Description
Предлагаемое изобретение относится к офтальмологии и предназначено для выбора тактики лечения при полном вывихе хрусталика в стекловидное тело.The present invention relates to ophthalmology and is intended to select a treatment tactic for complete dislocation of the lens into the vitreous body.
Известно, что при дислокации хрусталика различной этиологии на глазное дно могут быть использованы как «консервативные» средства лечения (очковая и контактная коррекция), при этом хрусталик не удаляют, так и хирургические, при которых обязательным этапом операции является удаление хрусталика и хирургическая коррекция афакии. Среди последних выделяют интраокулярную коррекцию и кераторефракционные операции (докторская диссертация Д.В.Липатова «Коррекция афакии при выраженной несостоятельности связочно-капсулярного аппарата хрусталика», 2003 год, стр.10-21). Считается, что очковая коррекция может быть использована только при двухсторонней афакии. Кроме того, очковым линзам высоких диоптрий присущ ряд существенных недостатков: увеличение размеров ретинального изображения, сужение и дефекты поля зрения, призматические феномены, концентрическая скотома на близкой периферии, искажение контуров по сторонам от центральной оптической зоны и некоторые другие. Контактная коррекция может быть затруднена из-за возраста пациентов и возникающих из этого «манипуляционных» трудностей с контактными линзами. Основными недостатками кераторефракционных операций являются сложность техники и необходимость дорогостоящего оборудования, возможность снижения остроты зрения в результате развития послеоперационного неправильного астигматизма и изменений кривизны трансплантата, трудность расчета его параметров, возможность плохой эпителизации роговицы и инфицирования трансплантата (вплоть до его отторжения). Таким образом, в настоящее время при дислокации хрусталика на глазное дно именно интраокулярная коррекция с транссклеральной фиксацией опорных элементов является оптимальным методом коррекции афакии.It is known that when the lens is dislocated with various etiologies on the fundus, both “conservative” treatment methods (spectacle and contact correction) can be used, while the lens is not removed, as well as surgical ones, in which the lens removal and surgical correction of aphakia are an obligatory stage of the operation. Lipatov's doctoral dissertation “Correction of aphakia with severe insolvency of the ligament-capsular apparatus of the lens”, 2003, pp. 10-21). It is believed that spectacle correction can only be used with bilateral aphakia. In addition, eyeglass lenses of high diopters have a number of significant drawbacks: an increase in the size of the retinal image, narrowing and visual field defects, prismatic phenomena, concentric scotoma on the near periphery, distortion of the contours along the sides of the central optical zone, and some others. Contact correction may be difficult due to the age of the patients and the resulting “manipulation” difficulties with contact lenses. The main disadvantages of keratorefractive operations are the complexity of the technique and the need for expensive equipment, the possibility of reducing visual acuity as a result of the development of postoperative irregular astigmatism and changes in the graft curvature, the difficulty of calculating its parameters, the possibility of poor corneal epithelization and infection of the graft (up to its rejection). Thus, at present, when the lens is dislocated to the fundus, it is intraocular correction with transscleral fixation of support elements that is the optimal method for correcting aphakia.
В настоящее время предложено большое количество способов удаления дислоцированного в стекловидное тело хрусталика, которые предусматривают выполнение тотальной витреоэктомии, что приводит к увеличению объема и усложнению хирургического вмешательства. Проведенные многолетние наблюдения за больными с вывихнутым в полость стекловидного тела хрусталиком показали, что нахождение хрусталика на глазном дне может протекать без осложнений. В то же время очковая или контактная коррекция у этих пациентов в большинстве случаев не могла считаться оптимальной. Наилучшие функциональные показатели при коррекции афакии, как известно, позволяет получить интраокулярная коррекция. Эти факты наталкивают на необходимость поиска путей, при которых можно сочетать сохранение вывихнутого хрусталика и интраокулярную коррекцию.At present, a large number of methods have been proposed for removing the lens dislocated in the vitreous body, which involve performing a total vitreoectomy, which leads to an increase in volume and complication of surgical intervention. Способ осуществляют следующим образом. С помощью современных ультразвуковых приборов определяют местоположение дислоцированного в стекловидное тело хрусталика, степень сохранности его капсулы и наличие процесса швартообразования в стекловидном теле. Определяют наличие симптомов воспаления и уровень внутриглазного давления (ВГД). При наличии интактной (неповрежденной) капсулы, отсутствии процесса швартообразования, симптомов воспаления и нормальном уровне ВГД производят интраокулярную коррекцию ложной афакии с транссклеральной фиксацией опорных элементов. При этом собственный хрусталик не удаляется и остается дислоцированным в полость стекловидного тела. При наличии одного из указанных выше симптомов (воспаление, повышение уровня ВГД, швартообразование в стекловидном теле и несохранная капсула хрусталика) производится удаление вывихнутого хрусталика с субтотальной витреоэктомией с интраокулярной коррекцией афакии тем же способом. При последующем появлении хотя бы одного из вышеуказанных симптомов всегда остается возможность дополнительно провести удаление дислоцированного хрусталика через плоскую часть цилиарного тела с одновременной суб- или тотальной витреоэктомией.The method is as follows. In the presence of one of the above symptoms (inflammation, increased IOP, mooring in the vitreous body and an unsaved lens capsule), the dislocated lens with subtotal vitreoectomy is removed with intraocular correction of aphakia in the same way. With the subsequent appearance of at least one of the above symptoms, it always remains possible to additionally remove the dislocated lens through the flat part of the ciliary body with simultaneous sub- or total vitreoectomy.
Пример 1. Пациент Иванников С.Ю., 67 лет, поступил в клинику НИИ ГБ РАМН с диагнозом: посттравматическая дислокация хрусталика (полный вывих) в течение 6 лет. Все это время пользовался афакичными мягкими контактными линзами (МКЛ). В последнее время резко ухудшилось зрение на другом глазу из-за глаукомы и возникли манипуляционные сложности с МКЛ. На глазу с ложной афакией симптомов воспаления, повышения уровня ВГД, процессов швартообразования в стекловидном теле отмечено не было, капсула хрусталика была интактной. В связи с этим было принято решение провести вторичную имплантацию ИОЛ с транссклеральной шовной фиксацией опорных элементов и передней витреоэктомией. Собственный хрусталик при этом был оставлен интактным на глазном дне. Во время операции и в послеоперационном переоде осложнений не было. Пациент выписан через 10 дней после поступления с остротой зрения 0.8 с коррекцией. Швы с конъюнктивы сняты через 10 дней, а с роговицы - спустя 2 месяца после операции. Через 6 месяцев после операции острота зрения составила 0.8-0.9 с коррекцией, симптомов воспаления, повышения ВГД и швартообразования не отмечается.Example 1. The patient was discharged 10 days after admission with visual acuity of 0.8 with correction. The sutures from the conjunctiva were removed after 10 days, and from the cornea - 2 months after the operation. 6 months after surgery, visual acuity was 0.8-0.9 with correction, symptoms of inflammation, increased IOP and mooring were not observed.

Пациентка Сергеева Л.Н., 65 лет, поступила в клинику НИИ ГБ РАМН с диагнозом: миопия высокой степени, полный вывих хрусталика в полость стекловидного тела после неудачной попытки экстракции катаракты в другом лечебном учреждении. При обследовании выявлены признаки воспаления (иридоциклит, увеит), повышение уровня ВГД до 34-35 мм рт.ст., разрыв хрусталиковой сумки и швартообразование в стекловидном теле (по данным ультразвукового исследования). В связи с этим была произведена субтотальная витреоэктомия через плоскую часть цилиарного тела, ядро хрусталика удалено с помощью фрагматома. Учитывая наличие вышеуказанных симптомов, имплантация интраокулярной линзы была признана нецелесообразной, а послеоперационная коррекция афакии осуществлена с помощью МКЛ. Острота зрения при этом составила 0.6. Пациентка находится под регулярным наблюдением.Example 2. Given the presence of the above symptoms, implantation of an intraocular lens was considered inappropriate, and postoperative correction of aphakia was performed using MKL. Visual acuity was 0.6. The patient is under regular supervision.

Таким образом, предложенный способ позволяет облегчить тактику хирургического вмешательства у пациентов с полной дислокацией хрусталика и добиться более высоких функциональных показателей в послеоперационном периоде из-за существенного уменьшения степени риска и объема хирургического вмешательства при данной патологии, особенно у лиц пожилого возраста, а также снизить экономические затраты в связи с отсутствием необходимости использования дорогостоящей эндовитреальной техники.Thus, the proposed method allows to facilitate the tactics of surgical intervention in patients with a complete dislocation of the lens and to achieve higher functional indicators in the postoperative period due to a significant reduction in the risk and volume of surgical intervention for this pathology, especially in the elderly, as well as to reduce economic costs due to the lack of the need to use expensive endovitreal equipment.
