Изобретение относится к области медицины, в частности к хирургической стоматологии, и может быть использовано при лечении радикулярных и фолликулярных кист, проросших в полость носа и верхнечелюстную пазуху.The invention relates to medicine, in particular to surgical dentistry, and can be used in the treatment of radicular and follicular cysts, sprouted into the nasal cavity and maxillary sinus.
Известен способ лечения одонтогенных кист, проросших в верхнечелюстную пазуху и полость носа, заключающийся в изготовлении выкройки в виде слизисто-надкостничного лоскута с вестибулярной стороны альвеолярного отростка, удалении части оболочки кисты и укладывании на место выкройки с наложением кетгутовых швов. Оболочку кисты удаляют вместе со слизистой верхнечелюстной пазухи с последующим объединением полостей кисты и пазухи и резекцией корней зубов, выступающих в полость кисты, после чего проводят тампонаду пазухи йодоформной турундой, выведенной в искусственное соустье в нижнем носовом ходу (см. книгу под ред. Технический результат достигается тем, что в способе лечения одонтогенных кист, проросших в верхнечелюстную пазуху и полость носа, заключающемся в изготовлении с вестибулярной стороны альвеолярного отростка выкройки соединительно-тканного лоскута, состоящего из слизисто-надкостничной и костной частей, удалении части оболочки кисты и укладывании на место выкройки соединительно-тканного лоскута с наложением кетгутовых швов, согласно изобретению выкраивание костной части надкостничного лоскута проводят пьезохирургическим скальпелем с одновременным отслаиванием оболочки кисты от костной части, а выкраивание слизисто-надкостничной части соединительно-тканного лоскута, отсечение отслоенной оболочки кисты и коагуляцию оставшейся части оболочки кисты осуществляют посредством диодного лазера с диаметром световода 500-600 мкм, мощностью 4-6 Вт, длиной волны 0,97-1,56 мкм и экспозицией 1-3 секунды в импульсном режиме.The technical result is achieved by the fact that in the method of treating odontogenic cysts, sprouted into the maxillary sinus and nasal cavity, which consists in making from the vestibular side of the alveolar process of the pattern of the connective tissue flap, consisting of the mucous-periosteal and bone parts, removing part of the cyst membrane and laying on the place of the pattern of connective tissue flap with the application of catgut sutures, according to the invention, the bone part of the periosteal flap is cut using a piezosurgical scalpel with simultaneous exfoliation of the cyst membrane from the bone part, and cutting out the mucoperiosteal part of the connective tissue flap, cutting off the exfoliated cyst membrane and coagulating the remaining part of the cyst membrane is carried out using a diode laser with a fiber diameter of 500-600 μm, 4-6 W, wavelength 0.97-1.56 microns and an exposure of 1-3 seconds in a pulsed mode.
Лечение с помощью предлагаемого способа и достижение технического результата поясняется на примере конкретного выполнения. Клинический пример: пациент Б., 60 лет, направлена лор-врачом на удаление фолликулярной кисты в области правого третьего моляра верхней челюсти, проросшей в верхнечелюстную пазуху. При объективном исследовании слизистая бледно-розовая, чистая, влажная, выбухает в преддверие на уровне первого, второго и третьего моляров верхней челюсти, справа определяется симптом «пергаментного хруста». На спиральной компьютерной томограмме костей лица диагностируется фолликулярная киста размером до 2,5 см в диаметре. Костная стенка в области дна пазухи и кисты отсутствует, в полости кисты располагается ретенированный третий верхний правый моляр. Пациент имеет кардиоимплантат - водитель сердечного ритма, поэтому электрохирургическое лечение противопоказано. Под местной анестезией с помощью лазерного и пьезохирургического скальпеля выкроен соединительно-тканный лоскут П-образной формы, отодвинут кверху. Выделена пьезохирургическим скальпелем оболочка кисты с костной части альвеолярного отростка до области спаивания ее со слизистой пазухи и отсечена лазерным скальпелем. Проведена коагуляция оставшейся части оболочки кисты лазерным лучом, полость заполнена «Коллапан-М», соединительно-тканный лоскут уложен на место, швы проленом, аппликации «Левомеколь», назначены анальгетики, диазолин, «цифран-ст» по схеме, холод, антисептические ванночки. Выкраивание слизисто-надкостничной части соединительно-тканного лоскута, отсечение выделенной части кисты и коагуляция оставшейся части оболочки проведена воздействием диодного лазера диаметром световода 500 мкм с мощностью на выходе 5 Вт, длиной волны 1,06 мкм и экспозицией 2 секунды в импульсном режиме. Последующий осмотр показал наличие незначительного отека, слабой болезненности на следующие сутки, исчезновение отека на 4-5 сутки, швы сняты на 7 сутки, заживление первичным натяжением. Данные спиральной компьютерной томографии через 6 и 12 месяцев показали восстановление структуры костного дефекта в полном объеме и отсутствие патологических изменений со стороны слизистой верхнечелюстной пазухи.Treatment using the proposed method and the achievement of a technical result is illustrated by the example of a specific implementation. Clinical example: patient B., 60 years old, was referred by an ENT doctor to remove a follicular cyst in the region of the right third molar of the upper jaw, germinated in the maxillary sinus. An objective examination of the mucous membrane is pale pink, clean, moist, swells on the eve of the first, second and third molars of the upper jaw, the symptom of “parchment crunch” is determined on the right. On a spiral computed tomogram of facial bones, a follicular cyst up to 2.5 cm in diameter is diagnosed. The bone wall in the area of the bottom of the sinus and cyst is absent; in the cavity of the cyst there is a retained third upper right molar. The patient has a cardiac implant - a heart rate driver, therefore electrosurgical treatment is contraindicated. Under local anesthesia, with the help of a laser and piezosurgical scalpel, a connective tissue flap of a U-shaped shape was cut, pushed up. The cyst membrane was isolated with a piezosurgical scalpel from the bony part of the alveolar process to the area of soldering it from the sinus mucosa and cut off with a laser scalpel. The remaining part of the cyst membrane was coagulated with a laser beam, the cavity was filled with “Kollapan-M”, the connective tissue flap was put in place, the sutures were wedged, the applications “Levomekol” were assigned analgesics, diazolin, “tsifran-st” according to the scheme, cold, antiseptic baths . The mucosal-periosteal part of the connective tissue flap was cut out, the selected part of the cyst was cut off, and the remaining part of the sheath was coagulated by the action of a diode laser with a fiber diameter of 500 μm with an output power of 5 W, a wavelength of 1.06 μm, and an exposure of 2 seconds in a pulsed mode. Subsequent examination showed the presence of slight edema, weak soreness on the next day, the disappearance of edema on the 4-5th day, sutures were removed on the 7th day, healing by primary intention. Data of spiral computed tomography after 6 and 12 months showed the restoration of the structure of the bone defect in full and the absence of pathological changes in the mucosa of the maxillary sinus.
Предлагаемый способ лечения позволяет повысить эффективность лечения одонтогенных кист, проросших в верхнечелюстную пазуху и полость носа, исключив осложнения во время проведения операции и послеоперационные осложнения за счет исключения риска разрыва и ожога слизистой пазухи и полости носа. Предлагаемый способ позволяет также расширить функциональные возможности, так как появилась возможность лечить пациентов с аллергическими реакциями на электроток или имеющих металлические имплантаты.The proposed method of treatment allows to increase the effectiveness of the treatment of odontogenic cysts sprouting in the maxillary sinus and nasal cavity, eliminating complications during surgery and postoperative complications by eliminating the risk of rupture and burns of the sinus mucosa and nasal cavity.
