Foreliggende oppfinnelse vedrører forbindelser som angitt i krav 1 og farmasøytiske sammensetninger som angitt i krav 9 som besitter nevronalaktivitet. Oppfinnelsen vedrører også anvendelser av slike forbindelser ved fremstilling av medikamenter som angitt i krav 14. Forbindelsene og sammensetningene som enten kan anvendes alene eller sammen med en nevrotrofisk faktor, slik som nervevekstfaktor, er egnet til å behandle eller forebygge nerveskade forårsaket av sykdom eller fysisk trauma.
Oppfinnelsens bakgrunn
Nevrologiske sykdommer er forbundet med død av eller skade på nerveceller. For eksempel er tapet av dopaminergiske nevroner i substansia nigra basisen for Parkinsons sykdom. Selv om den molekylære mekanisme av nevrodegenerasjon i Altzheimers sykdom ennå ikke er etablert, kan inflammasjon og avleiring av beta-amyloidprotein og andre slike midler kompromittere nevrona1funksjon eller overlevelse. Hos pasienter som lider av hjerneiskemi eller ryggmargsskader ob-serveres utbredt nervecelledød. For tiden er det ingen tilfredsstillende behandlinger for disse sykdommer.
En tilnærming for å behandle nevrologiske sykdommer involverer anvendelsen av legemidler som er i stand til å inhibere nervecelledød. En nyere tilnærmingsmåte involverer fremmingen av nerveregenerasjon med legemidler som stimulerer nevrittutvekst.
Nevrittutvekst kan stimuleres in vitro med nervevekstfaktor er, slik som NGF. For eksempel demonstrerer Glial cellelinje-avledet nevrotrofisk faktor (GDNF) nevrotrofisk aktivitet både in vivo og in vitro, og undersøkes for tiden for behandling av Parkinsons sykdom. Insulin og insulin-lignende faktorer har blitt vist å stimulere vekst av nevritter i rotte-feokromocytom PC12-celler og i dyrkede sym-patetiske og sensoriske nevroner [Recio-Pinto et al., J. Neurosci., 6, s. 1211-1219 (1986)]. Insulin og insulin-lignende vekstfaktorer stimulerer også regenerasjonen av skadede motoriske nerver in vivo og in vitro [Near et al., PNAS. s. 89, 11716-11720 (1992); og Edbladh et al., Brain Res.. 641, s. 76-82 (1994)]. Tilsvarende stimulerer fibro-blastvekstfaktor (FGF) nerveproliferasjon [D. Gospodarowicz et al., Cell Piffer., 19, s. 1 (1986)] og vekst [M.A. Wal-ter et al.. Lymphokine Cytokine Res.. 12, s. 135 (1993)].
Det er imidlertid flere ulemper forbundet med anvendelsen av nervevekstfaktorer for å behandle nevrologiske sykdommer. De krysser ikke lett blod-hjerne-barrieren. De er ustabile i plasma. Og de har dårlige legeflåttelleverings-egenskaper.
Nylig har små molekyler blitt vist å stimulere aksonalut-vekst in vivo. Hos individer som lider av nevrologiske sykdommer, kan denne stimulering av nevrittutvekst beskytte nevroner fra videre degenerasjon og akselererer regenerasjonen av nerveceller. For eksempel har østrogen blitt vist å fremme veksten av aksoner og dendritter, som er nevritter utsendt av nerveceller for å kommunisere med andre i en utviklende eller skadet voksen hjerne. [(C. Dominique Toran-Allerand et al., J. Steroid Biochem. Mol. Biol., 56, s. 169-78 (1996); og B.S. McEwen et al., Brain Res. Dev. Brain. Res.. 87, s. 91-95 (1995)]. Progresjonen av Alzheimers sykdom kan være langsom hos kvinner som tar østrogen. Østrogen er antatt å supplere NGF og andre nevrotrofiner og derved hjelpe nevroner til å differensiere og overleve.
Det har blitt rapportert at forbindelser med en affinitet for det FK506-bindende protein (FKBF) som inhiberer at pro-teiners rotamaseaktivitet også besitter nervevekststimulerende aktivitet. (Lyons et al., PNAS. 91, s. 3191-3195
(1994)] . Mange av disse forbindelsene har også immunosuppresiv aktivitet. FK506 (Tacrolimus), et immunosuppresivt legeflåttel, har blitt vist å virke synergistisk med NGF i stimulering av nevrittutvekst i PC12-celler samt sensoriske ganglier [Lyons et al., (1994)]. Denne forbindelsen har også blitt vist å være nevroprotektiv ved fokal cerebral iskemi [J. Sharkey og S.P. Butcher, Nature. 371, s. 336-339 (1994)] og å øke aksonal regenerasjonshastighet i skadet isjiasnerve [B. Gold et al., J. Neurosci.. 15, 7509-16 (1995)].
Anvendelsen av immunosuppresive forbindelser har imidlertid åpenbare ulemper. I tillegg til å kompromittere immunfunk-sjon kan forlenget behandling med disse forbindelser forårsake nevrotoksisitet [Kopp et al., J. Am. Soc. Nephrol., 1, s. 162 (1991)], nevrologiske mangler [P.C. DeGroen et al., N. Eng. J. Med., 317, s.861 (1987) og vaskulær hypertensjon [Kahan et al., N. Eng. J. Med., 321, s.1725 (1989)].
Mer nylig har sub-klasser av FKBP-bindende forbindelser som inhiberer rotamaseaktivitet, men som angivelig mangler immunosuppresiv aktivitet, blitt brakt for dagen for anvendelse i stimulering av nervevekst [se U.S. patenter 5,614,547; 5,696,135; WO 96/40633; WO 96/40140; WO 97/16190; J.P. Steiner et al., Proe. Nati. Acad. Sei. USA, 94, s. 2019-23 (1997); og G.S. Hamilton et al., Bioorg. Med. Chem. Lett., 7, s. 1785-90 (1997)]. Mens disse forbindelser formodentlig unngår visse uønskede bieffekter hos immunosuppresive FKBP-bindende forbindelser, binder de fremdeles til FKBP og inhiberer dens rotamaseaktivitet. Denne sistnevnte egenskap kan fremdeles føre til uønskede bieffekter hvilket kan skyldes andre roller FKBP kan spille i pattedyr.
Overraskende er det nå funnet at binding til FKBP ikke er nødvendig for nevronalaktivitet. Samtidig svevende U.S. patentsøknad med serienummer 08/748,447; 08/748,448 og 08/749,114 beskriver anvendelsen av ikke-FKBP-binding, ikke-immunosuppresive forbindelser for å stimulere nervevekst og forhindre nevrodegenerasjon. På grunn av deres mangel på affinitet for FKBP, unngår disse forbindelser fordelaktig enhver potensiell interferens med FKBP-forbin-dende biokjemiske veier. Disse forbindelser inhiberer imidlertid multi-legeflåttelresistens ("MDR") gjennom inhibering av p-glycoproteinet og MRP. Selv om det synes som om de nødvendige doseringer av disse forbindelser for å stimulere nervevekst og forhindre nevrodegenerasjon er lavere enn de som forårsaker MDR, vil det fremdeles være øn-skelig å oppnå forbindelser som er spesifike for nevronalaktivitet, uten andre signifikante virkningsmekanismer.

Internasjo-nale publikasjoner WO 94/07858 og WO 95/26337 bringer for dagen en familie av forbindelser som besitter multi-legef låttel-reverserende aktivitet. J. Med. Chem. 12,
(1969), s. 677-680 bringer for dagen syntetiserte forbindelser og studier på inhiberingen av saue-rød-blodcellerlyse med hemolysin og komplement fra marsvin. Anal. Biochem. 129 (1983), s. 502-512 viser syntesen av et fluoriserende substrat for grisepepsin.
Skjønt en bred variasjon av nevrologisk degenerative for-styrrelser kan behandles ved å stimulere nevrittutvekst, er det kjent relativt få midler som besitter disse egenskaper. Dessuten har de nyere ikke-immunosuppresive forbindelser bare nylig blitt begynt å testes i levende organismer. Således er det fortsatt et behov for nye farmasøytiske akseptable forbindelser og sammensetninger som har evnen til å stimulere nevrittutvekst og forhindre nevrodegenerasjon i pasienter uten å forårsake immunosuppresjon, uten å interferere med FKBP og uten å påvirke cellulære pumper, slik som p-glycoprotein eller MRP.
Oppsummering av oppfinnelsen
Søkerne har identifisert flere sub-klasser med forbindelser som ikke binder til FKBP, som ikke inhiberer MDR, men som har potent nevrologisk aktivitet.
Begrepet "nevrologisk aktivitet", som anvendt her, inkluderer stimulering av skadede nevroner, fremming av nerveregenerasjon, forebygging av nevrodegenerasjon og behandling av en nevrologisk forstyrrelse. Forbindelsene av denne oppfinnelse har aktivitet i både perifere nerver og sen-tralnervesystemet .
To av disse subgenus faller innenfor familien av forbindelser beskrevet i WO 92/21313. Disse forbindelser er karakterisert ved formelen:

Sammensetningene brakt for dagen her omfatter en forbindelse av denne oppfinnelsen, en bærer, og eventuelt en nervevekstfaktor.
De ovenfor nevnte forbindelser enten alene eller i kombina-sjon med en nevral vekstfaktor kan anvendes ved fremstilling av medikamenter som kan stimulere nervevekst og forhindre nevrodegenerasjon. Medikamentene er nyttige for å behandle eller forebygge nerveskade forårsaket av forskjellige nevrologiske sykdommer og fysiske traumer og også i ex vivo nerveregenerasjon.
DETALJERT BESKRIVELSE AV OPPFINNELSEN
Søkerne har oppdaget mange forbindelser med nevronal aktivitet, som ikke binder til FKBP, og som ikke har multi-legef låttel-resistans reverseringsaktivitet. Uten å være bundet av teori tror søkerne at forbindelsene brakt for dagen i denne søknad utøver sin nevronale aktivitet ved å øke cytoplasmiske Ca<2+->konsentrasjoner. Dette oppnås sannsyn-ligvis ved interaksjon, enten direkte eller indirekte, med en kalsium-frigivelsekanal, slik som ryanodinreseptoren eller inositol 1,4,5-trifosfatreseptoren, i den endoplasmiske retikulum av nervecellen.
Således, i henhold til en utførelse, frembringer oppfinnelsen medikamenter for å stimulere nervevekst eller forebygge nevrodegenerasjon ved å kontakte nerveceller med en forbindelse ifølge krav 1 som: a. øker cytoplasmisk Ca<2+->konsentrasjonen eller binder til ryanodinreseptoren;
b. ikke binder til FKBP; og
c. ikke besitter MDR reverseringsaktivitet.
I henhold til en beslektet utførelse frembringer den foreliggende oppfinnelse en forbindelse ifølge krav 1 som: a. har nevronal aktivitet;
b. øker cytoplasmisk Ca<2+->konsentrasjon eller binder til ryanodinreseptoren;
c. ikke binder til FKBP; og
d. ikke besitter MDR-reversibel aktivitet.
Begrepet "øker cytoplasmisk Ca2+-konsentrasjon", som anvendt her, betyr en detekterbar økning i kanal som regist-rert i den enkelte kanalregistrerende test beskrevet under i nærvær av en slik forbindelse sammenlignet med en passende kontroll. Alternativt betyr begrepet "øker cytoplasmisk Ca<2+->konsentrasjon", som anvendt her, en detekterbar end-ring i fluorescens-spektrummet i celletesten beskrevet her. Begrepet "binder til ryanodinreseptoren", som anvendt her, betyr at forbindelsen spesifikt konkurrerer med ryanodin for binding til mikrosomer i testen beskrevet under.
Begrepet "binder ikke til FKBP", som anvendt her, betyr at forbindelsen demonstrerer en Ki på 10 uM eller større i minst en av de rotamaseinhiberende tester beskrevet under.
Begrepet "besitter ikke MDR-reversibel aktivitet", som anvendt her,, betyr at ved en konsentrasjon på 2,5 uM har forbindelsen en MDR-ratio på mindre enn 7,0, og fortrinnsvis mindre enn 3,0 i minst en av MDR-testene beskrevet under.
Enkelt-kanalregistrerende eksperimenter er nyttige for å bestemme om forbindelsene av denne oppfinnelse forårsaker den nødvendige økning i cytoplasmisk Ca<2+->konsentrasjon. Disse eksperimenter utføres som beskrevet i E. Kaftan et al., Circulation Research, 78, s. 990-997 (1996), frem-bringelsen av hvilken inkorporeres her pr. referanse. Enkelt-kanalregistreringer utføres under spenningsklemmebe-tingelser med et par Ag/AgCl-elektroder som kontakter løs-ningene via CsCl-forbindelser. Vehikler tilsettes til cis-kammeret og kondenseres med plane lipid-bilag sammensatt av fosfatidyletanolamin/fosfatidylcholin (3:1, 30 mg/ml i de-kan, Avanti Polar Lipids). Trans-kammeret inneholder 250 niM HEPES og 53 mM Ba(OH)2, pH 7,35; cis-kammeret inneholder 250 mM HEPES-Tris pH 7,35. Forbindelser løst i metanol tilsettes til cis-kammeret. Kanalstrømmer forsterkes ved å anvende en bilag-klemmeforsterker (BC-525A, Warner Instruments) og tas opp på VHS-bånd (Dagen Corp.). Data fil-treres til et åtte-pols Besselfilter (Frequency Devices) til 500 Hz, digitaliseres ved 2 kHz, overføres til en per-sonlig datamaskin og analyseres med pClamp version 6,0 (Axon Instruments). Enkeltkanalregistreringer gjøres minst 3 ganger for hver forbindelsebetingelse.
Ryanodinbinding kan måles ved å inkubere mikrosomalprotein med <3>H-ryanodin i buffer inneholdende 36 mM Tris, pH 7,2 og 50 mM KCl i fravær eller nærvær av testforbindelser. Kon-troller for maksimal binding ble gjort i nærvær av 0,72 mM ATP og 36 mM CaCl2. Ikke-spesifik binding ble målt i nærvær av 25 uM umerket ryanodin. Bindings reaksjoner ble inkubert i 2 timer ved romtemperatur og deretter sentrifugert i 15 minutter ved 30.000 x g. Pelletene ble løst og radio-aktiviteten ble målt ved seintillasjonstelling.
Alternativt kan fluksen av cytoplasmisk Ca<2*> inn i cellen følges med cluorescens. For eksempel kan nerveceller in-kuberes med NGF og et kalsiumbindende fluorescent farge-stoff, slik som Fura-2, i en kalsiuminneholdende buffer. Celler avbildes kontinuerlig både før og etter tilsetningen av en testforbindelse av denne oppfinnelse. Forskjellen i fluorescensintensitet før og etter tilsetningen av forbindelser plottes deretter som et forhold av fluorescensenhe-ter ved 340 nm og 380 nm.
Testing av en forbindelse av denne forbindelse for å be-krefte at den binder til FKBP12 med en Ki på 10 uM eller høyere kan oppnås ved å anvende flere kjente tester i faget. Spesielt kan disse forbindelser testes for sin evne (eller mangel på) til å inhibere rotamase. Eksempler på tester som måler inhibering av FKBP12-rotamaseaktivitet er de hvor isomeriseringen av et kunstig substrat --N-succi-nyl-Ala-Ala-Pro-Phe-p-nitroanilid -- følges spektrofotometrisk [M.W. Harding et al., Nature. 341, s.758-60 (1989); ved J.J. Siekierka et al., Nature, 341, s. 755-57 (1989); og S.T. Park et al., J. Biol. Chem.. 267, s. 3316-24 (1992)]. Testen inkluderer cis-formen av substratet, FKBP12, forbindelsen som skal testes og kymotrypsin. Kymotrypsin er i stand til å spalte p-nitroanilid fra transformen fra substratet, men ikke eie-formen. Frigivelse av p-nitroanilid måles.
Andre FKBP-bindingstester inkluderer en konkurranse LH20-bindingstest som anvender FK-506 som en rapporterende li-gand. Disse har blitt beskrevet av [M.W. Harding et al., Nature, 341, s.758-60 (1989); og ved J.J. Siekierka et al., Nature. 341, s. 755-57 (1989).

Spesielt anvendes cellelinjer som er kjent å være resistente mot et spesielt legeflåttel. Disse cellelinjer inkluderer, men er ikke begrenset til, L1210, P388D, HL60 og MCF7 cellelinjene. Alternativt kan resistente cellelinjer utvikles. Cellelinjen eksponeres for legemidlet som det er resistent mot, eller testforbindelsen; cellelevedyktighet måles deretter og sammenlignes med levedyktigheten til celler hvilke eksponeres for legemidlet i nærvær av testforbindelsen ("MDR-forhold").
I en foretrukket utførelse av formel I, er ikke A og B samtidig hydrogen. Enda mer foretrukket er det at minst en av A eller B er en (Ci-Ce) -rettkjedet alkyl terminalt substituert med Ar (som selv er substituert eller usubstituert). Mer foretrukket er minst en av A eller B en (Ci-C6) - rettkjedet alkyl terminelt substituert med pyridin (som selv er substituert eller usubstituert).
I en annen foretrukket utførelse av formel I, er J en (Ci-C3)-rettkjedet alkyl.
I enda en annen foretrukket utførelse av formel I er K en Ar-substituert {Ci-C3)-rettkjedet alkyl. Mer foretrukket er det når K er (Ci-C3) -rettkjedet alkyl terminalt substituert med en usubstituert fenyl.
I henhold til en annen foretrukket utførelse av formel I, velges D fra (Ci-C6)-rettkjedet eller forgrenet alkyl. Ar, Ar-substituert-(Ci-C6)-rettkjedet eller forgrenet alkyl, Ar-substituert-(Ca-C6)-rettkjedet eller forgrenet alkenyl eller alkynyl , eller -CH3-S(0)a-(Ci-C4)-rettkjedet eller forgrenet alkyl.
Mer foretrukket velges D fra aminfenyl, nitrofenyl, isopropyl, benzyl, fluorfenyl, cyanofenyl, metoksyfenyl, dimetoksyfenyl, metylsulfonylmetyl, etylenfenyl, dinitroani-1 inf enyl, N, N-dimetylaminf enylazof enyl-N, N-dimetylamin-naftyl eller acetamidfenyl.
Mest foretrukket velges D fra 4-aminfenyl, 2-nitrofenyl, metylsulfonylmetyl, benzyl, etylenfenyl, 4-fluorfenyl, 4-cyanofenyl, 3,4-dimetoksyfenyl, 4-metoksyfenyl, 4-(2,4-di-nitroanilin)fenyl, 4-{(4-(N,N-dimetylamin)fenyl)azo)fenyl, 5-(N,N-dimetylamin)naftyl eller 4-acetamidfenyl.
I henhold til en annen foretrukket utførelse er Y metyl.
De mest foretrukne forbindelser med formel (I) er fremsatt i tabell 1 nedenfor:
Denne oppfinnelse inkluderer alle optiske og rasemiske iso-merer av forbindelsene med formel I, samt farmasøytiske akseptable derivater derav.
Et "farmasøytisk akseptabelt derivat" som anvendt her, be-tegner ethvert farmasøytisk akseptabelt salt, ester, prodroge eller salt av slik ester eller prodroge, av en forbindelse av denne oppfinnelse eller enhver annen forbindelse som ved administrasjon til en pasient, er i stand til å frembringe (direkte eller indirekte) en forbindelse av denne oppfinnelse, eller en metabolitt eller rest derav, karakterisert ved nevronal aktivitet.
Overraskende og uventet binder ikke forbindelsene med formel (I) av denne oppfinnelse til FKBP, inhiberer ikke dens rotamaseaktivitet og er ikke immunosuppresive. Dessuten, selv om forbindelsene av denne oppfinnelse bærer noen strukturell likhet med forbindelser som er kjent og rever-serer multi-legeflåttelresistans (se U.S. patent 5,543,423; WO 95/26337 og WO 94/07858), synes ikke de foreliggende forbindelser å ha noen aktivitet mot MDR. Således besitter de nylig viste forbindelser fordelaktig nevronalaktivitet, uten å interferere med andre veier kjent for å bli påvirket av strukturelt lignende forbindelser.
Nervevekstaktiviteten til forbindelsene av denne oppfinnelse kan testes initielt ved å anvende flere celledyrk-ningstester som er kjent i faget. For eksempel kan forbindelsene av denne oppfinnelse testes i en nevrittut-veksttest ved å anvende feokromocytom PC12-celler som beskrevet av Lyons et al., PNAS. 91, s. 3191-3195 (1994). En tilsvarende test kan utføres i SH-SY5Y humane nevroblastom-celler. Alternativt kan kylling-rygg-rot-gangliatesten beskrevet i U.S. patent 5,614,547 eller i G.S. Hamilton et al., Bioorg. Med. Chem. Lett.. (1997) og referanser sitert deri benyttes.
Forbindelsene av denne oppfinnelse kan også testes for nervevekst aktivi tet in vivo ved å anvende en musemodell for Parkinsons sykdom [J.P. Steiner et al., Proe. Nati. Acad. Sei. USA. 94, s. 2019-23 (1997), U.S. patent 5,721,256] eller etter kirurgisk isjiasnerveknusing i rotter.
I henhold til en annen utførelse frembringer oppfinnelsen sammensetninger omfattende en forbindelse med formel (I), og en farmasøytisk akseptabel bærer. Fortrinnsvis formuleres sammensetningene av denne oppfinnelse for administrasjon til et pattedyr (dvs. farmasøytiske sammensetninger).
Hvis farmasøytisk akseptable salter av forbindelsene av denne oppfinnelse benyttes i disse sammensetninger, er disse salter fortrinnsvis avledet fra uorganiske eller organiske syrer og baser. Inkludert blant slike syresalter er de følgende: acetat, adipat, alginat, aspartat, benzoat, benzensulfonat, bisulfat, butyrat, citrat, kamfer, kamfer-sulfonat, cyklopentanpropionat, diglukonat, dodecylsulfat, etansulfonat, format, fumarat, glukoheptanoat, glykerofos-fat, glykolat, hemisulfat, heptanoat, heksanoat, hydroklo-rid, hydrobromid, hydroiodid, 2-hydroksyetansulfonat, lak-tat, maleat, malonat, metansulfonat, 2-naftalensulfonat, nikotinat, nitrat, oksalat, palmoat, pektinat, persulfat, 3-fenylpropionat, fosfat, pikrat, pivalat, propionat, sa-licylat, succinat, sulfat, tartrat, tiocyanat, tosylat og undekanoat. Andre syrer, slik som oksalsyre, selv om de ikke i seg selv er farmasøytisk akseptable, kan anvendes i fremstillingen av salter som er nyttige som intermediater for å oppnå forbindelsene av oppfinnelsen og deres farma-søytiske akseptable syreaddisjonssalter.

Vann- eller oljeløselige eller dispersible pro-dukter oppnås derved.
Forbindelsene benyttet i sammensetningene i denne oppfinnelse kan også modifiseres ved å tilknytte passende funksjonaliteter for å øke selektive biologiske egenskaper. Slike modifikasjoner er kjent i faget og inkluderer de som øker biologisk penetrasjon inn i et gitt biologisk system (for eksempel blod, lymfesystem, sentralnervesystem), øker oral tilgjengelighet, øker løselighet for å tillate administrasjon ved injeksjon, endrer metabolisme og endrer eks-kresjonshastighet.

Cyklodekstriner slik som a- J3- og y-cyklodekstriner, eller kjemisk modifiserte derivater slik som hydroksyalkylcyklodekstriner, inklusive 2- og 3-hydroksy-propyl-p-cyklodekstriner, eller andre løselige derivater kan også fordelaktig anvendes for å øke levering av forbindelser av forbindelsene av denne oppfinnelsen.
I henhold til en annen utførelse omfatter de farmasøytiske sammensetninger av denne oppfinnelse ytterligere en nevrotrofisk faktor. Begrepet "nevrotrofisk faktor" som anvendt her, refererer til forbindelser som er i stand til å stimulere vekst eller proliferasjon av nervevev. Tallrike nevrotrofiske faktorer har blitt identifisert i
faget, og hvilke som helst av disse faktorer kan benyttes i sammensetningene av denne oppfinnelse. Disse nevrotrofiske faktorer inkluderer, men er ikke begrenset til, nervevekstfaktor (NGP), insulinvekstfaktor (IGF-1) og dens aktive,
avkortede derivater slik som gIGF-1, sur og basisk fibro-blastvekstfaktor (henholdsvis aFGF og bFGF), plateavledede vekstfaktorer (PDGF), hjerneavledet nevrotrofisk faktor (BDNF), ciliære nevrotrofiske faktorere (CNTF), glialcelle-linje-avledet nevrotrofisk faktor (GDNF), nevrotrofin-3 (NT-3), nevrotrofin 4/5 (NT-4/5), eller hvilke som helst av forbindelsene beskrevet i WO 97/36869; WO 96/41609; WO 97/16190; WO 96/40633; WO 97/18828; WO 96/40140 eller WO 98/13355. Den mest foretrukne nevrotrofiske faktor i sammensetningene av denne oppfinnelse er NGF.
Sammensetningene av den foreliggende oppfinnelse kan administreres oralt, parenteralt, ved inhalasjonsspray, topisk, rektalt, nasalt, bukkalt, vaginalt eller via et im-plantert reservoar. Sterile injiserbare former av sammensetningene av denne oppfinnelse kan være vandige eller oljelignende suspensjoner. For topiske applikasjoner kan de farmasøytiske sammensetninger formuleres i en egnet salve inneholdende den aktive komponent suspendert eller løst i en eller flere bærere.
Bærere for topisk administrasjon av forbindelsene av denne oppfinnelse inkluderer, men er ikke begrenset til, mineralolje, parafinolje, vaselin, propylenglycol, polyoksyetylen, polyoksypropylenforbindelse, emulgerende voks, stearinsyre, cetylstearat, cetylalkohol, lanolin, magnesiumhydroksid, kaolin og vann. Alternativt kan de farmasøytiske sammensetninger formuleres i en egnet lotion eller krem inneholdende de aktive komponenter suspendert eller løst i en eller flere farmasøytisk akseptable bærere. Egnede bærere inkluderer, men er ikke begrenset til, mineralolje, sorbi-tanmonostearat, polysorbat 60, cetylestere, voks, cetylalkohol, 2-oktyldodekanol, benzylalkohol og vann.
For oftalmisk anvendelse kan de farmasøytiske sammensetninger formuleres som mikroniserte suspensjoner i isoton, pH-justert steril saltløsning, eller, fortrinnsvis som løs-ninger i isoton, pH-justert steril saltløsning, enten med eller uten et konserveringsflåttel slik som benzylalkonium-klorid. Alternativt, for oftalmisk anvendelse, kan de far-masøytiske sammensetninger formuleres i en salve slik som petrolatum.
De farmasøytiske sammensetninger av denne oppfinnelse kan også administreres med nasal aerosol eller inhalasjon. Slike sammensetninger fremstilles i henhold til velkjente teknikker i faget for farmasøytisk formulering og kan fremstilles som løsninger i saltløsning, ved å anvende benzylalkohol eller andre egnede konserveringsmidler, absorp-sjonsfremmere for å øke biotilgjengelighet, fluorkarboner, og/eller andre konvensjonelle oppløsnings- eller disper-geringsmidler.

Syntesen av (S)-(N-Metyl)-2-(metyl-(4-amin-benzensulfanilamid) )-3-fenyl-N-(2-(pyridin-2yl) etyl)propionamid (forbindelse 2) er fremsatt under.
A. (N-Metyl)-2-(metyl-2-(tertbutyloksykarbonyl)amin-3-fenyl-N-(3-pyridin-4-yl)-propylbutylpropionamid.
Til en løsning av Boe-(N-Metyl)fenylalanin (5,0 g, 17,8 mmol) i metylenklorid (50 ml) ble EDC (6,0 g, 191,71, 31,2 mmol) tilsatt etterfulgt av tilsettingen av 2-(2-metylamin-etyl)pyridin (2,5 g, 18,4 mmol). Blandingen ble tillatt å omrøre ved omgivelsestemperatur i 24 timer. Løsningen ble fortynnet med 200 ml etylacetat og vann (50 ml) . Den vandige fase ble gjort basisk ved tilsetting av 2N NaOH inntil pH 12 ble oppnådd. De separerte organiske faser ble tørket over vannfri MgS04 og konsentrert under redusert trykk. B. (N-Metyl) - 2 - (metylamin) - 3 - f enyl-N- (2- (pyridin-2yl)etyl)propionamid.
Til forbindelsen fra trinn A (2,83 g, 7,1 mmol) i metylenklorid (25 ml) ble trifluoreddiksyre (10 ml, 130 mmol) tilsatt. Blandingen ble tillatt å omrøre i 2 timer ved omgivelsestemperatur. Løsningen ble konsentrert til tørrhet under redusert trykk. Residuet ble tatt opp i etylacetat (200 ml) og vann (50 ml). Den vandige fase ble gjort basisk ved tilsetting av 2N NaOH inntil ph 14-15 ble oppnådd.
De separerte organiske faser ble tørket over vannfri MgS04 og konsentrert under redusert trykk. Til en løsning av forbindelsen av trinn B {300 mg, 1,05 mmol) i metylenklorid (15 ml) ble trietylamin (2,0 ml, 101,19, 19,8 mmol) tilsatt etterfulgt av tilsettingen av 4-nitrobenzensulfonylklorid (330 mg, 1,56 mmol). En. løsning av forbindelsen fra trinn C (453 mg, 0,94 mmol) i etylacetat (20 ml) ble behandlet under omgivelsestemperatur med 150 mg 10 % palladium på karbon og hydrogenert i 24 timer under et svakt positivt hydrogentrykk. Blandingen ble filtrert og konsentrert in vacuo og råproduktet ble rensent via medium trykk væskekromatograf i ved å anvende 2:98 metanol/metylenklorid etterfulgt av 0,5:5:95 NH4OH/metanol/metylenkloridløsning som løseflåttelsystemet for å gi 194 mg (46 % utbytte) av tittelforbindelsen som en gulaktig olje. B. (S) -(N-metyl)-2-(metylamin)-3-fenyl-N-(3-(pyridin-4-yl) -1-(4-(pyridin-3yl)propyl)butyl)propionamid.
Til en løsning av trinn A (5,0 g, 9,2 mmol) i metylenklorid (20 ml) ble trifluoreddiksyre (20 ml, 260 mmol) tilsatt. Blandingen ble tillatt å omrøre i 3 timer under omgivelsestemperatur. Oppløsningen ble konsentrert til tørr-het under redusert trykk. Residuet ble tatt opp i etylacetat (200 ml) og en mettet natriumbikarbonatløsning (100 ml) . Til en løsning av forbindelsen av trinn B (1,5 g, 3,4 mmol) i metylenklorid (20 ml) ble trietylamin (5,0 ml, 101,19, 35,8 mmol) tilsatt etterfulgt av tilsettingen av 4-nitro-benzensulf onylklorid (1,0 g, 4,7 mmol). Blandingen ble tillatt å omrøre ved omgivelsestemperatur i 24 timer. Løs-ningen ble fortynnet med 150 ml etylacetat og vasket med vann (50 ml). D. (S)-(N-Metyl)-2- (metyl)-(4-amin-benzensulfanilamid))-3-fenyl-N-(4-(pyridin-3-yl)-1-(3-(pyridin-3-yl)propyl)butyl)propionamid (forbindelse 4).
En løsning av forbindelsen fra trinn C (1,75 g, 2,78 mmol) i etylacetat (50 ml) ble behandlet under omgivelsestemperatur med 1,0 g 10 % palladium-på-karbon og hydrogenert i 24 timer under et svakt positivt hydrogentrykk. Blandingen ble filtrert og konsentrert in vacuo og råproduktet renset via medium trykk væskekromatografi ved å anvende metylenklorid etterfulgt av 1:99 metanol/metylenklorid etterfulgt av 3:97 metanol/metylenkloridløsning som løseflåttelsyste-met for å gi 0,79 mg (47 % utbytte) av tittelforbindelsen som en gulaktig olje. TLC: Fr=0,36 (1:10:90 NH4OH/metanol/metylenklorid). HPLC: Rt=7,97min, [<H>l]-NMR (CDC13) overensstemmende med struktur.
Syntesen av andre forbindelsen av denne oppfinnelse, inklusive dem listet i tabell 1 over, kan oppnås ved å modifi-sere synteseskjemaene fremsatt i eksempel 1-4 ved å anvende passende reagenser som er velkjent i faget.
Eksempel 3
Rotamase inhibisjonstest.
Inhibering av FKBP enzymaktivitet ble bestemt i PPIase-testen beskrevet i S.T. Park et al., J. Biol. Chem.. 267, s. 3316-24 (1992) . Dette er en kymotrypsinkoblet test hvor FKBP katalyserer cis- til trans-isomerisering av Leu-Pro-bindingen i peptidsubstratet Suc-Ala-Leu-Pro-Phe-pNA. Transformen, men ikke cisformen, spaltes av kymotrypsin. Frigivelsen av Phe-pNA ble monitorert ved absorbanse på 400 nm. Kymotrypsinspaltingen er svært hurtig, og derfor er cis-til-trans-isomeriseringen hastighetsbegrensende.
Hver reaksjonsblanding besto av 0,1 M Trisbuffer, pH 7,8, 15 nM FKBP, 30 uM substrat og 0,5 nM til 10 uM forbindelse som skulle testes fortynnet i Me2SO. Blandingen ble inkubert ved 15°C i 5 minutter og deretter ble reaksjonen in-itiert ved tilsetting av kymotrypsin (100 ug/ml sluttkon-sentrasjon) og fulgt spektrofotometrisk i 5 minutter. MDR- sensebiliseringtest
For å demonstrere at forbindelsene i henhold til denne oppfinnelse ikke besitter MDR-reverseringsaktivitet, ble cellelinjer som er kjent å ha resistens mot et spesielt legeflåttel anvendt.
Multilegeflåttel-resistensreversering (MDR)tester ble ut-ført ved å anvende L1210vMDRC.06 eller HL60/Vinc cellelinjer. Tester anvendende HL60/Vinc-celler ble utført ved å spre celler i 96 brønns mikrotiterplater ved en konsentrasjon på 4 x 10<*> celler/brønn. Eksempel 5
Neurittutvekstkvantitisering i PC12- cellesystem
For å direkte bestemme den nevrotrofiske aktivitet til forbindelsene beskrevet i denne oppfinnelse ble nevrittut-veksttesten utført med feokromocytom PC12-celler som beskrevet av Lyons et al., (1994).
PC12-celler ble holdt ved 37°C og 5% C02 i Dulbecco's modifiserte Eagle's medium (DMEM) supplert med 10% varmeinaktivert hesteserum, 5% varmeinaktivert føtalt bovint serum (FBS), og 1% glutamat.
Cellene ble deretter spredd ved 105 pr brønn i 96 brønns plater belagt med 5 m/cm<2> rottehalekollagen og tillatt å feste over natten. Mediet ble deretter erstattet med DMEM, 2% varmeinaktivert hesteserum, 1% glutamat, 1-5 ng/ml NGF (Sigma) og varierende konsentrasjoner av forbindelsene av denne oppfinnelse (0,01 nM - 10.000 nM). Bakgrunnskon-trollkulturen ble administrert med 1-5 ng/ml NGF alene uten forbindelse. Positive kontrollkulturer ble administrert med høye konsentrasjoner av NGF (50 ng/ml). Cellene ble deretter inkubert ved 37°C ved 5% C02 i 72 timer, fiksert med 3% formaldehyd, og nerveutvekst ble bestemt visuelt på en skala fra 0 til 4.
Resultatene vist i tabellen under.
Forbindelsene av denne oppfinnelse, som eksemplifisert med forbindelser 2, 4-8 og 11-18 forårsaket en signifikant økning i nevrittutvekst over bakgrunnskontroilkulturer. Mangel på nervevekststimulerende aktivitet (som indikert ved "0") ved høye forbindelseskonsentrasjoner av visse forbindelser tilskrives de toksiske effekter av disse forbindelser på cellene ved høyere konsentrasjoner.
Andre forbindelser av denne oppfinnelse vil også demonstrere signifikant nervevekststimulerende aktivitet.

1. Forbindelse

Y er rettkjedet eller forgrenet (Ci-C6) -alkyl,
K er rettkjedet eller forgrenet Ar-substituert (Ci-C6) - alkyl,
n er 0;
J er rettkjedet eller forgrenet (Ci-C6)-alkyl, og
D er valgt fra gruppen bestående av rettkjedet eller forgrenet (Ci-C6)-alkyl, Ar, rettkjedet eller forgrenet Ar-substituert (Ci-C6)-alkyl, rettkjedet eller forgrenet Ar-substituert (C2-C6)-alkenyl eller -alkynyl og rettkjedet eller forgrenet -CH2-S{0)2- (Ci-C*) -alkyl, cyanofenyl, dinitroanilinofenyl, N,N-dimetylaminofenylazofenyl;
hvori
Ar er valgt fra fenyl, 1-naftyl og 2-naftyl og hvor
hver Ar eventuelt er substituert med 1 til 3 substituenter uavhengig valgt fra halogen, hydroksyl, nitro, -S03H, tri-fluormetyl, trifluormetoksy, rettkjedet eller forgrenet (Ci-Cs)-alkyl, rettkjedet eller forgrenet 0- [ (Ci-C6) - alkyl], 1,2-metylendioksy, -IKR<1>) (R<2>), karboksyl, rettkjedet eller forgrenet N-(C1-C5)-alkylkarboksamider,
hvor R<1> og R<2> uavhengig er valgt fra (Ci-C6) -rettkjedet eller forgrenet alkyl eller hydrogen, og
hvor forbindelsen med formel (I) ikke er (S)-(2-(5-(dime-tylamino)-1-naftalensulfanilamido)-3-fenyl)karboksylsyre, 1-(pyridin-4-yl)propylester eller (S)-(p-toluensulfanil-amido) -3-fenyl-N-(pyridin-2-yl)metyl)propionamid.

5. Forbindelse ifølge krav 4, karakterisert ved at Jeret rettkjedet (Cx-C3)-alkyl.
6. Forbindelse ifølge krav 4, karakterisert ved at Keret rettkjedet (Ci-C3)-alkyl terminalt substituert med fenyl.
7. Forbindelse ifølge krav 1 eller 4, karakterisert ved at Yer metyl.
8. Forbindelse ifølge krav 1 eller 4, karakterisert ved at forbindelsen er valgt fra enhver av forbindelsene i tabell 1.
9. Sammensetning,
karakterisert ved at den omfatter: a. en nevrotrofisk mengde av en forbindelse ifølge krav 1 eller 4; og b. en farmasøytisk akseptabel bærer.
10. Sammensetning ifølge krav 9, karakterisert ved at den formuleres som en farmasøytisk akseptabel sammensetning.
11. Sammensetning ifølge krav 10, karakterisert ved at den ytterligere omfatter en nevrotrofisk faktor.
12. Sammensetning ifølge krav 11, karakterisert ved at den nevrotrofiske faktor velges fra nervevekstfaktor (NGF), insulinvekstfaktor (IGF) og aktive trunkerte derivater derav, sur fibroblast-vekstfaktor (aFGF), basisk fibroblast-vekstfaktor (bFGF), plateavledet vekstfaktor (PDGF), hjerneavledet nevrotrofisk faktor (BDNF), ciliære nevrotrofiske faktorer (CNTF), glial celleavledet nevrotrofisk faktor (GDNF), nev-rotrof in -3 (NT-3) og nevrotrofin 4/5 (NT-4/5).
13. Sammensetning ifølge krav 12, karakterisert ved at nevrotrofinfak-toren er nervevekstfaktor (NGF).
14. Anvendelse av en nevrotrofisk mengde av en forbindelse ifølge kravene 1 eller 4 for fremstilling av et medikament for å stimulere nevronal aktivitet i en pasient eller i en ex vivo-nervecelle.
15. Anvendelse ifølge krav 14,
hvor medikamentet administreres til en pasient og formuleres sammen med en farmasøytisk akseptabel bærer i en farma-søytisk akseptabel sammensetning.

hvor medikamentet ytterligere omfatter en nevrotrofisk faktor enten som del av en multippel doseringsform sammen med forbindelsen eller som en separat doseringsform.

hvor medikamentet anvendes for å behandle en pasient som lider av trigeminal nevralgi, glossofaryngeal nevralgi, Bells lammelse, myasthenia gravis, muskulær dystrofi, mus-kelskade, progressiv muskulær atrofi, progressiv bulbær arvet muskulær atrofi, brokk, sprengt eller prolapset inver-tebra skivesyndromer, cervikal spondylose, pleksusforstyr-relser, torakale utløpsdestruksjonssyndromer, perifere nevropatier, slik som de forårsaket av bly, dapson, flått eller porfyri, andre perifere myelinforstyrrelser, Alzheimers sykdom, Gullain-Barre syndrom, Parkinsons sykdom og andre Parkinsoniserte sykdommer, ALS, multippel sklerose, andre sentrale myelinforstyrrelser, slag og iskemi forbundet med slag, nevral paropati, andre nervedegenerative sykdommer, motoriske nevronsykdommer, isjiaspress, nevropati forbundet med diabetes, ryggmargsskader, facialnervepressing og andre traumer, kjemoterapi- og andre medisinsk indiserte nevropatier og Huntingtons sykdom.

hvor medikamentet anvendes for å stimulere ex vivo nerveregenerasjon.
