Изобретение относится к медицине, а именно к травматологии и ортопедии, и может быть использовано для восстановления сферичности головки плечевой кости с использованием винтов и костного цемента в ходе оперативного лечения пациентов с застарелыми вывихами плеча.The invention relates to medicine, namely to traumatology and orthopedics, and can be used to restore the sphericity of the humeral head using screws and bone cement during surgical treatment of patients with chronic shoulder dislocations.
Застарелый вывих плеча характеризуется целым рядом патологических изменений, одним из которых является импрессионный перелом головки плечевой кости. В результате вправленный вывих может неоднократно рецидивировать в связи с инконгруэнтностью суставных поверхностей плечевого сустава.An old shoulder dislocation is characterized by a number of pathological changes, one of which is an impression fracture of the humeral head. Технический результат достигается тем, что из зоны дефекта резецируют субхондральную кость по краю дефекта с формированием костного навеса шириной 1-3 мм и глубиной 3-5 мм и вводят в дно дефектной полости три винта так, чтобы их оси располагались по ребрам условной трехгранной пирамиды с углами вершины 30°-40°, а головки винтов были погружены глубже условной восстанавливаемой сферической суставной поверхности, после чего заполняют полость дефекта костным цементом и под визуальным контролем точности перехода сферы хряща головки в сферическую поверхность цемента формируют суставную поверхность. Способ восстановления сферичности головки плечевой кости в ходе оперативного лечения пациентов с застарелыми вывихами плеча осуществляют следующим образом. После выделения головки плечевой кости 1 (Фиг. 1) с полостью костного дефекта 2 из последней удаляют рубцы 3 и субхондральную костную ткань 4, формируя циркулярный уступ 5 (шириной 1-Змм и глубиной 3-5 мм) (Фиг. 2) по краю костного дефекта 2. В дно 6 дефектной полости 2 вводят три винта 7, располагая оси 8 винтов 7 по условным ребрам 9 треугольной пирамиды 10 с углами вершины 11 в пределах 30°-40°. Концы 12 винтов 7 выводят через компактную кость шейки 13 плечевой кости 14 так, чтобы головки 15 винтов 7 оказались ниже условной поверхности сферы суставной поверхности 16. Необходимо, чтобы концы витков выстояли из компактного слоя шейки в соответствии с рекомендациями по остеосинтезу AO-ASIF. После введения винтов 7 в полость костного дефекта 2 заливают костный цемент 17 (Фиг. 3), из которого 17 во время полимеризации формируют сферическую поверхность 18, которая под визуальным контролем непрерывно продолжает сферическую суставную поверхность головки 1 плечевой кости 14. При наличии воспалительного процесса в области головки плечевой кости в анамнезе или признаков инфекционного процесса, выявленных во время операции, используют костный цемент с антибиотиком.The method for restoring the sphericity of the humeral head during the surgical treatment of patients with chronic shoulder dislocations is as follows. Клинический пример. Пациент Д., 47 лет, поступил с диагнозом застарелый вывих плеча, дефект головки плечевой кости. В ходе предоперационного обследования на основании результатов компьютерной томографии был выявлен массивный дефект головки плечевой кости с нарушением ее сферичности в задне-наружном отделе. Было принято решение выполнить оперативное вмешательство по предложенному способу.Clinical example. Срок иммобилизация плечевого сустава в мягкой повязке Дезо составил 4 недели. С 4-й недели начата пассивная разработка движений в плечевом суставе. Затем с 6-й недели со дня операции начата разработка активных движений в плечевом суставе. Через 3 месяца со дня операции функциональный результат со стороны плечевого сустава по шкале Constant составил 81 балл. Через 3, 6, 12 месяцев рецидивов вывихов отмечено не было.The term of immobilization of the shoulder joint in a soft Desot bandage was 4 weeks.
