Пациентка И., 38 лет, обратилась в клинику 15.01.2021 г. с жалобой на перелом коронки зуба 1.4. Проведено комплексное обследование. С помощью компьютерной томографии определено достаточное количество костной ткани в позиции 1.4 для установки дентального имплантата. Проведена инфильтрационная анестезия в позиции 1.4. и малотравматичное удаление зуба. Установили навигационный хирургический шаблон, используя специальный набор фрез провели препарирование костной ткани для создания ложа под дентальный имплантат и установили его. Затем сняли навигационный хирургический шаблон, пространство между имплантатом и вестибулярной стенкой плотно заполнили материалом ЛитАр. Далее с бугра верхней челюсти произвели забор свободного соединительно-тканного аутотрансплантата, его эпителиальный слой иссекли при помощи скальпеля 15С, в центре соединительно-тканного аутотрансплантата произвели сквозной звездчатый разрез и погрузили соединительно-тканный аутотрансплантат в раствор NaCl 0,9%. В донорской зоне для сохранения сгустка наложили швы. Затем зафиксировали соединительно-тканный аутотрансплантат на формирователь десны. Далее выкрутили формирователь десны и установили временную коронку на винтовой фиксации с усилием в 15н/см. Заживление проходило без осложнений. Через 4 месяца получили высокий эстетический результат. Пациентка проходит раз в полгода профилактические осмотры. Состояние мягких тканей стабильное, резорбции костной ткани, окружающей имплантаты, не наблюдается. Установили формирователь десны на дентальный имплантат, и сразу же сняли формирователь десны таким образом, чтобы соединительно-тканный аутотрансплантат остался вокруг дентального имплантата. После чего установили сканируемый абатмент и выполнили сканирование установленного дентального имплантата с получением STL-файла. После сканирования произвели установку формирователя десны на время моделирования и фрезерования временной ортопедической конструкции, а STL-файл отправили в собственную цифровую зуботехническую лабораторию, где в специальной программе произвели моделирование временной ортопедической конструкции. Затем выкрутили формирователь десны и устанавливали временную коронку на винтовой фиксации с усилием в 15 н/см. Заживление проходило без осложнений. Через 4 месяца получили высокий эстетический результат. Пациент проходит раз в полгода профилактические осмотры. Состояние мягких тканей стабильное, резорбции костной ткани, окружающей имплантаты, не наблюдается.Patient M., 45 years old, came to the clinic on December 24, 2020 with a complaint of a fracture of the crown of tooth 1.1. A comprehensive examination has been carried out. Using computed tomography, a sufficient amount of bone tissue was determined in position 1.1 for the installation of a dental implant. Conducted infiltration anesthesia in position 1.1. and minimally invasive tooth extraction. A navigational surgical template was installed, using a special set of cutters, bone tissue was prepared to create a bed for a dental implant and installed. Then the navigation surgical template was removed, the space between the implant and the vestibular wall was densely filled with LitAr material. Next, a free connective tissue autograft was taken from the tubercle of the upper jaw, its epithelial layer was excised with a 15C scalpel, a through stellate incision was made in the center of the connective tissue autograft and immersed in a 0.9% NaCl solution. The donor area was sutured to preserve the clot. Then the connective tissue autograft was fixed on the gum shaper. The gingiva former was installed on the dental implant, and the gingiva former was immediately removed so that the connective tissue autograft remained around the dental implant. After that, a scanned abutment was installed and the installed dental implant was scanned to obtain an STL file. After scanning, the gum former was installed for the time of modeling and milling the temporary orthopedic structure, and the STL file was sent to our own digital dental laboratory, where the temporary orthopedic structure was modeled in a special program. Then the gingiva former was unscrewed and the temporary crown was screwed in place with a force of 15 n/cm. Healing took place without complications. After 4 months, a high aesthetic result was obtained. The patient undergoes preventive examinations every six months. The condition of the soft tissues is stable, there is no resorption of the bone tissue surrounding the implants.
