Изобретение относится к реконструктивно-пластической хирургии в челюстно-лицевой хирургии, а именно к ринопластике, и может быть использовано для реконструкции рубцово-измененных дефектов кончика носа.The invention relates to reconstructive plastic surgery in maxillofacial surgery, namely rhinoplasty, and can be used for reconstruction of scar-modified defects of the tip of the nose.
Известен способ пластики дефекта кончика носа лоскутом с носогубной складки (патент РФ 2780132). В основе способа лежит формирование кожно-жировых лоскутов в области носогубной складки справа и слева и их перемещение в область дефекта кончика носа. Таким образом, послеоперационный рубец донорской зоны укрыт в складку латерального края крыла носа и носогубную складку. Однако данный способ имеет существенный недостаток: его использование при рубцово-изменённых дефектах кончика носа может привести к уменьшению объема донорского материала и возможности его перемещения в зону дефекта в связи с дополнительным иссечением рубцов.There is a known method for plastic surgery of a defect in the tip of the nose using a flap from the nasolabial fold (RF patent 2780132). Известен способ хирургической реконструкции кончика носа с использованием парамедианного лоскута лба (Пейпл А.Д. Пластическая и реконструктивная хирургия лица. Москва. 2007; с. 648-649). Способ включает в себя два этапа. Первым этапом выполняют формирование кожно-фасциального лобного лоскута с питанием на одной из надблоковых артерий. Форма лоскута в дистальном его отделе должна соответствовать по размеру и форме дефекту наружного носа. Далее сформированный лоскут ротируется в область дефекта примерно на 180° вокруг ножки в медиальном направлении. Дистальные отделы лоскута фиксируют узловыми швами по периметру дефекта. При заборе лоскута шириной до 3 см края донорской раны сопоставляются, и рана ушивается послойно наглухо с формированием в последующем длинного линейного рубца в лобной области. Второй этап проводится через три-четыре недели. Избыточные ткани с питающей ножкой лоскута резецируют, края раны освежают и закрывают донорский дефект окончательно. Данный способ позволяет восстановить обширные и сквозные дефекты наружного носа как с хрящевыми трансплантатами, так и без них. Описанный способ имеет ряд недостатков: методика является двухэтапной, заживление верхней части донорского дефекта вторичным натяжением, косметический дефект расположен на видном месте; возможна асимметрия контуров лица.There is a known method of surgical reconstruction of the tip of the nose using a paramedian forehead flap (Paple A.D. The distal sections of the flap are fixed with interrupted sutures around the perimeter of the defect. When harvesting a flap up to 3 cm wide, the edges of the donor wound are compared, and the wound is sutured tightly in layers, subsequently forming a long linear scar in the frontal region. The second stage takes place after three to four weeks. Excess tissue with the feeding pedicle of the flap is resected, the edges of the wound are refreshed and the donor defect is completely closed. This method allows you to restore extensive and through defects of the external nose, both with and without cartilage grafts. The described method has a number of disadvantages: the technique is two-stage, the upper part of the donor defect is healed by secondary intention, the cosmetic defect is located in a visible place; asymmetry of facial contours is possible.

Указанный результат достигается выкраиванием и опрокидыванием двух полнослойных кожных лоскутов - опрокидывающегося в области спинки носа и скользящего со лба, и фиксацией углов лоскута к внутренней поверхности основания крыльев носа. Способ осуществляется следующим образом. Пациенту предварительно производят измерения дефекта носа с целью дальнейшего моделирования кончика носа, приближенного к его первоначальной форме, используя фотодокументы до травмы. После нанесения предоперационной разметки производят дугообразный разрез кожи от верхней трети крыла носа справа, через спинку носа к верхней трети крыла носа слева, отступя от имеющегося рубцово-изменённого края дефекта по направлению к спинке носа на 1,5 см. Выкраивают и мобилизуют полнослойный кожный лоскут дугообразной формы в области спинки носа, соответственно размеру дефекта (1,5 см) и предварительно проведённым антропометрическим измерениям. Основанием данного лоскута является рубцово-измененный край дефекта. Ткани мобилизуют, лоскут опрокидывают в область имеющегося дефекта кончика носа, осуществляют гемостаз. С учётом опрокинутого лоскута в области спинки, кончика и крыльев носа образуется дефект размером 3,0×3,0 см.The method is carried out as follows. Длина разрезов равна трёхкратному вертикальному размеру дефекта (9,0 см). Выкроенный полнослойный кожный лоскут размером 9,0×3,0 см мобилизуют, производят гемостаз. Лоскут перемещают в область дефекта, фиксируют направляющими узловыми швами из резорбируемой полифиламентной нити 4,0. При этом углы лоскута фиксируют к внутренней поверхности основания крыльев носа. Края кожного лоскута фиксируют узловыми швами из монофиламентной нерезорбируемой мононити 4,0. Таким образом, дефект кончика носа устранен в один этап с использованием двух лоскутов - опрокидывающегося со спинки носа и скользящего со лба. Пациент Т., 1978 г., поступил в отделение челюстно-лицевой хирургии с жалобами на наличие дефекта в области кончика носа. Из анамнеза: травма в быту, укушенная рана. При осмотре: определяется дефект мягких тканей в области кончика носа с вовлечением подлежащего хряща неправильной формы 1,5×2,0 см. Дном дефекта является рубцовая ткань розового цвета, без отделяемого, гладкая, безболезненная при пальпации. Носовые ходы справа и слева проходимы, дыхание через них свободное. Диагноз: обширный дефект тканей кончика носа (М95). Patient T., 1978, was admitted to the department of maxillofacial surgery with complaints of a defect in the tip of the nose. Под ЭТН, отступя от имеющегося края дефекта на 1,5 см от верхней трети крыла носа справа через спинку носа к верхней трети крыла носа слева, произведен дугообразный разрез кожи (фиг. 1). Отсепарован полнослойный кожный лоскут, ткани мобилизованы, лоскут опрокинут, гемостаз методом коагуляции (фиг. 2). В области спинки кончика и крыльев носа образовался дефект размером 3,0×3,0 см (фиг. 3). Произведено 2 вертикальных разреза от верхней трети крыла носа справа и слева вдоль спинки носа с переходом на лобную область длиной 9 см (фиг. 4). Выкроен полнослойный лоскут размером 9,0×3,0 см, мобилизован, гемостаз методом коагуляции (фиг. 5). Лоскут перемещён в область дефекта и фиксирован погружными швами сабфилом 4,0 (фиг. 6); на кожу - швы этилоном 4,0 (фиг. 7). Таким образом, дефект кончика носа устранен с использованием опрокидывающегося лоскута со спинки носа и скользящего лоскута со лба (фиг. 8 а,б,в). Послеоперационный период протекал без осложнений, заживление раны первичным натяжением. Через 10 дней швы были сняты, пациент выписан с выздоровлением, социально адаптирован. Результат лечения через 1 месяц представлен на фиг. 9 а,б,в,г. A full-thickness flap measuring 9.0×3.0 cm was cut out, mobilized, hemostasis using the coagulation method (Fig. 5). The flap was moved to the defect area and fixed with submersible sutures using 4.0 subfil (Fig. 6); on the skin - stitches with ethylene 4.0 (Fig. 7). Thus, the nasal tip defect was eliminated using a tipping flap from the dorsum of the nose and a sliding flap from the forehead (Fig. 8 a, b, c). The postoperative period proceeded without complications, the wound healed by primary intention. After 10 days, the stitches were removed, the patient was discharged with recovery, and socially adapted. The result of treatment after 1 month is presented in Fig. 9 a,b,c,d.
Таким образом, предлагаемый способ позволяет провести одноэтапное оперативное лечение, избежать возможной асимметрии лица и косметического дефекта донорской зоны; добиться симметрии кончика носа и хорошего эстетического внешнего вида; использовать прилежащие к области дефекта ткани, обладающие идентичными характеристикам для лучшего косметического результата; сократить период нетрудоспособности и реабилитации пациента.Thus, the proposed method allows for one-stage surgical treatment, avoiding possible facial asymmetry and cosmetic defects in the donor area; achieve symmetry of the tip of the nose and a good aesthetic appearance; use tissue adjacent to the defect area that has identical characteristics for a better cosmetic result; shorten the period of incapacity and rehabilitation of the patient.
