Abstract
Description
Изобретение относится к области реконструктивной травматологии, т.е. к способам реконструкции коленного сустава при кистозном поражении мыщелка большеберцовой кости. Может быть использовано в специализированных отделениях реконструктивной микрохирургии. The invention relates to the field of reconstructive traumatology, i.e. to methods of reconstruction of the knee joint with cystic lesion of the condyle of the tibia. It can be used in specialized departments of reconstructive microsurgery.
Наиболее распространенным методом лечения кистозного поражения мыщелков большеберцовой кости является эндопротезирование коленного сустава. Однако данный способ лечения является органоуносящим, не дает долговременного функционального эффекта в силу анатомических и биомеханических особенностей, и не позволяет впоследствии выполнять хирургические вмешательства для сохранения функции конечности (1). В свою очередь, недостаточно только создать анатомичные взаимоотношения в суставе, необходимо создать условия для предотвращения образования и дальнейшего развития кистозного процесса. Такие условия создают путем улучшения питания пораженного костного участка за счет дополнительного источника кровоснабжения в виде микрохирургического мышечно-надкостнично-кортикального аутотрансплантата. Для остеогенетического и реваскуляризирующего влияния микрохирургического аутотрансплантата требуется создание хорошей контактной поверхности, что достигается путем формирования ложа по форме костной части микрохирургического аутотрансплантата на нехрящевой поверхности пораженного мыщелка. У больного с кистозным поражением мыщелка большеберцовой кости осуществляют доступ, выполняют остеотомию здорового мыщелка большеберцовой кости, перемещают и фиксируют его, создавая конгруэнтную суставную поверхность, замещают дефект объема в области перемещенного здорового мыщелка свободным костным трансплантатом, забирают микрохирургический мышечно-надкостнично-кортикальный аутотрансплантат, на нехрящевой поверхности пораженного мыщелка формируют ложе по форме костной части микрохирургического аутотрансплантата и выполняют микрохирургическую аутотрансплантацию. Лечилась в аппарате Илизарова. Сращение чрезмыщелкового перелома большеберцовой кости получено в неправильном положении: внутренний мыщелок опущен, развернут во фронтальной плоскости. Дважды выполнена лавсанопластика внутренней боковой связки правого коленного сустава. 6.03.96 г. - выполнена поднимающая остеотомия внутреннего мыщелка правой б/б кости с фиксацией болтами-стяжками, гофрирование внутренней боковой связки по Кэмпбеллу, остеотомия малоберцовой кости в верхней трети, высокая овальная остеотомия правой большеберцовой кости. 01.04.97 г. - наложен аппарат Илизарова на правую голень. Получено сращение.
