Известны способы лечения отслойки десцеметовой мембраны. Самым распространенным является способ, заключающийся в введении газа (например SF6, воздуха) в переднюю камеру. В ряде случаев удается добиться прилегания десцеметовой мембраны к строме роговицы, в этом случае жидкость из полости, возникшей в результате отслойки, вытекает через разрез в переднюю камеру. К сожалению, не во всех случаях этим способом удается получить прилегание десцеметовой мембраны.Known methods for the treatment of detachment descemet membrane. В переводе с английского на русский язык эта статья помещена в журнале "Новое в офтальмологии" за 1999 г. , N3, стр. 27. В этом же материале говорится о ненадежности этого способа: не всегда удается достичь прилегания мембраны, это подтверждает и наш практический опыт. Указанный технический результат может быть получен, если в способе лечения отслойки десцеметовой мембраны путем обеспечения прилегания ее к роговице в результате удаления жидкости из полости, образованной десцеметовой мембраной и стромой роговицы, в комбинации с внутрикамерным введением газа жидкость удалить наружу, для этого после введения газа в переднюю камеру дренируют образовавшуюся полость путем выполнения вкола изогнутой иглы в роговицу под углом 45-55"с выходом ее кончика в центральной зоне дренируемой полости вблизи стромы, затем поперечными движениями иглы расширяют и раскрывают просвет канала, а в конце дренирования иглой совершают плавный обратный ход. Отслойка десцеметовой мембраны чаще всего возникает при формировании доступа (разреза) в переднюю камеру. Место расположения доступа в большинстве случаев - это верхний сектор глаза (12 часов), там же берет начало и отслойка. При незначительной по площади отслойке ее можно устранить с помощью газа, введенного в переднюю камеру. Т. к. газ занимает верхнее положение в любой полости и обладает большой тампонирующей силой, то, находясь в передней камере, он давит на отслоенную десцеметову мембрану и вытесняет жидкость через разрез в переднюю камеру, мембрана прилегает. Если же отслойка обширная, то газ способствует лишь частичному выведению жидкости, а именно той ее части, которая находится около разреза (вверху), для остальной, удаленной от разреза жидкости газ сыграет негативную роль: в какой-то момент заблокирует ей выход. В результате часть жидкости, удаленная от разреза, окажется в замкнутом пространстве и будет продолжать обуславливать отслойку десцеметовой мембраны. Выходом является дренирование замкнутого пространства, образованного отслоенной десцеметовой мембраной и внутренней поверхностью стромы с выведением жидкости наружу. Осуществить дренирование позволяет канал, сформированный с помощью изогнутой режущей иглы, которая вводится в наружную поверхность роговицы с выходом ее кончика в центральной зоне дренируемой полости вблизи стромы. Для того чтобы канал выполнял роль дренажа, необходимо после заведения иглы поперечными движениями иглы его расширить и раскрыть просвет канала, что позволит жидкости выходить наружу. As a result, the part of the fluid remote from the cut will be in a confined space and will continue to cause detachment of the descemet membrane. The solution is the drainage of an enclosed space formed by a delaminated descemet membrane and the inner surface of the stroma with the removal of fluid to the outside. To carry out drainage allows the channel formed using a curved cutting needle, which is inserted into the outer surface of the cornea with the exit of its tip in the central zone of the drained cavity near the stroma. In order for the channel to play the role of drainage, it is necessary after the needle is inserted by transverse movements of the needle to expand it and open the lumen of the channel, which will allow the liquid to come out.
Важным моментом, обеспечивающим достижение положительного эффекта в заявляемом способе, является то, что дренажный канал формируют после заполнения передней камеры газом т. к. :

Газ, введенный в переднюю камеру, обеспечивает хорошую контуризацию образовавшейся при отслойке полости: хирург четко видит ее площадь и высоту, что важно для исключения травмирования десцеметовой мембраны иглой как в момент введения кончика иглы в образовавшуюся полость, так и в дальнейшем, при прилегании мембраны, когда жидкость выходит из полости.An important point, ensuring the achievement of a positive effect in the claimed method, is that the drainage channel is formed after filling the front chamber with gas, because:

Используя изогнутую иглу и выполняя пологий вкол иглы в роговицу (45-55o), удается при полном визуальном контроле атравматично и плавно ввести иглу, обеспечить отток жидкости из-под отслоенной мембраны и вывести иглу обратным ходом, не травмировав при этом десцеметовую оболочку, которая, поддавливаемая газом, плавно прилегает к строме (по мере выведения жидкости) и, таким образом, занимает правильное анатомическое положение. Способ осуществляется следующим образом. Под местной анестезией выполняют парацентез, желательно в зоне, в которой десцеметовая мембрана не отслоена. Через парацентез с помощью канюли в переднюю камеру вводят воздух: 2/3 объема передней камеры. Затем под углом 45-55o к роговице (пологий вкол) осуществляют вкол изогнутой режущей иглы (тип иглы: шпательная игла МИКРО-ПОЙНТ фирмы ЭТИКОН, длина 6 мм, диаметр 220 мк) в роговицу с выходом ее кончика в центральной зоне дренируемой полости вблизи стромы. В следующий момент хирург поперечными движениями иглы расширяет канал и создает просвет, через который жидкость начинает выходить наружу, десцеметовая мембрана, поддавливаемая газом, начинает прилегать. Затем, не переставая поддавливать иглу в поперечном направлении, прижимая кончик иглы к строме, по мере прилегания мембраны хирург начинает совершать плавный обратный ход иглой (с целью исключения травмирования мембраны иглой), выводя ее наружу вместе с остатками жидкости. При этом воздух, находящийся в передней камере, надежно поддавливает мембрану, заставляя остатки жидкости покинуть зону отслойки через канал, созданный иглой. Мембрана занимает свое анатомическое положение, воздух продолжает ее прижимать к роговице. Операция завершена.The method is as follows. Then, without ceasing to squeeze the needle in the transverse direction, pressing the tip of the needle against the stroma, as the membrane adheres, the surgeon begins to make a smooth return stroke with the needle (to prevent injury to the membrane with the needle), leading it out along with the remaining fluid. At the same time, the air in the front chamber reliably presses the membrane, forcing the remaining liquid to leave the detachment zone through the channel created by the needle. The membrane takes its anatomical position, the air continues to press it to the cornea. The operation is completed.
Заявляемым способом в ЕЦ МНТК "Микрохирургия глаза" прооперировано 11 пациентов после того, как не удалось добиться прилегания мембраны с помощью газа.
