В данном способе хирургическое лечение двухсторонних крыловидных складок шеи осуществляют следующим образом: основной разрез кожи проводят строго по гребню - свободному краю складки от сосцевидного отростка до основания складки на надплечье. Первый дополнительный разрез ведут кпереди и медиально под углом 60 градусов параллельно надплечью. Второй дополнительный разрез от сосцевидного отростка ведут также под углом 60 градусов в направлении задней поверхности шеи и надплечья латерально. Длина дополнительных разрезов должна быть равной. В результате образуются симметричные треугольные лоскуты, которые препаруют и формируют во всю толщу вместе с подкожной жировой клетчаткой и перемещают их встречно, при этом в ране иссекают фиброзно измененную жировую клетчатку, соединительнотканные тяжи и формируют контуры шеи. Вершину заднего лоскута перемещают кпереди, а переднего кзади и подшивают к концам разрезов. В результате раневой дефект закрывается встречными треугольными кожными лоскутами без натяжения и деформации, формируя контуры шеи. Способ осуществляют следующим образом.The method is as follows.
Под общим обезболиванием в положении больного на животе формируют трапециевидные кожные лоскуты по задней поверхности шеи. Для этого выполняют первый продольный разрез кожи, подкожной клетчатки и поверхностной фасции по средней линии шеи от большого затылочного бугра до остистого отростка 12 шейного позвонка. Второй поперечный разрез длиной 8-10 см осуществляют на уровне затылочного бугра, а третий длиной 10-12 см - на уровне остистого отростка 12 позвонка. Длина поперечных разрезов зависит от размера больного. Отслаивают кожные лоскуты в обе стороны шеи, разъединяя дубликатуры кожной складки. Затем формируют контуры шеи. При этом с незначительным натяжением соединяют кожные лоскуты по средней линии шеи, удалив избыток кожи на всем протяжении. Избыток кожи составляет до 4-5 см. Зашивают продольные разрезы. Такое подшивание обеспечивает хорошее кровоснабжение задней поверхности шеи, в результате чего происходит заживление ран первичным натяжением с мягким нежным косметическим рубцом. Затем сформировали шею, при этом расправили на всю толщу лоскутов кожные складки шеи, ликвидировали гемостаз по ходу операции, иссекли избыток лоскутов до 3 см. Наложили швы на кожу.
