Известно, что при некоторых заболеваниях желудочно-кишечного тракта хирурги вынуждены накладывать временные или постоянные свищи. Особенно часто необходимость в выполнении данного вида операции возникает при неоперабельном раке пищевода и желудка, а также после брюшно-промежностной экстирпации прямой кишки. В последние годы наблюдается увеличение заболеваемости раком указанных локализаций. Однако, существующие методики наложения свищей не обеспечивают герметичности полого органа, на который был наложен свищ, что приводит к постоянному выделению желудочного или кишечного содержимого. Это сопровождается воспалительной реакцией как кожи, так и подкожножировой клетчатки вокруг свища, что может привести к развитию перитонита и других тяжелых осложнений. Кроме этого, после наложения свища больные испытывают тяжелейший психический дискомфорт, а их нахождение в обществе становится невозможным. Это заставило хирургов искать различные пути в создании герметичности полого органа после наложения на него свища. Однако эти методы не нашли широкого клинического применения или за счет чрезмерной травматичности, или за счет развития тяжелых осложнений. This is accompanied by an inflammatory reaction of both the skin and subcutaneous fat around the fistula, which can lead to the development of peritonitis and other serious complications. In addition, after imposing a fistula, patients experience severe mental discomfort, and their presence in society becomes impossible. This led surgeons to look for different ways in creating the integrity of a hollow organ after applying a fistula to it. However, these methods have not found wide clinical application either due to excessive trauma, or due to the development of severe complications.

Целью настоящего изобретения является разработка способа, который позволяет надежно обеспечить герметичность полого органа после наложения на его стенку временного или постоянного свища на весь период его функционирования. Сущность изобретения состоит в том, что вокруг наружного свищевого отверстия создают полое кольцо, имеющее питающую ножку и погруженное под кожу, в просвет которого вводят две ленты из синтетической ткани, концы которых выводят в противоположном направлении на поверхность кожи для механического сужения просвета свищевого хода. Указанные цель и задачи достигаются за счет применения следующей методики операции. Вслед за этим начинают формировать полое кольцо из кожи вокруг образованного отверстия в передней брюшной стенке. Для этого, отступя на 2,5-3,0 см от края хода,производят циркулярный разрез до глубоких слоев кожи. Затем на поверхность образованного кожного лоскута помещают две ленты из синтетической ткани толщиной 0,3-0,5 см с таким расчетом, чтобы концы одной ленты приняли оральное, а другой аборальное направления. После этого над лентами на края кожных ран накладывают 8-образные капроновые швы. Для чего вначале нить проводят через край хода в брюшной стенке с последующим захватом края кожного лоскута. Затем концам нити крестообразно меняют направление, при этом дистальный от хода конец нити перекидывают к его краю, где кожу прокалывают снаружи вовнутрь с захватом брюшины, а проксимальный конец нити перемещают в противоположном направлении с прокалыванием кожи у наружного края раны, то есть прилежащей к краю образованного лоскута. При затягивании концов нити происходит приближение к одной точке всех краев ран с погружением образованного кожного лоскута под кожу, при этом лоскут принимает форму полого кольца, а наружное отверстие хода клиновидно расширяется, что имеет определенное значение в профилактике послеоперационного его сужения. С появлением перистальтики кишечника, когда начинает выделяться содержимое полого органа, производят обтурацию свищевого хода эластичным обтуратором, например раздутым пальцем от резиновой перчатки, с одновременным мягким сужением наружного отверстия свищевого хода при помощи затягивания концов лент, находящихся в просвете полого кожного кольца с завязыванием их на бантик. Это позволяет избежать некроза тканей от давления на них лент. Как только возникает необходимость в опорожнении кишечника, или в кормлении больного через гастростому, концы лент развязывают и обтуратор извлекают. После завершения процедуры вновь в свищевой ход вводят обтуратор и мягко затягивают концы лент. Швы снимают на 12-14 день. После заживления раны герметичности полого органа достигают только за счет затягивания лент, так как к этому времени наступает адаптация тканей к их механическому сдавлению. Инфицирования подкожножировой клетчатки не происходит, так как с внутренней стороны канала и по линии швов она прикрыта брюшиной, кроме этого рассечение кожи только до глубоких ее слоев при формировании кожного лоскута служит профилактикой данного осложнения. Свободное сообщение полого кожного кольца с внешней средой предотвращает скопление в нем тканевого детрита. С течение времени происходит омозоление кожи по внутренней поверхности кольца, что позволяет более жестко суживать наружное отверстие свища. При необходимости замены ленты это легко можно сделать. Для сужения свищевого хода можно использовать любую ткань, в том числе и резиновые трубки, однако ленты из синтетической ткани обладают эластичностью и хорошей механической прочностью и одновременно мало впитывают в себя экссудат. To narrow the fistulous course, you can use any fabric, including rubber tubes, however, ribbons made of synthetic fabric have elasticity and good mechanical strength and at the same time absorb little exudate.

Показано, что в области передней брюшной стенки, выбранной для наложения свища на полый орган, рассечена кожа 1, подкожножировая клетчатка 2, мышечная ткань 3 и брюшина 4, которая при этом подтянута до уровня кожи. Отступя от края 10 проделанного хода в брюшной стенке на 2,5-3,0 см, произведен циркулярный разрез кожи до глубоких ее слоев 11. При помощи 8-образных съемных швов 8 образовано вокруг наружного отверстия хода полое кольцо из аутокожи 6, при этом в просвете кольца находятся две ленты 7, концы которых выведены наружу в противоположном направлении. Больная С-ко 56 лет поступила в одну из клиник г. Краснодара 26 августа 1993 года с диагнозм рак прямой кишки на глубину 10 см от ануса. Через 10 дней произведена брюшно-промежностная экстирпация прямой кишки с наложением одноствольной колостомы по описанной выше методике удержания каловых масс. Осложнений после операции не наблюдалось. Швы были сняты на 14 день после операции. С 3-го дня после операции через стому стало отходить кишечное содержимое, которое в промежутках между актами дефекации полностью удерживалось при помощи обтуратора, сделанного из пальца резиновой перчатки, и мягкого затягивания вокруг него стенки выведенной кишки при помощи двух лавсановых лент, которые располагались в просвете кольца, сделанного из аутокожи. Через 2 месяца после операции герметизация кишки достигалась только при помощи затягивания лент. Отдаленных осложнений не наблюдалось. Продолжительность наблюдения 5 месяцев. Больной обходится без калоприемника. Стеноза наружного отверстия свищевого хода нет.
