Description
Oppfinnelsen vedrører nye anitmykotiske midler
i gelform til behandling av soppinfeksjoner i munnhulen, og som har en depot-virkning> gode klebeegenskaper og en høyere bio-disponerbarhet av det virksomme stoff, og derved muliggjør en korttidsterapi for behandling av infeksjoner overveiende på grunn av spross-sopp i munnhulen, er det allerede blitt kjent tilberedninger av antimykotiske forbindelser. Med disse tilberedninger var det for en fullstendig sanering nødvendig 10 til 21 dagers terapitid.
For å oppnå en forkortelse av terapivarighetene
krever man spesielt til eliminering av kimene resp. til å oppnå
en mykologisk sanering, en viss depotvirkning og en høyere bio-disponerbarhet av de virksomme stoffer. Dertil er de kjente formuleringer bare begrenset egnet. Når man nå uten ytterligere økning av den virksomme stoffkonsentrasjon vil oppnå en ned-settelse av terapivarigheten, må man sørge for en optimal biodispo-nerbarhet av det virksomme stoff.
Det var derfor nødvendig med formuleringer som på den ene side etter påføring på vekslingssiden har en tilstrekkelig klebeevne,på den annen side kan frigjøre de i formuleringene inneholdte virksomme stoffer tilstrekkelig også oppløst i spytt.
Det ble nå funnet at slike formuleringer av antimykotiske virksomme stoffer som som geldanner inneholder en celluloseeter, spesielt hydroksypropylcellulose, natriumalginat, eller propylenglykolalginat og dessuten inneholder de vanlige formuleringshjelpestoffer, muliggjør optimale klebeegenskaper og en optimal frigjøring av det virksomme stoff, og derved ned-settelse av terapivarigheten av oppnåelse av fungiside konsentra-sjoner av det virksomme stoff. Denne effekt oppnås ved at bio-disponerbarheten av de i formuleringen inneholdte virksomme stoffer, økes ved adhererende egenskaper, og derved kan den virksomme stoff-frigjøring i spyttet økes.
Spesielt ble det funnet at slike formuleringer av antimykotiske derivater som inneholder overnevnte geldannere i tillegg til linjære høymolekylære polysakkarider som stabiliserende geldannnere, vesentlig forbedrer varmestabiliteten, og formuleringene og dermed langtidsholdbarheten. Formuleringer uten høymolekylære polysakkaridtilsetning er bare holdbart til 30°C, over 30°C opptrer dette i kort tid en faseadskillelse, dvs. gel-strukturen ødelegges irreversibelt. Formuleringene som ved siden av geldanneren i tillegg inneholder mindre mengder enn linjær høymolekylært polysakkarid er stabile til 5 0°C, og derved økes vesentlig holdbarheten av slike tilberedninger.
De således fremstilte midler har også andre sed-vanlige egenskaper: Viskositeten påvirkes knapt av temperatursvigningene, de adhererende egenskaper av formuleringene forbedres.
Smaksutfoldelsen forbedres, og det oppnås en god munnføling.
Virksomme stoffer som kan formuleres på denne måte,
er alle antimykotiske virksomme derivater, spesielt imidazol-
og triazolderivater. De er tilstede imidlertid ifølge oppfinnelsen i mengder på 0,05 - 1 %, fortrinnsvis 1,1 - 1 %.
Eksempelvis skal det nevnes forbindelser av neden-
Tallrike ytterligere andre mykotiske virksomme azolderivater er kjent fra DE-OS 2 430 039. De kan likeledes tjene i midlene ifølge oppfinnelsen som virksomme stoffer.
Som geldanner kommer det på tale slike makromolekylære forbindelser som oppløser seg, resp. kan svelle så
vel i vann som også i organiske oppløsningsmidler. Fremfor alt skal det her nevnes cellulosetere, hvorav det kreves 2,5 til 17,5 %. Spesielt egnet er hydroksypropylcellulose, spesielt slike med mclvekter fra 1 000 000 til 60 000. Følger man en inndeling av makromolekylære hjelpestoffer, (Keipert et al, De Pharamzie 28, 145-183 (1973)) så kommer det fremfor alt til anvendelse ioniske makromolekyler i deres saltform. Dette er blandt annet natrium-karboksymetylcellulose, polyakrylsyre, polymetakrylsyre og deres salter, natriumamylopektinsemiglykolat, alginsyre og propylen-glykol-alginat, som natriumsalt,arabisk gummi, guar-gummi.
Amfotære makromolekyler av proteinderivater, f. eks. gelatin er likeledes egnet som ikke-ioniske polymere, f. eks. metylcellulose, hydroksypropylcellulose, oppløselige stivelser, påfører seg de overnevnte krav.
Som geldannere som også virker stabiliserende, kommer det i betraktning langkjedede linjære høymolekylære polysakkarider, med en molekylvekt på mer enn 1 000 000. Av slike stabilisatorer kreves 0,1 - 1,5 %.
Som oppløsningsmidler er vann egnet, også alle med vann blandbare oppløsningsmidler. I betraktning kommer f. eks. alkanoler som etanol og isopropylalkohol, bensylalkohol, propyl-englykol cl.
Som spesielt stabile gelkombinasjonsgrunnlag ble det funnet slike makromolekylære forbindelser som f. eks. hydroksypropylcellulose (molekylvekt 60 000 - 1 000 000) med et linjært høymolekylært polysakkarid (molekylvekt sannsynligvis 2 000 000).
Som bicpolysakkarider kommer det på tale "Xanthan Gum" (Keltrol, Keltzon) fra firma Kelco. Derved dreier det seg
om det høymolekylære naturlige kullhydrat med formel
som oppstår ved fermentering av Xantomonas campestris. Delene ifølge oppfinnelsen fremstilles idet til en 10%-ig oppløsning av bensylalkohol i avmineralisert vann som dessuten inneholder de vanlige formuleringshjelpestoffer, settes ved 6 0°C under hurtig omrøring 2,5 - 17,5 % celluloseeter som geldannere, og 0,1 - 1,5 % biopolysakkarid, avkjøles til 30°C, tilsettes 0,05 - 1 %, fortrinnsvis 0,1 - 1 % azolderivat, og blandingen avkjøles til værelsestemperatur.
Eksempel 1
I 94,89 kg avmineralisert vann, oppløses primært
1 kg bensylalkohol, 0,1 kg sakkarid, 0,01 kg Na-citrat, og opp-varmes til 60°C. Ved 60°C innarbeides under hurtig omrøring 2,5
kg hydroksypropylcellulose, (molekylvekt 1 000 000), og 0,5 kg biopolysakkarid og avkjøles til 30°C. Ved 30°C tilsettes 1,0 kg Clotrimazol, likeledes under hurtig omrøring. Blandingen av-kjøles til værelsestemperatur. På analog måte forarbeides resep-turene i følgende eksemplér.

1. Antimykotisk middel i gelform til behandling av soppinfeksjoner i munnhulen inneholdende azolderivater og vanlige formuleringshjelpestoffer, karakterisert ved at de inneholder 2,5 % - 17,5 % celluloseeter som geldanner, og 0,1 - 1,5 % biopolysakkarider som stabilisator, og/eller geldanner .

Fremgangsmåte til fremstilling av antimykotiske midler i gelform til behandling av soppinfeksjoner i munnhulen, karakterisert ved at den 1 %-ig oppløsning av en bensylalkohol i avmineralisert vann, som dessuten inneholder de vanlige formuleringshjelpestoffer, ved 60°C under hurtig omrøring, tilsettes 2,5 til 17,5 % celluloseter som geldanner, og 0,1 - 1,5 % biopolysakkarid, avkjøles til 30°C, tilsettes 0,05 - 1 %, fortrinnsvis 0,1 - 1 % azolderivat, og blandingen avkjøles til værelsestemperatur.
