Description

可撐張之椎間植入體
本發明係關於一種可撐張之椎間植入體,包括前壁、後壁,以及將前壁和後壁連接之二側壁,諸壁界定之空腔,具有開口底部和頂部,以及至少一構件,可在空腔內從第一位置(在此其表面不會突出空腔之外)和第二位置(在此其表面至少部份突出空腔之外)運動。植入體可從二側之任一側積極撐張。再者,植入體具有解剖性形狀。
椎間植入體是在椎間盤除去後插入,以穩定椎間區,直至同時充填之骨材成長至骨質連接和強化為止。
由US 6,176,882 B1已知一種可撐張之椎間植入體。椎間植入體包括二隔開側壁,在其一端連接側壁之前壁,在另一端連接側壁之後壁,諸壁界定在壁內相對應之空間、底面、頂面,各面橫向延伸至該諸壁,該諸壁所界定空間內設置之至少一結合構件,該結合構件具有朝底面或頂面之一定向的表面,以及二契形構件,利用螺軸支持在前壁和後壁內,心軸具有二端和二部份,其螺距相反,螺軸之一端係轉動自如地支持於前壁內,而他端則轉動自如地支持於後壁。二契形構件支持在空間內的方式是,螺軸在一方向轉動時,契形構件間之距離縮短,而螺軸以相反方向轉動時,契形構件間之距離加大。契形構件操作,往復推動結合構件。可撐張之椎間植入體外形呈長方形。所以,契形構件可支持在單一螺軸上。
另一種可撐張之椎間植入體,見於例如WO 2005/058209 A2。植入體包含本體,有縱向軸線,包含沿橫向軸線隔開之第一和第二軸向壁,第一和第二橫向末端壁在第一和第二軸向壁間延伸,並加以相連。椎間植入體包含撐張構件,與軸向壁共同作用,將本體沿橫向軸線撐張。植入體在平行於椎骨本體端板的平面之外形,亦呈長方形。具有長方形外形之椎間植入體,不能完美套合椎骨本體末端板之形狀。對於某些用途,需要解剖上造型之椎間植入體。
US 2005/0125062號揭示一種高度調節性椎間植入體,具有解剖性形狀。
本發明之目的,在於提供一種可撐張之椎間植入體,得以便利插入,同時具有撐張機制之改進簡便處理。
此目的係由申請專利範圍第1項之椎間植入體解決。進一步發展則列於申請專利範圍附屬項。
椎間植入體呈解剖上的造型,尤其是具有香蕉形狀或腎臟形狀。然而,由於本發明椎間植入體之撐張機制,可容易適於具有任何其他形狀,甚至更適於椎骨本體端板之自然形狀。
椎間植入體可藉從任一側進出而撐張。
可用各種聯結機制,把用來撐張植入體的一支持構件之轉動,聯結到另一支持構件。
本發明其他特點和優點,參見附圖詳述如下,即可明白,且更為清楚。
如第1-7圖所示,可撐張的椎間植入體包括前壁1,與前壁隔開之後壁2,在其一端連接前壁1和後壁2之第一側壁3,在其相反端連接前壁1和後壁2之第二側壁4。詳見第1圖,底面和頂面開啟,使四壁界定空腔5,具有開口底部和開口頂部。前壁1和後壁2長度大約相同,並按大約同樣方向彎曲。側壁3和4較短,連接前壁和後壁,其方式是由諸壁界定為實質上香蕉形或腎臟形輪廓。前、後壁以及側壁之長度和曲率,分別選擇適合中間插入植入體的椎骨本體相反對端板之形狀。前壁和後壁之高度朝壁中心稍微增加(見第21至24圖),以便適合椎骨本體端板之相對應凹部。前壁1和後壁2最好有複數通孔6,在圖示具體例內呈菱形。惟通孔形狀亦可為圓形、橢圓形,或其他形狀。
尤如第1和2圖所示,側壁3和4包括腔孔7,7',位於大約中心。由第4圖可見,腔孔7和相對應之對立腔孔7',各有第一部份8,設第一腔孔直徑,鄰接側壁之外側;並有後接之第二部份9,對空腔5開口,並設第二腔孔直徑,比第一腔孔直徑稍小。因而,第二部份9界定肩部。
軸承構件(或軸承軸頸)10,10',分別插入腔孔7,7'內。各軸承構件10,10'包含有第一外徑之第一圓筒部,和相鄰具有第二外徑之第二圓筒部,配合腔孔7,7'的第一部8和第二部9之內徑,使軸承構件置於肩部。軸承構件10,10'之尺寸,可藉壓套而把軸承構件10,10'保持在腔孔7,7'內。
植入體又包括二螺軸11,11',各有第一圓筒端12,12',在自由端有凹部13,13',以六面形凹部為佳,可與工具(圖上未示)結合。圓筒形第一端12,12'的外徑,比軸承10,10'的內徑稍小,使軸的第一端12,12'可插入軸承10,10'內,並可轉動自如保持於其內。各軸又有圓筒形第二端14,14',與第一端12,12'對立。
螺軸11,11'的螺距彼此相反。螺距的長度是,在軸保持在軸承10,10'內並突入空腔5內時,軸的第二端14,14'彼此不會相碰。
尤如第3至5圖所示,螺軸11,11'經由聯結構件15與第二端14,14'連接。在第1至7圖所示具體例內之聯結構件15,設計成彎曲管部,有開縫16,沿周緣方向延伸。開縫彼此偏離,其方式是,一沿周線之開縫蓋住相鄰沿周線至少不設開縫之部份。聯結構件15之內徑,使得聯結構件15可連接於螺軸的圓筒形端部14,14',例如利用壓套連接為之。聯結構件之曲率和長度,適合螺軸11,11'縱向軸線彼此包容之角度α。意即聯結構件15的曲率實質上相當於植入體前壁1和後壁2之曲率。選擇開縫16的數量、寬度和長度,以便對管提供所需彎曲可撓性,又有充分之扭力剛性。
由第3圖可見,圓形突緣17,17'可設在各軸11,11'的末端14,14',做為止動部,其功用詳後。突緣17,17'在兩側有凸部18,分別置於前壁1和後壁2內側所設肩部19上。突緣17,17'亦供導引和/或支持軸和聯結構件。
尤如第1圖和第3至5圖所示,各螺軸11,11'設有契形構件20,20'。各契形構件以頂面21,21'和相對立底面22,22'界定,包含彼此界定契形之角度。契形構件20,20'包括平坦前壁23,23'和彎曲後壁以及平坦側壁。平坦前壁23,23'以及頂壁21,21'和底壁22,22'之配置,使頂壁和底壁間之距離,朝背離前壁的方向而遞增。契形構件20,20'包括螺孔,有陰螺紋,分別與螺軸11,11'的陽螺紋相對應。契形構件20,20'螺合於相對應螺軸11,11'上,其狀態使相對應頂面21,21'和底面22,22'彼此相對傾斜。尤如第3圖所示,契形構件的後壁彎曲方式是,配合植入體側壁3,4之內側。
詳見第1、2和15至18圖,第一結合構件30和第二結合構件30',置於契形構件20,20'的彼此傾斜頂面21,21'和彼此傾斜底面22,22'之間,以下稱為契形面。各結合構件30,30'在朝向契形構件的下面,分別具有彼此傾斜之斜面31,32和31',32'。表面31,32和31',32'之傾角分別相當於契形構件之契形角度。結合構件30,30'在左、右斜面31,32和31',32'間,包括實質上彎曲圓筒形凹部33,33',其尺寸能夠容納包含突緣17,17'在內之聯結構件15。凹部33,33'分別延入斜面31,32和31',32'。結合構件30,30'的斜面和契形構件20,20'之契形面,可彼此滑動。為增進滑動能力,諸面可塗以塗料或磨光。
尤如第16和17圖所示,結合構件之輪廓大約香蕉形或腎臟形,並相當於第1和2圖所示植入體之本體輪廓。結合構件30的頂面34在圖示具體例中具有右部34a和左部34b,彼此相對稍微傾斜,其傾斜與斜面31,32下側的傾斜相反。第二結合構件31'之相對應頂面34'有斜部34a',34b'(圖上未示)。頂面34又有結合結構35,可設計成從表面突出之齒或肋條,或適於與椎骨本體端板表面結合之任何其他結構。
由第1、2和15、16圖可見各結合構件30,30'包括實質上開縫36,36',從頂面34,34'延伸至有一段距離。前壁1和後壁2各包括腔孔38,38',位於側壁間之中心,一在上半,一在下半。二銷39,39'延伸跨越空腔5通過對立腔孔38。銷39,39'分別與結合構件30,30'之U形開縫36,藉此形成止動部,以防結合構件30,30'脫出,U形開縫36亦形成結合構件30,30'之導件。
銷39,39'的配置和U形開縫36,36'的深度彼此相配,使各結合構件跨越底面和頂面的向外運動最大高度,是由銷39,39'的相對位置和開縫36深度決定。
契形構件20,20'、螺軸11,11'和結合構件30,30'的維度,以及螺距之設計,使得結合構件可從第21和23圖所示第一位置(此時頂面34,34'位於空腔5內),移動至第22和24圖所示第二位置(此時頂面34,34'突出植入體的空腔5之上)。
植入體係由生物相容性材料(諸如鈦)、生物相容性塑膠材料,或其他相容性材料製成。聯結構件15係例如由不銹鋼或鈦製成,以提供充分強度。
操作上,首先把契形構件20,20'帶入第3圖所示位置,藉轉動螺桿之一,使後壁接觸到植入體側壁34的內側。如此造成各結合構件30,30'到達最低位置,至此,頂面34,34'的結合結構35不會突出到植入體空腔5之外。植入體即可容易插入到二椎骨間之處,故無傷及椎骨端板軟件之虞。由於植入體的輪廓大略呈香蕉形,插入時比長方形植入體插入時方便。植入體插入方式為,前壁定向於背側方向,而後壁定向於腹側方向,而側壁定向側面。前壁、後壁和側壁之指示,不限制植入體使用於插入之特別方式,僅僅是分辨諸壁彼此相關而已。在椎骨間之植入體正確定位之後,以工具(例如結合軸的凹部13,13'之Allen扳手)從側壁3,4之一進出,結合螺軸11,11'之一,使二契形構件20,20'彼此相反運動。轉動螺軸之一,利用聯結構件15把轉動轉移至其他螺軸。在第1至7圖所示具體例中,聯結構件因其開縫16形成弱部之故,具有充分扭力剛性之屈曲能力,令一軸之轉動傳至他軸。轉動軸之一,二對立契形構件20,20'之彼此傾斜契形表面21,22和21',22',即對相對應結合構件30,30'之斜面31,32和31',32'施力,加以提升到頂面34,34'的結合構件30,30'突出於空腔5之外為止,因而結合各椎骨之端板為止。結合構件30,30'之提升運動,受到銷39,39'所形成止動件之限制,特別如第24圖所示。突緣17,17'亦可提供契形構件運動之止動件。
工具轉動透過一螺軸傳輸至其他螺軸,並從契形構件至結合構件,得以精確調節結合構件之撐張,並使植入體分別適應個別患者椎骨端板的解剖性形狀。
螺軸防止結合構件本身鬆脫。結合構件30,30'只能用工具逆向轉動而縮回,於是椎骨施於結合構件上之壓力,把結合構件逼回到植入體之空腔5內。此舉可從椎骨端板釋放結合結構。
第19和20圖表示結合構件之變通具體例。結合構件300與結合構件30,30'不同在於鄰近U形凹部36具有二對立斜面301a,301b,其傾斜是與結合構件300下側的斜面31,32之傾斜平行。斜面301a,301b無結合結構。利用此具體例之結合結構,在具有最高骨骼強度之端板外部具有支持部。
第10至12圖表示聯結機制把轉動從一軸傳送至他軸之又一改變。聯結機制可由斜齒輪驅動所形成,包括二小齒輪150,151,彼此結合。各小齒輪係非轉動式連接於軸11,11'。
聯結機制把一軸的轉動傳送至他軸之又一改變具體例,如第13和14圖所示。包括可撓性管160,其曲率和維度類似第1至7圖所示聯結構件15,而其尺寸可壓套連接於軸之末端14,14'。聯結構件160利用可撓性材料變形,以傳送轉動。聯結構件160可例如由鎳鈦合金,或具有屈曲可撓性又有扭力剛性的可撓性塑膠材料製成。
第25a至25e圖表示植入體之組裝步驟。在第25a圖所示第一步驟中,首先把上面螺合相對應契形構件之第一軸,插入空腔和相對應軸承內。次一步驟是把聯結構件15連接於第一軸。再插入第二契形構件,把第二軸導引穿過第二腔孔,並旋經第二契形構件,直到可與聯結構件的突緣連接。然後,把第二軸承插入(第25a和25b圖)。如第25c和25d圖所示,當軸包含契形構件插入,並利用聯結構件加以聯結時,即可插入結合構件,把銷39,39'插穿腔孔38,38',如第25e圖所示。
雖然所述壁為前壁、後壁和側壁,但不視為對椎骨間植入體定向之限制。植入體形狀不限於香蕉形或腎臟形。例如可呈環形、四分之一環、半環,或任何其他形狀。
另構想可設二以上的契形構件和二以上的支持構件,彼此形成角度。例如設有三個支持軸彼此呈120°和三個調節或契形構件。
亦可設想其他聯結機制,例如波形管、伸縮接頭或bowden纜線。甚至非機械性之聯結器,例如磁性聯結器,亦屬可用。

Claims (15)

  1. 一種可撐張之椎間植入體,包括:一前壁(1);一後壁(2),與該前壁隔開;和二側壁(3,4),在其各端連接該前壁(1)和該後壁(2);該諸壁界定一空間(5);一底面;一頂面;至少一結合構件(30,30'),有一結合表面(34,34'),可與椎骨本體之端板結合;一第一調節元件(20)和一第二調節元件(20'),分別以一第一支持構件(11)和一第二支持構件(11')支持,並與各該支持構件合作,使該結合構件(30,30')可在一第一末端位置(此時該結合表面(34,34')不會突出超過該底面或該頂面)和一第二末端位置(此時該結合表面(34,34')至少部份向外突出超過該底面或該頂面)之間往復運動;其中該第一和該第二支持構件(11,11')係利用一聯結機制(15;150,151;160)加以聯結;其中該第一支持構件(11)可轉動,該第二支持構件(11')可轉動,而該聯結機制(15;150,151;160)則把該第一支持構件的轉動,傳送給該第二支持構件;又其中該第一支持構件(11)之轉動軸線和該第二支持構件(11')之轉動軸線,包含角度非零者。
