、 バウチ 1は切断し た分だけ短くなるが、 便の無い状態になり、 その後そのまま使える。 臭気は便が 外気に触れないので全く出ない。
この作用を第 1図 Bの中央の断面図と見なせる第 2図でさらに細かく説明する と、 ヒートシ一ラー 3の一方のフレーム 4の内部にはヒーター 5が、 即ちアルミ 製の台 6に絶縁用のガラステープ 7を貼った上に張られた直径 0. 6 mmの半円 のニクロム線であるヒータ一線 8と、 さらにその上に貼られた粘着剤付きフッ素 樹脂テープ 9からなるが、 設けられており、 他方のフレーム 4には同じくアルミ
5 製の台 6に貼られたシリコンゴム 1 0が設けられている。
バウチ 1を固形物が挟まらないよう、 外からしごいてきれいにしてィンパルス ヒートシ一ラー 3で挟むことによって、 ノ ゥチ 1はヒーター 5とシリコンゴム 1 0に挟まれ、 圧迫される。
そして電源回路より適当な電流を流すと、 ヒータ一 5が発熱し、 1秒前後でパ 0 ゥチ 1のその個所は第 3図のように、 互いに溶け合い、 中央は薄くなる。 2秒程 そのままにして冷却した後、 その部分を強く手で引っ張るなり、 中央部をハサミ で切ると、 両方の袋は密封されたまま分かれる。 電流が強ければシールしながら 同時にヒ一トカツ 卜することもある。
ヒータ一 5は第 4図 Aのように幅 2〜 5 m m程度の平線 1 1を用いたものにし i s て、 シールした後ハサミでカッ 卜してもよいし、 第 4図 Bのようにニクロム線の 厚さ 0. l m m、 幅 2 mm程度の平線に直径 0. 3 mm程度の丸線を溶接した、 いわゆる玉シール線 1 2にして、 シールと同時にカッ トしてもよい。 しかしこれ らの場合は、 袋の内面に付着した便や液体が、 シーラ一で挟んで圧迫された時、 途中に残るので、 カツ 卜された断面から便又は臭気が多少出る恐れがある。
2 0 これに対して第 2図の半円線 8とか、 第 4図 Cのように台 6に突起を設けて凸 にして、 ^面を湾曲させたリボン線 1 3からなるヒーターだと、 中央部が凸で、 その両端がなだらかに広がっている、 言い換えれば常に凸であると、 圧力を掛け た時にバウチの内部に残った液体や固体をシール部分から排除し易いし、 ほんの 少しであるが袋の素材が伸びるので、 シール及びカツ 卜した時にバウチの切断面
2 5がよりきれいになる。
しかしヒータ一形状がいく ら良くても、 そして予め出来るだけバウチの外側か らしごいたりしてシール部分から排除しても、 便の状態により固形物、 特に織維 状のものが挟まることが無いとは言えない。 そのカツ 卜した縁に粘着テープをべ つたり貼ってもよいが、 その縁は熱で皺が寄っているので完全には密閉すること が出来ない。
その場合はそのカツトした縁 1 4を別の袋に密封すれば良く、 第 5図の下部に 描かれたような一辺を二つ折りして他の一辺をシールした密封用袋 1 5をその縁 1 4に被せ、 バウチ 1と重なった部分に縁 1 4と平行にシール 1 6、 及び開放さ 5れている辺にシール 1 7を施すことにより、 カットした縁 1 4を完全に密封する ことが出来る。 この方法は密封用袋 1 5の代わりにフイルムでも三辺をシールし た袋でも出来るが、 やり辛い。 縁 1 4を入れるのは二辺シールがよく、 そのうち 一辺は二つ折りでもシールでもよい。
本発明の方法では力ットした時に臭気が出ないということは、 ガスも出ていか
1 0ないということなので、 バウチにはガス抜きが必須になる。 簡単にはバウチの上 部の一部に孔を開け、 ガスを押し出した後、 その孔をシールして塞げばよい。 し かし脱臭装置を結合してガス抜きをしたり、 時にはバウチの内部を清掃したりす る場合もあるので、 第 1図 は第 5図のようにそのストーマ用孔 1 8よりも上部 に本格的な密閉する栓又は蓋を用意した孔 1 9を設けても良い。
i s 本発明に使用するバウチは、 切り取り長さを 1回 5 c mで 1日 3回とすると、
3日分では 4 5 c mとなる。 この長さを粘着部に加わえると、 全長では 6 0 c m にもなる。 これでは困る場合もあるので、 最初はある程度の長さにしておいて、 使用中に短くなつたら別の袋を増設することが出来ればよい。
その為には第 6図のように従来の形で言えば面板等からなる粘着部 2 0から下
2。方に、 幅いっぱいの薄いポリエステルフィルム 2 1を挟み、 外側に増設用の袋又 はチューブ 2 2を被せ、 それらの上からヒートシール 2 3するのである。 すると 5枚全部のフィルムに熱が通り Γί:いに溶着するが、 ポリエステルフィルムは溶藿 しないので剥がれ内部は通じ、 外側同志は溶着し、 漏れない一つの袋又はチュー ブになる。 チューブの場合はポリエステルフィルムを取り外すことが出来、 その
2 5後底をシールする。
シールさせない効果は印刷面でも同じである。 従つて第 6図のポリエステルフ イルム 2 1と同じ位置に、 即ち粘着部から下部の適当な長さで、 その幅のほぼ全 部に渡り内側に、 少なくとも一つの側にメジユームゃ他のィンキで印刷を施して おけば、 必要なときに袋を増設することが出来るし、 さらにその袋をそシールの すぐ上で切断して除去すれば、 さらに又別の袋を増設することも出来る。
本発明に用いるヒートシ一ラーは熱板式でも使えないことはないが、 1秒前後 の短時間大電流を流して加熱するィンパルス式の方が予熱をしないですぐ使えて 便利である。 しかし従来のイ ンパルスヒ一トシ一ラーは^產坰で大きく、 机上で
5使うものでも トランスを内蔵するので重く、 持ち運びし辛いものであった。
そこで本発明に用いる使い易いィンパルスヒートシ一ラーの側面図を第 7図に 示す。 ポリカーボネートのような剛性と煮沸消毒にも耐える耐熱性を持ったブラ スチックを用い、 向き合った必要な長さのフレーム 4を作り、 一方にはアルミ台 の上にヒータ一を、 他方には同じくアルミ台の上にシリコンゴムを設け、 それら 1 0の一端を支点 2 4として回転するように止めて、 他端は力点として手で握ってフ レーム同志に挟圧力を加えられるようなハンドル 2 5とした。
さらにフレーム内にはトランスゃスィツチを含めた電源回路を入れず他に設け て配線だけにして、 軽く洗浄可能にし、 机上でも使えるが空中で手で持ったまま でも、 バウチのような長い袋の途中をヒートシール又はヒートカツ ト出来る構造 i sとした。 なお 2 6はコードである。
この時さらに第 7図のようにハンドルの端部に、 口ック又はクランプ 2 7を設 け、 弱い力でレバ一を動かすことにより、 梃子の原理で強い挟圧力が生ずるよう にすると、 握力の弱い人でも楽に使うことが出来る。 この時クランプのある段階 で、 バウチを挟みながら移動できる程度の緩さにすると、 バウチ内の排泄物を片
2 0側に押しやるのが楽に出来た。
外に出した電源回路は家庭用 1 0 0 Vからトランスを用いて電圧を下げるが、 この時 1 2 Vにすると、 幅が 1 5 c m程度のバウチだとシール時間が 2秒以上に 長くなるが、 使うことが出来る。 これにより自動車の電源からも取れるので、 本 体を軽くすることと合わせて、 何処でも使うことの出来るィンパルスヒ一トシ一
2 5ラ一となつた。
次に本発明の便処理と従来の方法との比較をした実験結果を第 8図のグラフに 表した。 実験は次の条件で行った。
実験 A—紙コップに便を出し、 それを一旦床に置き、 バウチの裾をゴムで閉鎖 し、 紙コップをポリ袋に入れ室外に持ち出す。 実験 B—紙コップに便を出し、 すぐにポリ袋に入れ袋の口を縛り、 その後裾の 処理をする。
実験 C—バウチ内に予め消臭剤を 6滴入れておき、 実験 Bと同様の処理をする。
消臭剤にはバニッシュを使用
実験 D—実験 Bの処置後、 消臭スプレーを 2秒噴霧する。
実験 E—シーラーを使用して便処理をする。
測定は人の嗅覚と臭気測定器とによるものとし、 測定器はアルケア社の二オイ センサ一 X P— 3 2 9を使用、 2 0 0までの数値を実線で表した。 便は健康な 3 才児のもので各 5 0 g、 測定は便処理より 5 0 c m離れた地点で行った。 室温 25度( 、 湿度 6 0 %の室内で、 処理は窓を閉めたまま行い、 処理直後窓を開放し た。 所用時間はバウチ着用者が処理をして、 着衣を整え、 その部屋を退出する迄 の時間とした。
嗅覚での測定は健康な女性 3名の中間値とし、 以下 6段階の基準により点線で しに。
臭気強度 0—無臭
臭気強度 1一やつと感知できる臭い
臭気強度 2—何の臭いか分かる弱い臭い
臭気強度 3—楽に感知できる臭い
臭気強度 4一強い臭い
臭気強度 5—強烈な臭い
この第 8図の実験からもはっきり言えるように、 従来の A〜Dの方法ではどう 工夫しても強烈な臭いを取り除くことは出来なかったが、 実験 Eのようにシ一ラ —を用いれば臭いを全く感じないで処理を行うことが出来た。
又シ一ラ一を用いれば実験 Eのように所用時間も半分以下になり、 処理する者 も手を汚すことなく清潔であった。 さらにこの方法は身体に直接使用するもので もないので安全性にも問題が無かった。 産業上の利用可能性
本発明はバウチをシール機でシールするだけなので簡単であり、 バウチ装着者 や介護者が年寄りや身体の不自由な人でも出来たし、 周囲を汚したり、 後で漏れ るような失敗もしないように出来た。
便及び臭気が一切外に出ないので、 電源さえあれば処理が何処でも出来たし、 介護する人にも気安く頼めた。
シ一ラーと本発明の方法により、 バウチは何時でも好きなように増設出来るの で、 従来のバウチを用いることも出来るし、 普段は必要最小限に短く して使用す ることも出来た。
これら本発明の効果によりバウチ装着者が今まで行動が狭められたり、 対人恐 怖症的に生活していたことから開放され、 ずつと普通の生活が行えるようになつ た o
又その費用も経済的で、 長くなつた袋と極少量の電気代だけなので、 今までの バウチのその都度洗浄や乾燥をしなければならなかつた温水等の光熱費に比べて はるかに低くなった
これらにより従来生産過程でしか利用されなかったヒートシ一ラーが、 軽く持 ち運びが出来て、 力の弱い人にも使える形状とすることにより、 バウチ処理機と して身近に利用されるようになった。

Claims

請求の範囲
1 . バウチがポリェチレン等の熱可塑性樹脂を用いた袋であることを利用して、 バウチに溜まつた排泄物を袋の底部に集め、 そこと残りの部分との間をヒ一トシ ーラ一によりヒートシールし、 同時又は後でシールの中程を力ッ 卜することによ り、 臭気も外に出すことなく排泄物を除去することを特徴とする、 バウチの排泄 物の処理法。
2. 少なくともストーマ用孔とその周囲の面板等の粘着部を設けてなるバウチに 於いて、 粘着部よりも下部の長さが従来のものより長く、 1回以上の切り取りに 耐える充分な長さを持つことを特徴とする、 請求項 1の方法に用いるバウチ。
3. バウチのシールをカッ トした縁に、 別の袋又はフイルムを被せ、 ヒ一トシ一 ラ一によりバウチと重なった部分と開放されている端部をシールして、 先程の縁 の直ぐ外側を囲う四辺をシール線又は折り返しで囲って、 その縁を完全に密閉す ることを特徴とする、 請求項 1のバウチの排泄物の処理法。
4. シールするか折り曲げることにより、 少なくとも二辺が密封された袋である ことを特徴とする、 請求項 2の方法に用 t、る密閉用袋
5. 少なくともストーマ用孔とその周 の粘着部を設けてなるバウチに於いて、 粘着部から下部で、 適当な長さでその内側に、 その幅のほぼ全部に渡り、 少なく とも片側にメジユームゃ他のィンキで印刷を施すなり、 その幅のポリエステル等 の薄いフィルムを挟んだりして、 内側がヒートシ一ルしないようにしたことを特 徴とする、 別の袋又はチューブを増設することが可能な、 請求項 1の方法に用い るバウチ及び増設用袋又はチューブ。
6. 一方にはヒータ一を、 他方にはシリコンゴムをそれぞれ内蔵したアルミ台に 設けた、 必要な長さの、 向き合った剛性のあるフレームを、 一端を支点として回 転するように止めて、 他端は力点となるハンドルとして、 手で握ってフレーム同 志を、 その結果としてヒータ一とシリコンゴムに挟圧力を加えられるようにして、 さらにフレーム内にはトランスゃスィツチを含めた電源回路を入れず他に設けて 配線だけにして、 軽く、 洗浄可能な構造としたことを特徴とするイ ンパルスヒ一 トシ一ラー。
7. ハンドルの有る側の端部にハンドルと合わせて、 又はハンドルを省略して、 口ック又はクランプを設けた構造を特徴とする請求項 6のィンパルスヒートシ一 ラー。
8. ロック又はクランプのある段階では、 挟んだバウチが軽く圧迫されながら移 動できる程度の緩さが有る構造を特徴とする請求項 7のィンパルスヒ一トシーラ ο
9. ヒータ一線に粘着剤付きフッ素樹脂テープを被せたヒータ一が、 その断面で 見て中央部から両端迄、 常に凸である曲線を描くような形状であることを特徴と する請求項 6のィ ンパルスヒー トシーラー。
