Description
Foreliggende oppfinnelse vedrører anvendelse av en sammensetning omfattende tre eller flere anti-atypiske mycobakterielle midler valgt blant klaritromycin, rifabutin, klofazimin og etambutol for fremstilling av et medikament for behandling av inflammatorisk tarmsykdom, slik som Crohn's sykdom.
Inflammatorisk tarmsyndrom, IBD, er en mangel av ukjent etiologi som typisk karakteriseres ved diaré, kramper, abdominalsmerter, vekt-tap og rektal blødning. Den omfatter slike mangler som Crohn's sykdom, ulcerativ colit, indeterminat-colit, mikroskopisk colit og collagen colit. Årsaken er ukjent. I den senere tid har det imidlertid vært tegn som tyder på at mycobacterium paratuberkolosis, Mp, og kanskje dennes forskjellige substammer, kan spille en infeksiøs rolle ved å trenge inn i cellene som utgjør tarmveggen. Kilden for denne bakterie er uklar men kan ligge hos andre dyr som sauer, kveg, kaniner så vel som hos andre mennesker. Den kan overføres til mennesker muligens via melk, kontaminerte vannforråd, dårlig kokt kjøtt og så videre. Selv om det har vært en lengevarende kontrovers når det gjelder involveringen av Mp når det gjelder årsakene til Crohn's sykdom begynner nyere applikasjoner av PCR-bruk å bekrefte at de fleste Crohn's tilfeller virkelig er infisert med denne organisme som sannsynligvis er det kausale, infeksiøse middel. Tidligere har terapi rettet mot eradikasjon av Mp ved bruk av kombinerte anti-TB-medikamenter, for eksempel INH, pyrazinamid, streptomycin, etambutaol, rifampisin og PAS, generelt vært til liten hjelp. Med andre ord og selv om transientforbedring hos en del av pasientene inntrådte, ble ingen pasient helbredet. Selv om Mp var årsaken til denne sykdom var det ingen effektiv terapi tilgjengelig for Mp fordi det dreiet seg om en "atypisk mycobakterie" og for atypiske mycobakterier forelå det ingen kjent terapi. Fordi Mycobakteriumet paratuberkulosis har en lang delingstid kreves multiple antimikrobielle medikamenter for å behandle infeksjonen som må gjennomføres over lange tidsperioder tilsvarende behandling som benyttes ved terapi av mycobakterium tuberkulosis. Videre resulterer mycobakterium tuberkulosis terapi med dagens medikamenter i dannelse av resistente stammer. Slike resistente stammer blir ikke utryddet med kjente anti-mikrobielle midler. Således foreligger det ingen effektiv kur for resistent TB.
I henhold til dette foreligger det et behov for en effektiv behandling av inflammatorisk tarmsyndrom og særlig Crohn's sykdom.
Comprehensive Therapy, vol. 21, no. 6,1995, side 296-302 omtaler en studie som vedrører behandling av inflammatorisk tarmsyndrom, inkludert bruk av anti-mycobakteriell kemoterapi ved behandling av Crohn's sykdom. En placebo kontroll studie viste ingen signifakant effekt av en kombinasjon av rifampicin, isoniazid og etambutol på pasienter med aktiv Crohn's sykdom.
J. Antimicrobial Chemotherapy, vol.21, no.6,1995, side 296-302 omtaler en studie som vedrører effekten av rifabutin og makrolid antibiotika terapi på pasienter som lider av Crohn's sykdom. Forfatterne konkluderte med at anvendelsen av rifabutin i kombinasjon med makrolid var trygg.
American Journal of Gastroenterology, vol 89, no. 4,1994, side 513-518 omtaler en studie vedrørende effekten av anti-mycobakterielle medikamenter ved behandling av Crohn's sykdom. Pasientene ble behandlet med et ni måneders regime av etambutol, klofazimin og dapson samt en en dags dose med rifampicin. Det ble konkludert med at et slikt regime kan være gunstig ved behandling av symptomer hos enkelte pasienter, men at videre studer er nødvendig.
Videre er det fra New England Journal of Medicine, vol. 335, no. 6,1996, side 377-383 kjent at en kombinasjon av rifampicin og klofazimin ikke var velfungerende mot Mycobacterium avium kompleks hos AIDS pasienter.
Antim. Agents and Chemotherapy, vol. 40, no. 3,1996, side 743-749 omtaler en rekke
(132) forskjellige in vitro kombinasjoner for behandling av Mycobacterium avium kompleks. Det blir verken foreslått eller antydet en anvendbar behandling av Mycobacterium avium paratuberkulose.
Pharmacology in Medicine, 1984, side 847-858 omtaler anti tuberkulose midler.
Overraskende har foreliggende oppfinnere oppdaget at metabolismen til mycobakterien som antas å være ansvarlig for symptomene på inflammatoriske tarm-mangler kan inhiberes lenge nok til å kurere infeksjonen og derved lindre symptomene, ved til pasienten å administrere en kombinasjon av anti-atypiske mycobakterielle midler og/eller en immuniserende mengde av et mycobakterielt produkt.
Et første aspekt ved oppfinnelsen vedrører anvendelse av en sammensetning omfattende tre eller flere anti-atypiske mycobakterielle midler valgt blant klaritromycin, rifabutin, klofazimin og etambutol for fremstilling av et medikament for behandling av inflammatorisk tarmsykdom, inkludert Crohn's sykdom og colit. Anvendelsen ifølge oppfinnelsen resulterer i en herding av infeksjonen og en reversering av den kliniske tilstand.
Som nevnt ovenfor omfatter nevnte medikament tre eller flere anti-atypiske mycobakterielle midler. I enkelte situasjoner, for eksempel der det foreligger resistente stammer, kan det være nødvendig med en kombinasjon av flere enn tre anti-atypiske mycobakterielle midler. Fire, fem eller sågar seks medikament-kombinasjoner kan være nødvendig hos pasienter med resistente mycobakteirum-paratuberkulosis-stammer. Egnede anti-atypiske, mycobakterielle midler omfatter, men er ikke begrenset til, klaritromycin, rifabutin, rifampicin, rifapenten, azitromycin, roxitromycin, cefazolin, minocyklin, streptomycin, amikacin, klofazimin, etambutol, ofloxacin, sparfloxacin, pefloxacin, grepafloxacin, trovafloxacin, norfloxicin, enoxacin, lomefloxacin, ciprofloxacin og oksazolidinon.
I en utførelsesform ifølge det første aspekt ved oppfinnelsen omfatter nevnte medikament rifabutin og klaritromycin, og mer foretrukket rifabutin, klaritromycin og klofazimin.
Karakteristisk kan nevnte medikament inkludere mellom 10 og 500 mg av hver av tre eller flere anti-atypiske, mycobakterielle midler. Mere typisk kan nevnte medikament omfatte mellom 15 og 250 mg av hvert av tre eller flere anti-atypiske, mycobakterielle midler. Ennå mer typisk kan nevnte medikament inkludere rifabutin tilstede i en mengde mellom 50 og 250 mg og mer typisk rundt 150 mg, klaritromycin i en mengde mellom 200 og 300 mg og mer typisk rundt 250 mg og klofazimin i en mengde av 10 til 150 mg og mer typisk rundt 50 mg. Typisk kan andre atypiske, mycobakterielle midler være tilstede i en mengde i overensstemmelse med kjente doser.
Typisk kan nevnte medikament være tilgjengelig i form av en tablett som inneholder hver av tre eller flere anti-atypiske, mycobakterielle midler tilstede i en presset pulverform. Alternativt kan nevnte medikament være tilgjengelig i form av en tablett/kapsel inneholdende et eller flere av de anti-atypiske, mycobakterielle midler i en mikro-innkapslet form. Som en ytterligere mulighet kan nevnte medikament være tilgjengelig i form av en tablett/kapsel inneholdende en av de tre eller flere anti-atypiske mycobakterielle midler tilstede i pulverform og de gjenværende, anti-atypiske, mycobakterielle midler tilstede i en mikro-innkapslet form. Som en ytterligere mulighet kan nevnte medikament være tilstede i form av en tablett/kapsel inneholdende hvert av tre eller flere anti-atypiske, mycobakterielle midler tilstede i mikrogranulert form. I nok
ytterligere muligheter kan nevnte medikament være tilgjengelig i form av en eller flere
tabletter inneholdende en eller flere av de anti-atypiske, mycobakterielle midler i en kapsel, en eller flere kapsler inneholdende et eller flere av de anti-atypiske, mycobakterielle midler i en tablett, en eller flere kapsler inneholdende et eller flere av de anti-atypiske, mycobakterielle midler i en ytre kapsel inneholdende de andre anti-atypiske, mycobakterielle midler, eller en hvilken som helst kombinasjon derav.
I en foretrukket form består nevnte medikament av en indre kapsel inneholdende rifabutin med en ytre kapsel inneholdende klaritromycin og klofazimin, der klaritromycin og klofazimin kan være tilstede i pulver-, mikro-innkapslet eller mikrogranulert form.
Nevnte medikament kan typisk administreres i form av en eller flere tabletter/kapsler inneholdende hver av tre eller flere anti-atypiske, mycobakterielle midler som beskrevet i avsnittet ovenfor, eller ved administrering av hver av tre eller flere anti-atypiske, mycobakterielle midler separat.
Nevnte anti-atypiske mycobakterielle midler blir vanligvis benyttet kontinuerlig over en periode på 3 til 36 måneder, fortrinnsvis 24 måneder. Doser av de anti-atypiske mycobakterielle midler ligger generelt i overensstemmelse med kjente doseringsområder. For eksempel er typiske doseringsområder for klaritromycin fra 250 mg til 1,5 g/dag, typisk rundt 750 mg/dag, den typiske dose for rifabutin er fra 150 til 750 mg/dag og karakteristisk rundt 450 mg/dag; den typiske dose klofazimin er fra rundt 1 mg/kg til 6 mg/kg, karakteristisk rundt 2 mg/kg; den typiske dose etambutol er opp til rundt 15 mg/kg; og den typiske dose azitromycin er fra 250 mg til 1000 mg/dag, karakteristisk rundt 500 mg/dag.
Nevnte medikament kan også kombineres med et eller flere mildere anti-TB-midler som salazopyrin, olsalazin eller mesalazin, såvel som andre mindre kjente aminosalisylsyrer. 4-aminosalisyl-syrene eller 5-aminosalisylsyrene kan kombineres med hvilke som helst tre eller flere av de anti-typiske mycobakterielle midler som nevnt ovenfor. Doseringer av disse midler er generelt kjent. For eksempel ligger det typiske doseringsområdet for salazopyrin i områder rundt 500 mg til 4 g/dag; for olsalazin eller mesalazin ligger dosen fra rundt 500 mg til rundt 3 mg/dag. Således kan nevnte medikament ytterligere inkludere et middel som er effektivt mot tuberkulose.
Foretrukket omfatter nevnte medikament videre et mycobakteriell ekstrakt eller produkt (levende eller dødt, eller dets ekstraherte vegg og DNA-komponenter) som et immuniserende middel for å stimulere leukocytter i den immuniserte pasient. Nevnte ekstrakt eller produkt er foretrukket fra ikke-patogene mycobakterier, slik som M. vaccae eller M. phlei, og mer foretrukket er nevnte produkt eller ekstrakt regressin som er kommersielt tilgjengelig fra Bioniche of London, Ontario, Canada. Det er foretrukket å anvende nevnte ekstrakt eller produkt i en mengde fra rundt 25 ug til rundt 500 ug og mer typisk 50 ug til 500 \ xg. Som her benyttet betyr uttrykket "mycobakteriell ekstrakt eller produkt" hele, avlivede mycobakterier eller mycobakteriell ekstrakt, med eller uten adjuvanter.
Nevnte mycobakterielle produkt kan administreres på en hvilken som helst av flere måter, for eksempel oralt, intravenøst, intramuskulært eller subkutant. Slik immunisering kan befri pasienten for Mp infeksjon, og har evnen til å helbrede sykdommen eller plassere pasienten i en forlenget remisjon. Administrering av det mycobakterielle produkt eller ekstrakten skjer karakteristisk ukentlig eller månedlig men kan være mer eller mindre hyppig. Et egnet behandlingsregime kan oppnås på enkel måte av fagmannen på området i ethvert spesielt tilfelle ut fra den her gitte lære.
Mycobakterium phlei-ekstrakt (for eksempel regresin) kan fordelaktig administreres ukentlig med økning av doseringen med 50 \ ig ekstrakt hver uke inntil pasienten utvikler feber, kramper og neusea. Dosen reduseres så med 50 ug til det nedre nivå og pasienten forsetter å opprettholde immuniseringen på månedlig basis. Behandlingen kan vare fra 6 uker og opp til et månedlig immuniseringsprogram på 2 år eller mer.
Den inflammatoriske prosess kan overvåkes ved kolonoskopi og biopsi såvel som forskjellige blodparametre, under hele behandlingens varighet.
Nevnte medikament kan prepareres på i og for seg kjent måte for preparering av medikamenter (slik den foreligger i den farmasøytiske teknologi) inkludert blanding, oppmaling, homogenisering, suspendering, oppløsning, emulgering, dispergering og der det har noen hensikt, blanding av de anti-atypiske, mycobakterielle midler sammen med valgte drøyemidler, diluenter, bærere og adjuvanter.
For oral administrering kan det farmasøytiske medikament foreligge i form av tabletter, lozenges, piller, dragéer, kapsler, elexirer, pulvere inkludert lyofiliserte pulvere, oppløsninger, granuler, suspensjoner, emulsjoner, siruper og tinkturer. Former med langsom frigivning eller forsinket frigivning kan også fremstilles, for eksempel i form av belagte partikler, flersjiktstabletter eller mikrogranuler.
Faste former for oral administrering kan inneholde farmasøytisk akseptable bindemidler, søtnere, disintegreirngsmidler, diluenter, smaksstoffer, belegningsmidler, preservativer, smøremidler og/eller tidsforsinkelsesmidler. Egnede bindemidler omfatter gummi acacia, gelatin, maisstivelse, tragacanth-gummi, natriumalginat, karboksymetylcellulose eller polyetylenglykol. Egnede søtnere er sucrose, lactose, glucose, aspartam eller saccharin. Egnede disintegreringsmidler omfatter mais-stivelse, metylcellulose, polyvinylpyrrolidon-xantangummi, bentonit, alginsyre eller agar.
Egnede diluenter er laktose, sorbitol, mannitol, dextrose, kaolin, cellulose, kalsium-karbonat, kalsiumsilikat eller dikalsiumfosfat.
Egnede smaksmidler er peppermynteolje, olje av vintergrønn, kirsebær-, appelsin- eller bringebærsmak. Egnede belegningsmidler er polymerer eller kopolymerer av akrylsyre eller metakrylsyre og/eller estere derav, vokser, fettalkoholer, zein, shellac eller gluten. Egnede preserveringsmidler er natriumbenzoat, vitamin E, a-tocoferol, ascorbinsyre, metylparaben, propylparaben eller natriumbisulfit. Egnede smøremidler er magnesium-stearat, stearinsyre, natriumoleat, natriumklorid eller talkum. Egnede tidsforsinkelsesmidler er glycerylmonostearat eller glyceryl-distearat.
Flytende former for oral administrering kan i tillegg til midlene ovenfor inneholde en flytende bærer. Egnede flytende bærere er vann, olje som oliven, jordnøtt-, sesam-, solsikke-, safran-, arachis- eller kokosnøtt-olje, flytende paraffin, etylenglykol, propylenglykol, polyetylenglykol, etanol, propanol, isopropanol, glycerol, fettalkoholer, triglycerider eller blandinger derav.
Suspensjoner for oral administrering kan videre inkludere dispergerings- eller suspenderingsmidler. Egnede suspenderingsmidler er natriumkarboksymetylcellulose, metylcellulose, hydroksypropylmetylcellulose, polyvinylpyrrolidon, natriumalginat eller cetylalkohol. Egnede dispergeringsmidler er lecitin, polyoksyetylenester av fettsyrer som stearinsyre, polyoksyetylensorbitolmono- eller -dioleat, -stearat eller -laurat, polyoksyetylensorbitan-mono- eller -dioleat, -stearat eller -laurat og lignende. Emulsjonene for oral administrering kan videre inkludere et eller flere emulgeringsmidler. Egnede slike er dispergeirngsmidler som eksemplifisert ovenfor eller naturgummier som acacia-gummi eller tragachant-gummi.
Eksempler
Eksempel 1: Behandling av pasienter med inflammatorisk tarmsvkdom ved bruk av en kombinasjon av anti- atypiske. m<y>cobakterielle midler.
Femten pasienter i alderen 13 til 58 år ble behandlet i henhold til forskjellige protokoller av anti-mycobakterielle midler. Tolv pasienter hadde Crohn's sykdom og tre hadde
ulcerativ colit. Nærvær av Mycobacterium paratuberculosis ble identifisert i ni av disse pasienter. En kombinasjon av klaritromycin (250 mg til 1,5 g/dag), rifabutin (150 mg til 750 mg/dag) og klofazimin (3 mg/kg til 10 mg/kg) ble benyttet. Hurtig klinisk remisjon ble oppnådd i disse pasienter med cessasjon av prednison, azatioprin og 5ASA-forbindelser og klinisk setning av den inflammatoriske tarmsykdom.
Efter 4 måneders behandling ble 5 pasienter undersøkt colonoskopisk. To av disse pasienter hadde normalisert den koloniske og terminale ileum mucosa mens tre fortsatt hadde damlignende inflammatoriske endringer og histologisk nærvær av minimal inflammatorisk infiltrat med noen eosionfiler.
I disse tre pasienter var en kombinasjon av klaritromycin og rifabutin (samme dose som ovenfor) sammen med 2 mg/kg tilsatt klofazimin, en foretrukken terapi. 70 % av pasientene ble i løpet av 8 måneder drastisk forbedret med fjerning av alt behov for medikamenter mot anti-inflammatorisk tarmsykdom. Intet prednison, azatioprin eller 5-azaforbindelser ble benyttet. Den inflammatoriske prosess hos disse pasienter var ikke lenger påvisbar og heller ikke histologisk var bevis på IBD tilstede, betraktet under mikroskop. Imidlertid ble hos en pasient som var sensitiv for rifabutin (markert hodepine og feber) rifabutin byttet ut mot etambutol ved den dose på 400 mg 2 ganger daglig. Denne dose ble øket i 1 trinn til 50 mg/kg i et forsøk på å reversere den inflammatoriske prosess og derefter redusert til 10 mg/kg. Pasienten oppnådde også reversering av den inflammatoriske prosess og der diaré og blødning forsvant og der til slutt akuttbehovet forsvant. Hos nok en ytterligere pasient ble 4 medikamenter benyttet samtidig på grunn av resistens mot klaritromycin. Acitromycin i en mengde av 500 mg hver morgen (området 250 til 1500 mg) ble benyttet i kombinasjon med rifabutin, klofazimin og etambutol.
Eksempel 2: Behandling av pasient med inflammatorisk tarms<y>kdom ved bruk av en mikrobiell ekstrakt.
En 34 år gammel pasient med to tarmreseksjoner og en strikturotomi og som var på standard Crohn's sykdom-terapi bestående av azopiatrin, prednison og mesalazin, fikk en immunostimulatorisk injeksjon av Regressin. Denne ble gitt intramuskulært og senere oralt i en startdose på 500 ug fulgt av 500 ug ukentlig i 4 uker og deretter månedlig.
To år etter recurrent oral immunisering på en ukentlig og så månedlig basis forble pasienten symptomfri og av all terapi, noe som antydet reversering og forsvinnen av Crohn's sykdom. Ved colonoskopi av anastomose-sete var dette fritt for Crohn's sykdom.
Eksempel 3: Behandling av alvorlig Crohn' s sykdom ved bruk av Rifabutin-Macrolid- Klofazimin- kombinasion.
Pasienter som ikke kunne underkastes maksimal konvensjonell terapi ble prospektivt begynt gitt en kombinasjon av rifabutin (450 mg/dag), klaritromycin (750 mg/dag), og klofazimin (2 mg/kg). Azatiaprin ble terminert mens 5-ASA og steroider ble avtrappet og så oppgitt. Forløpet ble overvåket ved kolonoskopi, tverrsnittsultralyd, hematologi-verdier og Harvey-Bradshaw-aktivitetsindeksen. Etter 8 til 12 måneder oppnådde 10 pasienter så og si fullstendig kontroll av Crohn's sykdom på kombinasjonsterapien alene. Ileal-strikturer dilaterte til normal ultralyd-veggtykkelse til alle de fem undersøkte pasienter. Utstrakt pseudopolyp-legemer regresserte fra colon hos pasienten som led fra denne tilstand, defunksjonerende ileostomi ble lukket etter 11 måneder hos pasienten som led av denne tilstand, reversering fra inflammert til histologisk ikke-inflammert ileal- og colonisk mucosa ble observert hos fem av tolv pasienter som led av denne tilstand. Alle pasientene hadde i det vesentlige normaliserte hematologiske verdier etter 8 til 12 måneders behandling. Hos to pasienter progresserte Crohn's sykdom 2 til 3 måneder etter cessasjon av steroider som deretter ble re-introdusert mens man opprettholdt kombinasjonsterapien ifølge oppfinnelsen. Harvey-Bradshaw-indeksen falt fra 15,5 til 2,5.
Eksempel 4: Preparat for oral administrerin<g> til en pasient med inflammatorisk tarmsykdom.
Det ble fremstilt et preparat inneholdende 150 mg rifabutin, 250 mg klaritromycin og 50 mg klofazimin. Preparatet ble presentert i form av en kapsel inneholdende hvert av de anti-atypiske, mycobakterielle midler i en mikro-innkapslet form.
Eksempel 5: Preparat for oral administrerin<g> til en pasient med inflammatorisk tarms<y>kdom.

Claims (10)
1.
Anvendelse av en sammensetning omfattende tre eller flere anti-atypiske mycobakterielle midler, for fremstilling av et medikament for behandling av inflammatorisk tarmsykdom, hvor de anti-atypiske mycobakterielle midler er valgt blant klaritromycin, rifabutin, klofazimin og etambutol.

Anvendelse ifølge krav 1, hvor preparatet videre omfatter et mycobakteriell ekstrakt eller produkt.

Anvendelse ifølge krav 1 eller 2, hvor preparatet omfatter rifabutin og klaritromycin.
4.
Anvendelse ifølge krav 1 eller 2, hvor preparatet omfatter rifabutin, klaritromycin og klofazimin.
5.
Anvendelse ifølge et hvilket som helst av kravene 1 til 4, hvor preparatet videre omfatter minst en 5-aminosalisylsyre-forbindelse eller 4-aminosalisylsyre-forbindelse.
6.
Anvendelse ifølge krav 5, hvor 5-aminosalisylsyre-forbindelsen eller 4-aminosalisylsyre-forbindelsen er valgt blant mesalazin, olsalazin, salazopyrin og para-amino-salisylsyre.
7.
Anvendelse ifølge krav 2, hvor det mycobakteriell ekstrakt eller produkt omfatter et ekstrakt eller produkt fra ikke-patogene mykobakterier.

Anvendelse ifølge krav 7, hvor de ikke-patogene bakterier omfattende M. vaccae eller M. phlei.

Anvendelse ifølge et hvilket som helst av kravene 2, 7 eller 8, hvor det mycobakteriell ekstrakt eller produkt anvendes i en mengde fra rundt 25 \ ig til rundt 500 ug.

Anvendelse ifølge et hvilket som helst av kravene 2 eller 7-9, hvor det mykobakterielle ekstrakt eller produkt er regressin.
