Description
Изобретение относится к терапевтической стоматологии. Цель -сокращение сроков лечения и повышение качества отбеливания. Эмаль зубов, пораженную флюорозом, обрабатывают отбеливающим средством и постоянным током. Перед воздействием средством, эмаль дополнительно обрабатывают 10-20%-ной НС1 в течение 35 мин, а после воздействия средства на нее наносят 3%-ный раствор ремодента на 3040 мин, затем эмаль покрывают клеем МК-42 на 12-14 дней.
Изобретение относится к медицине, а именно, терапевтической стоматологии.
Цель изобретения - сокращение сроков лечения и повышение качества отбеливания.
Способ осуществляют следующим образом.
Больного усаживают в стоматологическое кресло с запрокинутой головой. В полость рта вводят пластмассовые губоудерживатели. На вестибулярную поверхность верхних фронтальных зубов (зубов, подлежащих отбеливанию) на 3-5 мин накладывают ватный тампон, смоченный 10-20%-ной НС1, По истечении этого времени зубы обрабатывают 2%-ным раствором двууглекислого натрия с целью нейтрализации кислоты. После ополаскивания в полость рта вводят устройство и больной зажимает его зубами.
Через иглу посредством шприца в полость устройства вводят отбеливающий раствор. Устройство подключают к аппарату ОД-2 и воздействуют на окислитель постоянным током силой 100-150 мкА при экспозиции 15-20 мин. Во время процедуры периодически меняют полярность тока. Отработанный раствор выпускают через иглу в подставленный почкообразный лоток. Не извлекая устройства из полости рта, отбеливающий раствор меняют через каждые 5-7 мин. Общее время обработки зубов продолжается 20-15 мин. После этого устройство извлекают, ополаскивают полость рта водой и на 30-40 мин в преддверие и собственно в полость рта вводят ватные валики, обильно смоченные 3%-ным раствором ремодента. Спустя 10-15 мин валики меняют на свежие. После удаления валиков зубы высушивают сжатым воздухом и покрывают медицинским клеем МК-2 на 12-14 дней.

Пример. Больной К. обратился в поликлинику. После обследования поставлен диагноз: флюороз зубов, тяжелая форма, III степень. Определена интенсивность окрашивания по полутоновой стандартной шкале плотностей. Установлено интенсивное коричневое окрашивание верхних фронтальных зубов, которое условно принято нами за 100%.
Проведено отбеливание зубов по предложенному способу. Для этого в полость рта вводят пластмассовые губоудерживатели, десну обработали вазелином. На вестибулярную поверхность верхних фронтальных зубов наносят на 5 мин тампон, смоченный 10%-ной НС к. После этого полость рта прополаскивают 2%-ным раствором двууглекислого натрия.
В полость рта вводят устройство, с помощью которого создают емкость над фронтальной группой зубов, подлежащих отбеливанию. Устройство подключают к аппарату ОД-2. В емкость, созданную над верхними фронтальными зубами, с помощью шприца вводят 33 %-ный НгОг и воздействуют на окислитель постоянным током с силой 100 мкА при экспозиции 15 мин, периодически меняя при этом полярность.
После извлечения устройства из полости рта прополаскивают рот водопроводной водой. Определяют эффект отбеливания - в местах темнокоричневого окрашивания, т.е.
при однократном использовании способа эффект составил 90%.
В полость рта вводят тампон, смоченный 3%-ным раствором ремодента на 30 мин и после этого вестибулярную поверхность фронтальных зубов покрывают медицинским клеем МК-2 и на 12-14 дней. Клей бесцветный, поэтому практически не определяется на зубах.
Осмотр больного через 2 недели и через год показал стойкий эффект отбеливания. Болевых ощущений в зубах, подлежащих отбеливанию, не отмечалось.
Предложенный способ позволяет получить стойкий результат отбеливания, срок лечения сокращается в 4-5 раз.

The purpose of the invention is to reduce the treatment time and improve the quality of whitening.
The method is as follows.
The patient is seated in a dental chair with his head thrown back. Plastic mouthpieces are introduced into the oral cavity. A cotton swab moistened with 10–20% HC1 is placed on the vestibular surface of the upper frontal teeth (teeth to be whitened) for 3-5 minutes. After this time, the teeth are treated with a 2% sodium bicarbonate solution to neutralize the acid. After rinsing, a device is inserted into the oral cavity and the patient clamps it with his teeth.
Through a needle through a syringe in the cavity of the device enter the bleaching solution. The device is connected to the device OD-2 and affect the oxidizer with a constant current of 100-150 μA with an exposure time of 15-20 minutes. During the procedure, periodically change the polarity of the current. The spent solution is discharged through a needle into a substituted kidney-shaped tray. Without removing the device from the oral cavity, the whitening solution is changed every 5-7 minutes. The total time of dental treatment lasts 20-15 minutes. After that, the device is removed, rinsed mouth with water and for 30-40 minutes on the eve and wadded rollers, which are abundantly moistened with 3% remoment solution, are introduced into the oral cavity. After 10-15 minutes, the rollers change to fresh. After removing the rollers, the teeth are dried with compressed air and covered with medical glue MK-2 for 12-14 days.

Example. Patient K. turned to the clinic. After the examination, a diagnosis was made: dental fluorosis, severe form, III degree. The intensity of staining was determined according to a semitone standard density scale. An intensive brown staining of the upper anterior teeth was established, which we conventionally assumed to be 100%.
Held teeth whitening on the proposed method. To do this, injected into the oral cavity plastic pads, gum treated with petroleum jelly. A tampon moistened with 10% HC is applied to the vestibular surface of the upper anterior teeth for 5 minutes. After this, the oral cavity is rinsed with 2% sodium bicarbonate solution.
A device is introduced into the oral cavity with the help of which a container is created above the frontal group of teeth to be whitened. The device is connected to the device OD-2. A 33% HgOg is injected into a container created above the upper frontal teeth with a syringe and acts on the oxidizer with a direct current of 100 μA at an exposure time of 15 minutes, periodically changing the polarity.
After removing the device from the oral cavity, rinse the mouth with tap water. Bleaching effect is determined - in the places of dark brown coloring, i.e.
with a single use of the method, the effect was 90%.
A tampon is injected into the oral cavity, moistened with a 3% solution of remoden for 30 minutes and after that the vestibular surface of the anterior teeth is covered with medical glue MK-2 and for 12-14 days. The glue is colorless, therefore it is practically not detected on the teeth.
Examination of the patient after 2 weeks and a year later showed a persistent bleaching effect. Pain in the teeth to be bleached, was not observed.
The proposed method allows to obtain a stable bleaching result, the duration of treatment is reduced by 4-5 times.
