Abstract
Description
The invention relates to medicine, namely to therapeutic dentistry.
[Bustrom A., Cleasson R., Sundqvist G. The antibacterial effect of camphorated paramonochlorophenol, camphorated phenol and calcium hydroxide in the treatment of infected root canals. Endodontics and Dental Traumatology 1: 170-175, 1985]. [Ogilntebi R. Infection of dentinal tubules and endodontic therapy. Int Endodontic J 27:314-323, 1994].A known method of intracanal treatment of apical periodontitis using pastes based on calcium hydroxide [Bustrom A., Cleasson R., Sundqvist G. The antibacterial effect of camphorated paramonochlorophenol, camphorated phenol and calcium hydroxide in the treatment of infected root canals. Endodontics and Dental Traumatology 1: 170-175, 1985]. The method consists in introducing into the root canals of periodontitis teeth medical pastes containing calcium hydroxide. However, this method does not allow to fully influence the entire spectrum of microflora of apical periodontitis and, therefore, does not provide for the rehabilitation of foci of infection, often leading to a protracted, recurrent course of the disease [Ogilntebi R. Infection of dentinal tubules and endodontic therapy. Int Endodontic J 27: 314-323, 1994].
A method is also known in which for the preparation of root canals an ultrasound-activated aqueous solution of sodium hypochlorite and a 17% solution of sodium ethyleneiminetetracetate are used, then local antibiotic therapy is carried out by introducing a mixture of equal parts by weight of metronidazole, ciprofloxacin and minocycline into the root canals, and to stimulate reparative processes Calcium hydroxide preparation is injected intrachannel (Pat. RF No. 2158123. MPK7 A61K 6/00. Method for the treatment of apical periodontitis. Barer G.M., Ovchinnikova I.A., Tsarev V.N. . №30, 2000). This method is selected for the prototype.
However, ciprofloxacin and minocycline are antibiotics, and antibiotics form resistance to bacteria, and their subsequent use according to vital indications is not effective, and Ca (OH) 2 is an alkali, which leads to intra-channel resorption of dentin and cement. Sodium hypochlorite is highly toxic in high concentrations and tends to cause tissue irritation at the site of contact, therefore, the higher the concentration, the stronger the damage to vital cells, and, according to the method suggested by the author, special equipment is required to sound the reagent.
The objective of the invention is to increase the effectiveness of treatment by reducing the toxic effects of drugs on periodontal tissue and the rapid removal of pain, reducing treatment time, simplifying the treatment process.
This is achieved due to the fact that, as the tooth canal expands, a 5% metronidazole solution is repeatedly injected, after which turunda moistened with Polyoxidonium solution is inserted into the canal and left until the next doctor’s visit, the canal mouth is closed with Sandarak Plus, after 48 hours the tooth canal is opened, treated with a 2% aqueous solution of sodium hypochlorite, 5% solution of metronidazole and fill.
2%.Reducing the toxicity of the effect on the patient's body consists in eliminating antibiotics, reducing the concentration of sodium hypochlorite from 5.25 to 2%.
Repeated administration of a 5% solution of metronidazole in the process of channel expansion in combination with Polyoxidonium provides relief of inflammation in the early stages and prevents exacerbation after applying a bandage. The presence of turunda moistened with a solution of Polyoxidonium in the tooth canal within 48 hours prevents the formation of purulent exudate. Closing the mouth of the canal with the “Sandarak Plus” preparation provides air permeability of the tissue to reduce the pressure resulting from the accumulation of products of inflammation in the damaged periodontium. Carrying out the treatment of the canal with a 2% aqueous solution of sodium hypochlorite makes it possible to irrigate with any anatomical form of the apical delta and at the same time removes the smeared layer, the process does not transform into a purulent phase of inflammation.
The method is as follows:
The patient undergoes mechanical necroectomy from the root canals, their mechanical cleansing and expansion. As the tooth channel expands, during treatment, a 5% metronidazole solution is repeatedly injected into it, after which turunda moistened with Polyoxidonium solution is inserted into the canal and left until the next doctor’s visit after 48 hours, the canal mouth is closed with the Sandarak Plus preparation. On the second visit to the doctor, the tooth canal is opened, the turundu is removed, treated with a 2% aqueous solution of sodium hypochlorite, then with a 5% solution of metronidazole and filled with gutta-percha.
