Abstract
Description
The invention relates to medicine, namely to maxillofacial surgery, and can be used to eliminate clefts of the alveolar process of the upper jaw in patients with congenital cleft lip and palate.
Patients with congenital clefts of the alveolar process of the upper jaw have a defect in the bone tissue of the alveolar process.
To restore the integrity of the alveolar process of the upper jaw and prevent deformation of the upper dentition, such patients are shown bone grafting of the alveolar process.
There is a method of eliminating a cleft of the alveolar process of the upper jaw, including a bordering incision of the scar-modified mucosa in the area of the edges of the cleft and closing the nasal anastomosis by suturing the mucosa, with further peeling and mobilization of soft tissues and mucous flaps from the vestibular surface to the base of the pear-shaped alveolar opening of the pear-shaped alveolar opening .
Mucous flaps are sutured, forming the posterior and upper walls of the bed for placement of the autograft. The lower wall is formed by stitching mobilized mucous flaps from the palatine side. Then the bed is lined with a biodegradable Bio-Gide membrane, a smooth surface to soft tissues, rough to a bone autograft, taken from the iliac crest in the form of a spongy substance or bone block of a cortical plate.
The edges of the cleft of the alveolar process are refreshed with a chisel or boron.
[Патент РФ №2594945, А61В 17/56, 2015].A bone autograft is placed in the formed bed, which is covered with a Bio-Gide membrane on top, with a rough surface to the bone, overlapping the edge of the cleft by 1-2 mm, with continuation to the lower wall of the bed. The membrane is laid so that under it there is a small space for the implementation of directional regeneration. Mucous flaps are laid on top, the wound is sutured with U-shaped or single interrupted sutures. [RF patent No. 2594945, A61B 17/56, 2015].
The disadvantages of this method are the discrepancy of sutures in the postoperative period due to the strong tension of the mucous flaps with exposure of the bone structure and the rapid pathological resorption of the autograft in the area of the cleft alveolar process due to the lack of blood supply to the bone tissue.
Closest to the proposed method is a method of eliminating a cleft of the alveolar process of the upper jaw, in which a fringing incision of the scar-modified mucosa is performed in the region of the edges of the cleft of the alveolar process, closing the mouth cavity by suturing the mucosa, exfoliating and mobilizing mucous flaps from the vestibular side of the maxilla , skeletalization of the alveolar ridge from the vestibular surface of the upper jaw to the pear-shaped opening.
[J.C. Posnick, Principles and practice orthognathic surgery. - Published by Saunders, an imprint of Elsevier Inc., 2014, P. 1058].Then, the upper jaw osteotomy is performed according to Le Fort I. The segments of the upper jaw obtained during cuts are brought together in the cleft region and osteosynthesis is performed using miniplates and screws [J.C. Posnick, Principles and practice orthognathic surgery. - Published by Saunders, an imprint of Elsevier Inc., 2014, P. 1058].
The disadvantages of this method are the low functional and aesthetic effects due to excessive narrowing of the upper jaw after the convergence of its segments, which leads to even greater deformation of the upper jaw and dentition.
There is also a high probability of aseptic necrosis of the bone of the alveolar process as a result of circulatory disorders in the area of exfoliated mucous flaps and the occurrence of a through defect, which complicates the rehabilitation of patients in the postoperative period.
The technical result of the proposed method is to increase the functional and aesthetic results due to the full blood supply to the revascularized femoral periosteal-cortical autograft.
The technical result is achieved by the fact that in the method of eliminating the cleft of the alveolar process of the upper jaw by conducting a bordering incision of the scar-modified mucosa in the region of the cleft edges, closing the nasal anastomosis by suturing the mucosa, exfoliating and mobilizing the mucous flaps along the entire perimeter of the cleft, skeletizing the alveolar from the crest of the alveolar openings and closures of the cleft with a bone autograft, a distinctive feature is that they carry out the allocation of facial arteries and veins by intraoral access from the same side of the mucosa of the cheek, and as a bone autograft use a revascularized femoral periosteal-cortical autograft taken from the anteromedial surface of the epiphysis of the femur 2-3 cm long, 1.5-2 cm wide, 0.5 thick -1 cm with the periosteum, the size exceeding 1.5-2 times the length and width of the bone part of the autograft on the vascular pedicle, including the descending knee artery and vein, extending from the superficial femoral arteries and veins, roti ovannyh and conducted through the tunnel under the mucous membrane to the same side cheek cleft to the vestibular surface of the upper jaw and anastomosed over facial artery and vein.
1-3):The method is as follows (Fig. 1-3):
Based on multispiral computed tomography (MSCT) of the bones of the facial skeleton, the sizes of the bone cleft (1) of the alveolar ridge of the upper jaw are estimated and intraoperative stereolithographic templates for the collection and modeling of the autograft are made.
Prepare a receptive bed. After the bordering incision (2) of the scar-modified mucosa along the edges of the cleft (1) of the alveolar process of the upper jaw, it is sutured to close the rotonosal anastomosis with inverted sutures. Peeling and mobilization of mucous flaps (3) from the vestibular and palatine sides along the entire perimeter of the cleft (1) of the alveolar process is carried out, the front surface (4) of the upper jaw is skeletonized to the pear-shaped opening (5). Assess the defect.
(6).Then, the isolation and preparation of recipient vessels (facial artery and vein) is performed (6).
On the same side, a vertical incision is made in the area of the mucosa of the cheek (7), the buccal muscle is bluntly exfoliated, visualized, isolated and taken on the holders of the facial artery and vein. A tunnel (8) is formed under the mucosa of the cheek from the selected vessels in the direction of the cleft of the alveolar process (1) along the vestibular surface of the upper jaw. After preparing the receiving bed and isolating the vessels, the femoral periosteal-cortical autograft is sampled (9) along the access line in the lower third of the thigh.
From the anteromedial pineal gland (10) of the femur, a L-shaped incision (11) of the skin, subcutaneous fat (PUF) is 10-12 cm long along the projection line of the femoral artery from the medial gland of the femur up. Dissect the skin, subcutaneous fat (PUFA), superficial fascia of the thigh without damage to the medial cutaneous nerve and large saphenous vein. Mobilize the inner edge of the medial abdomen of the quadriceps femoris and extend along it to the bone. A vascular pedicle (12) is isolated, including the descending knee artery and vein, departing from the superficial femoral arteries and veins, which are taken to the holder.
The periosteum is dissected along the surface of the femur, 1-2 cm apart at the edges of the planned autograft. Using oscillating and reciprocal saws, osteotomy lines are drawn along the anteromedial surface of the pineal gland (10) of the femur in accordance with an intraoperative template 2-3 cm long, 1.5-2 cm wide, 0.5-1 cm thick. Bandaging of the vascular pedicle (12) in the proximal region is performed. The vascular leg (12) of the autograft (9) is cut off.
Adapt and fix the femoral periosteal-cortical autograft (9) in the area of the cleft (1) of the alveolar process using microscrews and / or miniplates. The proposed method has several advantages, such as:
1. The possibility of eliminating the cleft of the alveolar process of the upper jaw using a revascularized femoral periosteal-cortical autograft, which has independent nutrition, which is independent of the blood supply to the receiving bed and cicatricial changes in the oral mucosa.
2. The absence of pathological resorption of the autograft due to the presence of a blood supply periosteum with high osteotropic ability in its composition allows its volume to be preserved, which creates the conditions for subsequent dental implantation in the area of the transplanted autograft.
3. Elimination of the cleft of the alveolar process of the upper jaw by the femoral periosteal-cortical autograft to the entire height of the bone cleft from the alveolar process to the edge of the pear-shaped opening allows stabilization of the segments of the upper jaw in the cleft and creates bone support for supporting the nose wing on the cleft side, which leads to cleft aesthetic results after subsequent corrective operations.
