Острые нарушения кровообращения в сетчатой оболочке относятся к числу наиболее тяжелых форм патологии глаза, приводящих к частичной, а в ряде случаев к полной потери зрения. В последние десятилетия в связи с общим ростом сосудистой патологии отмечается значительное увеличение числа случаев острых нарушений кровообращения органа зрения. Несмотря на огромное количество исследований по этиологии патогенезу и лечению таких состояний до настоящего времени гарантированно помочь больному не представляется возможным.Acute circulatory disorders in the retina are among the most severe forms of eye pathology, leading to partial, and in some cases complete loss of vision. Известно лечение острого нарушения кровообращения путем назначения лекарственных средств - антикоагулянтов, ангиопротекторов, фибринолитических агентов. Однако это лечение опосредованное, к тому же достаточно высок риск возникновения кровоизлияния в стекловидное тело (Л.В.Кацнельсон, Т.И.Форофонова, А.Я.Бунин. Для этого используют фотокоагуляторы - аргон зеленый, ягнеодимовый зеленый, желтый на красителях, диодный инфракрасный в режиме коагуляции. Параметры подбираются индивидуально в зависимости от прозрачности оптических сред, пигментации сетчатки. Коагуляция проводится методом Grid либо комбинированно Grid + коагуляция пораженной зоны. В случае неоваскуляризации осуществляют панретинальную лазерную коагуляцию (Steven M.Bloom, Alexander S.Brocer. Laser Surgery of the Posterior Segment. 1990. - P.112-123). Всем больным до лечения, а также через неделю и месяц после него проводят офтальмологическое обследование: визометрия, биомикроскопия, оптическую когерентную томографию (ОСТ) сетчатки и ДЗН. Перед операцией проводят инстилляционную анестезию Sol.Dicaini 1%. Непосредственно на пораженные сосуды сетчатки проводят лазерную инфракрасную транспупилярную термотерапию при мощности излучения 300-400 мВт, с длиной волны 810 нм, диаметром пятна 0,8-1,2 мм. Длительность воздействия - 1 минута.
