Description
Innledning
Den foreliggende oppfinnelse vedrører en forbindelse valgt fra en gruppe av asymmetriske dinukleosidtetrafosfater, et farmasøytisk preparat omfattende en slik forbindelse, og anvendelser av en slik forbindelse eller et slikt farmasøy-tisk preparat for fremstilling av et medikament for ulike behandlinger.
Teknisk område
Oppfinnelsen omhandler visse dinukleotider som øker hydrati-seringen av beholdte slimsekresjoner, stimulerer produksjonen av muciner og øker ciliær støtfrekvens for å øke renselse av beholdte sekresjoner.
Bakgrunn for oppfinnelsen
Kronisk obstruktiv lungesykdom (chronic obstructive pulmonary disease, COPD) rammer 15 millioner pasienter i USA og er den 6. ledende dødsårsak. Den er karakterisert ved retensjon av slimsekresjoner i lungene. Mange pasienter som har fått diagnosen COPD har en lidelse kalt kronisk bronkitt (chronic bronchitis, CB) og 600.000 pasienter innlegges på sykehus hvert år på grunn av en akutt forverring av CB. Cystisk fibrose og primær ciliær dyskinesi (Primary Ciliary Dyskinesia, PCD) er andre eksempler på lungelidelser som antar en klinisk profil som ligner på COPD. Ciliær dyskinesi, enten den er primær eller sekundær, resulterer i beholdte sekresjoner som kun kan renses ved hosting.
En annen sykdomstilstand karakterisert ved akkumulasjon av beholdte mukøse sekresjoner er sinusitt. Sinusitt er en innflammasjon i de paranasale sinuser som typisk forbindes med en øvre respiratorisk infeksjon. Den er USA's mest vanlige helserelaterte lidelse og rammer anslagsvis 31 millioner mennesker . (A. Moss og V. Parsons, National Center for Health Statistics, 1986: 66-7, DHHS Publication No. (PHS)86-1588 (1985)).
Otitis media (OM) er en viral eller bakteriell infeksjon i mellomøret som primært rammer barn under en alder på tre år. Den fremskyndes vanligvis av en øvre respiratorisk infeksjon som sprer seg inn i mellomøret via nasofarynks og det eusta-kiske rør. Omtrent 25-50 millioner legebesøk utføres hvert år for diagnose og behandling av OM. Ved en alder på tre år vil omtrent 75% av barn ha hatt minst ett tilfelle av akutt OM (J. Klein, Clin. Infect. Dis. 19, 823-33 (1994)). Etter passende behandling med antibiotika er akkumulert fluid til-bake i mellomøret, hvilket forårsaker svekket hørsel og mulige forsinkelser i språklig og kognitiv utvikling. Økt evne til å rense sekresjoner i mellomøret vil redusere eller eliminere signifikante ettersykdommer av otitis media.
En ytterligere lidelse som resulterer fra beholdte sekresjoner er pneumoni. Pasienter som er immobilisert av en rekke ulike årsaker har høy risiko for å utvikle pneumoni. ' På tross av ekstra årvåkenhet og tallrike tiltak utvikles pneumoni i over 400.000 pasienter pr. år, med signifikant morbiditet og mortalitet.
Det er også situasjoner hvor det er terapeutisk ønskelig å øke drenering av det lakrimale system. Når det lakrimale dreneringssystem ikke fungerer på passende måte kan resul-tatet være overdreven tåredannelse (epifora), mukopurulent uttømming og tilbakevendende dakryocystitt. Vanlige behandlinger for tilstoppelse av nese-tåre-kanalen er oftest inntrengende kirurgiske prosedyrer, og forskere har forsøkt å finne ikke-inntrengende farmasøytiske behandlinger.
Tåresekresjon kan stimuleres fra lakrimale aksessoriske vev via P2Y2 og/eller P2Y4 purinerg reseptor-medierte mekanismer som ligner på dem som hydratiserer luftveisepiteler. Tørt øye-sykdom er den generelle betegnelsen for indikasjoner som frembringes ved abnormaliteter i den prekorneale tårefilm som er karakterisert ved en reduksjon i tåredannelse eller en økning i tårefilmfordampning, sammen med den okulære over-flatesykdom som resulterer. For tiden er den farmasøytiske behandling av tørt øye-sykdom oftest begrenset til administrasjon av kunstige tårer (saltoppløsning) for midlertidig å rehydratisere øynene. Lindringen er imidlertid kortvarig og hyppig dosering er nødvendig.
Normalt fjernes mukøse sekresjoner via det mukociliære renselses (MCC) system. MCC er avhengig av den integrerte virkning av tre komponenter: 1) slimsekresjon ved begerceller og submukosale kjertler, 2) bevegelsen av cilier på epitelceller som driver slimet tvers over den luminale overflaten, og 3) ionetransport inn i og ut av luminale epitelceller som samtidig kontrollerer strømmen av vann inn i slimet.
Det er nå kjent at nukleosidfosfater slik som uridin-5'-trifosfat (UTP) modulerer alle komponentene i MCC-systemet. For det første er UTP blitt vist å øke både mengden pr. tidsenhet og den totale mengde mucinsekresjon ved begerceller in vitro (M. Lethem, et al., Am J. Respir. Cell Mol. Biol. 9, 315-22
(1993) ). For det andre er UTP blitt vist å øke støtfrekvens for cilier i humane luftveis-epitelceller in vitro (D. Drutz et al., Drug Dev. Res. 37(3), 185 (1996)). Og for det tredje er UTP blitt vist å øke Cl" sekresjon og således vannsekre-sjon fra luftveis-epitelceller in vitro (S. Mason et al., Br. J. Pharmacol. 103, 1649-56 (1991)). I tillegg menes det at frigivelse av surfaktant fra type II alveolarceller som respons på UTP (Gobran, Am. J. Physiol. 267, L625-L633
(1994) ) bidrar til at lungene fungerer optimalt og kan hjelpe til med maksimalisering av MCC (M. Knowles, et al., N. Engl. J. Med. 325, 533-38 (1991)). UTP er blitt vist å øke intra-cellulær Ca<++> på grunn av stimulering av fosfolipase C ved P2Y2 reseptoren (H. Brown et al., Mol. Pharmacol. 40, 648-55
(1991))•
UTP's modulasjon av alle komponenter av det mukociliære eska-latorsystem resulterer i en 2,5-ganger forbedring i mukoci-liær renselse av lunger hos normale frivillige uten noen signifikante bivirkninger (K. Olivier, et al., Am J. Respir. Crit. Care Med. 154, 217-23 (1996)). I tillegg økte UTP signifikant renselse ved hosting (renselse av beholdte sekresjoner ved hosting) i pasienter med PCD (P. Noone, et al., Am. J. Respir. Crit. Care Med. 153, A530 (1996)).
Det skal anføres at publisert PCT-søknad WO 96/40059, publisert 19. desember 1996, beskriver nukleotidforbindelser som er anvendbare i behandling av luftveissykdommer.
På grunn av UTP's demonstrerte evne til å øke renselsen av beholdte mukøse sekresjoner, ble søkerne motivert til å undersøke hvorvidt andre nukleosidfosfater kunne være like, om ikke mer, terapeutisk effektive. Den foreliggende oppfinnelse er basert på denne undersøkelsen.

Den farmasøytiske anvendelighet av forbindelser i henhold til oppfinnelsen er indikert ved inositolfosfat-analysen for P2Y2 og annen P2Y reseptoraktivitet. Denne alminnelig anvendte analyse, som beskrevet i E. Lazarowski et al., Brit. J. Pharm. 116, 1619-27 (1995), beror på måling av inositolfos-fatdannelse som et mål på aktivitet av forbindelser som aktiverer reseptorer forbundet via G-proteiner til fosfolipase C.

De farmasøytiske preparater inneholdende forbindelser i henhold til foreliggende oppfinnelse kan være i en form som er egnet for oral anvendelse, f.eks. som tabletter, piller, vandige eller oljeaktige suspensjoner, dispergerbare pulvere eller granuler, emulsjon, harde eller myke kapsler, eller siruper eller eliksirer. Preparater som ment for oral anvendelse kan fremstilles i samsvar med en hvilken som helst metode som er kjent på området for fremstilling av farma-søytiske preparater og slike preparater kan inneholde ett eller flere midler som er valgt fra gruppen bestående av søtningsmidler, aromamidler, fargemidler og konserveringsmidler for å tilveiebringe farmasøytiske elegante og vel-smakende preparater. Tabletter inneholder den aktive bestanddel i blanding med ikke-toksiske farmasøytisk akseptable eksipienser som er egnet for fremstilling av tabletter. Forbindelser med generelle formler I, IA eller IB kan administreres parenteralt i et sterilt medium. Eksempel 7
P<1->(tymidin-5'-)P<4->(uridin-5•-)tetrafosfat(UP4T)
En oppløsning av uridin-5'-trifosfat (UTP) trinatriumsalt (ProBioSint, 5,86 g, 0,01 mol) i vann (5 ml) ble ført gjennom en kolonne med BioRad AG-MP 50 sterk kationbytterharpiks i sin tributylamin-form (50 ml sjiktvolum) og eluert med destillert vann (omtrent 300 ml). Til denne oppløsningen ble det tilsatt tributylamin (5 ml), og suspensjonen ble rystet inntil pH i den vandige fraksjonen hadde steget til 8. Lagene ble separert og den vandige oppløsningen inndampet til et lite volum og deretter frysetørket over natten. Resten ble oppløst i tørr dimetylformamid (DMF, 20 ml) og løsnings-middelet avdampet ved 0,1 mmHg. Det tørkede tributylaminsalt ble tilberedt til 100 ml med vannfritt aceton til å gi en lageroppløsning (0,1 M i UTP). Dicykloheksylkarbodiimid (DCC) Baker, 0,1 g, 0,5 mmol) ble tilsatt til en delmengde av den foregående UTP-oppløsningen (1,0 ml, 0,1 mmol) og opp-løsningen ble omrørt ved romtemperatur i 3 0 min. Det utfelte dicykloheksylurea ble fjernet ved filtrering, reaksjonsblandingen ble ekstrahert med eter (10 ml) og resten ble oppløst i tørt deuterert dimetylsulfoksyd (DMSO-d6, 0,3 ml). Denne oppløsningen av uridin-5<1->cyklisk metafosfat (UcTP) ble tilsatt til en oppløsning av tymidin-5<1->monofosfat (TMP, 0,064 g, 0,2 mmol) og tributylamin (0,2 ml) i DMSO-d6 (0,3 ml) og satt til side ved 50°C i 24 timer. P1-(inosin-5 ' -) P4-(uridin-5'-)tetrafosfat (UP4I)
Kondensering av uridin-5<1->cyklisk trimetafosfat (UcTP) og inosin-5<1->monofosfat ble utført hovedsakelig som beskrevet i det foregående, unntatt at reaksjonsblandingen ble lagret ved 25°C i 5 uker før avdampning av løsningsmiddelet. Resten ble oppløst i vann (1,0 ml), filtrert og separert i 150 [ il delmengder ved ionebytterkromatografi på en Hamilton PRP X-100 kolonne, under eluering med isokratisk 1,0 M ammoniumbikarbonat, 5 ml/min., 30 min. Fraksjonene som eluerer mellom 6 og 9 minutter ble inndampet og frysetørket over natten for å fjerne buffer. Eksempel 9
P<1->(4-tiouridin-5'-)P<4->(uridin-5'-)tetrafosfat (UP4(4-SH-U)) 4-tiouridinmonofosfat-natriumsalt (25 mg, 0,057 mmol) ble oppløst i vann (0,5 ml), tilført til en kolonne med BioRad AG-MP50 sterk kationbytterharpiks (2 ml sjiktvolum, 3 mekv.) i sin tributylatform, kolonnen ble eluert med vann (-10 ml) og eluatet frysetørket. Det resulterende tributylaminsalt av 4-tio-UMP ble kondensert med uridin-51-cyklisk trimetafosfat (0,1 mmol) fremstilt ved aktivering av UTP med dicykloheksylkarbodiimid (2 06 mg) hovedsakelig som beskrevet i eksempel 7 (72 timer reaksjonstid). Etter avdampning av DMSO fra reaksjonsblandingen ble resten oppløst i vann (~1 ml) og separert i 200 /il delmengder ved ionebytterkromatografi på en Hamilton PRP X-100 kolonne, under eluering med isokratisk 1,0 M ammoniumbikarbonat, 5 ml/min., idet elueringen overvåkes ved 328 nm. Fraksjonene som eluerer mellom 15 og 25 min ble frysetørket til å gi den i overskriften angitte forbindelse (9,7% utbytte, 99,7% ren ved HPLC(AUC)), som ble kvantitert ved sammenligning av sin ultrafiolette absorpsjon ved 332 nm med den for 4-tio-UMP ved den samme bølgelengden.

Eksempler 10-12 ble fremstilt fra uridin-5<1->cyklisk trimetafosfat (0,1 mmol) og det relevante nukleosid 5<1->monofosfat (0,2 mmol) hovedsakelig som beskrevet i eksempel 7, unntatt at heksan ble anvendt i stedet for eter til å ekstrahere overskuddet av DCC fra reaksjonsblandingen.

P1- (3' -azido-3' -deoksytymidin-5 ' -) -P4- (uridin-5 ' -) tetrafosfat (UP4(AZT))
3<1->azido-3'-deoksytymidin-5'-monofosfat (AZTMP) natriumsalt (50 mg, 0,135 mmol) ble oppløst i vann (1 ml) og tilført til en kolonne med Biorad AG-MP50 sterk ionebytterharpiks i sin tributylamin-form. Kolonnen ble eluert med vann (- 10 ral) og eluatet ble frysetørket. Det resulterende tributylaminsalt ble kondensert med uridin-5<1->cyklisk trimetafosfat (0,1 mmol) fremstilt ved aktivering av UTP med dicykloheksylkarbodiimid (206 mg) hovedsakelig som beskrevet i eksempel 7. Uridin-5'-trifosfat (5,0 g) i vann (18 ml) ble ført gjennom en kolonne med Dowex 50 H+, og tributylamin (3,0 g) ble tilsatt til eluenten. Blandingen ble konsentrert til en olje, tørket ved inndampning med tørt DMF og på ny oppløst i tørt DMF (18 ml). Dicykloheksylkarbodiimid (DCC, 3,5 g) ble tilsatt, oppløsningen ble omrørt ved romtemperatur i 3 0 min. og presipitatet ble fjernet ved filtrering, heksan (70 ml) ble tilsatt til filtratet, og bunnlaget ble separert og vasket igjen med heksan (70 ml) for å fullføre fjerningen av DCC. Denne oppløsningen av uridin-5'-cyklisk metafosfat (UcTP) ble anvendt i de følgende forsøk: Eksempel 17 P1- (2 ' -deoksyguanosin-5 ' -) P4- (uridin-5 1 -) tetrafosfat (UP4dG) 2<1->deoksyguanosin-5<1->monofosfat (d-GMP, Sigma, 500 mg) ble oppløst i DMF ( 4,5 ml), tributylamin (1,0 ml) ble tilsatt og oppløsningen ble konsentrert til en olje under vakuum. En tredjedel av oppløsningen av uridin-5<1->cyklisk metafosfat (UcTP, ovenfor) ble tilsatt og oppløsningen ble oppvarmet ved 40°C i 24 timer. Oppløsningen ble inndampet til en olje, oppløst i vann (10 ml) og tilført til en kolonne med Sephadex DEAE (350 ml i en 4,5 x 22 cm kolonne) i sin bikarbonat-form, for-ekvilibrert med 0,25 M ammoniumbikarbonat. Kolonnen ble eluert suksessivt med 0,25, 0,30, 0,35, 0,40 og 0,50 M ammoniumbikarbonat . Elueringen ble overvåket ved HPLC (SynchroPak AX-300, 75% 0,50 M KH2P04, 25% MeCN 1,0 ml/min. , UV 254 nm), og fraksjonen inneholdende UP4dG ble konsentrert til et faststoff, og deretter ko-inndampet 6-7 ganger med vann til å gi ammoniumsaltet av dinukleotidet som et orange-gult faststoff (140 mg, estimert renhet ved HPLC (AUC) 94%).
Eksempel 19
pl_ (2 1 -deoksyinosin-5' -) P4- (uridin-5 ' -) tetrafosfat (UP4dI) 2'-deoksyinosin-5'-monofosfat (d-IMP, Sigma, natriumsalt 1,0 g) omdannes til den frie syreform med Dowex 50 (H<+>) harpiks som beskrevet for UTP ovenfor. Elueringsmiddelet nøytrali-seres med tributylamin (2,0 ml) og blandingen konsentreres til en olje under vakuum. Det resulterende tributylaminsalt tørkes ved inndampning med DMF, resten oppløses i DMF (4,5 ml) og behandles med uridin-51-cyklisk trimetafosfat hovedsakelig som beskrevet ovenfor til å gi P<1->(2'-deoksyinosin-51-)P<4->(uridin-5<1->)tetrafosfat.

pl_(2'-deoksycytidin-5'-)P4- (uridin-5'-) tetrafosfat (UP4dC) 2'-deoksycytidin-5'-monofosfat (d-CMP, Sigma, 500 mg) behandlet hovedsakelig som ovenfor ga P<1->(2'-deoksycytidin-51-)P<4->(uridin-5'-)tetrafosfat som et hvitt faststoff (130 mg), estimert HPLC renhet 82%.

5. Anvendelse av en forbindelse som beskrevet i krav 1 eller et farmasøytisk preparat som beskrevet i krav 2 for fremstilling av et medikament for behandling av tørt øye i et pattedyr.
6. Anvendelse av en forbindelse som beskrevet i krav 1 eller et farmasøytisk preparat som beskrevet i krav 2 for fremstilling av et medikament for behandling av retinal løsning i et pattedyr.
