Abstract
Description
The invention relates to medicine, namely to ophthalmology, and is intended for the surgical treatment of aphakia associated with retinal detachment.
Today, most ophthalmic surgeons recognize that simultaneous microinvasive surgery of the anterior and posterior segments is a highly effective and safe way to treat combined eye pathology.
Aphakia is a pathological condition of the eye in which the natural lens in the eyeball is missing. The causes of this pathology can be congenital or acquired dislocation of the lens, various types of injuries and operations. An important feature of aphakia surgery is the implantation of an intraocular lens (IOL) with subsequent stable fixation.
Retinal detachment is a severe disease characterized by separation of the retina from the choroid, which, if left untreated, will inevitably cause complete loss of vision in a short time. There are various methods of surgical treatment of retinal detachment. In severe cases, one has to resort to more complex operations, such as microinvasive subtotal vitrectomy followed by tamponade of retinal tears with silicone oil or long-acting gas of the vitreal cavity. This is necessary to ensure reliable fixation of the retina after laser coagulation during the formation of chorioretinal adhesions. Currently, there is no unified approach to surgical treatment, as well as a unified view on the mechanisms of development of this pathology.
The closest analogue is the "Method of fixing an intraocular lens on the eyes without capsular support when performing endovitreal interventions."
(Патент РФ на изобретение №2652576).The method consists in the fact that to fix the IOL in the eyes without capsular support with an installed anterior chamber or pupillary IOL during endovitreal interventions with tamponade of the vitreal cavity with one of the vitreous body (ST) substitutes, when performing endovitreal interventions, before tamponade of the vitreal cavity, additional fixation of the IOL to the sclera is performed a thread with a needle, which is carried out twice above the anterior surface of the IOL, retreating 1-2 mm from the edges of the optical zone. The thread is carried out in such a way that the two sections of the thread that fix the IOL are parallel to each other. (RF patent for invention No. 2652576).
The disadvantages of this method of operation include the following: Firstly, the need for dissection of the mucous membrane to the sclera to perform IOL fixation.
Secondly, the need for repeated manipulations of the needle in the anterior chamber and in the sclera, which increases the risk of hemorrhages, lengthens the time and increases the invasiveness of the operation.
Thirdly, the above method is achieved by spending more time, since the more time spent on the operation, the more the patient needs anesthesia, the likelihood of postoperative complications increases, and the risk of metabolic disorders in the structures of the eye increases.
The objective of the invention is to create a method for the surgical treatment of aphakia, combined with retinal detachment, which improves visual functions and restores binocular vision. The technical result of the invention is to ensure a stable position of the IOL with pupillary fixation in the aphakic eye in the absence of capsular support and the simultaneous implementation of effective surgical treatment of retinal detachment with minimal trauma, thereby reducing the number of postoperative complications, increasing the reliability of the operation, maintaining optimal conditions for reliable adaptation of the retina, while maintaining anatomical and topographic relationships in the eye, which ultimately allows you to achieve optimal anatomical and functional results.
The proposed method is carried out as follows. The operation is performed under local anesthesia with intravenous administration of painkillers and sedatives. The surgical field is processed, the eyelid expander is installed.
Four ports are installed with trocars: three ports with a caliber of 27 G and a fourth port of 29 G 3.5-4 mm from the limbus in a self-sealing (two-plane) way. Ports for surgical instruments are set at 2 and 10 o'clock as standard. The port for the cannula with the supply of a balanced irrigation saline solution is set depending on the eye: for the right eye at 8 o'clock, for the left eye at 4 o'clock. The fourth accessory port is placed at 6 o'clock (29G) and is used for permanent placement of a chandelier (illuminator), which allows the use of a bimanual vitreoretinal technique. To detail the structure of the posterior cortical layers of the vitreous body and the internal limiting membrane (ILM), MembraneBlue-Dual is used, the posterior hyaloid layers of the vitreous body are separated from the surface of the retina using aspiration technique, starting from the optic nerve head, gradually raising the ILM to the periphery, then 0, 1-0.2 ml of MembraneBlue-Dual solution in the direction of the macular area, remove residual dye using passive aspiration. The next step is to perform a full vitrectomy using a vitreotome, the frequency is 8000-10000 cuts per minute, the vacuum is -650 mm Hg. then the vitreous cavity is filled with 1-2 ml of a perfluoroorganic compound (PFOS) to the level of retinal rupture. During the filling of the vitreal cavity with PFOS, subretinal fluid drains through the existing peripheral retinal tears. Perform two paracentesis at 3 and 9 o'clock and the main tunnel incision at 10 o'clock with a diameter of 2.2 mm. Viscoelastic is introduced into the anterior chamber. With the help of an injector, the IOL RSP-3 type "Mushroom" is implanted through the main tunnel incision, the posterior haptic part of the lens is filled through the pupil behind the iris, the anterior haptic part of the lens is left on the iris. For faster capture of the implanted lens by the pupil, a solution of myochol is injected into the anterior chamber. A tightening suture is applied to the edge of the pupil at 12 o'clock. To do this, use a curved needle, thread polypropylene 10.0. The needle is inserted into the anterior chamber through the paracentesis at 9 o'clock, the iris is sutured at 12 o'clock 1-2 mm from the pupillary edge of the iris and it is removed through the paracentesis at 3 o'clock, then a knot is formed: with a hook through the paracentesis at 9 o'clock, the distal in relation to the free end part of the thread (distal to the paracentesis at 9 o'clock) in the form of a loop outward and with two tweezers for tying through the loop, the proximal (free) end of the thread is passed three times. The opposite ends of the thread are pulled, tying a knot, as a result of which it slides to the place of contraction of the pupil. Repeat the above steps to form the node two more times. There are three nodes in total. The advantage of the tension knot is that when it is used, the peripheral part of the iris and haptics of the IOL does not move from the desired anatomical positions. The threads are cut with vitreal scissors in the anterior chamber. After that, two basal colobomas of the iris are performed through any of the free ports using a vitreotome at 3 o'clock and at 8 o'clock, so that the haptic elements of the RSP-3 IOL do not overlap them, i.e. in a place free from haptic elements, and silicone oil, when floating up, did not block the circulation of intraocular fluid. Replace PFOS with air. After that, along the edges of the existing retinal ruptures, endolaser coagulation is performed in 2-4 rows in a checkerboard pattern with a power of 0.15-0.18 mW, and silicone oil is injected into the vitreal cavity in air to tamponade the retina. The operation is completed by removing the ports and applying transconjunctival interrupted 8-00 vicryl sutures to the sites of their installation. Produce subconjunctival injection of 0.2 ml of dexamethasone and 0.2 ml of antibiotic. Apply a monocular bandage.
