Известен способ лечения радикулярных кист (цистэктомия), заключающийся в выкраивании и отслойки слизисто-надкостничного лоскута с вестибулярной стороны альвеолярного отростка, резекции и или ретроградном пломбировании верхушек зубов, выстоящих в полость кисты, удалении оболочки кисты и укладывании на место лоскута с наложением швов, введением в полость йодоформного тампона, который меняется периодически до полного заживления (В.В. Причем стерилизацию и удаление эпителия в области верхушек корней зубов, выступающих в полость кисты, осуществляют путем электрокоагуляции в импульсном режиме 2 секунды 3-5 раз с мощностью тока на выходе 90-100 Вт, пломбирование осуществляют путем погружения верхушки корня зуба в пломбировочный материал, предварительно размещенный в конусообразном углублении глубиной 3-5 мм в фиксирующей пластинке, выполненной в виде плоской губки из остеокондукторного материала, с последующим заполнением послеоперационного дефекта костной ткани дополнительным остеокондукторным материалом, причем в качестве остеокондукторного материала используют колаполовую губку, а в качестве пломбировочного материала используют материал «Pro-root» (см. патент РФ №2446768, МПК А61С 5/00, опубл. 20.09.2011 г.) и способ лечения периапикального воспалительного процесса (SU 1210798 А, 15.02.1986), при котором после удаления патологически измененных тканей часть корня, выступающего в полость кисты, покрывают герметичным веществом, что позволяет снизить травматичность и предупредить рецидив.The closest in technical essence is a method of filling the tops of the roots of the teeth that stand in the cyst cavity during their treatment, including the preparation of the carious cavity, instrumental and antiseptic treatment of the root canals of the “causal” teeth, cutting out the mucous-periosteal-bone flap and removing the shell of the cyst and epithelium from the tops of the roots of the teeth standing in the cavity of the cyst, their drying and constant filling. Недостатком известного способа является то, что при проведении лечения не представляется возможным в полной мере обеспечить сухость в каналах корней зубов, выстоящих в полость кисты из-за наличия большого объема кистозной жидкости, поступающей под давлением в корневой канал, и быстрое пропитывание колаполовой губки биологическими жидкостями - транссудатом и кровью, а покрытие герметиком корневой части также не обеспечивает герметичность пломбирования канала, так как процедура пломбирования проводится перед операцией и без обеспечения сухости в канале из-за поступления экссудата из полости кисты, гранулемы. Кроме того, создание сухости при удалении патологических тканей в области гранулемы и кисты возможно только вакуум-аспиратором, который одновременно с биологическими жидкостями удаляет и жидкую клеевую композицию. При этих условиях невозможно осуществить надежное пломбирование, так как наличие влаги препятствует герметичности пломбирования корневых каналов. Не герметичное пломбирование каналов способствует реинфицированию периапикальных тканей, рецидиву кисты, что может привести к дополнительным воспалительным осложнениям и необходимости удаления зубов, располагающихся в области кисты. Поэтому после постоянного пломбирования каналов в таких неблагоприятных условиях дополнительно применяют операцию резекции верхушек корней зубов, выстоящих в полость кисты и их ретроградное пломбирование. Однако обеспечить сухость в полости кисты для ретроградного пломбирования также не представляется возможным даже при мизерной кровоточивости и выделения транссудата, причем для выполнения этой манипуляции требуется дополнительное дорогостоящее оборудование и препараты, а применение методик многократного временного пломбирования с применением кальцийсодержащих препаратов, снижающих явления воспаления и экссудации, занимает длительный период времени (15-18 месяцев), что также снижает качество жизни пациента, так как не исключает возможности нагноения кист и удаления зубов вместе с кистой.The disadvantage of this method is that during the treatment it is not possible to fully ensure dryness in the canals of the roots of the teeth, standing in the cavity of the cyst due to the presence of a large volume of cystic fluid coming under pressure into the root canal, and the quick soaking of the bell sponge with biological fluids - transudate and blood, and the sealant coating of the root part also does not ensure the tightness of the filling of the canal, since the sealing procedure is carried out before the operation and without I have dryness in the canal due to the flow of exudate from the cavity of the cyst, granuloma. Therefore, after constant filling of the canals in such adverse conditions, the operation of resection of the apices of the roots of the teeth standing in the cyst cavity and their retrograde filling are additionally used. However, to ensure dryness in the cavity of the cyst for retrograde filling is also not possible even with scanty bleeding and transudate excretion, moreover, additional expensive equipment and preparations are required to perform this manipulation, and the use of multiple temporary filling techniques using calcium-containing preparations that reduce inflammation and exudation, takes a long period of time (15-18 months), which also reduces the quality of life of the patient, as it does not exclude the possibility zhnosti suppuration cysts and tooth extraction with cyst.

Клинический пример №1. Пациентка Т., 45 лет, направлена на операцию цистэктомии в области 3.1, 3.2, 3.3, 3.4.Clinical example No. 1. Patient T., 45 years old, is directed to a cystectomy operation in the area 3.1, 3.2, 3.3, 3.4.
При объективном исследовании слизистая бледно-розовая, чистая, влажная, выбухает в преддверие на уровне премоляров и первого моляра нижней челюсти, справа определяется симптом «пергаментного хруста». На ортопантомограмме просветление овальной формы с четким границами размером до 3,5 см в наибольшем измерении, каналы зубов частично запломбированы. Верхушки корней зубов 3.1, 3.2, 3.3, 3.4 в полости кисты. Под 2-сторонней торусальной анестезией 4% раствором артикаина 1:200000 распломбированы каналы, механическая, антисептическая обработка каналов 3.1, 3.2, 3.3, 3.4, высушивание. Высушить не удается из-за постоянного подтекания кистозной жидкости в канал зуба. Выкроен трапециевидный слизисто-надкостнично-костный лоскут, удалена оболочка кисты, удален эпителий с верхушек корней всех зубов петлевидным электродом, наблюдается умеренная кровоточивость. Полость кисты заполнена гемостатическим средством (мембрана «Тахо-Комб»), область верхушек корней зубов плотно закрыта мембраной «Пародонкол», после чего при повторном высушивании каналов зубов бумажными штифтами обеспечена полная сухость в каналах и их герметичное пломбирование «Эндометазоном» и холодной гуттаперчей. Наложены редкие швы проленом, аппликации «Эпланом». Контрольная рентгенограмма показала равномерное пломбирование каналов на всем протяжении. Назначены холод по схеме, анальгетики, десенсибилизирующие и антибактериальные препараты по схеме, антисептические ванночки. Последующий осмотр показал наличие незначительного отека, слабой болезненности на следующие сутки, исчезновение отека на 3-4 сутки, швы сняты на 7 сутки, заживление без осложнений. Rare sutures were applied by prolemen, appliqués by Eplan. The control x-ray showed uniform filling of the channels throughout. На прицельной рентгенограмме выявлено просветление до 2,0 см с прорастанием дна верхнечелюстной пазухи, каналы 1.5, 1.6 зубов частично запломбированы.Clinical example No. 2. Ultracaini 4% - 1,8 мл выкроен слизисто-надкостнично-костный лоскут, удалена оболочка кисты и эпителий с верхушек корней зубов петлевидным электродом. Полость кисты заполнена мембраной «Тахо-Комб», область верхушек корней зубов плотно закрыта мембраной «Пародонкол», поэтому при дополнительной антисептической обработке и повторном высушивании каналов зубов бумажными штифтами обеспечена полная сухость в каналах и их герметичное пломбирование «Эндометазоном» и холодной гуттаперчей. Наложены редкие швы «Проленом», аппликации «Эпланом». Контрольная рентгенограмма показала равномерное пломбирование каналов на всем протяжении. Назначена антибактериальная, противовоспалительная, десенсибилизирующая терапия. Последующий осмотр показал уменьшение боли на следующие сутки, исчезновение отека на 4 сутки.
