Abstract
Description
The invention relates to medicine, namely, to ophthalmic surgery, and can be used for reposition of an intraocular lens (IOL), dislocated together with a capsule bag.
М., 2007. С. 237-243.With phacoemulsification of cataract, pronounced displacement of the IOL in the postoperative period occurs in 0.2-2.8% of cases and requires repeated surgical intervention (Stebnev S. D. Dislocation of intraocular lenses. Causes, nature, surgical tactics, treatment results / Stebnev S. D. Dislocation of intraocular lenses. , Malov V.M. // Modern technologies of cataract surgery: collection of scientific articles M., 2007. S. 237-243.
The main reasons for the pronounced dislocation of the IOL are the initial weakness of the ligamentous-capsular apparatus of the lens (SCA), its damage during the operation, or the surgeon's incorrect assessment of the safety of the supporting parts of the SCA during IOL implantation.
There is a method of repositioning a monoblock IOL, dislocated together with a capsule bag, providing for suture fixation of the IOL-capsule bag block to the iris, during which the institution is carried out through the flat part of the ciliary body under the IOL-capsule bag block of the injection needle and centering the said block , the block is pressed upward and transcorneal fixing sutures are applied to the IOL support elements (patent RU No. 2527912), prototype.
However, when manipulating the injection needle to center the block, there is a risk of damage to the intraocular structures, which increases the invasiveness of the intervention and the operation time.
The proposed invention solves the problem of developing an atraumatic and reliable method for repositioning an IOL of any design, deployed together with a fibrously changed capsular bag.
The essence of the invention lies in the fact that after performing the surgical access to the dislocated IOL, the aspiration tip is brought in the lumen of the pupil, the tip is fixed to the optical part of the IOL by means of occlusion on the switched on vacuum, and the IOL is repositioned and centered until the contours of the lens on the iris are visualized. Continuing to hold the IOL with a vacuum, a suture needle is passed through the cornea and iris under the support element to the place of its future fixation to the iris, the vacuum is turned off and suture fixation of the IOL support element to the iris is performed. Then the aspiration cannula is brought under the second support element, the cannula is fixed to it, and the support element is repositioned on the switched on vacuum until its contour is visualized on the iris. A suture needle is passed through the cornea and iris under the support element, the vacuum is turned off and suture fixation of the second support element to the iris is performed.
The technical result is to reduce the trauma of surgical intervention and operation time, as well as to improve the convenience of the surgeon.
The use of the tip of the aspiration cannula for IOL reposition reduces the risk of damage to intraocular structures during IOL reposition and centering and atraumatically center the dislocated IOL without unnecessary rotations in the anterior chamber.
In the case of suturing the posterior chamber lens, holding the fibrous capsular bag with a vacuum allows you to create opposition to the injection of the needle at the time of puncture of the capsule bag.
The method is carried out as follows.
After performing anesthesia, the main incision and paracentesis, an aspiration cannula is brought to the dislocated IOL (dislocated block "IOL-capsule bag") in the lumen of the pupil, the cannula tip is fixed to the optical part of the IOL by occlusion on the switched on vacuum, and the IOL is repositioned and centered until visualization of the contour lenses on the iris. The operation is completed by hydration of paracentesis and administration of an antibiotic.
