Description
本發明關於用於輸送藥物給人或動物的藥物傳輸裝置,更特定言之係關於具有一可替換藥筒的電子控制自動注射器。此等裝置常被糖尿病患者用來投送胰島素。
藥物傳輸裝置常被未經正式醫學訓練的人(亦即患者)使用,這些人的自我健康管理日益常見。這些情況對於此類藥物傳輸裝置設下許多要求。注射器應當構造堅固,但就其供使用者之操作性及部件操控性方面來說應易於使用。在罹患糖尿病之人的情況,許多人可能視力受損且可能身體也柔弱。因此太大或笨重的裝置可能難以使用,對於動作靈敏度減低的人來說特別是如此。
美國專利說明書第5,989,221號敘述一種電子控制注射裝置,其中該裝置之投藥準備工作及後續自其輸送藥物皆由一電子控制單元控制。特定言之,該控制單元包括一用於發出一信號的位置或姿勢感測器,使得該裝置除非其注射藥筒之縱軸處於一預定方向否則無法進行準備工作。此係基於去除空氣或混合藥物組份的前提。該控制單元亦可操作驅動一活塞桿一預定距離以輸送一劑藥物。
在此等習知裝置中,使用者在安裝好藥筒之後仍然必須進行裝置之發射準備工作。此外,此等裝置可能給藥不足,特別是在使用者將藥筒插入之後藥筒並未正確地安置在裝置內的情況。
本發明之一目標是提出一種讓使用者易於進行藥筒之更換的藥物傳輸裝置。
依據本發明,提出一種用於輸送一藥物給一患者的藥物傳輸裝置,其中該藥物傳輸裝置包括:一殼體;一在該殼體內用於接收一藥筒的支架;一用於驅動該藥筒之一塞子的活塞桿;一驅動機構,其包含一用於提供一輸出驅動作用給該活塞桿以輸送該藥物的電動機;及用於控制該裝置之操作的控制構件;該裝置之特徵為:提供一用於感測該活塞桿何時與該塞子接觸的塞子感測器;其中該控制構件可操作以促進該活塞桿之驅動朝向該塞子。
在一較佳實施例中,該控制構件可操作促進該活塞桿之驅動直到該塞子感測器感測到該活塞桿與該塞子接觸為止。可提供一閂鎖感測器以在該支架從一開啟位置移至一關閉位置時感測該支架之位置。在此情況中,該控制構件可操作為僅在該閂鎖感測器感測到該支架係處於該關閉位置時促進該活塞桿之驅動。
此外,可提供一藥筒感測器以感測一藥筒在該支架內之存在。在此實施例中,該控制構件可操作為在該藥筒感測器感測到該藥筒之存在且該閂鎖感測器感測到該支架係處於一關閉位置時促進該活塞桿之驅動。
該控制構件可操作為在下列條件下停止該活塞桿之進一步推進:a)當該塞子感測器感測到該活塞桿與該塞子接觸之時;或b)在該活塞桿與該塞子之間初始接觸之後該藥筒已移至該殼體內之一安頓位置之時。在此情況中,作動該塞子感測器所需要的力大於該藥筒與該支架之間的滑動摩擦力。該支架可包含該藥物傳輸裝置之一門。該控制構件可操作為當該藥筒感測器感測到該藥筒相對於該支架處於一預定(亦即“正確”或“安頓”)位置時促進該活塞桿驅動以輸送一劑藥物。
在一替代實施例中,該閂鎖感測器及該塞子感測器可各自產生感測器信號,該等信號經組合使得發送給該控制構件的組合信號表示當該活塞桿接觸該藥筒塞子時該藥筒係存在且/或處於正確位置。
在另一實施例中,該控制構件可操作為當該塞子感測器感測到該活塞桿與該塞子接觸之時使該活塞桿往回轉一預定量。這在該塞子感測器係呈一圓頂形開關或其他壓力開關(包含固態開關及壓力開關)之形式的案例中係有利,從而讓圓頂形開關留在一開路位置。為進行劑量設定,該控制構件驅動該活塞桿一補償此往回運動的量。
藥筒、閂鎖和塞子感測器當中任一者或全部可為開關,其向該控制構件發信各自之感測功能。
本發明之實施例的有利之處在於該藥物傳輸裝置在一藥筒存在且注射器之門/支架係關閉之時自動地將該活塞桿朝該藥筒塞子推進。這簡化使用者的操作動作數量,包括免除使用者撥選一空氣發射劑量以將該活塞桿定位為抵住該塞子的需求。電池壽命因避免在藥筒不存在時驅動該活塞桿而節約使用。由於藥筒被正確地或完全地安置在支架內,進一步的好處是在一新的或使用過的藥筒插入之後發生給藥不足的可能性降低。
依據本發明,“藥物傳輸裝置”一辭應係指一種單劑或多劑或預設劑量、拋棄式或可多次使用型裝置,其經設計用於施配一藥品之一使用者可選擇或預定劑量,較佳係用於施配多劑,該藥物例如係胰島素、生長激素、低分子量肝素、及/或以上之類似物及/或衍生物等。該裝置可為任何形狀,例如緊湊密實或為筆型。劑物傳輸作用可為透過一機械式(視情況為手動)或電動式驅動機構或是儲能式驅動機構譬如彈簧等提供。劑量選擇作用可為透過一手動機構或電子機構提供。此外,該裝置可含有經設計用以監測生理特質譬如血糖濃度等的組件。再者,該裝置可包括一針頭或可為無針頭。特定言之,“藥物傳輸裝置”一辭可係指一提供多份劑量且具有一電動驅動機構的有針頭裝置,其經設計供未經正式醫學訓練的人譬如患者使用。較佳來說,該藥物傳輸裝置為自動化類型,亦即一自動注射器。
依據本發明,“殼體”一辭較佳應係指任何外部殼體(“主殼體”、“本體”、“外殼”)或內部殼體(“嵌件”、“內體”),具有一單向軸向耦接作用以阻止指定組件之近端方向運動。殼體可被設計為促成該藥物傳輸裝置或其任一機構之安全、正確且舒適的操作。一般而言,其被設計為藉由限制對於污染物譬如液體、灰塵、泥土等的暴露而容納、固定、保護、導引、且/或接合該藥物傳輸裝置之任一內部組件(譬如驅動機構、藥筒、柱塞、活塞桿)。整體而言,殼體可為管狀或非管狀形狀的一體或多件式組件。一般而言,外部殼體係用於容納一藥筒,多份劑量的藥品可從該藥筒施配出。
依據本發明,“電動機”一辭較佳應係指任何用於驅動齒輪系統且最終驅動輸入驅動構件的電動化構件。在本發明中較佳使用步進馬達,然亦可將任何用於驅動齒輪系統或驅動構件的構件、包括機械或手動致動構件納入該裝置內。
裝置或裝置之一組件的“近端”應係指離裝置之施配端最遠的末端。
裝置或裝置之一組件的“遠端”應係指最靠近裝置之施配端的末端。
實施本發明的裝置可被有效地部署在包括可替換藥筒的可多次使用藥物傳輸裝置且亦可被部署在自動注射器裝置內。
以下參照隨附圖式以舉例方式進一步說明本發明,其中以相同參考數字標示相同元件。
在圖1中,一藥物傳輸裝置1包括一有一顯示器3的外殼2,該顯示器用於顯示與該藥物傳輸裝置之操作有關的功能資訊,包括所設劑量、藥筒中之剩餘劑數等。提供使用者介面按鈕4、5和6以允許使用者操作注射器,包括進行發射準備、設定一劑量、開啟一藥筒架及門7、以及作動所設劑量之施配。提供一有螺紋針頭附接處8讓一針頭附接以進行劑物傳輸以及隨後移除丟棄。可提供一蓋(圖中未示)套在外殼2之下部部分上以協助保護該裝置不被顆粒物和流體侵入。圖2顯示藥物傳輸裝置1,其中藥筒架及門7係處於一開啟位置以接收一替換藥筒9。
圖3更詳細地示出藥筒架及門7,藥筒9安置於其內。支架7藉由一附接件11鉸接固定於外殼2使得支架7可在一開啟位置(見圖2)與一關閉位置(見圖1)之間移動。一臂13伸入外殼2內且有一擋止15以限制門/支架7在箭頭B之方向中的外伸程度。擋止15亦可接合一閂鎖開關或感測器16,如圖6所示,該感測器係用於感測支架7何時從一開啟或一關閉位置移動。提供一致動器/操縱桿17以感測外殼2內的藥筒9之存在及支架7之關閉。圖4係一沿圖3之線A-A取得的剖面圖且示出藥筒9及致動器/操縱桿17。在此實施例中,操縱桿17構成具有一可移位接觸件21之一接觸式開關19的一部分。此可移位接觸件21在藥筒收納至支架7內時被移動以改變開關19之狀態。操縱桿17、接觸式開關19及可移位接觸件21共同構成一藥筒感測器。
圖5係一伸縮活塞桿總成23的透視圖,該總成包括在一殼體29內伸縮的可滑移組件25和27。總成23在其接觸藥筒9之一塞子20的末端處有一圓頂形開關31。圓頂形開關31連接至一在組件25和27內延伸的可撓PCB元件33。元件33自其另一端冒出,穿過殼體29以終結於一接觸件35。接觸件35耦接至一控制構件(圖中未示),該控制構件可操作以控制該裝置,包含該活塞桿之驅動。
圖6係一圖2藥物傳輸裝置的透明圖且示出該裝置內之閂鎖開關或感測器16及擋止15之大致位置。儘管圖中所示係一接觸式開關,可理解到該閂鎖開關或感測器亦可為此技藝中已知之一機械式閂鎖或任何其他開關或感測器。
藥物傳輸裝置1之操作如下。提供閂鎖感測器16以感測支架7何時從一開啟或一關閉位置移動,且塞子感測器31可操作為感測活塞桿25、27何時與塞子20發生接觸。該控制構件可操作為一旦閂鎖感測器16感測到支架7已被移至一關閉位置就使活塞桿25、27之驅動件朝塞子20推進。該控制構件可操作促進活塞桿25、27之驅動直到塞子感測器31感測到該活塞桿與塞子20接觸為止。
提供一藥筒感測器17、19、21以感測支架7內一藥筒9之存在。在此情況中,該控制構件可操作為當藥筒感測器17、19、21感測到藥筒之存在且閂鎖感測器16感測到支架7已從一開啟位置移至一關閉位置時促進活塞桿25、27之驅動。
亦可能有利的是藥筒感測器17、19、21僅在閂鎖感測器16偵測到支架7已被開啟或關閉之後才運作。當該支架不處於一關閉位置時,沒必要監測該藥筒感測器。這可延長裝置之電池壽命。
在此情況中,為將一藥筒9裝入裝置1內,可將支架7(無藥筒)開啟至圖2所示位置。支架7自該關閉位置至該開啟位置的移動係由閂鎖感測器16偵測。來自閂鎖感測器16的信號促使該控制構件察看藥筒感測器17、19、21以判斷藥筒9是否存在。在本例中,藥筒不存在,故未偵測到信號。因此,該控制構件不會促進活塞桿25、27之驅動。這也有助於電池壽命。
然後可將一藥筒9放入支架7並關閉該支架。支架7一旦關閉,閂鎖開關16偵測到該支架之移動,促使該控制構件察看藥筒感測器17、19、21。由於一藥筒9出現在支架7內且該支架被關閉,可移位接觸件21與開關19接觸,表示有一藥筒存在。由於閂鎖感測器16及藥筒感測器17、19、21二者均已感測到支架7關閉且藥筒9存在,該控制構件可操作促進活塞桿25、27之驅動。
該控制構件可操作為在下列條件下停止活塞桿25、27之進一步推進:a)當塞子感測器31感測到該活塞桿與塞子20接觸之時;或b)在該活塞桿與該塞子之間初始接觸之後藥筒9已移至該支架內之一安頓位置之時(見圖3)。在此情況中,作動塞子感測器31所需要的力大於藥筒9與支架7之間的滑動摩擦力。支架7可包含藥物傳輸裝置1之一門。該控制構件可操作為當藥筒感測器17、19、21感測到藥筒9相對於該支架處於一預定(亦即“正確”或“安頓”)位置時促進該活塞桿驅動以輸送一劑藥物。

Claims (6)
- 一種用於輸送一藥物給一患者的藥物傳輸裝置,其中該藥物傳輸裝置包括:一殼體;一在該殼體內用於接收一藥筒的支架;一用於驅動該藥筒之一塞子的活塞桿;一驅動機構,其包含一用於提供一輸出驅動作用給該活塞桿以輸送該藥物的電動機;及用於控制該裝置之操作的控制構件;該裝置之特徵為:提供一用於感測該活塞桿何時與該塞子接觸的塞子感測器;其中該控制構件可操作促進該活塞桿之驅動朝向該塞子;其包括一用於在該支架從一開啟位置移至一關閉位置時或從一關閉位置移至一開啟位置時感測該支架之位置的閂鎖感測器;其中該控制構件可操作為當該閂鎖感測器感測到該支架係處於一預定位置或一關閉位置時促進該活塞桿之驅動;其包括一用於感測一藥筒在該支架內之存在的藥筒感測器;及其中該控制構件可操作為當該藥筒感測器感測到該藥筒之存在且該閂鎖感測器感測到該支架係處於一關閉位置時促進該活塞桿之驅動。
