Die Erfindung betrifft HMW-Endostatine.
Description
HMW-Endostatin zur Hemmung des Wachstums von Tumoren und von Gefaß ucherungen und zur Diagnose von Gefäß- und Tumorerkrankungen
Gegenstand der vorliegenden Erfindung sind Peptide mit angiostatischen Eigenschaften, die als HMW-Endostatin (High Molecular Weight - Endostatin) bezeichnet werden, sowie ein Arzneimittel enthaltend die natürlichen und synthetischen Peptide zu therapeutischen Zwecken bei Tumor- und Gefäßerkrankungen.
Diese Stoffe sind dadurch gekennzeichriet, daß sie insbesondere aus Hämofiltrat oder Hämodialysat, das aus menschlichem Blut abfiltriert wird, gewonnen werden können. Die Peptide werden als HMW-Endostatin bezeichnet und können zum Zwecke der Analyse von Erkrankungen und als Medikament verwendet werden.
Die HMW-Endostatine wurden erstmals aus dem Hämofiltrat Nierenkranker nach Ultrafiltration am Hämodialyseapparat gewonnen und wurden anhand ihrer molekularen Masse und den 30 Aminosäuren des N-Terminus charakterisiert. Zur Darstellung des humanen HMW-Endostatin wurde ein patentiertes Verfahren (Forssmann, 1988; DE 36 33 707 C2), welches die für Gewinnung von Eiweißstoffen aus Hämofiltrat beschreibt, verfeinert. Aus den mit diesem Verfahren gewonnenen Molekülen mit einem Molekulargewicht unter 30 kDa, die bei veno- venöser oder arterio-venöser Shuntverbindung abfiltriert werden, können die HMW- Endostatin enthaltenden Fraktionen durch Massenspektrometrie erkannt werden. Es wurde weiter festgestellt, daß mittels weiterer Verfahren diese Substanzen aufgereinigt werden können, bis schließlich einheitliche Eiweißstoffe isoliert und in ihrer Zusammensetzung und Sequenz aufgeklärt wurden. Die Stoffe sind ein humanes Peptid, welche überraschenderweise die Eigenschaft besitzt, sehr potent das Wachstum von Tumoren und Gefäßen zu hemmen. Überraschenderweise besitzt das Molekül eine extrem hohe Plasmahalbwertszeit, was es für die Therapie als beson-
ders wertvoll erscheinen lässt. Bislang ist schon von der Arbeitsgruppe Folkman (Harvard Medical School, Boston, USA) ein verwandtes Protein der Maus mit der Eigenschaft beschrieben worden, das Wachstum von Tumoren und Gefäßen zu hemmen (O'Reilly et al, Cell 88, 277-285, 1997).
Die erfindungsgemäßen HMW-Endostatine, deren Struktur bisher unbekannt war und deren Bildungsstätte im Körper noch ungeklärt ist, zeigen überraschenderweise eine therapeutische Bedeutung als wichtige zirkulierende Peptid des menschlichen Blutes.
Die HMW-Endostatine sind weiter dadurch gekennzeichnet, daß sie durch chemische Synthese und durch gentech ologische Produktion gewonnen werden können und überraschenderweise für zahlreiche weitere Belange genutzt werden können, unter anderem für die Analyse im menschlichen Blut als Diagnosemerkmal von Erkrankungen des Gefäßwachstums, des Wachstums von Tumoren und von Metastasen.
Die vorliegende Erfindung betrifft also neue Peptide, die HMW-Endostatine, ihre Herstellung, die Peptide enthaltende Arzneimittel, sowie natürliche und pharmako- logisch verträgliche Derivate von HMW-Endostatin, insbesondere amidierte, acety- lierte, phosphorylierte und glycosylierte HMW-Endostatin-Derivate und Fragmente des Peptides. Durch Massenspektrometrie konnten die Molekulargewichte für die vier HMW-Endostatine mit 22001 ± 0,02% (1), 19904 + 0,02% (2), 20995 ± 2.5 (3) und 21843 ± 0,02% (4) Dalton bestimmt werden.
Natürlich vorkommende Peptiden können insbesondere glykosyliert sein.

Es handelt sich möglicherweise um Fragmente des Kollagen-alpha 1 (XVIII).
Die vollständigen Ammosäuresequenzen wurden aus den eigenen Sequenzanalysen bestimmt bzw. aus der bereits bekannten cDNA-Sequenz für Kollagen-alpha 1 (XVIII) hergeleitet (Oh, S.P. et al., Genomics 19, 494-499, 1994).

Es handelt sich vermutlich um Fragmente von Kollagen-alpha 1 (XV).
Diese vollständigen Aminosäuresequenzen wurden aus der Sequenzanalyse bestimmt bzw. aus der bereits bekannten cDNA-Sequenz für Kollagen-alpha 1 (XV) hergeleitet (Muragaki, Y. et al., J. Biol. Chem., 269, 4042-4046, 1994).
HMW-Endostatin (1) und HMW-Endostatin (4) sind vorzugsweise am Rest Nummer 9 (entspricht T125 gemäß der Nummerierung von Sasaki et al., EMBO J. 17 (1998) 4249-4256). Bei einer stufenweisen Deglycosylierung des natürlichen HMW-Endostatin (1), das einen Gal-GalNac-NANA Rest trägt (MW = 22,001 Da), werden Derivate mit einen Gal-GalNAc Rest (MW = 21,710 Dalton) und ein vollständig deglycosyliertes Derivat mit einem Molekulargewicht von 21,345 Dalton erhalten (NANA: N-Acetylneuraminsäure; Gal: Galactose; NAc: N-Acetyl).
Für HMW-Endostatin (4) ergibt sich ausgehend von der gleichen Glycosylierung mit einem Molekulargewicht von 21,843 Dalton ein teilweise deglycosylieres Derivat mit einer Masse von 21,552 Dalton und ein deglycosyliertes Derivat mit einer Masse von 21,187 Dalton.
Bevorzugte glycosylierte Derivate und Fragmente von HMW-Endostatin (2) und (3) weisen Massen von 20,995, 19,904 und 16,373 Dalton auf.
Die vorliegende Erfindung betrifft weiter verschiedene Verfahren zur Herstellung der HMW-Endostatine oder ihrer Derivate durch prokaryontische oder eukaryonti- sche Expression, durch Isolierung aus menschlichem Blut über Chromatographie- Verfahren in bekannter Weise, und schließlich durch die üblichen Verfahren der Festphasen- und Flüssigphasen-Synthese aus den geschützten Aminosäuren mittels Kopplung, Deblockierung und chromatographischer Reinigung.
Die Arzneimittelzubereitung enthält HMW-Endostatin oder ein physiologisch verträgliches Salz der HMW-Endostatine. Die Form und Zusammensetzung des Arzneimittels, welches HMW-Endostatin enthält, richtet sich nach der Art der Verabreichung. Das HMW-Endostatin kann parenteral, intranasal, oral und mittels Inhalation verabreicht werden. Vorzugsweise wird HMW-Endostatin zu einem Injektionspräparat, entweder als Lösung oder als Lyophilisat zur Auflösung unmittelbar vor Gebrauch, konfektioniert. Die Arzneimittelzubereitung kann außerdem Hilfsstoffe enthalten die z.B. abfülltechnisch bedingt sind, einen Beitrag zur Löslichkeit, Stabilität oder Sterilität des Arzneimittels leisten oder den Wirkungsgrad der Aufnahme in den Körper erhöhen.
Die zu verabreichende Tagesdosis für HMW-Endostatin hängt von der Indikation und der Anwendung bestimmter Derivate ab. Sie liegt bevorzugt im Bereich von 1 μm bis 1 g pro Dosis.
Das erfindungsgemäße Peptid HMW-Endostatin ist dadurch gekennzeichnet, daß es sich besonders auch für die Langzeit-Therapie bei Tumorerkrankungen oder anderen Erkrankungen, die durch ein unkontrolliertes Gefäßwachstum gekennzeichnet sind, eignet und bei Dauerbehandlung keine Immunreaktion auslöst. Überraschenderweise besitzt das Molekül eine extrem hohe Plasmahalbwertszeit, was es für die Therapie als besonders wertvoll erscheinen läßt. Das erfindungsgemäße Präparat ist insbesondere für die Kombinationstherapie mit Chemo- oder Strahlentherapie oder in Anschluß an Chemo- oder Strahlentherapie bei Krebs geeignet.
Das erfindungsgemäße Präparat ist weiter als Mittel zur Therapie und Diagnose bei Gefäßerkrankungen des Stütz- und Bindegewebes, der Atemwege, des Herz- Kreislaufsystems und Urogenitalapparates, des Nervensystems und des Auges einzusetzen, da es zur Herstellung von human verträglichen Antikörpern verwandt werden kann, die geeignet sind, Änderungen des Gefäßwachstums in diesen Orga-
nen festzustellen oder zu beeinflussen.
Beispiele
Isolierung und Charakterisierung von zirkulierendem HMW-Endostatin (1) und von zirkulierendem HMW-Endostatin (2) aus humanem Hämofiltrat
Als Ausgangsmaterial wurde Hämofiltrat verwendet, das in großen Mengen bei der Behandlung niereninsuffizienter Patienten anfällt und alle Plasmabestandteile bis zu einer Molekülgröße von etwa 20.000 Dalton enthält.
I. Gewinnung des Rohpeptidmaterials
Das Hämofiltrat wurde mittels einer Hämofiltrationsanlage der Firma Sartorius unter Verwendung von Cellulosetriacetat-Filtern mit einer Ausschlußgröße von 20.000 Dalton (Typ SM 40042, Sartorius, Göttingen, BRD) gewonnen. Das Filtrat stammte von niereninsuffizienten Patienten, die sich durch Langzeit-Hämofiltration in einer stabilen Stoffwechsellage befanden, und wurde unmittelbar nach der Gewinnung mittels Ansäuern und Abkühlung auf 4°C gegen proteolytischen Abbau geschützt. In zwölf Extraktionen mit einer Kationenaustauschersäule (TSK SP 650(M), Merck, Darmstadt, DE) wurden insgesamt 10.000 1 Hämofiltrat verarbeitet. Die gepoolten Extrakte wurden auf dem obengenannten Säulenmaterial nacheinander durch verschiedene Puffer mit unterschiedlichen pH- Werten (sogenannte pH- Pool Eluate) eluiert (Schulz-Knappe, P., Schrader, M., Ständker, L., Richter, R., Hess, R., Jürgens, M., & Forssmann, W.G. (1997) A peptide bank generated by large scale preparation of circulating human peptides. J. Chromatogr.A, 776, 125- 1329). Anschließend erfolgte eine RP-Fraktionierung der einzelnen pH-Pool Eluate auf einer präparativen RP-Säule (Pharmacia, Fine-line) mit 80% Acetonitril, 10 mM HC1 als Elutionspuffer (Gradient 0-60%B in 60 min) wie in der oben bezeichneten Literaturstelle bereits beschrieben.
Die entstehenden salzfreien Rohfraktionen wurden anschließend gefriergetrocknet.
I. Präparative RP -Chromatographie
67 mg der Rohfraktion Nr 25 entstammend aus dem pH-Pool-Eluat 7 (Charge 04/1997) wurden mittels präparativer RP-Chromatographie grob nach Hydrophobi- zität getrennt. Von einer PrepPak Cartridge der Firma Vydac mit den Dimensionen 47 x 300 mm wurden Fraktionen abgesammelt.

Die Fraktionen 27 + 28 enthielten die erfindungsgemäßen Peptidsubstanzen.
III. Analytische RP-HPLC
In einem Gradienten auf einer Vydac-Säule (10 x 250 mm Stahlmantel, Material: RP C4, 300 Ä, 5 μm) konnte eine weitere Auftrennung der Fraktionen 27 + 28 aus der vorhergehenden Chromatographiestufe erzielt werden. Die Eluenten waren Wasser mit 0,1 Vol% Trifluoressigsäure und 80% Acetonitril mit 0,1 Vol% Trifluo- ressigsäure.

Die Fraktionen 41 enthielt die erfindungsgemäße Peptidsubstanz HMW-Endostatin (2) und HMW-Endostatine (3).
Die Fraktionen 43 enthielt die erfindungsgemäße Peptidsubstanz HMW-Endostatin (1).
IV. Bestimmung der molekularen Masse von zirkulierendem HMW-Endostatin m ittels Elektrospray-Massenspektrometrie Die Molekularmassen wurden von den nativen, gereinigten HMW-Endostatin Molekülen aus der Präparation im Schritt III gemessen. Für die drei Substanzen wurden drei molekulare Massen mit Hilfe eines Elektrospray-Massenspektrometer (Sciex API III, Perkin-Elmer, Langen, DE) gemessen. Es zeigten sich Peaks von den acht- bis elffach protonierten Molekülen. Die Molekularmassen betrugen 19904±0,02% für HMW-Endostatin (2), 20995±2.5 Dalton für HMW-Endostatin (3) sowie 21999±4.2 Dalton für HMW-Endostatin (1). Bei den HMW-Endostatin Molekülen handelt es sich möglicherweise um Sequenzvarianten oder unterschiedlich glykosi- lierte Formen des humanen Kollagen-alpha 1 (XV) bzw. Kollagen-alpha 1 (XVIII),
da die gemessenen Molekularmassen nicht mit den aus der Aminosäuresequenz des humanen Kollagen-alpha 1 (XV) bzw. des Kollagen-alpha 1 (XVIII) herleitbaren Massen übereinstimmen.
V. Reinheitsbestimmung mittels Kapülar-Zonen-Elektrophorese
Die aufgereinigten Peptide wurden direkt zur Messung in der Kapillar-Zonen- Elektrophorese verwendet. Das Elektropherogramm zeigt lediglich einen Peak und keine weiteren Peaks von Nebenbestandteilen. Dieses Ergebnis zeigt, daß in der Endreinigungsstufe hochreine HMW-Endostatin Moleküle vorlagen.

VI. Sequenzanalyse von HMW-Endostatin
Die aufgereinigten nativen Peptide werden mittels Edman- Abbau auf einem ABI 473 A Sequenzer unter Verwendung des Standard-Programms analysiert. Die Proben wurden auf eine Polybrene-Membran in Mengen zwischen 100 und 400 pmol aufgetragen. Es ergaben sich für die HMW-Endostatine

VII. Bestimmung der biologischen Wirksamkeit von HMW-Endostatin Alle mit Hilfe des beschriebenen Verfahrens aus humanem Hämofiltrat isolierten hochreinen HMW-Endostatine wurde zur Bestimmung der biologischen Funktion in Endothelzell-Proliferationsassays eingesetzt. Für diesen Assay wurden bovine Kapillarendothelzellen aus der Nebennierenrinde von frisch geschlachteten Kälbern, wie in der Literatur beschrieben (Folkman, J. et al., 1979), in Kultur genommen.
Die Ausführung des Proliferationsassays wurde, wie in der Literatur beschrieben (O'Reilly, M., et al. Cell 88, 277-285, 1997), durchgeführt. Das isolierte hochreine HMW-Endostatin wurde den bovinen Kapillarendothelzellen zugegeben und hemmte in konzentrationsabhängigerweise die Proliferation dieser Zellen. Eine halbmaximale Hemmung der Proliferation in diesem Assay wurde bei einer Konzentration von 20 ng/ml HMW-Endostatin erreicht, was überraschenderweise auf eine sehr potente anti-angiogene Wirkung hinweist. In Proliferationsassays, die mit nicht-endothelialen Zellen durchgeführt wurden, z.B. NIH 3T3-Zellen und LMTK- Zellen, zeigte HMW-Endostatin keine antiproliferative Wirkung.

4. Verfahren zur Herstellung des HMW-Endostatin nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß das HMW-Endostatin durch prokaryonti- sche oder eine eukaryontische Expression hergestellt und chromatographisch gereinigt wird.
5. Verfahren zur Herstellung des HMW-Endostatin nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß man es aus menschlichem Blut über Chromatographie- Verfahren isoliert.
6. Verfahren zur Herstellung des HMW-Endostatin nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, daß man das HMW-Endostatin durch Festphasen- und Flüssigphasen-Synthese aus den geschützten Aminosäuren, die in der ausgegebenen Sequenz enthalten sind, herstellt, deblockiert und es mit den gängigen Chromatographie- Verfahren reinigt.
7. Arzneimittel, enthaltend das HMW-Endostatin nach einem der Ansprüche 1 bis 3 neben üblichen Hilfs- und Zusatzstoffen.
8. Verwendung des Arzneimittels nach Anspruch 7 zur Behandlung von Erkrankungen des menschlichen Organismus, insbesondere in Verbindung mit Gefäßwucherungen, Krebs oder Erkrankungen des Herzkreislauf- und Nervensystems.
9. Verwendung des Arzneimittels nach Anspruch 7 zur Behandlung von Erkrankungen des menschlichen Organismus, insbesondere mit Beteiligung des Intu- gementes und der Sinnesorgane, insbesondere des Auges.
10. Verwendung des Arzneimittels nach Anspruch 7 zur Behandlung von Systemerkrankungen bei Mangel von HMW-Endostatin insbesondere durch Anwendung von HMW-Endostatin zur Substitutionstherapie.
11. Verwendung des Arzneimittels nach Anspruch 7 zur Behandlung von chronischen Erkrankungen, teils vergesellschaftet mit Erkrankungen gemäß Ansprüchen 8 bis 10, indem es in geeigneter Form für die Behandlung für die Elektrolytwirkung bei Tumor- und Gefäßerkrankungen benutzt wird.
12. Verwendung des Arzneimittels nach Anspruch 7 zur Behandlung von akuten Erkrankungen gemäß Ansprüchen 8 bis 10, indem es in geeigneter Form für die Behandlung in der Intensivpflege dieser Erkrankungen benutzt wird.
13. Antikörper dadurch gekennzeichnet, daß sie gegen HMW-Endosatin nach einem der Ansprüche 1 bis 3 oder synthetische Teilstücke gerichtet sind.
14. Arzneimittel nach Anspruch 7 in galenischen Applikationsformen, insbesondere der lyophilisierten, mit Mannit aufgenommenen Form in sterilen Ampullen zur Auflösung in physiologischer Kochsalzlösung und/oder Infusionslösungen zur wiederholten Einzelinjektion und/oder Dauerinfusion in Mengen von 30 Mikrogramm bis 30 Milligramm reines HMW-Endostatin pro Therapie- Einheit.
15. Diagnostikmittel enthaltend spezifische Antikörper gemäß Anspruch 13.
16. Nukleinsäuren kodierend für HMW-Endostatin nach einem der Ansprüche 1 bis 2.
17. Diagnostisches Verfahren umfassend den Schritt der Bestimmung der Konzentration des HMW-Endostatin nach einem der Ansprüche 1 bis 3 im Blut.
18. Arzneimittel enthaltend Antikörper nach Anspruch 13 zur Behandlung der Überexpression von HMW-Endostatin.
