С интраоперационными кровотечениями сталкивается каждый хирург при выполнении любой манипуляции. Широко известны способы временной остановки внутреннего кровотечения, к которым относятся: пальцевое прижатие сосуда в ране, наложение кровоостанавливающего зажима, временное шунтирование сосуда, тампонада раны [Гостищев В.К. Общая хирургия, 5-е издание, 2015; 121-122]. С целью мобилизации, выделения, маркировки, оттягивания и перетяжки деликатных структур, таких как вены, артерии, сухожилия, нервы, в хирургии широко используют силиконовые сосудистые турникеты разных производителей и свойств (DEV-O-LOOPS, DEVON™ Vessel Loops) [Критерии оценки эффективности применения турникета / С.А. Известен способ для остановки кровотечений при повреждениях таза (патент RU 2143235 C1, 14.04.1998) с помощью устройства, которое имеет заостренную спицу с хвостовой частью. Кровотечения останавливают путем сдавливания тканей в области кровоточащих сосудов за счет введения в кость спиц. Хвостовая часть спицы выполнена с петлей для зацепления за подобный устройству элемент, создавая цепочку. Для придавливания париетальной брюшины вместе с кровоточащими сосудами к костной ткани, средняя часть спицы изогнута под углом 90° на расстоянии не менее 15 мм от заостренного конца, который выполнен в виде рыболовного крючка. Способ осуществляют путем введения в тазовые кости в области гематомы через один кортикальный слой с помощью инструмента и молоточка. Низкая эластичность пены способна дополнительно травмировать прижимаемый сосуд. За счет силиконового валика, который надежно сдавливает сосуд, прекращается кровоток и обеспечивается остановка внутреннего кровотечения, при этом эластичность валика позволяет снизить вероятнось травматизации сосуда. Способ апробирован на 10 больных. С помощью силиконового валика, во всех случаях удавалось надежно сдавливать сосуд, обеспечивая остановку внутреннего кровотечения. Эластичное прижимание сосуда позволяет щадяще пережимать сосуд и окружающие мягкие ткани. За счет надежной остановки кровотока улучшается экспозиция операционного поля.The method was tested on 10 patients. Пример №1. Больная, N, 28 лет поступила в экстренное хирургическое отделение с диагнозом: «Язвенная болезнь двенадцатиперстной кишки. Перфорация» через 6 часов после проявления первых клинических признаков перитонита. После лапаротомии при ревизии в брюшной полости до 1200 мл кишечного содержимого с фибрином, по передней стенке луковицы двенадцатиперстной кишки, в корпоральном отделе обнаружено перфоративное отверстие диаметром до 0,9 см, из которого поступает содержимое ДПК с примесью желчи. Через перфоративное отверстие произведена поперечная дуоденотомия. При ревизии: перфоративной язвы выявлено, что язва со стороны слизистой - язвенный кратер до 1,5 см в диаметре. Зона инфильтрации вокруг язвенного кратера до 2,0 см, края плотные. Интрадуоденально при ревизии выявлено на заднебоковой стенке с переходом на большую кривизну, язвенный кратер размером до 3,0 см, глубиной до 0,4 см с переходом на боковую стенку с пенетрацией в головку поджелудочной железы и гепатодуоденальную связку. Гепатодуоденальная связка инфильтрирована, инфильтрат распространяется по большой кривизне луковицы двенадцатиперстной кишки, отечна, покрыта фибрином. В дне пенетрирующей головку поджелудочной железы - язвы; два тромбированных сосуда диаметром до 0,15 см. Учитывая большие размеры язвы, риск рецидива кровотечения, молодой возраст, стабильность состояния больной, принято решение выполнить сегментарную дуоденопластику. Начата мобилизация луковицы двенадцатиперстной кишки по Кохеру. Во время мобилизации у больной открывалось профузное кровотечение из области гепатодуоденальной связки, куда пенетрирует язва. Попытка выявить источник кровотечения затруднилась тем, что операционное поле при снятии прижатия мгновенно заполнялось большим количеством венозной крови. Попытка осуществить гемостаз путем наложения зажима и силиконового сосудистого турникета не представлялась возможным ввиду выраженной инфильтрации гепатодуоденальной связки. Принято решение: с целью выявления источника кровотечения, взяты два силиконовых валика плотностью по шкале А. Шора 40 ед. диаметром 20,0 мм, длиной 60,0 мм Путем прижатия проксимальной и дистальной части гепатодуоденальной связки и смещения в проксимальном и дистальном направлениях на 5,0-6,0 см кровотечение было остановлено. При ревизии выявлено, что язва пенетрирует в портальную вену, которая расположена атипично, кверху от холедоха. Портальная вена была мобилизирована и ушита проленом 5/0. Прижатие убрано, кровоток восстановлен. Произведена сегментарная дуоденопластика с наложением дуодено-дуоденального анастомоза однорядным швом монофиламентной нитью биосин 5/0.Example No. 1. The hepatoduodenal ligament is infiltrated, the infiltrate spreads along the greater curvature of the duodenal bulb, is swollen, and covered with fibrin. There are ulcers in the bottom of the pancreas penetrating the head; two thrombosed vessels with a diameter of up to 0.15 cm. Considering the large size of the ulcer, the risk of recurrent bleeding, the young age, and the stability of the patient’s condition, a decision was made to perform segmental duodenoplasty. Mobilization of the duodenal bulb according to Kocher has begun. During mobilization, the patient experienced profuse bleeding from the area of the hepatoduodenal ligament, where the ulcer penetrates. An attempt to identify the source of bleeding was complicated by the fact that the surgical field, when the pressure was removed, was instantly filled with a large amount of venous blood. An attempt to achieve hemostasis by applying a clamp and a silicone vascular tourniquet was not possible due to severe infiltration of the hepatoduodenal ligament. A decision was made: in order to identify the source of bleeding, two silicone rollers with a density on the A. Shore scale of 40 units were taken. diameter 20.0 mm, length 60.0 mm By pressing the proximal and distal parts of the hepatoduodenal ligament and displacing it in the proximal and distal directions by 5.0-6.0 cm, the bleeding was stopped. During the audit, it was revealed that the ulcer penetrates into the portal vein, which is located atypically, above the common bile duct. The portal vein was mobilized and sutured with 5/0 prolene. The pressure is removed, blood flow is restored. A segmental duodenoplasty was performed with a duodeno-duodenal anastomosis using a single-row suture using a 5/0 biosin monofilament thread.

Осуществить временный гемостаз, провести сосудистые турникеты не представлялось возможным в виду геморрагической имбибиции окружающих мягких тканей, гематомы в области нижней полой вены. Путем прижатия дистальной и проксимальной частей вены, с последующим сдвиганием (расскатыванием) на 5,0-6,0 см от края дефекта силиконовых валиков плотностью по шкале А. Шора 40 ед. диаметром 50,0 мм, длиной 120,0 мм с минимальной травматизацией, был произведен временный гемостаз. Четкая экспозиция кровоточащего сосуда позволила оценить уровень и размер дефекта и произвести мобилизацию вены с дальнейшим ушиванием монофиламентной нитью (пролен 4/0). Данный способ остановки абдоминального кровотечения позволил минимизировать количество кровопотери, улучшить экспозицию операционного поля и с минимальной затратой времени осуществить окончательный гемостаз без травматизации сосуда и окружающих тканей.Example No. 2. This method of stopping abdominal bleeding made it possible to minimize the amount of blood loss, improve the exposure of the surgical field and achieve final hemostasis with minimal time without traumatizing the vessel and surrounding tissues.
