Description of the invention
The useful model relates to medicine, namely to surgical dentistry and maxillofacial surgery and 2 Can be used as an element of surgical intervention in the surgical treatment of diseases of the temporomandibular joint.
Performing surgical interventions in the area of the temporomandibular joint is associated with difficulties caused by its topographical anatomy. The joint is located in close proximity to important anatomical formations - the branches of the facial and trigeminal nerves, large vessels, the parotid salivary gland, the organ of hearing, the base of the skull, etc. The success of the surgical intervention as a whole depends on the adequacy of the surgical access, which should provide good visualization of the area of surgical intervention, the optimal "angle of attack" when performing surgical manipulations, the formation of an aesthetically acceptable scar, and at the same time create a minimal risk of injury to important anatomical formations located near the joint.
Existing methods of surgical access to the temporomandibular joint do not fully satisfy these 79 requirements.
Thus, there is a well-known method of surgical access to the temporomandibular joint according to A.E. Rauer, according to which an angular incision of the skin, subcutaneous tissue and superficial fascia of the parotid-masticatory area is performed, which is started by retreating forward from the external auditory canal by 1.5 cm. carried out parallel to the zygomatic arch to the auricle and further down parallel to the outer edge of the branch of the lower jaw. Next, the underlying soft tissues are layered with a hemostatic clamp or separating scissors, avoiding trauma to the branches of the facial nerve, and access to the temporomandibular joint from the external GI side is provided.
-оThe disadvantage of the described method is a significant risk of injury to the branches of the facial nerve, poor visualization and the difficulty of manipulations in the area of the lateral and medial pterygoid muscles, and the presence of a visible scar 29 on the face. -at
Згідно з цим способом на волосистій частині скроні проводять дугоподібний розріз шкіри і підшкірної клітковини довжиною 4см, розсікають поверхневий листок скроневої фасції по поверхні скроневого м'язу, і о 30 відшаровують його допереду, забезпечуючи візуалізацію виличної дуги і верхніх відділів капсули «3 скронево-нижньощелепного суглоба |21.The closest analogue (prototype) of the proposed useful model is the method of surgical access to the zygomatic arch according to (N.O. Shev), which is also used when performing operations on the temporomandibular joint. According to this method, an arcuate incision is made on the hairy part of the temple skin and subcutaneous tissue with a length of 4 cm, dissect the surface sheet of the temporal fascia along the surface of the temporal muscle, and peel it forward at 30, providing visualization of the zygomatic arch and the upper parts of the capsule "3 of the temporomandibular joint |21.
--The disadvantages of the method are poor visualization of the elements of the temporomandibular joint and the difficulty of performing surgical manipulations during typical surgical interventions used in the treatment of joint diseases. This increases the risk of injury to important anatomical formations, in particular large vessels and nerve branches. --
The useful model is based on the task of providing better access to the elements of the temporomandibular joint, articular sprout, lateral and medial pterygoid muscles in order to achieve good visualization, sufficient convenience of surgical manipulations on the specified anatomical formations, while simultaneously reducing the risk of intraoperative complications associated with with injury of the branches of the 70th facial nerve. Technical result - simplification of surgical manipulations during surgical interventions in the area of the temporomandibular joint, reduction of the duration of the operation and reduction of the frequency of intraoperative complications.
The task is solved by the fact that in the known method of surgical access to the 45th temporomandibular joint, which includes an arcuate incision of the skin and subcutaneous tissue - on the hairy part of the temple, dissection of the superficial sheet of the temporal fascia on the surface of the temporal muscle and av | its detachment before visualization of the zygomatic arch, according to a useful model, after detachment of the temporalis muscle, two vertical osteotomies of the zygomatic arch are performed in its anterior and posterior parts, and the formed fragment of the zygomatic arch is shifted downwards on the fibers of the masticatory muscle, and upon completion of the main o 20 stage of surgical intervention, return to its place and fix in this position.
A distinctive feature of the proposed method of surgical access is its improvement due to the fragmentation of the zygomatic arch through its osteotomy and the displacement of the formed fragment downwards on the fibers of the masticatory muscle, which eliminates anatomical obstacles on the way to the anterior parts of the joint and the pterygoid muscles located in the subtemporal fossa. Due to this, good visualization and simplification of subsequent surgical manipulations are achieved. At the same time, such access allows bypassing the branches of the facial nerve, which are located more superficially in relation to the area of surgical manipulation. According to known literary data, such a method is unknown.
The essence of the useful model is presented graphically on a schematic figure. The patient's temporal area is shaved and antiseptic treatment of the skin is carried out. An arcuate incision of 1 60 skin, subcutaneous tissue is performed on the hairy part of the temple. The incision is started from the bridge of the ear and continues upwards, its length is determined individually taking into account the scope of the planned surgical intervention and the features of the anatomical structure of the patient's face. An incision is made in the surface sheet of the temporal fascia along the surface of the temporalis muscle.
Exfoliation of tissues in front is performed under the superficial sheet of fascia in the interfascial space, thus avoiding injury to the temporal branch of the facial nerve. Visualization of the zygomatic arch is provided. Two vertical osteotomies of the 2 zygomatic arches in its front and back sections are performed with a thin bur or a circular cutter, having previously created holes for the oblique fixator. Fragment C of the zygomatic arch is shifted downwards on the fibers of the masticatory muscle 4. At the same time, a good visualization of the joint 5, medial and lateral pterygoid muscles, located in the subtemporal fossa, is provided. They provide hemostasis during the operation. Perform the main stage of surgical intervention. At the end of the surgical intervention, the fragment of the zygomatic arch Z is lifted up, placed in its place, focusing on the pre-made holes, and fixed in the correct position with oblique fixators. The wound is sutured in layers.
