Через переднюю брюшную стенку производят установку одного оптического роботического, трех рабочих роботических троакаров хирургической системы Да Винчи и двух ассистентских троакаров по стандартной методике для тазовой робот-ассистированной хирургии [5]. В зависимости от объема гиперплазии простаты, используют нить, которой прошивают основание аденомы и производят тракцию узлов в различных направлениях. Далее под визуальным контролем простатическую уретру пересекают над уровнем семенного бугорка и отсеченную гиперплазированную ткань извлекают в брюшную полость. При необходимости производят биполярный или лигатурный гемостаз капсулярных сосудов. Под эндотрахеальным наркозом в положении больного лежа на спине с разведенным на 30 градусов ногами, был выполнен лапароцентез в верхней параумбиликальной точке, с помощью иглы Вереша брюшная полость заполнена углекислым газом до достижения давления 12 мм рт. ст. Затем установили троакары роботической системы da Vinci в типичных местах по стандартной методике для тазовой робот-ассистированной хирургии, а также дополнительные ассистентские троакары в количестве двух. При ревизии в брюшной полости патологии не выявлено. Через переднюю стенку париетальной брюшины осуществили доступ в предпузырное пространство. Выполнили продольное рассечение верхушки мочевого пузыря длиной 6 см, наложили две нити держалки на стенку мочевого пузыря. Затем выполнили ревизию мочевого пузыря, идентификацию устьев мочеточников, маркировку монополярной коагуляцией линии инцизии шейки мочевого пузыря отступя на 1 см от внутреннего отверстия уретры. Далее выполнили инцизию шейки мочевого пузыря по задней полуокружности между капсулой предстательной железы и узлами гиперплазии и сформировали тоннель до апекса предстательной железы. Тупым и острым путем порционно отделили всю гиперплазированную ткань от хирургической капсулы аденомы предстательной железы, для чего использовали тракционную нить, которой прошивали узлы гиперплазии предстательной железы и производят тракцию этих узлов в различных направлениях. Под визуальным контролем простатическую уретру пересекли над уровнем семенного бугорка и выделенную гиперплазированную ткань извлекли в брюшную полость. Далее выполнили лигатурный и биполярный гемостаз капсулярных сосудов ложа аденомы. Затем с помощью рассасывающейся нити V-loc, циркулярно, непрерывно, произвели реконструкцию шейки мочевого пузыря путем наложения эндолюминарного (внутрипростатического) пузырно-уретрального анастомоза. В завершении хирургического этапа в полость мочевого пузыря установили двухпросветный силиконовый катетер Фолея для отведения мочи в послеоперационном периоде. Дефект стенки мочевого пузыря был ушит послойно: сначала отдельным абсорбируемым швом ушивается слизистая оболочка, затем мышечная оболочка и дефект брюшины, наглухо. Выполнили тест на герметичность мочевого пузыря. Брюшная полость не дренируется. Удаленную ткань в специальном экстракционном мешке удалили наружу через расширенный параумбиликальный доступ. Контроль гемостаза, при котором источников продолжающегося кровотечения не обнаружено. Десуффляция углекислого газа из брюшной полости. The entire hyperplastic tissue was separated in a blunt and sharp way from the surgical capsule of prostate adenoma, for which a traction thread was used, which was used to suture the nodes of prostatic hyperplasia and traction of these nodes in various directions was used. Under visual control, the prostatic urethra was transected above the level of the seminal tubercle and the isolated hyperplastic tissue was removed into the abdominal cavity. Then, using an absorbable V-loc thread, circularly, continuously, the bladder neck was reconstructed by applying an endoluminal (intraprostatic) vesicourethral anastomosis. At the end of the surgical stage, a double-lumen silicone Foley catheter was installed in the bladder cavity to divert urine in the postoperative period. The bladder wall defect was sutured in layers: first, the mucous membrane was sutured with a separate absorbable suture, then the muscular membrane and the peritoneal defect, tightly. A bladder leak test was performed. The abdominal cavity does not drain. The removed tissue in a special extraction bag was removed outside through an extended paraumbilical approach. Control of hemostasis, in which no sources of ongoing bleeding are found. Desufflation of carbon dioxide from the abdominal cavity. Surgical wounds at the trocar sites were sutured and covered with aseptic stickers.
Послеоперационный период ведения пациента. Учитывая коморбидность пациента, госпитализация была продлена до момента удаления уретрального катетера. Уретральный катетер удален на 6 сутки. Самостоятельное мочеиспускание восстановлено. Скорость потока струи мочи при урофлоуметрии составила 19,4 мл/сек. Остаточной мочи нет. Уровень ПСА - 12,8 нг/мл, по поводу чего за три месяца до госпитализации по месту жительства пациенту выполнена мультифокальная биопсия предстательной железы из 12 точек, клеток аденокарциномы не обнаружено. Послеоперационный период ведения пациента. Учитывая отдаленное место жительства пациента, госпитализация была продлена до момента удаления уретрального катетера. Уретральный катетер удален на 4 сутки. Самостоятельное мочеиспускание восстановлено. Скорость потока струи мочи при урофлоуметрии составила 17,8 мл/сек. Остаточной мочи нет. Примесь крови в моче незначительная.Postoperative period of patient management. Considering the remote location of the patient, hospitalization was extended until the urethral catheter was removed. С использованием заявляемого способа в клинике урологии АО «Ильинская больница» выполнено 32 робот-ассистированных лапароскопических аденомэктомии с реконструкцией шейки мочевого пузыря. В периоперационном, раннем и позднем послеоперационном периодах не зафиксировано каких-либо осложнений, связанных с хирургическим методом, а средние сроки госпитализации составили 3,5 дня.Using the proposed method in the clinic of urology JSC "Ilyinskaya Hospital" performed 32 robot-assisted laparoscopic adenomectomy with reconstruction of the bladder neck. Таким образом, заявляемый способ проведения робот-ассистированной аденомэктомии позволяет ускорить ранее восстановление адекватного мочеиспускания в послеоперационном периоде и обеспечить профилактику таких осложнений как, послеоперационное кровотечение, формирование стеноза шейки мочевого пузыря, ухудшение эректильной функции и развитие недержания мочи.Thus, the claimed method of robot-assisted adenomectomy allows to accelerate the early recovery of adequate urination in the postoperative period and prevent complications such as postoperative bleeding, formation of bladder neck stenosis, deterioration of erectile function and the development of urinary incontinence.
