Настоящее изобретение относится к медицине, а именно к офтальмохирургии, и может быть использовано при операциях на глазном яблоке для фиксации интраокулярной линзы (ИОЛ) при отсутствии или недостаточности капсульной поддержки.The present invention relates to medicine, namely to ophthalmic surgery, and can be used in operations on the eyeball to fix an intraocular lens (IOL) in the absence or insufficiency of capsular support.

Метод подходит для подшивания линз с широкой массивной гаптической частью. При этом сохраняется риск полного или частичного прорезывания нити в следствие малой толщины и мягкости современных ИОЛ подобного типа. Метод нельзя использовать для репозиции ранее имплантированной ИОЛ без ее удаления из глаза. При накладывании шва можно случайно подтянуть и фиксировать к радужке стекловидное тело, что вызывает задержку внутриглазной жидкости и нарушение путей ее циркуляции. Способ осуществляют следующим образом. Накладывают векорасширитель (1), выполняют анестезию. Проводят необходимые подготовительные этапы операции (факоэмульсификация катаракты, интракпсулярное удаление сублюксированного хрусталика, репозицию вывихнутой ИОЛ из стекловидного тела, переднюю витректомию и т.п.). Если фиксируется имплантированная ранее и впоследствии дислоцированная ИОЛ, то выполняют три парацентеза (Фиг. 1), через которые производят все последующие манипуляции. Два в противоположных меридианах в проекции предполагаемых точек фиксации нитей (5) и по направлению к ним (для облегчения манипуляций желательно выполнять их прямыми) (7), и еще один вспомогательный (6) в удобном для хирурга секторе по классической двухступенчатой технике (8). В обоих случаях нить на гаптике завязывается «дорожкой» (9) из пяти последовательных узлов как можно ближе к оптической части линзы (Фиг. 2). Используют атравматическую изогнутую иглу (10) не менее 12 мм длиной и нитью от 9 до 11 «00» (2). После завязывания первой нити, иглу проводят через радужную оболочку изнутри-наружу и выводят наружу через соответствующий парацентез. Прокол осуществляют отступя 2/3 ширины радужки от сфинктера зрачка. Затем повторяют всю последовательность действий для второго гаптического элемента. Из анамнеза: 4 года назад прооперирована по поводу незрелой катаракты, была имплантирована мягкая ИОЛ Alcon Acrysof IQ (S-образная ИОЛ с двумя тонкими гибкими гаптическими элементами). Острота зрения после операции 0,9. Около 2х месяцев назад заметила прогрессирующее снижение зрения, периодический «туман» перед правым глазом. Зрачок округлый, фотореакция ослаблена. ИОЛ в задней камере, смещена книзу. В просвете зрачка виден гаптический элемент ИОЛ. Рефлекс с глазного дна розовый, детали за флером. При ультразвуковом В-сканировании выявлены плавающие помутнения в стекловидном теле, оболочки прилежат. Установлен диагноз «OD-артифакия, частичный вывих ИОЛ в стекловидное тело».Upon admission to the hospital: visual acuity of the left eye - 0.05, IOP 22 mm Hg The conjunctiva is calm. На следующий день пациентку прооперировали. Операцию производили описываемым способом. Под местной анестезией выполнили 3 парацентеза (1 мм) на 5, 11 и 3 часах условного циферблата (ЧУЦ). Первые два парацентеза выполнили «прямыми», в направлении предполагаемых швов радужки, третий парацентез сделали по классической двухэтапной ступенчатой технике. Ввели вискоэластик. С помощью крючка и шпателя ИОЛ вывели в переднюю камеру. Нить 10-«00» с атравматической иглой 12 мм подшили к гаптическому элементу максимально близко к оптической части ИОЛ. Процедуру выполнили путем перемещения иглы через парацентезы с использованием инсулиновой иглы (для профилактики прошивания роговицы) и наложения «дорожки» из пяти последовательных узлов. Затем на 11 ЧУЦ провели иглу через радужку изнутри-наружу, отступив 2/3 ее ширины от сфинктера зрачка. Иглу вывели наружу через соответствующий парацентез. Ко второму гаптическому элементу так же фиксировали нить, иглу вывели наружу через парацентез на 5 ЧУЦ. ИОЛ погрузили в заднюю камеру, позиционировали и центрировали линзу, попеременно потягивая за обе нити. Иглой на 11 часах повторно прошили радужку снаружи внутрь и обратно. Получившуюся петлю связали 3 узлами со свободным концом нити, фиксируя гаптику к радужке. Концы нити обрезали цанговыми ножницами в передней камере. Действия повторили для другого гаптического элемента. После этого методом ирригации-аспирации отмыли вискоэластик из передней камеры, разрезы герметизировали методом гидратации. В послеоперационном периоде проводили противовоспалительную и антибактериальную терапию.The next day, the patient was operated on. The operation was performed as described. Then, at 11 CJC, a needle was passed through the iris from the inside out, retreating 2/3 of its width from the pupil sphincter. The needle was brought out through appropriate paracentesis. A thread was also fixed to the second haptic element, the needle was brought out through paracentesis at 5 PM. IOLs were immersed in the rear chamber, positioned and centered on the lens, alternately pulling on both threads. The needle at 11 o'clock repeatedly flashed the iris from the outside inwards and back. The resulting loop was tied with 3 knots to the free end of the thread, fixing the haptic to the iris. The ends of the thread were cut with collet scissors in the front chamber. The steps were repeated for another haptic element. After this, the viscoelastic was washed from the anterior chamber by irrigation-aspiration, the incisions were sealed by hydration. In the postoperative period, anti-inflammatory and antibacterial therapy was performed.
На следующий день после операции отмечался слабый отек роговицы, ИОЛ расположена правильно, фиксирована к радужке двумя узловыми швами. Острота зрения составила 0,5, ВГД пальпаторно в пределах нормы. При контрольном осмотре спустя 7 дней острота зрения составила 1,0 без дополнительной коррекции, ВГД 19 мм рт.ст. Роговица прозрачная, передняя камера глубокая, влага чистая. Зрачок округлый, фотореакция слегка ослаблена. ИОЛ в задней камере, расположена правильно, фиксирована двумя узловыми швами к радужке.
