Одним из факторов развития одонтогенного верхнечелюстного синусита является попадание стоматологических материалов через нижнюю стенку верхнечелюстной пазухи в ее полость. В ряде случаев патологический субстрат остается фиксированным на дне верхнечелюстной пазухи, способствуя развитию очага хронического воспаления. При выраженной пневматизации альвеолярного углубления нижняя стенка верхнечелюстной пазухи становится еще более труднодоступной зоной для стандартных эндоскопических доступов, функциональность которых во многом определяется малым размером антростомы в среднем носовом ходе. Расширенная антростомия в среднем носовом ходе, а также различные варианты доступов через переднюю стенку верхнечелюстной пазухи сопряжены с выраженной морбидностью. Известен способ формирования антростомы через грушевидный гребень при помощи боров (Якобашвили И.Ю., Чумаков П.Л., Федоткина К.М., с соавт. Способ хирургического лечения хронических верхнечелюстных синуситов через грушевидный гребень при помощи боров). После гидроотсепаровки области грушевидного гребня проводится разрез от места прикрепления переднего конца нижней носовой раковины до основания передней носовой ости. Мягкие ткани отсепаровываются в нижнем носовом ходе и от лицевой стенки верхнечелюстной пазухи. Бором формируется овальное трепанационное отверстие 0,3×0,5 см в лицевой и латеральной стенке верхнечелюстной пазухи и расширяется до переднего конца нижней носовой раковины. После элиминации хронического очага воспаления инфратурбинальный лоскут укладывается на костный остов латеральной стенки полости носа, полностью закрывая сформированную антростому. Нижнюю носовую раковину латерализуют. Разрез закрывается биодеградируемым гемостатическим материалом. After elimination of the chronic focus of inflammation, the infraturbinal flap is placed on the bone skeleton of the lateral wall of the nasal cavity, completely closing the formed anthrostoma. The lower turbinate is lateralized. The incision is closed with biodegradable hemostatic material. To ensure adequate postoperative care, a resection of the hook process is performed.

Пациентка С., 66 лет поступила в ЛОР-отделение с диагнозом «Правосторонний одонтогенный верхнечелюстной синусит. Инородное тело правой верхнечелюстной пазухи». При поступлении жалобы на стекание слизисто-гнойного отделяемого из полости носа по задней стенке глотки, ощущение тяжести в проекции правой верхнечелюстной пазухи. Вышеуказанные жалобы возникли 10 лет назад после пломбирования зуба верхней челюсти справа. Объективно: носовое дыхание не затруднено. Слизистая оболочка полости носа розовая, влажная. Перегородка носа существенно не искривлена. Слизистая оболочка нижних носовых раковин неотечная. Патологического отделяемого в полости носа нет. Пациентке выполнена эндоскопическая операция на правой верхнечелюстной пазухе, удаление инородного тела инфратурбинальным доступом по предложенной методике. На фоне лечения пациентка отметила отсутствие стекания слизи по задней стенке глотки, дискомфорта в проекции правой верхнечелюстной пазухи. Через месяц полость носа осмотрена эндоскопом, нижняя носовая раковина латерализована, инфратурбинальный лоскут состоятелен, васкуляризован, полностью закрывает антростому латеральной стенки полости носа.Patient S., 66 years old, was admitted to the ENT department with a diagnosis of “Right-sided odontogenic maxillary sinusitis. Against the background of treatment, the patient noted the absence of mucus draining along the back wall of the pharynx, discomfort in the projection of the right maxillary sinus. After a month, the nasal cavity was examined with an endoscope, the lower nasal concha was lateralized, the infraturbinal flap was consistent, vascularized, completely covered the anthrostome of the lateral wall of the nasal cavity.
Авторами прооперировано 15 пациентов с диагнозом «Одонтогенный верхнечелюстной синусит» по предложенной авторами методике. В послеоперационном периоде пластическое закрытие антростомы инфратурбинальным лоскутом состоятельно. Таким образом, удалось добиться создания хирургического коридора, который позволяет свободно манипулировать в труднодоступной для стандартной эндоскопической техники зоне, при этом максимально сохраняются анатомические структуры.
