Острый кератоконус характеризуется возникновением отека стромы роговицы из-за проникновения влаги из передней камеры вследствие разрыва Десцеметовой мембраны (ДМ). Острый кератоконус, как правило, возникает внезапно, сопровождается резким болевым синдромом и выраженным отеком роговицы. Зона отека и помутнения стромы может быть различной - от локального помутнения в центре роговицы до ее тотального отека. Ряд авторов полагают, что поскольку данное состояние может разрешиться самопроизвольно, то активного лечения оно не требует. С другой стороны, поскольку разрешение процесса может занимать до 3-4 месяцев, ряд офтальмологов считают целесообразными придерживаться более активной лечебной тактики. В частности выполнять хирургические вмешательства, включая введение в переднюю камеру глаза крови (гемо-пломбирование), пузырька воздуха или газа, коагуляцию роговицы, ее бандажное укрепление и ряд других. Есть мнение о целесообразности выполнения экстренной сквозной кератопластики, однако при этом повышается риск помутнения трансплантата роговицы. Активная медикаментозная терапия для снятия воспаления и более скорого клинического успокоения глаза, общепризнанно считается целесообразной.Acute keratoconus is characterized by the occurrence of edema of the corneal stroma due to the penetration of moisture from the anterior chamber due to rupture of the Descemet's membrane (DM). Acute keratoconus, as a rule, occurs suddenly, accompanied by a sharp pain syndrome and severe corneal edema. The zone of edema and clouding of the stroma can be different - from local clouding in the center of the cornea to its total edema. A number of authors believe that since this condition can resolve spontaneously, it does not require active treatment. On the other hand, since the resolution of the process can take up to 3-4 months, a number of ophthalmologists consider it expedient to adhere to a more active treatment tactics. In particular, perform surgical interventions, including the introduction of blood into the anterior chamber of the eye (hemo-filling), an air or gas bubble, coagulation of the cornea, its bandage strengthening, and a number of others. There is an opinion about the expediency of performing emergency penetrating keratoplasty, however, this increases the risk of corneal graft opacification. Active drug therapy to relieve inflammation and achieve faster clinical calm of the eye is generally considered to be appropriate.

После чего, используя нить нейлон 10-0, авторы накладывали непроникающие пред-Десцеметовые швы в количестве от 3 до 7 перпендикулярно разрыву ДМ. Недостатками данного способа являются наложение непроникающих пред-Десцеметовых швов и заполнение ПК стерильным воздухом, который оставляли на длительный срок. Непроникающие пред-Десцеметовые швы не дают полной аппланации роговицы, что увеличивает срок резорбции отека. Технический результат достигается тем, что способ согласно изобретению, включает наложение от 2 до 5 узловых швов, проводимых через всю толщу роговицы с проникновением в переднюю камеру по предварительно нанесенным меткам в пораженном участке. Сам шов уплощает роговицу, приближая ее строму к участку отслоенной ДМ, а участок нити, проходящий внутри передней камеры, позади роговицы, служит для непосредственной аппланации ДМ к задней поверхности стромы. При этом при наличии видимого участка разрыва ДМ, швы накладываются перпендикулярно его длине, а в отсутствие участка разрыва ДМ - крестообразно или в виде решетки. The method is performed under the control of intraoperative optical coherence tomography (IOCT).

Затем в парацентез вводят 0,1 мл дисперсивного вискоэластика (ВЭ), располагая его каплю в передней камере (ПК) непосредственно у входа в парацентез. Далее выполняют иОКТ и определяют границы отслойки ДМ, которые отмечают маркером на передней поверхности роговицы. Данные метки послужат ориентиром для последующего прокола роговицы иглой при проведении швов. Вкол изогнутой иглы (игла длиной 15 мм, изгиб игольного наконечника 1/4) с нитью полипропилен (10-0) проводят по внешней границе метки, проводят ее сквозь роговицу через переднюю камеру и выкол делают снаружи метки в противолежащем меридиане. Нить завязывают, а узел шва сдвигают к месту входа иглы в роговицу. В случае необходимости, добавляют воздух в переднюю камеру, проводят иОКТ и накладывают следующий шов. Далее, максимально выпускают пузырек воздуха и замещают объем ПК сбалансированным солевым раствором. The operation is completed with an injection of dexamethasone and an antibiotic under the conjunctiva.

Из анамнеза: в 30 лет выставлен диагноз OU Кератоконус 3 стадии. При обращении острота зрения OD 0,05 не коррегируется. Объективно: роговица изменена в виде конуса, центр смещен книзу и кнаружи, резкий отек всех слоев, роговица помутнена, включая поверхностные слои стромы, разрыв ДМ не обнаружен, отслойка ДМ и эндотелия от задних слоев. Передняя камера глубокая, радужка структурна. Глубжележащие среды не офтальмоскопируются. Учитывая данные анамнеза (ранее установленный диагноз), дооперационной диагностики и объективного исследования - был выставлен диагноз: OD Острый кератоконус.
