Существует множество различных хирургических подходов при санирующих вмешательствах на височной кости у больных хроническим гнойным средним отитом (ХГСО), и каждый из них имеет свои показания и особенности. Одним из них является эндауральный подход. Данный подход выполняется при ограниченном распространении холестеатомы (апикальная, антральная) и особенностях строения височной кости (предлежание сигмовидного синуса и низкое расположение дна средней черепной ямки), что обеспечивает минимально инвазивную хирургию этой области с сохранением здоровых тканей и сокращение послеоперационного периода вследствие быстрого заживления раны. При данном подходе существенным недостатком является дефицит тканей для пластического этапа - уменьшения (облитерации) созданной операционной полости при распространенном деструктивном процессе.There are many different surgical approaches for sanitizing interventions on the temporal bone in patients with chronic purulent otitis media (HCHO), and each of them has its own indications and features. This approach is performed with a limited spread of cholesteatoma (apical, antral) and structural features of the temporal bone (presentation of the sigmoid sinus and low location of the bottom of the middle cranial fossa), which provides minimally invasive surgery in this area with the preservation of healthy tissues and a reduction in the postoperative period due to rapid healing of the wound. With this approach, a significant drawback is the lack of tissue for the plastic stage - reduction (obliteration) of the created operating cavity during a common destructive process.
Аналогами настоящего изобретения явились способы уменьшения (облитерации) мастоидальных полостей при «открытых» санирующих операциях, выполненных заушным и эндауральным доступами. Одним из них является «Способ мастоидопластики» с использованием мышечного лоскута на питающей ножке, сформированного из височной мышцы при заушном подходе [1 - прототип]. Необходимо отметить, что мастоидопластика при заушном подходе всегда применялась для закрытия обнаженной костной ткани созданной полости и ускорения ее эпителизации. Учитывая, что созданная хирургически антромастоидальная полость значительно ухудшает слуховую функцию (за счет изменения акустического резонанса) и качество жизни больных ХГСО (необходимость регулярной ее очистки), то современной тенденцией плановой санирующей хирургии при ХГСО является обязательное выполнение не только тимпанопластики, но и мастоидопластики (или облитерации). В последние годы для этого используются комбинированные «жесткие» материалы с обязательным их закрытием надкостничным, мышечным и фасциальным лоскутом для уменьшения (облитерации) созданной полости в сосцевидном отростке [2, 3]. При эндауральном подходе вследствие недостаточности пластического материала для уменьшения (облитерации) полости используют синтетические материалы (остеокондуктивное биостекло - Биоситал), реже аутокость (забранную по ходу операции), которые покрывают хондро-перихондральными фрагментами из козелка ушной раковины пациента и кожными лоскутами из слухового прохода [4]. При этом подходе костной ткани можно получить немного, а использование хряща, как и мышечного трансплантата требует дополнительных разрезов.Analogs of the present invention were methods of reducing (obliterating) mastoid cavities during "open" sanitizing operations performed by behind-the-ear and endaural accesses. It should be noted that mastoidoplasty with the behind-the-ear approach has always been used to close the exposed bone tissue of the created cavity and accelerate its epithelization. Considering that a surgically created antromastoid cavity significantly worsens auditory function (due to changes in acoustic resonance) and the quality of life of patients with chronic hypertension (regular cleaning), the current trend in planned sanitizing surgery for chronic hypertension is not only tympanoplasty, but also mastoidoplasty (or obliteration). In recent years, combined “hard” materials have been used for this with their mandatory closure by the periosteal, muscle and fascial flap to reduce (obliterate) the created cavity in the mastoid process [2, 3]. In the endaural approach, due to insufficient plastic material, synthetic materials (osteoconductive bioglass - BioSital) are used to reduce (obliterate) the cavity, less often autobone (taken during the operation), which is covered with chondro-perichondral fragments from the patient’s tragus and skin flaps from the ear canal [ four]. With this approach, bone tissue can be obtained a little, and the use of cartilage, as well as muscle transplant, requires additional incisions.
Техническим результатом предлагаемого способа является достижение уменьшения (облитерации) созданной антромастоидальной полости при эндауральном подходе без дополнительного расширения зоны операционного поля посредством забора мышечно-надкостничного лоскута из края височной мышцы и надкостницы заушной области и перемещения его на нижней ножке в созданную антромастоидальную полость.The technical result of the proposed method is to achieve reduction (obliteration) of the created antromastoid cavity during the endaural approach without additional expansion of the area of the surgical field by taking the muscle-periosteal flap from the edge of the temporal muscle and periosteum of the behind the ear and moving it on the lower leg to the created anthromastoid cavity.
Методика операции заключается в следующем.The technique of the operation is as follows.
Под эндотрахеальным наркозом и инфильтрационной анестезией выполняется межхрящевой разрез (по Heermann) между козелком и завитком до костной части наружного слухового прохода. Из этой зоны продолжением разреза является циркулярный разрез (по Rosen) по задней стенке наружного слухового прохода с формированием меатотимпанального кожного лоскута (кожа костного отдела наружного слухового прохода с барабанной перепонкой). Далее выполняется латеральное смещение кожи наружного слухового прохода с надкостницей с обнажением кости задней и верхней стенок слухового прохода и сосцевидного отростка (фиг. 1 Схема межхрящевого (по Heermann) и циркулярного (по Rosen) разрезов, смещения кожи для обнажения сосцевидного отростка (в положении лежа)). После установки ранорасширителя, отодвигающего мягкие ткани, в хирургическом поле оказываются площадка сосцевидного отростка, наружный слуховой проход и барабанная перепонка. Вначале выполняется ревизия барабанной полости после смещения меатотимпанального лоскута и вскрытия барабанной полости. В зависимости от распространения холестеатомы в среднем ухе выполняется поэтапная аттико-адито-антромастоидотомия с помощью фрез или долот.Under endotracheal anesthesia and infiltration anesthesia, an inter-cartilage incision (according to Heermann) is performed between the tragus and curl to the bony part of the external auditory meatus. Depending on the spread of cholesteatoma in the middle ear, a phased attico-adito-anthromastoidotomy is performed using cutters or chisels.
В случаях антромастоидотомии с удалением стенки сосцевидного отростка («открытый» вариант операции) после завершения санирующего этапа операции и снятия ранорасширителя выше площадки сосцевидного отростка под кожей выделяется распатором и выкраивается ножницами в височно-заушной области мышечно-надкостничный лоскут 5,0×1,5 см (надкостница сосцевидного отростка и краевой участок m. temporalis) на нижней питающей ножке. Лоскут перемещается в сформированную полость после выполнения элементов тимпанопластики под меатотимпанальный кожный лоскут (фиг. 2. Схема подготовки и мастоидопластики мышечно-надкостничным лоскутом из височно-заушной области через эндауральный разрез: а. - выкраивание мышечно-надкостничного лоскута: 1 - разрез по Heermann, 2 - мышечно-надкостничный лоскут; б. - перемещение лоскута на нижней ножке в созданную антромастоидальную полость (стрелки - направление смещения)). Операцию заканчивают укладкой на барабанную перепонку и кожу костного отдела наружного слухового прохода резиновых или силиконовых полосок и тампонадой наружного слухового прохода губками с антибиотиком. The operation is completed by laying on the eardrum and skin of the bony section of the external auditory canal of rubber or silicone strips and tamponade of the external auditory canal with antibiotic sponges. The interchondral section is sutured in layers.
Описанный хирургический способ уменьшения (облитерации) антромастоидальной полости при эндауральном подходе может выполняться при различном распространении деструктивного холестеатомного процесса, в условиях близкого расположения важных анатомических структур - сигмовидного синуса и средней черепной ямки (осложняющих проведение операции заушным подходом), что расширяет возможности эндаурального подхода при санирующих операциях. Это сокращает сроки заживления и реабилитации после операции, улучшает качество слуха и жизни пациента вследствие уменьшения объема операционной полости.The described surgical method for reducing (obliterating) the antromastoid cavity during the endaural approach can be performed with different distribution of the destructive cholesteatomic process, in the conditions of close proximity of the important anatomical structures - the sigmoid sinus and middle cranial fossa (complicating the operation by the behind-the-ear approach), which expands the possibilities of the endaural approach with sanitizing operations. This reduces the healing and rehabilitation time after surgery, improves the quality of hearing and life of the patient due to a decrease in the volume of the operating cavity.
В течение 2 лет способ был применен у 5 пациентов с ХГСО.Within 2 years, the method was applied in 5 patients with HGSO.

При поступлении предъявляла жалобы на снижение слуха на правое ухо, ощущение влажности в правом ухе, слизисто-гнойные выделения из уха.Upon admission, she complained of hearing loss in the right ear, a feeling of moisture in the right ear, mucopurulent discharge from the ear.
Из анамнеза известно, что страдает ХГСО с детства. Во взрослом возрасте прогрессивного снижение слуха не было. В последние два года отмечала постоянную влажность в правом ухе и слизисто-гнойные выделения из этого уха. Проведенная компьютерная томография (КТ) височных костей показала: признаки правостороннего хронического среднего гнойного отита с холестеатомой аттико-антральной области при низком расположении дна средней черепной ямки. При отомикроскопии - наружный слуховой проход правого уха широкий, свободный. Барабанная перепонка в заднем отделе натянутой части с краевым дефектом. Латеральная стенка аттика-адитуса разрушена, с эпидермизацией этой области и густым слизистым отделяемым. Слуховая труба проходима по Вальсальве, Левое ухо - без видимых изменений.ENT status: nose, pharynx, nasopharynx, larynx - no visible changes. On examination: behind the ear, auricles unchanged. Интраоперационно обнаружено: латеральная стенка аттика и адитуса разрушены и покрыты стелющимся по рубцовой выстилке эпидермисом, распространяющимся в антрум. В барабанной полости выявлено разрушение цепи слуховых косточек. Стремя подвижное, арка его смещена к каналу лицевого нерва (ЛН). В фациальном синусе - рубцы. Наковальня отсутствует. Головка молоточка - кариозно изменена, окутана плотным эпидермисом и удалена. Канал ЛН в тимпанальном отделе частично оголен вследствие разрушения костной стенки, и покрыт эпидермисом до сухожилия мышцы, натягивающей барабанную перепонку. С помощью моторной системы удалена латеральная стенка аттика, адитуса и антрума до верхушки сосцевидного отростка. Сосцевидный отросток пневматического типа строения, антрум небольшой с большим количеством клеток, в которые погружается эпидермис холестеатомы. У верхушки сосцевидного отростка и в перифациальных клетках выявлены плотные гроздьевидные грануляции. Над сформированной в сосцевидном отростке полостью в височно-заушной области отсепарован и выделен мышечно-надкостничный лоскут на питающей нижней ножке. Под дефект барабанной перепонки в натянутом отделе уложена аутохрящевая полупластина (взятая из хряща ушной раковины пациента через дополнительный разрез 1 см), с опорой на головку стремени и под рукоятку молотка, которая укрывается аутофасцией (подготовленной через эндауральный доступ). В созданную антромастоидальную полость уложены фрагменты аутохряща, аутофасция, которые прикрыты смещенным мышечно-надкостничным лоскутом, поверх которого разложены остатки меатального кожного лоскута. Созданная полость, неотимпанальная мембрана и слуховой проход выстланы резиновыми протекторами и тампонированы губкой с антибиотиком. Fragments of auto-cartilage, autofascia, which are covered by an offset muscular-periosteal flap, on top of which the remains of the meatal skin flap are laid, are placed in the created antromastoid cavity. The created cavity, the non-tympanic membrane and the ear canal are lined with rubber protectors and are plugged with an antibiotic sponge. The interchondral section is sutured in layers.
По данным ТПА при выписке на 5 сутки отмечалась стабильность порогов костной проводимости. Тампоны и протекторы в слуховом проходе сохранялись до 16 суток.According to TPA, at discharge on day 5, stability of bone conduction thresholds was noted. При динамическом наблюдении через 1, 3 и 6 месяцев после операции: заушная область не изменена, безболезненна. Отоскопическая картина справа: наружный слуховой проход широкий без отделяемого. Мастоидальная и антральная области отсутствуют. Область аттика открыта, эпидермизирована. Неотимпанальная мембрана состоятельна, подвижна при продувании. Слуховая труба проходима. По данным ТПА на стороне оперированного уха отмечено сокращение КВИ до 20 дБ на низких и разговорных звуковых частотах.During dynamic observation after 1, 3 and 6 months after surgery: the behind-the-ear area is not changed, it is painless.
