Abstract
The method of laparoscopic balloon dilatation of the large papilla of the duodenum and lithoextraction from the common bile duct includes the imposition of pneumoperitoneum, laparoscopic cholecystectomy, balloon catheter through the stump of the vesical duct or choledochometal up to 5 atm and exposure up to 10 minutes, contact lithotripsy for large stones, reduction of stones in the duodenum, ligation of the duct of the bladder or suturing the wound of the choledochus, suturing laparoscopic incisions in the skin. With the help of fibrogastroduodenoscopy before pneumodilation, a puncture of the large duodenal papilla is performed and an antispasmodic (for example, 0.5-1 ml of 0.1% solution of atropine sulfate) is injected into the drug for its drug dilatation with subsequent reduction of concretions up to 20 mm from the choledochus into the duodenum.
Description
A useful model belongs to the field of medicine, namely endoscopic surgery, and can be used to remove calculi from the common bile duct in patients with cholangiolithiasis, mechanical jaundice, cholangitis.
In the world, the number of patients with gallstone disease, which in 14-57 96 cases is complicated by choledocholithiasis, mechanical jaundice, cholangitis, is progressively increasing. After laparotomy interventions, 4-22 95 complications are noted, the mortality rate is 0.9-12.3 95. Currently, a two-stage tactic for the treatment of mechanical jaundice and cholangitis is adopted. At the first stage, endoscopic external drainage of the gallbladder or duct is carried out by one of the methods. After the disappearance of jaundice, the second stage is carried out - a radical operation. One of the minimally invasive methods of treating choledocholithiasis is balloon papillodilatation and lithoextraction.
There is a known method of antegrade lithotripsy and lithoextraction (1), with percutaneous transhepatic access with a diameter of 28Eg after preliminary bulging of the primary puncture access and the formation of a sealed transhepatic channel on the cholangiostomy drainage for further manipulations. A balloon catheter with a balloon diameter of 8 mm at a working pressure of up to 5 atm was used for papillodilation. and exposure up to 10 min. The balloon was installed in the region of stenosis of the large duodenal papilla through transhepatic cholangiostomy access. After using the balloon, stones with a diameter of up to 0.8 cm independently moved into the duodenum. For larger calculi, contact lithotripsy and lithoextraction were performed.
Disadvantages of this method are: injury to the liver tissue and impossibility of removal of calculi larger than 8 mm through the large duodenal papilla.
The closest analogue is the method with the use of antegrade balloon papillodilatation (21), which is performed in patients with a high surgical and anesthetic risk, with endoscopically "difficult" megacholedocholithiasis, as well as in patients with a history of gastric resection.
This creates conditions for ensuring adequate release of choledochal stones after lithotripsy, allows successful antegrade reduction of stones up to 8 mm in size into the lumen of the duodenum when it is impossible to remove them transpapillary endoscopically.
This method is an alternative to endoscopic retrograde papillosphincterotomy due to the absence of serious complications. This method consists in imposing a pneumoperitoneum,
By passing a balloon catheter through the stump of the duct or a choledochotomy incision, its installation in the area of stenosis of the large duodenal papilla with a balloon diameter of 8 mm, at a working pressure of up to 5 atm. and exposure for up to 10 minutes, contact lithotripsy for large stones, removal of calculi with a diameter of up to 8 mm into the duodenum, ligation of the cystic duct or suturing of the choledochal incision, suturing of laparoscopic skin incisions.
The disadvantage of the closest analogue is also the impossibility of descending into the intestine through the large duodenal papilla of concretions larger than 8 mm. In addition, with pronounced spasm of the papilla, it is not possible to dilate it.
The basis of a useful model is the task of creating such a method of laparoscopic balloon dilatation of the large papilla of the duodenum and lithoextraction from the common bile duct, which would be devoid of the mentioned disadvantages.
The task is solved by the fact that the method of laparoscopic balloon dilatation of the large papilla of the duodenum and lithoextraction from the common bile duct, which includes applying a pneumoperitoneum, performing a laparoscopic cholecystectomy, passing a balloon catheter through the stump of the cystic duct or a choledochotomy incision, installing it in the area of stenosis of the large duodenal papilla with with a cylinder diameter of 8 mm, at an operating pressure of up to 5 atm. and exposure for up to 10 minutes, contact lithotripsy for large stones, removal of calculi into the duodenum, ligation of the bladder duct or suturing of the choledochal wound, suturing of laparoscopic incisions on the skin, according to a useful model, with the help of fibrogastroduodenoscopy, a puncture of the large intestine is performed before pneumodilatation duodenal papilla and with the help of an injector inject an antispasmodic into it (for example, 0.5-1 ml of a 0.195 solution of atropine sulfate) for its medicinal dilatation with further lowering through it of concretions up to 20 mm in size from the choledochal into the duodenum.
The specified digital parameters are optimal, which has been proven by experience. At other digital values, no positive effect was achieved.
Example.
Patient A., 59 years old, was hospitalized on September 14, 2019 with a diagnosis of "Chronic calculous cholecystitis, choledocholithiasis, obstructive jaundice." For 5 years, he was troubled by attacks of pain in the right hypochondrium. An ultrasound scan revealed gallstones in the gall bladder. She refused surgical treatment. Two days ago, after another attack of pain, jaundice appeared. An ultrasound scan revealed calculi in the gallbladder and three stones measuring 1.1x1.9 cm in the dilated common bile duct. After a comprehensive examination, on September 16, 2019, an operation was performed according to the proposed method - laparoscopic cholecystectomy, antegrade balloon and retrograde medical dilation of the large papilla of the duodenum and washing out of calculi from the common bile duct into the intestine.
During control intraoperative cholangiography, there are no calculi in the choledochus, the patency in the intestine is free. The stump of the duct is clipped. Laparoscopic wounds are sutured. Aseptic dressing. There were no complications in the postoperative period. Recovery.
Thus, the advantages of the proposed method of laparoscopic balloon dilatation of the large papilla of the duodenum and lithoextraction from the common bile duct are: preservation of the autonomy of the biliary tree; absence of complications that occur during endoscopic retrograde papillosphincterotomy (bleeding, acute pancreatitis, etc., the possibility of removing calculi with a diameter of up to 2.0 cm.
The proposed method is available for use in surgical hepatology departments. Approbation of the method in clinic conditions confirmed its high efficiency and reliability.
