Description
Foreliggende oppfinnelse vedrører anvendelsen av en kombinasjon av diol og en alfa-hydroksysyre for lokal behandling av hyperkeratotiske hudsykdommer.
Kroniske hyperkeratotiske hudsykdommer kjennetegnes ved en forstyrrelse i keratiniseringsprosessen og inkluderer blant annet sykdommene psoriasis, hyperkeratotisk eksem og iktyose.
Psoriasis karakteriseres av rødaktige avskallende hudlesjoner. Sykdommen har som regel et kronisk forløp. Lokale midler for behandlingen er ofte klissete og/eller misfarvende, f .eks. ditranol og tjære. Steroidpreparater er kosmetisk attraktive, men har vesentlige bivirkninger. Bløtgjørende preparater inneholdende f .eks. salisylsyre og urea i en krem- eller salvebasis, har en viss avskrellende effekt, men er ikke alene tilstrekkelig virksomme.
Hyperkeratotiske eksemer karakteriseres ved fortykket skjellaktig hud, ofte med sprekker (ragader).
Lamellær iktyose er en sjelden, medfødt genodermatose som karakteriseres ved generalisert hyperkeratose, intens tørrhet av huden og store skjell, spesielt over ekstremitetene (H. Traupe, et ai, (1989) The Ichtyoses: A guide to clinical dragnosis, genetic counseling and therapy, 1. Utg., Springer Verlag, Berlin). Selv om sykdommen vanligvis ikke er livstruende, kan den være meget vansirende og forårsake betydelig bekymring gjennom livet for dem som lider av denne. Behandlingen av lamellær iktyose involverer orale vitamin A-derivater (retinoider) kombinert med supplerende lokal behandling med forskjellige bløtgjøringsformuleringer, inneholdende f.eks. urea, propylenglykol og alfa-hydroksysyrer (AHA). De terapeutiske resultatene er i likhet med andre hyperkeratotiske hudsykdommer, ofte skuffende og assosiert med bivirkninger.
E.J. Van Scott, et al., (1974) Arch. Dermatol. 110:586-590 gjorde et pioner-arbeid ved å benytte alfa-hydroksysyrer mot iktyotiske tilstander. Videre beskrev E.J. Van Scott, et al, (1984) J. Am. Acad. Dermatol. 11:867-879 en keratolytisk gel (5-10% alfa-hydroksysyre i en bærer bestående av vann, etanol og propylenglykol (4:4:2) som imidlertid gikk i glemmeboken siden den var irriterende og ikke egnet for påføring over hele kroppen.
I EP-0-292495 er det beskrevet blandinger som i det vesentlige består av en blanding av propylenglykol, urea og eventuelt melkesyre. Blandingene ble angitt å ha en gunstig effekt på hyperkeratotiske hudsykdommer etter lokal påføring på huden. Enkel påføring av blandingen på huden er mulig, men har i praksis vist seg å være meget vanskelig å oppnå effektivt. Blandingen er flytende og tørker langsomt (fordamper) og bruk av plastere og/eller bandasjer ansees nødvendig. Denne frem-gangsmåten ansees å være for upraktisk for generell bruk. Dessuten kan det som følge av de høye konsentrasjonene av propylenglykol (40-80%) og urea (5-20%), forekomme bivirkninger, så som irritasjon, som også er en alvorlig ulempe for daglig påføring av slike preparater over et lengre tidsrom.
US-patent 4.105.783 omtaler blandinger som inneholder et produkt, fremstillet ved, i vandig eller alkoholisk vandig løsning, å omsette én eller flere alfa- eller beta-hydroksysyrer og en base valgt fra gruppen bestående av ammoniumhydroksyd og et organisk alkylamin, i en totalmengde på fra 1 til 20%, for lokal behandling av tørre hudlidelser, så som psoriasis og iktyose. Basene benyttes med hell til å heve pH av blandingene som inneholder hydroksysyrene, uten å ødelegge den terapeutiske virkning av virkestoffene, hvorved hudirritasjonen reduseres. Selv om det ble hevdet at reaksjonsproduktene, dvs. de således dannede ammoniumsaltene og amidene, ikke trenger noen isoleringsprosess og kan inkorporeres direkte i den terapeutiske blandingen, fordrer god tilberednings- og klinisk praksis idag en omhyggelig bestemmelse av virkestoffet både kvalitativt og kvantitativt, og omfattende farma-kologisk og toksikologisk testing av det dannede reaksjonsprodukt.
Det er således fortsatt behov for et effektivt produkt basert på velkjente og veldefinerte virkestoffer, eller en metode som kan påføres lokalt på hyperkeratotisk hud uten å forårsake alvorlige bivirkninger, så som irritasjon.
Formålet med foreliggende oppfinnelse er å muliggjøre anvendelse av en kombinasjon av en diol og en alfa-hydroksysyre i en bærer for fremstilling av et medikament for lokal behandling av hyperkeratotiske hudsykdommer, hvor bæreren har semi-okkluderende egenskaper.
Det er nå oppdaget at blandinger som inneholder en kombinasjon av en diol og en alfa-hydroksysyre i en semi-okkluderende bærer, ved lokal påføring til huden til pasienter som har en hyperkeratotisk hudsykdom, er effektiv og oppviser en betydelig reduksjon av bivirkninger, som f .eks. irritasjon, sammenlignet med blandinger som inneholder de samme mengder av diol og alfa-hydroksysyre i en ikke-okkluderende bærer, så som en gel. I tillegg til dette oppviser blandingene ifølge oppfinnelsen en bemerkelsesverdig høyere effekt enn blandinger som omfatter enten diol eller alfa-hydroksysyre i samme mengde.
Eksempler på dioler som kan benyttes er propylenglykol, butylenglykol, pentandiol og heksylenglykol. Fortrinnsvis benyttes propylenglykol og heksylenglykol, som kan anvendes i en konsentrasjon opptil 40% og fortrinnsvis fra 10-20%.
Eksempler på alfa-hydroksysyrene eller derivater derav, er melkesyre, sitronsyre, glykolsyre, glukuronsyre, galakturonsyre, pyrodruesyre, etc, men fortrinnsvis benyttes melkesyre. Alfa-hydroksysyren kan benyttes i en mengde opptil 10%, men fortrinnsvis benyttes opptil 5%.
De semi-okkluderende bærere som benyttes kjennetegnes ved at det i in vitro og in vivo tester, observeres en okkluderende virkning, som er lavere enn for vaselin, som ansees å være en okkluderende bærer. Den nedre grense for okklusjonsaktiviteten bestemmes av verdier som oppnås for fremstillinger kjent for å være ikke-okkluderende. I in vitro testen beskrevet i Eksempel 7, ansees f .eks. preparater som har en okklusjonsfaktor lavere enn ca. 70, å være ikke-okkluderende preparater. En passende in vivo test, hvor effektiviteten av lokale preparater for gjenopprettelse av hudbarrierefunksjonen av syk hud bestemmes, er omtalt i Eksempel 8. Eksempler på slike semi-okkluderende bærere er fete kremer inneholdende en viss mengde okkluderende fettstoffer, som vaselin, eller kremer som inneholder en vandig suspensjon av faste lipide nanopartikler, f.eks. hvor det faste lipid er parafinvoks, som har et smeltepunkt varierende fra 54 til 57°C.
Kombinasjonen av kun 5% melkesyre og 20% propylenglykol i en fet krembase, kan lett aksepteres av de fleste pasienter og har vist seg å være meget mer effektiv og mindre irriterende enn monoterapi, med hver av de to forbindelsene i samme, eller endog to ganger disse konsentrasjoner. Den forbausende sterke effekt av blandingen av diol og alfa-hydroksysyre i den semi-okkluderende bærer, tyder på at ingrediensene virker synergistisk ved å reversere hyperkeratose.
Blandingene kan benyttes i flere trinn av behandlingen av hyperkeratotiske hudsykdommer. Ved forbehandingen av psoriasis-plakk før behandling med f.eks. kortikosteroider settes inn, har blandingene vist seg å være like gode som standardproduktet, som er 5% salicylsyre i vaselin. De kosmetiske egenskapene til blandingene ble av pasientene ansett som bedre enn de for standardproduktet. Behandlingen av lamellær iktyose består vanligvis av systemisk behandling med retinoider kombinert med lokal støttebehandling. Blandingene som anvendes ifølge oppfinnelsen har vist seg fordelaktige ved bruk under slik lokal støttebehandling. For mindre alvorlige hyperkeratotiske hudlidelser kan blandingene som anvendes i henhold til oppfinnelsen benyttes uten systemisk terapi.
De etterfølgende eksempler illustrerer oppfinnelsen.
Eksempel 1
<*> Den fete krembase består av 3% polyoksyetylen 1000 monocetyleter (Cetomacrogol® 1000), 6% cetostearylalkohol, 18% flytende parafin, 42% vaselin, konserveringsmidler, q.s. buffere, q.s. og vann opptil 100%.
Eksempel 2
<*> Nanopartikkel-suspensjonen består av 30% parafinvoks (smeltepunktområde 54-57°C), 5% polyoksyetylen 1000 monocetyleter og 65% vann.
Eksempel 3
<*> Nanopartikkel-kremen består av 2% cetyl-dimetikon-kopolyol, 10% isopropyl-myristat, 16,67% cyklometikon, 0,33% natriumklorid, 10% parafinvoks, 1,67 polyoksyetylen 1000 monocetyleter, buffere q.s. konserveringsmidler, q.s. og vann opptil 100%. Eksempel 4
<*> Kremen består av 32,5% cetostearylalkohol, 5% polyoksyetylen 1000 monocetyleter og 62,5% vann.

<*> Bæreren består av en blanding av nanopartikkel-suspensjonen i Eksempel 2 og kremen i Eksempel 4.
Eksempel 6
Ti pasienter (7 kvinner: alder 2-38 år) med alvorlig lamellær iktyose, hvorav to var under behandling med peroral acitretin (50-75 mg/dag) ble utvalgt etter sam-tykke. Pasientene ble instruert om å påføre formuleringen i henhold til Eksempel 1, to ganger per dag på leggene og avstå fra all annen lokal medisinering. Tegn på avskalling, hyperkeratose og xerosis ble gitt separate skåre etter en skala fra 0 (trekk ikke forekommende) til 4 (trekk forekommende i stor utstrekning) og summen av disse skåre for disse trekkene (maks.: 12) ble beregnet for hver pasient før og etter endring av terapi. Bruk av formuleringen ifølge Eksempel 1 resulterte etter 1 måned,
i en reduksjon av den gjennomsnittlige totale skåre fra 9,6 ± 2,2 til 3,5 ± 2,3 (p<0,001; paret t-test). Alle pasientene rapporterte øket avflassing av skjell etter noen få dagers behandling, etterfulgt i løpet av 1-2 uker av opptreden av en meget glattere, nesten normalt utseende hud. Ni pasienter anså effekten «bedre enn noen gang tidligere» og valgte senere å utvide behandlingen til resten av kroppen med like gode resultater. En pasient var senere i stand til å redusere sin acitretin-dosering fra 75 mg til 25 mg per dag uten forverring av hudsymptomer. Enkelte pasienter
opplevde en forbigående hudirritasjon etter påføring av kremen. Ingen andre bivirkninger ble registrert.
Eksempel 7
En beholder i form av et begerglass ble benyttet. Beholderen hadde en diameter på 5,5 cm og en høyde på 7 cm og var konstruert for å lukkes øverst med et standard laboratorie-filterpapir (TVN, omsatt av Schut, Nederland, overflate 23,8 cm<2>). Testen ble foretatt ved å anbringe 50 g destillert vann i beholderen, lukke beholderen med papirfilteret, hvor det på den øvre overflate jevnt ble fordelt 200 mg av preparatet som skulle testes, hvoretter den lukkede beholderen ble anbrgat i 72 timer i en ovn ved 33°C og 58% RH. Når alle øvrige betingelser ble holdt like, avhang vekttapet av vann fra beholderen (vannfluks) etter 72 timer utelukkende av okklusiviteten av det testede preparat.
Okklusjonsfaktoren F av det testede preparat ble beregnet etter ligningen:
hvor A er vannfluksen gjennom udekket filter og B vannfluksen gjennom filteret dekket med testpreparatet.
Alle preparatene ble testet i tre paralleller, og det maksimale avvik mellom resultatene for et preparat var 10%. Følgende tabell viser gjennomsnittet av de okklusjonsfaktorer F som ble funnet.
Eksempel 8
Hospitaliserte kvinnelige pasienter (8) med kronisk konstitusjonell eksem, ble innlemmet i en undersøkelse for å vurdere effektiviteten av forskjellige lokale preparater til gjenopprettelse av hudbarrierefunksjonen ved hjelp av TEWL-målinger (Trans Epidermal Water Loss). Fleksor-siden av høyre underarm ble benyttet gjennom hele forsøket. De inkluderte stedene (området mellom en avstand på ca. 5 cm fra håndledd til albue) ble undersøkt for å klassifiseres som lett, moderat eller alvorlig angrepet. Pasientene fikk ikke lov til å benytte noen annen lokal medisinering 24 timer før undersøkelsesstart. Forsøkene ble foretatt i et eget rom med konstant fuktighet og temperatur. Disse parameterne ble kontrollert før starten av forsøkene. Pasientene fikk sitte rolig i minst 10 minutter før starten av målingene. Målingene ble foretatt ved å benytte et Servo Med EP1 Evaporimeter (Servo Med., Stockholm, Sverige), kalibrert i henhold til produsentens instruksjoner. 6 områder på ca. 3 x 3 cm ble markert på hver pasients underarm. 0,5 mL av preparatene ble påført på de markerte flekkene. For hver pasient ble preparatet påført på det samme sted. Etter massering i 0,5 minutter med en fingertupp og et tidsrom på 2 minutter, ble gjenværende preparat fjernet med en treskrape som deretter forsiktig ble strøket av med tørkepapir. Den sjette flekken forble ubehandlet for å muliggjøre kontrollmålinger. Initiale TEWL-verdier ble bestemt for hver flekk før påføring av dressingene (t=0-verdier). De markerte flekkene ble målt 0,5,1,1,5, 2, 3, 4, 6 og 8 timer etter behandling. Alle målinger ble foretatt i tre paralleller.
Resultatene viste at vaselin alltid er det beste preparat med hensyn til for-hindring av TEWL og kan ansees som et okkluderende preparat. Bæreren i Eksempel 1 resulterte i en lavere normalisering av hudbarrierefunksjonen enn vaselin, men preparatet kan ansees som semi-okkluderende.

1. Anvendelse av en kombinasjon av en diol og en alfa-hydroksysyre i en bærer for fremstilling av et medikament for lokal behandling av hyperkeratotiske hudsykdommer, hvor bæreren har semi-okkluderende egenskaper.
