Известен способ лечения радикулярных кист - цистэктомия, заключающийся в изготовлении выкройки в виде слизисто-надкостничного лоскута с вестибулярной стороны альвеолярного отростка, удалении оболочки кисты и укладывании на место выкройки с наложением швов, при цистотомии - отличие в том, что оболочка иссекается частично, в образованную полость с оставшейся оболочкой кисты вводится йодоформный тампон, который меняется периодически до полного заживления (В.В. Клинический пример №1. Пациентка Т., 50 лет, направлена на операцию цистэктомию в области 4.4, 4.5.Clinical example No. 1. Patient T., 50 years old, is directed to cystectomy surgery in the area 4.4, 4.5.
При объективном исследовании слизистая бледно-розовая, чистая, влажная, выбухает в преддверие на уровне премоляров и первого моляра нижней челюсти, справа определяется симптом «пергаментного хруста». На спиральной компьютерной томограмме костей лица диагностируется радикулярная киста размером до 2,5 см в диаметре. Верхушки корней зубов в полости кисты, оболочка располагается непосредственно у ментального отверстия нижнего альвеолярного нерва. При выполнении цистэктомии возможно повреждение сосудисто-нервного пучка ментального отверстия. Поэтому проведена фотодинамическая терапия кисты - трансканальная и наружная с вестибулярной стороны. Проведена антибактериальная терапия корневых каналов 1.5, 1.6 зубов, затем в полость кисты введено 2 мл фотосенсибилизатора метиленового синего с экспозицией 7 мин, облучение кисты в течение 1 мин диодным лазером с игольчатым световодом через корневой канал, а затем облучение пластинчатым световодом с вестибулярной поверхности в течение 1,0 мин с мощностью 1 Вт, длиной волны 630 нм, в импульсном режиме. Затем герметично пломбируются каналы горячей гуттаперчей с использованием силлера «Seal apex» с предварительным выведением в полость кисты остеоиндуктора «Коллап-Ан» геля с метрогилом. Пломба светофотополимер. Назначены холод по схеме, анальгетики, диазолин, «цифран СТ-500» по схеме, антисептические ванночки. Последующий осмотр показал наличие незначительного отека, слабой болезненности на следующие сутки, исчезновение отека на 3-4 сутки. Then the channels of hot gutta-percha are hermetically sealed using a Seal apex sealer with preliminary excretion of a gel with metrogil into the cyst cavity of the osteoinducer “Collap-An”. Облучение кисты диодным лазером через корневой канал в течение 1,0 мин игольчатым световодом 1.5, 1.6 и пластинчатым световодом с вестибулярной поверхности в течении 1,5 мин с мощностью 1 Вт, длиной волны 630 нм, в импульсном режиме. Затем в полость кисты введено 1,5 мл остеоиндуктора «Коллап-Ан» геля с метрогилом, герметично запломбированы каналы горячей гуттаперчей с использованием силлера «Seal арех», пломба из светофотополимера. Назначены холод по схеме, анальгетики при болях, диазолин 1 табл. наночь, «цифран СТ-500» по схеме, антисептические ванночки. Последующий осмотр показал уменьшение боли на следующие сутки, исчезновение отека на 4 сутки. Предлагаемый способ лечения позволяет повысить эффективность лечения радикулярных кист, устранить постлечебные осложнения и обеспечить полное удаление оболочки кисты за счет эффекта фотодинамической терапии. Предлагаемый способ позволяет также расширить клинический диапазон возможностей лечения данной патологии, избежать потери трудоспособности пациентов.The proposed method of treatment improves the treatment of radicular cysts, eliminates post-treatment complications and ensures complete removal of the cyst membrane due to the effect of photodynamic therapy.
