Abstract
Description

Из практической медицины для лечения кожной формы синдактилии известен способ (Джанелидзе Ю.Ю. Пересадка кожи с помощью длинных трубчатых полосок. - Нов. хир. архив. - 1924. - 3., С.22), который состоит в следующем: после линейного разделения пальцев в целях предупреждения сморщивания кожи производится свободная ее пересадка. Однако недостатком данного способа является то, что при этом приходится брать кожу с какого-то определенного участка тела, нанося дополнительную травму пациенту, кожа не всегда хорошо приживается в результате ее некроза.From practical medicine for the treatment of the skin form of syndactyly, a method is known (Dzhanelidze Yu. При этом для разделения пальцев формируют треугольные лоскуты, затем дефект одного из пальцев закрывают собственной кожей этого пальца, а для устранения дефекта на другом пальце используют свободную пересадку кожи. Поставленный технический результат достигается тем, что в способе оперативного лечения кожной формы синдактили, включающем пилообразные разрезы, последующее перемещение треугольных кожных лоскутов навстречу друг другу с закрытием кожного дефекта местными тканями - при формировании треугольных лоскутов разрезы проводят до края пальца.The technical result is achieved by the fact that in the method of surgical treatment of the skin form of syndactyly, including sawtooth incisions, the subsequent movement of the triangular skin flaps towards each other with the closure of the skin defect by local tissues - when forming triangular flaps, the incisions are made to the edge of the finger.
Способ осуществляют следующим образом. Вначале двумя инъекционными иглами, проведенными через сращение между пальцами, обозначают основания треугольных лоскутов. Затем, как показано на схеме (рис.1), выкраивают на каждом пальце по два треугольных лоскута, таким образом, чтобы при встречном их перемещении можно было закрыть дефект кожи на соседнем пальце, причем разрезы проводят не до середины, как в предыдущих методиках, а до края пальца, что обеспечивает большую растяжимость лоскутов. Большая податливость кожных лоскутов достигается, также, за счет того, что треугольные лоскуты межпальцевого промежутка выкраиваются более проксимально, чем это было принято ранее, и за счет лучшей смещаемости кожи тыла кисти, что позволяет закрыть большой участок дефекта между пальцами. Тыльный и ладонный треугольные лоскуты сшивают по типу “ласточкина хвоста” (рис.2).The method is as follows. Клинический пример 1. Больной И., 1996 г.р. (2 года 5 месяцев) поступил в клинику с диагнозом: кожная синдактилия II и III пальцев правой кисти. В связи с этим заболеванием 23 марта 1999 года выполнено разделение II и III пальцев правой кисти с последующей зигзагообразной кожной пластикой местными тканями. Операция выполнена под внутривенным наркозом. На среднюю треть предплечья наложен жгут. Через сращение между пальцами проведены 2 инъекционные иглы для обозначения оснований треугольников. Затем на каждом пальце выкроены по 2 треугольных лоскута, разрезы при этом произведены до краев пальцев. Пальцы разъединены. Иссечен избыток жировой клетчатки. Лоскуты на каждом пальце перемещены навстречу друг другу, пришиты по краям дефектов. Межпальцевой промежуток ушит по типу “ласточкина хвоста”. Наложена асептическая повязка. Иммобилизация гипсовой лонгетой. Послеоперационный период протекал без осложнений. Рана зажила первичным натяжением.Clinical example 1. Через год после операции у больного жалоб нет. По поводу данной патологии 25 января 1999 года было выполнено разделение II и III пальцев левой стопы с последующей зигзагообразной пластикой местными тканями. На среднюю треть голени наложен жгут. Проведены две инъекционные иглы через сращение между пальцами для обозначения оснований треугольников. Затем на каждом пальце выкроены по два треугольных лоскута, разрезы при этом произведены до краев пальцев. пальцы разъединены. Иссечен избыток жировой клетчатки. Лоскуты на каждом пальцы перемещены навстречу друг другу, пришиты по краям дефектов. Межпальцевой промежуток ушит по типу “хвоста ласточки”. Наложена асептическая повязка.
