Description
本發明關於一種醫藥,且尤其攸關一種鼻咽癌治療的醫藥。
依據我國行政院衛生福利部的統計資料,癌症(亦稱「惡性腫瘤」)至民國102年為止已連續蟬聯國人十大死因榜首三十一年。目前,癌症的臨床治療方式有手術切除、化學治療、標靶治療、或放射治療。
本發明之目的在於提供一種新穎的藥劑,其可使用於鼻咽癌的治療,以期賦予治療多樣化的選擇。
是以,本發明之第一方面,提出一種白血病抑制因子抑制劑用於製備鼻咽癌治療之醫藥的用途。
本發明之第二方面,提出一種白血病抑制因子接受器抑制劑用於製備鼻咽癌治療之醫藥的用途。

為讓本文更能明顯易懂,特別針對本文使用的術語予以定義,如下:
用語「白血病抑制因子(leukemia inhibitory factor,LIF)」,是指一屬於介白素-6(interleukin-6,IL-6)家族的細胞素,其可藉由抑制細胞分化來調控細胞生長。
用語「白血病抑制因子接受器(leukemia inhibitory factor receptor,LIFR)」,又稱CD118(cluster of differentiation 118),是指一可與白血病抑制因子結合的穿膜蛋白,這種結合可活化一連串膜內的訊號傳遞(signal transduction)。
用語「抑制劑」,是指一可抑制一特定因子活性的物質。因此,文中「白血病抑制因子抑制劑」可抑制白血病抑制因子的活性,文中
「白血病抑制因子接受器抑制劑」可抑制白血病抑制因子接受器的活性。
用語「可溶性接受器」,是指一非穿膜接受器,其可與一特定因子結合。因此,文中「白血病抑制因子可溶性接受器(soluble leukemia inhibitory factor receptor,sLIFR)」可與白血病抑制因子結合。
用語「抗體」,是指一由二條H鏈與二條L鏈以雙硫鍵鍵結構成的Y型蛋白,其可辨識並中和一特定抗原。因此,文中「白血病抑制因子抗體」可辨識並中和白血病抑制因子,文中「白血病抑制因子接受器抗體」可辨識並中和白血病抑制因子接受器。
用語「siRNA」,是指一長度約為20-25個核苷酸的雙股RNA,其可特異地干擾一特定基因的表現。因此,文中「白血病抑制因子siRNA」可特異地干擾白血病抑制因子基因的表現,文中「白血病抑制因子接受器siRNA」可特異地干擾白血病抑制因子接受器基因的表現。
用語「拮抗劑」,是指一可與一特定因子競爭的物質。因此,文中「白血病抑制因子拮抗劑」可與白血病抑制因子競爭。
根據本發明,本發明人意外地發現:白血病抑制因子可溶性接受器可抑制鼻咽癌細胞的生長。於是,本發明人認為只要能阻斷鼻咽癌細胞之白血病抑制因子接受器與白血病抑制因子的結合,即可抑制鼻咽癌細胞的生長。
本發明之第一實施方式提出一種白血病抑制因子抑制劑的用途,此用途為用於製備鼻咽癌治療的醫藥。本實施方式的醫藥可投予至一有治療需求的個體。於個體內,醫藥可透過抑制白血病抑制因子的活性來阻止個體之腫瘤的白血病抑制因子接受器與白血病抑制因子結合,這麼
一來,可達到抑制個體腫瘤的療效。
於本實施方式中,白血病抑制因子抑制劑可以為但不限於,白血病抑制因子可溶性接受器、白血病抑制因子抗體、白血病抑制因子siRNA、或白血病抑制因子拮抗劑,而較佳地,為白血病抑制因子可溶性接受器。
須指出的是,本實施方式的醫藥可視需要地呈現不同的劑型,像是錠劑、膠囊劑、顆粒劑、散劑、流浸膏劑、溶液劑、糖漿劑、懸液劑、乳劑、酊劑、或靜脈注射液。而且,本實施方式的醫藥可搭配放射治療來提升療效;舉例而言,於個體亦有放射治療的需求時,可於放射治療前、時、或後投予至個體。一般而言,可由醫師對個體的實際情況配合放射治療的療程來決定醫藥的投予劑量、時機、或途徑、以及放射線的使用劑量、或時間。另外,醫藥可經血管內、椎管內、肌肉內、皮下、腹膜內、口服、直腸、陰道、鼻部、或腫瘤之途逕投予至個體。
本發明之第二實施方式提出一種白血病抑制因子接受器抑制劑的用途,此用途為用於製備鼻咽癌治療的醫藥。本實施方式的醫藥可投予至一有治療需求的個體。於個體內,醫藥可透過抑制個體之腫瘤的白血病抑制因子接受器的活性來阻止白血病抑制因子接受器與白血病抑制因子結合,如此一來,可達到抑制個體腫瘤的療效。
於本實施方式中,白血病抑制因子接受器抑制劑可以為但不限於,白血病抑制因子接受器抗體、或白血病抑制因子接受器siRNA。
須指出的是,本實施方式的醫藥可視需要地呈現不同的劑型,例如錠劑、膠囊劑、顆粒劑、散劑、流浸膏劑、溶液劑、糖漿劑、懸
液劑、乳劑、酊劑、或靜脈注射液。而且,本實施方式的醫藥可搭配放射治療來提升療效;舉例而言,於個體也有放射治療的需求時,可於放射治療前、時、或後投予至個體。一般而言,可經醫師對個體的實際情況搭配放射治療的療程來決定醫藥的投予劑量、時機、或途徑、與放射線的使用劑量、或時間。再者,醫藥可經血管內、椎管內、肌肉內、皮下、腹膜內、口服、直腸、陰道、鼻部、或腫瘤之途逕投予至個體。
茲以下述實例,予以詳細說明本發明上述的實施方式。
《實例1:鼻咽癌病患放射治療前之血清LIF濃度與疾病之復發或緩解的關聯》
首先,取得132位鼻咽癌病患放射治療前的血清。接著,所有病患完整地接受放射治療,治療使用的放射線劑量為6840-7600cGy/7-8週。於治療後的追蹤期內,診斷全部病患的疾病為復發(recurrence)、或為緩解(remission),其中「復發」定義為治療時疾病症狀已痊癒,但於追蹤期內症狀又出現;而「緩解」定義為治療時疾病症狀已痊癒,且於追蹤期內未發現症狀。請參閱第1A圖,91位病患的疾病診斷為緩解,剩餘41位病患的疾病診斷為復發。且,疾病緩解之病患放射治療前的血清LIF濃度大多為低於疾病復發之病患放射治療前的血清LIF濃度。請參閱第1B圖,將這132個病患放射治療前的血清LIF濃度與診斷結果作成一接受者操作特徵曲線(receiver operating characteristic curve,ROC),而當血清LIF濃度為4.96pg/ml時,可區分疾病的復發或緩解。
另外,將這132個病患分成二群組分別為:高表現群、及低表現群,而高表現群病患之放射治療前的血清LIF濃度不低於4.96pg/ml,低
表現群病患之放射治療前的血清LIF濃度低於4.96pg/ml。請參閱第1C及1D圖,相較於低表現群病患,高表現群病患具有較為不佳的無復發存活期(recurrence-free survival)與無惡化存活期(progression-free survival)。
依據本實例,鼻咽癌病患放射治療前的血清LIF濃度可作為預測鼻咽癌之復發或緩解的基準。
《實例2:鼻咽癌病患之鼻咽腫瘤組織與鼻咽正常組織的LIF和LIFR表現》
首先,利用免疫組織化學染色法(immunohistochemistry,IHC)對鼻咽癌病患的鼻咽腫瘤組織與鼻咽正常組織染色。如第2A及2B圖所示,腫瘤組織的LIF表現量明顯高於正常組織的LIF表現量,且腫瘤組織的LIFR表現量亦明顯高於正常組織的LIFR表現量。
為進一步驗證上述結果,採用西方點墨法(Western blotting)。同樣地,腫瘤組織的LIF表現量高於正常組織的LIF表現量,且腫瘤組織的LIFR表現量也高於正常組織的LIFR表現量(如第2C圖)。
綜合本實例,鼻咽癌病患之腫瘤組織的微環境(microenvironment)表現有大量的LIF與LIFR。
《實例3:LIF對鼻咽癌細胞於活體內(in vivo)或於活體外(in vitro)之生長的影響》
首先,種植6×103個TW06細胞,並於37℃中培養隔夜後,投予PBS、sLIFR(1μg/ml)、LIF(10ng/ml)、或LIF與sLIFR至細胞。最後,於種植後的特定時間點,使用xCelligence即時細胞分析儀(Roche Applied Science)測定細胞的生長。請參見第3A圖,單獨投予LIF之細胞的生長為最
佳;反觀,單獨投予sLIFR之細胞的生長為最差。此外,同時投予LIF及sLIFR之細胞的生長仍較單獨投予PBS之細胞的生長為差。
另外,將有螢火蟲螢光素酶2(firefly luciferase 2)報導基因的轉染TW06_Luc2細胞經皮下地注射至七週大的NOD/SCID(non-obese diabetic/severe-combined immunodeficient)小鼠大腿。待小鼠大腿的腫瘤成長至50mm3的體積時,經腫瘤地注射PBS、LIF(150-200ng,每週二次,共四週)、或sLIFR(1-2μg,每週二次,共四週)至小鼠。最後,於注射後的特定時間點,觀察小鼠的腫瘤並利用活體分子影像系統(in vivo imaging system,IVIS)測量腫瘤的活性及體積大小。請參見第3B及3C圖,注射LIF之小鼠的腫瘤體積為最大;相對地,注射sLIFR之小鼠的腫瘤體積為最小。
依照本實例,LIF能提升鼻咽癌細胞於活體內或於活體外的生長;然而,sLIFR能抑制鼻咽癌細胞於活體內或於活體外的生長。
《實例4:LIF與鼻咽癌細胞於活體外受放射線照射後之生長的關聯》
首先,種植6×103個CNE1細胞,並於37℃中培養隔夜。接著,先投予PBS、或LIF(10ng/ml)至細胞,再照射細胞不同劑量的放射線。最後,於照射後的特定時間點,利用xCelligence即時細胞分析儀來測定細胞的生長。請參照第4A圖,投予LIF搭配照射放射線之細胞的生長確實較投予PBS搭配照射放射線之細胞的生長為佳。且,如第4B圖,於照射後第48小時,搭配投予PBS之放射線的IC50值為1.5Gy,而搭配投予LIF之放射線的IC50值為3.7Gy;另,於照射後第55小時,搭配投予PBS之放射線的IC50值為1.3Gy,而搭配投予LIF之放射線的IC50值為5.8Gy。
另外,種植6×103個CNE1細胞或TW06細胞,並於37℃中培養隔夜。之後,先投予PBS、sLIFR(1μg/ml)、LIF(10ng/ml)、或LIF及sLIFR至細胞,再照射細胞4Gy的放射線。最後,於照射後的特定時間點,利用xCelligence即時細胞分析儀來測定細胞的存活。請參見第4C圖,投予LIF搭配照射放射線之細胞的生長為最佳,而投予sLIFR搭配照射放射線之細胞的生長為最差。另外,同時投予LIF與sLIFR搭配照射放射線之細胞的生長仍較投予PBS搭配照射放射線之細胞的生長為差。
再者,如第4D圖所示,於照射後第96小時,與投予sLIFR搭配照射放射線的細胞比較,投予LIF搭配照射放射線的細胞含有相對低的Caspase3/7表現量。又如第4E及4F圖所示,於照射後第10分鐘,投予sLIFR搭配照射放射線的細胞比投予LIF搭配照射放射線的細胞強烈地活化(磷酸化)細胞死亡相關的蛋白及與DNA損傷反應(DNA damage response)有關的蛋白(ATM、p53、γH2AX、NBS-1、CDC25C)。
根據本實例,LIF可透過抑制細胞死亡的途徑來降低鼻咽癌細胞受放射線照射造成的死亡;而,sLIFR可透過促進細胞死亡的途徑來增加鼻咽癌細胞受放射線照射造成的死亡。
《實例5:LIF與鼻咽腫瘤於活體內受放射線照射後之生長的關聯》
將有螢火蟲螢光素酶2報導基因的轉染TW06_Luc2細胞經皮下地注射至七週大的NOD/SCID小鼠大腿。當小鼠大腿的腫瘤成長至60-100mm3的體積時,經腫瘤地注射PBS、LIF(150-200ng,每週二次,共四週)、或sLIFR(1-2μg,每週二次,共四週),並於第一次注射後照射小
鼠7Gy的放射線。最後,於照射後的特定時間點,觀察小鼠的腫瘤並利用活體分子影像系統測量其活性及體積大小。請參看第5A及5B圖,注射LIF搭配照射放射線之小鼠的腫瘤體積為最大,而注射sLIFR搭配照射放射線之小鼠的腫瘤體積為最小。
為進一步地確認上述結果,各別將有螢火蟲螢光素酶2報導基因與sLIFR基因的轉染TW06_Luc2_sLIFR細胞、及有螢火蟲螢光素酶2報導基因與綠色螢光蛋白(green fluorescent protein,GFP)基因的轉染TW06_Luc2_GFP細胞經皮下地注射至七週大的NOD/SCID小鼠大腿。直到小鼠的腫瘤成長至60-100mm3的體積時,照射小鼠7Gy的放射線。最後,於照射後的特定時間點,觀察小鼠的腫瘤並利用活體分子影像系統測量其活性及體積大小。請參閱第5C及5D圖,相較於注射轉染TW06_Luc2_GFP細胞的小鼠,注射轉染TW06_Luc2_sLIFR細胞之小鼠的腫瘤體積相對地小。
依照本實例,LIF可降低鼻咽腫瘤於活體內受放射線造成的死亡,相反地,sLIFR可促進鼻咽腫瘤於活體內受放射線造成的死亡。
上述實例僅為說明本發明之原理及其功效,然非限制本發明。習於本技術之人士對上述實例所做的修飾與變化仍不違背本發明之精神。本發明之權利範圍應如後續之申請專利範圍所示。
Claims (11)
- 一種白血病抑制因子抑制劑用於製備鼻咽癌治療之醫藥的用途。
- 如請求項第1項所述之用途,其中該白血病抑制因子抑制劑係為白血病抑制因子可溶性接受器、白血病抑制因子抗體、白血病抑制因子siRNA、或白血病抑制因子拮抗劑。
- 如請求項第2項所述之用途,其中該白血病抑制因子抑制劑係為白血病抑制因子可溶性接受器。
- 如請求項第1項所述之用途,其中該醫藥係於放射治療前、時、或後投予至一有該鼻咽癌治療與該放射治療之需求的個體。
- 如請求項第4項所述之用途,其中該醫藥係經血管內、椎管內、肌肉內、皮下、腹膜內、口服、直腸、陰道、鼻部、或腫瘤之途逕投予至該個體。
- 如請求項第5項所述之用途,其中該醫藥係經腫瘤之途逕投予至該個體。
- 一種白血病抑制因子接受器抑制劑用於製備鼻咽癌治療之醫藥的用途。
- 如請求項第7項所述之用途,其中該白血病抑制因子接受器抑制劑係為白血病抑制因子接受器抗體、或白血病抑制因子接受器siRNA。
- 如請求項第7項所述之用途,其中該醫藥係於放射治療前、時、或後投予至一有該鼻咽癌治療與該放射治療之需求的個體。
- 如請求項第9項所述之用途,其中該醫藥係經血管內、椎管內、肌肉內、皮下、腹膜內、口服、直腸、陰道、鼻部、或腫瘤之途逕投予至 該個體。
- 如請求項第10項所述之用途,其中該醫藥係經腫瘤之途逕投予至該個體。
