Abstract
Description
The invention relates to medicine, namely to physiotherapy exercises, and can be used for the treatment and prevention of manifestations of osteochondrosis.
The prior art methods of treating manifestations of osteochondrosis by means of massage, manual therapy and physical therapy (RU 2034530, RU 2005456), which are aimed at restoring mobility of the entire spine in general and lumbar in particular.
As the closest analogue, we consider a method for the treatment of lumbar osteochondrosis (AS 1562998), which includes manual therapy (massage, postisometric muscle relaxation and mobilization of blocked vertebral motor segments (PDS), as well as subsequent laser puncture and methods of auto-mobilization or auto-stabilization. methods of auto-stabilization of hypermobile vertebral motor segments and auto-mobilization blocked as exercises, it is based on the idea of bone -muscular nature of the manifestations of osteochondrosis without taking into account the leading participation in the nervous system. This leads to the conclusion that relaxation of overstressed is sufficient and toned muscles are relaxed and posture is brought to template (actually youthful) standards to solve the problem.Muscular tone disorders are considered as accumulation of muscle fatigue due to long-term existing errors in the form of irrational postures and movements: the musculoskeletal system using manual therapy, including post-isometric relaxation, Methods for automobilizatsii and autostabilizatsii is given to the physiological norm, and the patient returns allegedly initially correct movement patterns, serving human needs throughout their lives.
The disadvantage of this method is the lack of accounting for the inevitable age-related changes expressed in the degeneration of the intervertebral discs, a decrease in their height, sagging segmental ligaments, the appearance of vertebral hypermobility in the segment, the appearance of a fundamentally new phenomenon that is impossible in childhood and adolescence, extrusion of a morphologically changed intervertebral disc towards the spinal canal extension and twisting of the vertebrae of the segment. Removing functional blocks and eliminating pain with laser therapy returns spinal mobility (in fact, hypermobility, since it is not the youthful spine, but the spine is in a state of osteochondrosis, or dorsopathy, and the intervertebral discs are degenerated), the temporary absence of pain allows you to restore the previously existing stereotype, including bending, extension and twisting of the spine, which inevitably leads to the extrusion of a morphologically altered disk towards the spinal canal, as well as avmatizatsiyu spinal roots and facet joints due to appearing after removal of the functional blocks segmental hypermobility. That is why all treatment methods (including gymnastic exercises) aimed at restoring normal spinal mobility - that is, restoring the motor stereotype characteristic of a person before degeneration of the intervertebral disc, lead to a worsening of the condition and an increase in the manifestations of osteochondrosis.
п.The claimed method takes into account the fact that age-related changes are mandatory, statistically significantly occurring in the spine at the end of growth. The method aims at creating a motor stereotype that protects the nervous, vascular and connective tissue structures that are inevitably damaged by movements that are standard for the period when the spine did not remain in the age state of osteochondrosis. First of all, the most important, neurological aspect of the problem is solved, when nerves and blood vessels are mechanically injured due to squeezing the intervertebral disc into the lumen of the spinal canal, which leads to the most serious manifestations of osteochondrosis, such as myeloradiculoischemia with severe pain, paresis of the foot, pelvic disorders, etc. .P.
The method is based on a fundamental idea of osteochondrosis (dorsopathy, degenerative disc disease) as a natural age-related process that programmatically changes the bone-cartilage structures of the spine. The new condition of the spine excludes a return to the motor stereotype, which was adequate to the youthful spine, and requires a new motor stereotype that protects the nervous and vascular structures from fragments of the naturally destroyed intervertebral disc falling into the lumen of the spinal canal.
With the help of functional stabilization of the spine, movements that squeeze the intervertebral disc in the direction of the spinal canal are eliminated and manifestations of osteochondrosis are prevented. In addition, it should be borne in mind that osteochondrosis (dorsopathy) is always hypermobility, because a decrease in the height of the intervertebral disc leads to sagging of the segmental ligamentous apparatus and increased mobility of the vertebrae, and it is local hypermobility that creates the conditions for reflex muscle spasm and functional blocking of the segment. Removing the functional block again restores the segment's hypermobility, which leads to trauma to the spinal nerves emerging from the intervertebral holes, and to trauma to the articular facets of the arched joints. At the same time, the removal of functional blocks is necessary to protect the spine from the pathological consequences of overstrain of segmental muscles and infringement of the meniscoid tissue in the arched joints. Protective pathogenetic fixation of the vertebrae of the segment is replaced by a specially developed protective functional stabilization at the level of the entire spine, which protects the patient from self-injury and avoids segmental functional blocks with their characteristic pathological manifestations.
The purpose of the method is not to build muscle to create a muscle corset, not to remove functional blocks to restore spinal mobility, but to compensate for functional insufficiency of the ligamentous apparatus of the spine by means of functional muscle stabilization on a permanent, permanent basis.
The gymnastics used by the method is coordination and is aimed at consolidating the skill of constant (permanent) stabilization of the spine at the level of the lumbar region with removed pathological functional blocks. Gymnastics only illustrates examples of permanent protective coordination and helps to consolidate a protective motor skill, and a person’s physical culture consists in the fact that any movement and posture is a physical exercise that trains and strengthens the skill of safely solving a motor task.
Thus, permanent stabilization of the spine by the alternative locomotion method (raising a protective motor stereotype, protective coordination, creating a functional motor corset) eliminates the triggering factor for the occurrence of manifestations of osteochondrosis (dorsopathy) and solves the problem of treating these manifestations and preventing them if they are possible. Permanent stabilization of the spine, becoming the basis of the motor stereotype, allows the patient to solve any domestic motor tasks without the risk of osteochondrosis. Permanent stabilization of the spine in the form of physical culture preserves and increases the physical activity of the patient, allowing you to effectively deal with physical inactivity without the risk of osteochondrosis. Classes using the PSP method in physical education groups increase the emotional background, self-esteem of patients, which positively affects their quality of life.
In the claimed method, treatment and prophylaxis of the manifestations of lumbar osteochondrosis are carried out by removing functional blocks by the method of manual therapy with simultaneous functional stabilization of the lumbar spine. At the same time, a new motor stereotype is formed in stages, which excludes flexion, extension of the lumbar spine in all planes, as well as twisting during physical exertion and solving everyday motor problems. The lumbar spine is stabilized along the longitudinal axis of the body due to muscle tension in the body. The formation of a new motor stereotype is possible by performing a set of gymnastic exercises. The complex may include various exercises, in particular the following: the exercise is performed lying on the bed: lying on your back, hands at the seams, breathe evenly, deeply and calmly. Slowly, with the whole body, without twisting the spine, they turn sideways, lying on their sides, the upper arm is placed arbitrarily along the body, and the lower arm rests on the edge of the bed, resting the whole body on the lower arm, bend the knees at an angle of 90 degrees relative to the line of the spine, after which they lower their feet to the floor, and, slowly, resting on their hand, lift their bodies and sit on the edge of the bed. In this case, the shoulder girdle should be kept in a constant position relative to the pelvis, sitting on the edge of the bed, the right foot is brought forward, and the left foot is placed underneath, raised by putting the pelvis under the shoulders, keeping the back line stable, while the spine is placed in a vertical position relative to the support . Combine half-bent legs and bring the pelvis under the shoulders, straighten the legs, occupying an upright position. Another exercise is also performed on the bed: starting position - lying on his stomach, arms under his head, face on his hands. Both arms are lifted above themselves, then with the whole body, without twisting the spine, they make a turn on their side, lying on their sides the upper arm is laid along the line of the body, and with the lower hand rest on the edge of the bed, resting with the whole body on the lower arm, bend the knees at an angle of 90 degrees relative to the line of the spine. After that, they lower their legs to the floor, and, resting on their arm, raise their bodies and sit on the edge of the bed, while maintaining a stable position of the shoulder girdle relative to the pelvis. Sitting on the edge of the bed, bring the right foot forward, and put the left foot underneath; rise by removing the pelvis under the line of the shoulders, while the spine is displayed in a vertical position relative to the support.
The complex may also include an exercise performed as follows: starting position - standing, legs shoulder-width apart, arms at the seams, the pelvis located under the shoulder line, bend the left foot at the knee and take the right foot back a distance of one and a half shoulder widths, with the knee should be above the big toe of the left foot. Slowly lowering, they take the pelvis back beyond the support area, maintaining a stable position of the shoulder girdle relative to the pelvis, dropping to the lower position, they are delayed for 5-10 seconds, then they begin to rise by straightening the left leg in the knee and bringing the right leg forward, the pelvis is located under the line of shoulders, occupy the starting position. In addition, they form a new motor stereotype when boarding a car: starting position - standing next to a passenger car, bend your knees and bring your pelvis back beyond the support area, leaning in, you should maintain a stable position of the shoulder girdle relative to the pelvis, sit down, then turn the body into familiar position in the car. And they also form a new motor stereotype when getting out of the car: with the whole body, without twisting the spine, they make a turn to the exit. After that, both legs are taken out and lowered from the car to the ground, and, resting on the hands, the pelvis is moved to the edge of the seat, while the position of the shoulder girdle relative to the pelvis should be kept unchanged, one leg should be brought forward, and the other leg should be placed next to the car. They are raised by removing the pelvis under the shoulder line, keeping the back line stable, while the spine is displayed in a vertical position relative to the support.
A set of gymnastic exercises may also include pole exercises. I.p. standing, feet shoulder-width apart, a pole held in the hands, the pelvis is located under the shoulder line, bend the left foot at the knee and take the right foot back at a distance of one shoulder width, while the knee of the left foot should be above the big toe of the left foot, right and left the legs rest completely on the feet, the right hand covers the pole and presses it to the hip, the left hand holds the pole in the middle in front of you, the tip of the pole points forward and is at the neck level. Perform a sliding movement with the left foot from the right supporting leg, keeping the back line stable, while the spine is in a vertical position relative to the support, and the pelvis is located under the shoulder line, the right leg is pulled back to the same distance of the same shoulder width, at the same time as the movement, both hands move pole forward, while the lower hand fixes the tip of the pole at the level of the solar plexus, repeat the sliding movement forward, returning to its original position, combined with the movement of the pole for 10 minutes.
According to modern concepts, the human spinal column is a primary-stressed structure, the internal tension of which is provided by the tension of the anterior and posterior longitudinal vertebral ligaments, and the external one is practically incompressible (90% fluid), pushing the vertebral bodies by the gelatinous (pulpous) nucleus of the intervertebral disc (MTD). At the same time, the vertebral column (PS) is a mobile, resistant to external influences motor system that provides support function for the human body and a protective function for the neuromedullary apparatus (spinal cord, spinal roots, ganglia, nerves, blood vessels). The functional unit of PS is the vertebral motor segment (PDS), which consists of adjacent vertebrae, intervertebral disc, segmental ligamentous apparatus (yellow, interspinous, intervertebral ligaments, articular bags of the arched joints) and segmental intervertebral muscles. Violation of the motor function of the vertebral motor segment is the main reason for the development of osteochondrosis of the spinal column (OPS) N.A. Zagorskaya, BC Goidenko, 1998 (in foreign sources, OPS is referred to as degeneration of the intervertebral disc, and a disease associated with degeneration of the disc is called a denatured disc disease - degenirative disc desease). According to the above concept, if VDS does not move, OPS will occur (i.e., osteochondrosis occurs as a result of physical inactivity, and active mobility in the VDS prevents OPS).
However, in our opinion, the causal relationship is the opposite, that is, osteochondrosis (OPS) of the spinal column, spontaneously developing as a genetic aging program, is a cause of dysfunction of the vertebral motor segment of the PDS, in other words, osteochondrosis develops first, and as a result, normal motor function of the spinal column.
Такая картина наблюдается в массовом порядке у гимнасток, артистов балета, цирковых артистов, демонстрирующих номера типа "каучук" или "человек-змея", а также у спортсменов и физкультурников, усиленно работающих над гибкостью позвоночного столба во время общефизической подготовки (ОФП).Moreover, special work on the preservation and enhancement of mobility in PDS leads to early manifestations of aging in the form of a disease (premature manifestations of osteochondrosis with the formation of spondylarthrosis and protrusion of the intervertebral discs into the lumen of the spinal canal with all the syndromes inherent in these processes). This picture is observed in droves by gymnasts, ballet dancers, circus performers demonstrating numbers such as "rubber" or "snake-man", as well as among athletes and athletes who work hard on the flexibility of the spinal column during general physical training (GP).
This knowledge defines a fundamentally new approach to the treatment and prevention of manifestations of OPS. You should not go against nature, but help it by fixing the lumbar spine in a timely manner with protective muscle coordination, which means consciously creating a new protective motor stereotype as the basis for human postures and movements at any point in space (this is different from private conservation tips lower back in a particular risk situation). These are effective, rapid movements of a person, allowing to solve any motor problem with an always stable lower back. The point is not in small amplitudes and slow movements of gymnastics, but in targeted coordination stabilization of the lower back at any amplitudes and body speeds. This approach fundamentally changes the goals and objectives of existing health and sports disciplines, supposedly "returning youth and flexibility of the spine", but in reality faster or slower leading to illness. For manifestations of osteochondrosis, osteochondrosis per se is a necessary but not sufficient factor. The trigger moment that triggers the onset of the disease is a movement that does not take into account the existence of osteochondrosis. Two mechanisms of occurrence of manifestations of osteochondrosis are known, and in both, the mechanical factor is the starting point.
1st mechanism:
Г.С. Е. 1973.“A decrease in the height of the intervertebral space and (as a result) pathological mobility between the vertebral bodies (degenerative spondylolisthesis) lead to a subluxation of the articular processes in this segment. Due to the constant trauma of these processes, spondylarthrosis will inevitably develop. "G.S. Yumashev, M.E. Furman. 1973.
Further, trauma to the articular processes leads to the inflammatory process of the articular surfaces-facets with a sharp pain syndrome, pathological myofixation of the compromised motor segment.
Manual therapy, in our opinion, removes blockages in the PDS associated with “subluxations”, and coordination muscle stabilization of the PDS prevents trauma to the articular surfaces, which prevents the development of spondylarthrosis and spondylitis with inflammation of the articular surfaces of the facets and, therefore, prevents the occurrence of pain spreading from the articular surfaces on the corresponding areas of bones, muscles, ligaments, skin (pain on the osteotome, myotoma, sclerotoma and dermatoma).
2nd mechanism:
At the vertebra up and down from its arc, paired upper and lower articular processes depart. The base of the articular processes forms the upper and lower vertebral notches. In the vertebral segment, where two vertebrae are adjacent, the lower notch of the superior vertebra and the upper notch of the inferior vertebra form intervertebral openings through which the spinal nerves formed by the roots and the blood vessels pass. Loss of disk fluid leads to a decrease in its height, and it no longer serves as a full-fledged spacer between the two vertebrae. The ligaments connecting the vertebrae and stretched by a spacer disk, holding the displacement of the vertebrae in the horizontal plane, while sagging and stop holding the vertebrae, giving them an additional amplitude of horizontal displacement. The intervertebral openings are periodically deformed due to the mutual displacement of the vertebrae, their diameter decreases, and the spinal nerves and blood vessels passing there are mechanically compressed (impaired). This injures the spinal nerves formed by the nerve roots, the spinal nerves, and the blood vessels that leave the vertebral openings, which causes traumatic damage to the nerves and blood vessels, and disorders called radiculitis, radiculopathy, and lumbar ischialgia.
Stabilization of PDS by the upbringing of a protective motor stereotype, functional muscle corset, protective coordination, gymnastics stops the traumatization of the spinal nerves and stops the development of the pathological process, which is a treatment in the case of developed pathology and prevention in the event of a temporary absence.
3rd gear
The upper and lower articular processes in the vertebral motor segment form joints between them, mating with articular surfaces (articular facets). A decrease in the height of the intervertebral disc with fluid loss leads to an increase in the amplitude of the mutual displacement of the vertebrae above the physiological volumes and the displacement of the facets. The displacement of the facets in these joints causes infringement and trauma of the meniscoid tissue located there, rich in nerve endings, and with prolonged trauma, inflammation of the joint with clinical manifestations in the form of pain.
Stabilization of the PDS (protective motor stereotype, muscle corset, protective coordination, gymnastics) stops the pathological displacement of the articular facets, causing trauma to the joints, and stops the development of the pathological process, which is a treatment in the event of its development and prevention in the absence of such.
Symptoms of osteochondrosis reduce physical and social activity, cause sleep disturbance, worsen the quality of life of patients and cause financial costs. Despite the development of effective methods of treatment and rehabilitation, osteochondrosis remains a serious medical and social problem due to the high prevalence in the population, progressive course, high frequency of damage to people of working age, early occurrence of reduced functional abilities, loss of professional and social skills, and significant disability of patients. In connection with age-related degeneration of the intervertebral discs (osteochorosis), instability of the vertebral motor segment appears, as well as a tendency to extrude the intervertebral disc towards the spinal canal during flexion-extensor and rotational movements in the segment.
As a result, there is a mechanical compression of the neurovascular structures of the spinal canal with the occurrence of lumbar ischialgia, radiculitis, myeloradiculi ischemia with paresis of the feet, spinal strokes with paresis of the legs and pelvic dysfunction. Thus, the trigger mechanism for osteochondrosis complications is the mechanical factor (flexion-extensor and rotational movements in the segment). If these movements are excluded from the motor stereotype of a person who is the carrier of osteochondrosis (osteochondrosis is a natural age process), biological complications of osteochondrosis can be avoided.
The exclusion of the mechanical link of triggering complications of osteochondrosis leads to their prevention and contributes to the success of the treatment of an existing disease.
Having excluded dangerous movements in the lumbar spine, it is necessary to compensate for them with the movements of other parts of the musculoskeletal system, in connection with which a new motor stereotype arises, called the protective one.
The method differs from existing treatment methods in that it does not aim to restore the active mobility of the lumbar spine, which in the presence of compromised vertebral motor segments inevitably leads to repeated injuries of the meniscus tissue and compression of the neurovascular structures by intervertebral disc fragments with the obligatory start of the pathological process.
Physiotherapy exercises and traditional methods of manual therapy are aimed at restoring the “normal” motor stereotype, patterned for the youthful age, when there are no clinically significant degenerative changes in the intervertebral discs
In the existing methods of exercise therapy, it is practiced to gradually, as the pain syndrome stops, extending to all parts of the musculoskeletal system, restore the previous mobility, which with compromised vertebral motor segments inevitably leads to repeated pinching of the meniscus tissue and compression of the neurovascular structures by fragments of the intervertebral discs with mandatory the launch of the pathological process. Thus, the end result of existing methods of exercise therapy is an exacerbation of the disease.
The method of developing a protective motor stereotype is similar to the methods practiced in manual therapy, in that the removal of functional blocks and the restoration of passive mobility of the vertebral motor segments are used to decompress meniscus tissues, neurovascular structures and relieve pathological muscle spasm.
The method differs from the formation of the "optimal motor stereotype" practiced in manual therapy in that it does not aim to restore the mobility of the lumbar motor segments in active movements, it is not intended to return the motor stereotype of a patient with lumbar osteochondrosis to a youthful pattern as the pain aggravation is removed (therefore that the intervertebral discs do not correspond to the youthful state).
The youthful motor stereotype adopted as the standard, considered the only correct and normal, starting from the age of clinically significant degenerative changes in the intervertebral discs, is the source of the disease.
Thus, the theoretical basis for protective coordination is the provision that morphological changes in the intervertebral discs of the vertebral motor segments are a consequence of natural age-related processes, and therefore are not subject to clinically significant regeneration phenomena. Morphological changes in the intervertebral discs ultimately lead to mechanical compression of the neurovascular structures by the elements of the musculoskeletal system. Thus, if it is impossible to clinically significantly resist age-related biochemical and morphological changes in the musculoskeletal system, it makes sense to prevent the development of the pathological process by mechanically protecting the neurovascular structures from the bone-cartilage elements of the biokinematic chain.
The method of muscular stabilization of the spine in the form of a protective motor stereotype reliably fixes the spine, does not need additional devices, does not limit physiological needs and is always available to a trained patient.
Exercise
1. At the very beginning it is necessary to learn to understand the differences between bending and bending, straightening and bending. You should also learn to bend, not bending, and to straighten, not bending. In this case, the back as a functional unit can interact with other functional units: arms, shoulders, hips and legs when solving everyday and sports motor tasks.
2. The implementation of exercises involves maintaining a comfortable position when performed in a moderately limited amount available to the patient.
3. All movements are performed while maintaining the vertical axial load, which avoids excessive loads on the spine.
4. Movements are performed due to the hip, knee, ankle joints, which allows you to relax the back muscles and prevents trauma to the tissues of the affected PDS.

2. Slowly lower the height of the body by bending the left leg in the knee and slowly pulling the right leg back to a distance of about one shoulder width, while the knee of the left leg should be above the big toe of the left leg.
3. The right and left legs rest completely on the feet.
4. The area of the support is the shape of an isosceles triangle.
5. With the right hand, we embrace the pole and press it to the hip.
6. The left hand holds the pole in the middle in front of you.
7. The tip of the pole points forward and is at neck level.
8. We make a sliding movement with the front foot from the back supporting leg, keeping the back line stable, while the spine is in a vertical position relative to the support, and the pelvic line is located under the shoulder line.
9. The knee of the left foot should remain above the big toe of the left foot. The right leg is pulled back to the same distance, about the same shoulder width.
10. At the moment of movement, the legs rest completely on the feet.
11. Simultaneously with our movement with both hands we carry out the movement of the pole forward, while the lower hand fixes the tip of the pole at the level of the solar plexus.
12. Repeat the sliding forward movement (returning to the starting position) combined with the movement of the pole for 10 minutes, breathing smoothly, deeply and calmly, looking ahead.
Approbation of work.
1. A study of 509 patients with lumbar pain syndrome using a complex method, including questionnaires, neurological examination, manual testing, radiography, electromyography, magnetic resonance imaging, showed that the absence of a motor stereotype adjustment adequate to age-related changes in the spine is the cause of lumbar pain.
For the prevention and treatment of lumbar pain, a method of protective coordination has been developed (a way to educate an analgesic protective motor stereotype), which is a system of motor skills adapted to the conditions of domestic and labor activity and aimed at functional stabilization of the vertebral motor segments of the lumbar. According to the five-point digital rating scale and visual analogue scale of pain characteristics, the frequency of positive outcomes using the method of protective coordination was 87%, using traditional therapy 54% (p <0.001)
Observation of patients for 3 years established the absence of complaints and pathological neurological symptoms in 66% of patients using the method of protective coordination, and only 26% of patients with standard treatment (p <0.05)
45%.5. According to magnetic resonance imaging, the phenomenon of protrusion of the intervertebral disc, which was observed in 25% of patients by the beginning of the observation, during the observation period decreased in patients who used the method of protective coordination to 20%, in people involved in standard methods of prevention, this indicator increased to 45 %
Practical value
40%.Subjective assessment of the effectiveness of their treatment by patients practicing a protective motor stereotype during the first 3 months coincided with the results of independent studies: 88% of those tested reported a decrease in pain symptoms, there was a qualitative improvement in such indicators as working capacity - up to 35%, quality of life - by 100 %, exercise tolerance increased to 40%.
