Изобретение относится к области медицины, а именно к офтальмологии, и может быть использовано при хирургическом лечении с необходимостью имплантации искусственного хрусталика глаза (ИХГ) у пациентов с отсутствием адекватной капсульной поддержки.The invention relates to the field of medicine, namely to ophthalmology, and can be used in surgical treatment with the need to implant an artificial eye lens (ICL) in patients with a lack of adequate capsular support.
Отсутствие адекватной капсульной поддержки у пациентов с катарактой, афакией и необходимостью имплантацией ИХГ на сегодняшний день является одним из актуальных вопросов офтальмохирургии. Основными причинами данного состояния являются травмы глаза, заболевания соединительной ткани, интраоперационные осложнения в хирургии глазного яблока, состояния после раннее выполненной интракапсулярной экстракции катаракты. Новые модели ИХГ для обеспечения наилучших хирургических и клинических результатов и по сей день разрабатываются во всем мире. Существуют ИХГ с различными способами фиксации: внутрикапсульный, в цилиарной борозде, в углу передней камеры, зрачковый, к радужке, транссклеральный. Каждый из них имеет свои неоспоримые преимущества и недостатки.The lack of adequate capsule support in patients with cataracts, aphakia and the need for ICG implantation is one of the topical issues in ophthalmic surgery today. The main causes of this condition are eye injuries, connective tissue diseases, intraoperative complications in eyeball surgery, conditions after an early intracapsular cataract extraction. New models of ICG to ensure the best surgical and clinical results are still being developed around the world. There are ICG with various methods of fixation: intracapsular, in the ciliary sulcus, in the angle of the anterior chamber, pupillary, to the iris, transscleral. Each of them has its undeniable advantages and disadvantages.
Ближайшим аналогом является искусственный хрусталик глаза (патент РФ на полезную модель № 31 954). Данный хрусталик содержит оптическую часть и S-образные опорные элементы, отогнутые в сторону передней поверхности оптической части, диаметр которой лежит в интервале 5-7 мм, расстояние между максимально удаленными друг от друга точками, лежащими на опорных элементах, равно 12,5-13 мм, на каждом опорном элементе выполнено по одному отверстию для фиксации к склере, при этом эти отверстия расположены по одну сторону главной оси оптической части, а на конце одного из опорных элементов выполнено дополнительное отверстие также для фиксации к склере, расположенное по другую сторону главной оси оптической части по отношению к основным отверстиям.The closest analogue is an artificial lens of the eye (RF patent for utility model No. 31 954). Места перехода боковых сторон в дуги окружности скруглены. Внутренние контуры первого и второго участков повторяют внешний, а ширина этих участков составляет 350 мкм. От центра вогнутости отходит третий участок в виде якоря плоского сечения, который имеет длину 1,5 мм. Этапы хирургического лечения: выполняют два парацентеза, формируют корнеосклеральный разрез в 2 мм, выполняют капсулорексис и основной этап факоэмульсификации катаракты. После вымывания кортикальных слоев, в переднюю камеру глаза вводят вискоэластик и производят имплантацию ИХГ в полость глаза с помощью инжекторной имплантации через картридж по описанному способу. Производят вымывание вискоэластика и гидратацию парацентезов.Stages of surgical treatment: two paracentesis are performed, a corneoscleral incision of 2 mm is formed, capsulorhexis is performed and the main stage of cataract phacoemulsification is performed.
