Предлагаемый способ лечения РН осуществляется следующим образом. Первым этапом проводят трехкратные инсталляции циклоплегических препаратов (0,5%-раствор тропикамида, 1,0%-раствор циклопентолата и 2,5%-раствор фенилэфрина) за час до процедуры, с интервалом между закапываниями 15-20 минут.The proposed method for the treatment of pH is as follows. Для иммобилизации век используют специальные векорасширители для недоношенных новорожденных. Процедуру коагуляции проводят при помощи роговичной контактной 3-х зеркальной линзы с пятном контакта 13 мм (Ocular instruments) за стационарной щелевой лампой. Наводка лазерного луча на глазное дно осуществляется с помощью метки. При пороговых стадиях активной РН, когда наблюдается распространение экстраретинальной пролиферации на 5 последовательных прилегающих по окружности часовых меридианов, лазерные коагуляты в количестве от 300 и более в виде матричных паттернов 5×5 накладывают на аваскулярную сетчатку от вала к периферии в зоне локализации патологического процесса. Продолжающийся рост пролиферации является показанием для проведения дополнительной процедуры коагуляции относительно патологической зоны сетчатки. При постпороговых стадиях РН, когда на периферии имеется локальная отслойка сетчатки с протяженностью 2-3 часовых меридиана, коагуляты в количестве от 300 и более в виде матричных паттернов 5×5 наносят на аваскулярные участки, а затем в виде матричных паттернов 3×3 накладывают на васкуляризированную часть сетчатки, отступя от места ее отслоения на расстояние 1-1,5 диаметра диска зрительного нерва. Во всех случаях добиваются интенсивности коагулятов 2-й степени по классификации F.L′Esperance [L′Esperance F., 1983]. Мощность излучения варьирует от 120 до 220 мВт, длительность импульса - 0,02-0,03 секунды, диаметр одного коагулята на глазном дне - 100-150 мкм, общее количество коагулятов может достигать 1100-1500. В результате применения предложенного способа достигнуто повышение эффективности лечения детей с активной ретинопатией недоношенных. Преимуществом применения лазерного излучения в желтом спектре (577 нм) является раннее проявление коагулятов на глазном дне, что позволяет контролировать объем и площадь зоны коагуляции; возможность применения при полупрозрачных средах глаза и кровоизлияниях сетчатки; более щадящее действие излучения, исключающее возможность повреждения фоторецепторного слоя сетчатки, особенно при локализации патологического процесса в парацентральной зоне. После проведенного обследования в условиях Уфимского НИИ глазных болезней был поставлен диагноз: OU - ретинопатия недоношенных III стадия, активная фаза. Глазное дно: ДЗН бледно-серый, границы четкие. Сосуды умеренно расширенные, артерии извитые. В III зоне в верхне-наружном секторе (5 часовых меридианов) глазного дна определяется демаркационный вал с экстраретинальной фиброваскулярной пролиферацией. На 38 неделе постконцептуального возраста (ПКВ) была проведена паттерная лазерная коагуляция сетчатки с длиной волны излучения 577 нм. При этом коагуляты в количестве 300 и более в виде матричных паттернов (5×5) наносили на аваскулярную зону сетчатки позади вала в верхне-наружном секторе. Общее число коагулятов составило 900-950. При осмотре через неделю отмечалось дальнейшее прогрессирование и рост пролиферации в нижнем секторе (3 часовых меридиана). Проведена дополнительная ПЛКС аваскулярной зоны в нижнем секторе. Общее число коагулятов составило 1000-1500. Через 3 недели после проведенного лечения отмечалась стабилизация процесса и регресс заболевания.Example 2. ПКВ на момент лечения составлял 42 недели. Проведена паттерная лазерная коагуляция сетчатки с длиной волны излучения 577 нм, при этом коагуляты в количестве 300 и более в виде матричных паттернов 5×5 наносили на аваскулярные участки, а затем в виде матричных паттернов 3×3 накладывали на васкуляризированную часть сетчатки, отступя от места ее отслоения на расстояние 1-1,5 диаметра диска зрительного нерва.
