Description
Oppfinnelsen vedrører sammensetninger egnet som elementær medisinsk mat og hypoellergen, medisinsk mat. Disse er forbedrede enterale nærende hypoallergene sammensetninger og mer spesielt hypoallergene blandinger som smaker godt. I tillegg vil det nærende produkt i henhold til oppfinnelsen tilveiebringe en næringsrik, komplett hypoallergen næring til individer med multiple matvareallergier, kort tarmsyndrom, cystisk fibrose, pankreassykdom, gastroenteritt, inflammatorisk tarmsykdom, ukontrollert diaré, feil-
ernæring, proteinufordøyelighet, infeksjonssykdommer eller for pasienter med hypermetabolisme, slik som brann- og traumaofre eller kreftpasienter.
Oppfinnelsens Bakgrunn
Hypoallergene formler eller sammensetninger som også er omtalt som elementalblandinger, er særpreget ved at de inneholder immunologisk ikke-reaktive proteinhydrolysater eller frie aminosyrer. Proteinhydrolysatene omfatter korte peptidfragmenter og/eller frie aminosyrer i stedet for intakt protein som eksempelvis finnes i kumelk- og soya-proteinbaserte blandinger. Disse korte peptidfragmenter og frie aminosyrer er funnet å være mindre immunogene eller allergene enn intakte proteiner.
I tillegg til proteinhydrolysatene og/eller frie amino-
syrer, vil de fleste næringsrikt balanserte elementære eller hypoallergene sammensetninger inneholde karbohy-
drater, lipider, vitaminer og mineraler for å gi en næringsrik fullstendig sammensetning. Disse sammensetninger anvendes for å mate spedbarn, barn og voksne som har allergier eller er følsomme for intakt protein og blir ofte anvendt medisinsk ved behandling av cystisk fibrose,
kronisk diaré, tynntarmsreaksjon, steatore og protein-kalorifeilernæring.
Ett velkjent problem ved fremstilling av elementære eller hypoallergene sammensetninger er produktets ustabilitet. Ekstensiv hydrolyse ay proteinet med syrer eller enzymer er nødvendig for å gi de korte peptider og aminosyrer som anvendes i blandingene for å gjøre slike hypoallergene. Disse veldig forkortede proteiner eller frie aminosyrer har uønskede karakteristika så som dårlig smak og tap av evne til å emulgere fett og er derved ikke i stand til å danne fysiologisk stabile emulsjoner som ikke viser tegn på fase-separasjon.
U.S. patent nr. 4.414.238 til Schmidl, et al., viser en elementær diettblanding omfattende karbonhydrater, aminosyrer og/eller lavmolekylære peptider og lipider. Ele-mentærdietten ifølge dette patent har en lipidkomponent i form av en emulsjon bestående av lipider, en emulgator valgt fra gruppen bestående av mono- og diglyserider, og en stivelse modifisert med ravsyreanhydrid.
U.S. patent nr. 4.670.268 til Mahmoud viser en enteral næringsrik hypoallergen blanding som anvender en stivelse modifisert med oktenylravsyreanhydrid, som anvendes som den eneste lipidemulgator for å gi en næringsrik, velbalansert blanding som utviser utmerket fysikalsk stabilitet.
U.S. patent nr. 5.411.751 til Crissinger viser spedbarn-blandinger som inneholder ikke mer enn ikke-irriterende nivåer av frie, langkjedede (Ci6-C2a) fettsyrer og triglyserider derav. Dette patent angir at fordøyelsesprodukter av triglyserider, spesielt langkjedede fettsyrer og monoglyserider, kan ødelegge intestinal epitelium hos spedbarn og kunne fremskynde sykdommer så som dødelig enterokolitt. Dette patent verken viser eller antyder en velsmakende, elementær eller hypoallergen medisinsk matvare som i det vesentlige er fri for L-glutaminsyre og L-asparginsyre.
U.S. patent nr. 5.234.702- til Katz, et al., viser et pulverformig nærende produkt, hvor det for å beskytte oljekomponenten anvendes et antioksidantsystem omfattende askorbylpalmitat, betakaroten og/eller blandede tokoferoler og citrat.
U.S. patent nr. 3 5.233 viser en fremgangsmåte ved behandling av atropi av skjelettmuskler og tarmslimhinner . som omfatter enteral administrering av glutamin eller et funksjonelt derivat derav i en mengde på 0,4 - 3,0 g/kg/- døgn.
Den nærende sammensetning i henhold til denne oppfinnelse er omhyggelig sammensatt for å gi et nærende produkt som kan være den eneste næringskilde for pasienter som for-bruker den. For å gi effektiv næring til spedbarn, barn og voksne, så tas det i dens mer spesifikke utførelsesformer hensyn til biotilgjengeligheten av spor- og ultraspormineraler og de diettsamspill som involverer sporelementene. U.S. patentene 5.326.569 og 5.550.146 til Acosta, et al., viser en generisk pulverbase, rik på fett, karbohydrater, vitaminer, mineraler og sporelementer som kan blandes med spesifikke aminosyrer til å gi flere forskjellige tera-peutiske produkter som anvendes som næringsbidrag til voksne og barn med forskjellige fordøyelsessykdommer. Disse patenter verken antyder eller viser næringsrike, komplette hypoallergene blandinger ifølge oppfinnelsen som i det vesentlige er fri for L-glutaminsyre og L-aspartinsyre.
Etiologien for et antall gastrointestinale tilstander er vist å være forårsaket i visse tilfeller av matproteinallergener. Én slik gastrointestinal tilstand er eosinofil gastroenteritt. Eosinofil gastroenteritt er særpreget ved perifer eosinofili, eosinofil infiltrering av bukhulen og forskjellige gastrointestinale symptomer. I pediatritil-feller er matantigener den vanligste grunn til eosinofil gastroenteritt. Administrasjon av en elementær hypoallergen diett kan være nødvendig for rask heling fordi selv ekstensivt hydrolyserte kaseinbaserte blandinger slik som Alimentum<®>Protein Hydrolysate Fonnula With Iron (Ross Products Division, Abbott Laboratories, Columbus, Ohio) inneholder peptidfragmenter som er tilstrekkelig store til å fremkalle en allergisk reaksjon for de mest følsomme allergiske individer.
U.S. patent nr. 5.492.899 til Masor, et al., viser en enteral næringsrik blanding inneholdende ribonukleotider ved spesifikke nivåer. Blandingen i henhold til patentet omfatter karbohydrater, lipider, proteiner, vitaminer og mineraler og fire (4) spesifikke nukleotider i spesifikke nivåer og forhold. Én foretrukket utførelsesform av foreliggende oppfinnelse innbefatter tilstedeværelse av nukleotider i den elementære hypoallergene medisinske mat, og det henvises derfor til U.S. patent 5,492,899.
U.S. patent nr. 5.340.603 til Neylan, et al., vedrører en høykaloriblanding som tilveiebringer næringsstøtte for spedbarn med kronisk lungesykdom. Blandinger ifølge patentet har en kaloritetthet på minst 800 kcal/l av blandingen, hvor ikke mindre enn 56 % av kaloriene i blandingene er avledet fra fett. I tillegg inneholder høykaloriblandingen i henhold til dette patent ikke mer enn 15 % totale kalorier avledet fra en høykvalitetspro-teinkilde og fra 20 - 27 % av totale kalorier fra en karbohydratkilde. Blandingen ifølge patentet innbefatter også m-inositol i en konsentrasjon på minst 50 mg/l av blandingen.
U.S. patent nr. 5.411.757 til Buist, et al., vedrører en balansert, velsmakende medisinsk diett for behandling av pasienter med medfødte feil ved essensiell aminosyreme-tabolisme. I dietten ifølge dette patent anvendes L-aminosyrer ved spesifikke vekter og forhold. Mer spesielt viser dette patent en velsmakende medisinsk diett hvori protein-ekvivalenten av L-aminosyrer dannes ved acetylering eller esterifistering av L-aminosyrene. I motsetning til dette anvendes ifølge oppfinnelsen L-aminosyrer for å tilføre amin<p>nitrogen (proteinekvivalent) bortsett fra at L-asparagin erstattes av aspartinsyre og L-glutamin erstattes med glutaminsyre.
Et for tiden tilgjengelig produkt kjent som Vivonex<®>Pediatric markedsfører av Clinical Products Division ofSandoz Nutrition, Minneapolis, Minnesota for gastroin-testinalt svekkede barn.Vivonex<®>Petriatric inneholder 100% frie aminosyrer og har en osmolalitet ved standard fortynning på 1 kcal/ml på 490 mOsm/kg H20. Dette produkt avleder 25% av totale kalorier fra fett med 68 % av disse kalorier som stammer fra triglyserider med midlere kjede-1engde.
Neocate<®>One + markedsført av Scientific Hospital Supplies, Inc. (SHS) North America, Gaithersburg, Maryland, er et pediatrisk elementært næringsmiddel som markedsføres i flytende og pulverform og er beregnet for barn over ett år. Dette flytende produkt inneholder 100% av dets aminonitrogen i form av frie L-aminosyrer og har en osmolalitet ved standard fortynning (100 kcal/lOOrnl) på 835 mOsm/kg H20.
Neocate<®>One+pulver (ikke-smakssatt) utblandet til å gi 100 kcal/100 ml har en osmolalitet på 610 mOsm/kg H20. Disse produkter inneholder 3,5 g fett pr. 100 ml ved en kaloridensitet på 100 kcal/100 ml. Neocate<®>, også markedsført av SHS i U.K., Skandinavia og U.S.A. anvender maltodekstrin som karbohydratkilde, 11,4 kalorier fra frie L-aminosyrer og 44,3 % kalorier stammer fra fett, hvor fettet foreligger som en blanding av raffinert talg, peanøttolje og kokos-nøttolje.
Generelt er de kjente elementære eller hypoallergene blandinger belemret med dårlig smak.(organoleptiske egenskaper, høy osmolaritet og lav kaloridensitet. I motsetning har blandingen ifølge oppfinnelsen akseptable organoleptiske egenskaper hvilket letter en pasients orale inntak som også øker pasientens forenelighet med diett- restriksjoner. De forbedrede organoleptiske egenskaper oppnås delvis ved i det vesentlige å utelukke L-glutaminsyre og L-aspartinsyre fra blandingen og erstatte disse med L-glutamin og L-asparaginmonohydrat. Ytterligere atskiller foreliggende oppfinnelse seg fra den kjente teknikk ved at den har et høykaloriinnhold fra fett som forbedrer de organoleptiske egenskaper, nedsetter osmolaliteten og øker den totale kaloridensitet. Ytterligere vil den spesifikke fettblanding ifølge oppfinnelsen gi en bedre balanse av de essensielle fettsyrene, linolsyre og a-linolensyre, i en lett absorberbar form.
Sammendrag av oppfinnelsen
Foreliggende oppfinnelse vedrører en forbedret elementær sammensetning omfattende karbohydrater, lipider, frie L-aminosyrer, vitaminer og mineraler. Oppfinnelsen er delvis basert på at det er funnet at anvendelsen av triglyserider med midlere kjedelengde (TCMT'er) eller fraksjonert kokosnøttolje, soyaolje, forestrede glyserolemulgatorer og høyoleinsaflorolje som lipidkomponent, i kombinasjon med anvendelsen av L-asparginmonohydrat i stedet for L-aspartinsyre og L-glutamin i stedet for L-glutaminsyre, vil vesentlig forbedre de organoleptiske egenskaper for den hypoallergene blanding. I henhold til oppfinnelsen anvendes også et høyere fettnivå til å gi et produkt med en lav osmolaritet og som dessuten tilveiebringer de nødvendige energibehov i et lite volum.
Ett trekk ved foreliggende oppfinnelse vedrører elementær medisinsk mat som i det vesentlige er fri for L-glutaminsyre og L-aspartinsyre. I henhold til foreliggende oppfinnelse er L-glutaminsyre (C5H9NO4, molvekt 147,13) erstattet med L-glutamin {C5H10N2O3, molvekt 146,15) monoamidet av glutaminsyre og L-aspartinsyre (C4H7NO4, molvekt -133,10) er erstattet med L-asparaginmonohydrat (C4HeN203' H20, molvekt 150,13), monoamidet av aspartinsyre med ett molekyl vann. Anvendt heri og i kravene betyr betegnelsen Må være i det vesentlige fri for L-glutaminsyre og L-aspartinsyre" at mindre enn 5 mg av hver syre skal være tilstede i 100 g pulverisert produkt {mindre enn 50 ppm) .
Foreliggende oppfinnelse vedrører også en komplett pediatrisk hypoallergen sammensetning egnet som næring til barn på en allergenfri diett. Blandingen i henhold til oppfinnelsen anvender frie L-aminosyrer som kilde for aminonitrogen og tilfredsstiller de anbefalte diett-tilskudd (RDA) for 1 - 3 år gamle barn i 1000 kcal og for 4 - 6 år gamle barn i 1500 kcal. Anvendt i det etterfølgende og i kravene betyr betegnelsen "proteinekvivalent" mengden av aminonitrogen i g til stede i blandingen multiplisert med 6,25..
Spesielt vedrører oppfinnelsen en sammensetning egnet som elementær medisinsk mat for administrasjon til mennesker omfattende a) en karbohydratkomponent som omfatter 3 8-56 % av det totale kaloriinnhold i matvaren; b) en lipidkomponent som omfatter fra 38-50% av det totale kaloriinnhold i maten, og hvor lipidkomponenten er en blanding av høy-oleinsaflorolje, fraksjonert kokosnøttolje (triglyserider med midlere kjedelengde), soyaolje og forestrede glyseroler; samt c) en aminosyrekomponent som utgjør 10-2 0% av det totale kaloriinnhold i matvaren, og hvor aminosyrekomponent en utgjør mindre enn 5 mg av L-glutaminsyre og L-aspartinsyre per 100 g av maten.
Sammensetningene i henhold til oppfinnelsen kan benyttes som næringstilskudd av et menneske som lider av en sykdom valgt fra gruppen bestående av multiple matallergier, avkortet tarmsyndrom, cystisk fibrose, pankreassykdom, gastroenteritt, inflammatorisk tarmsykdom, intraktabel diaré, feilernæring, mangelfull proteinfordøyelse, smitt-somme sykdommer så som HIV, hypermetabolisme, trauma, eosinofil gastroenteritt og gastroesofogeal refluks.
Mer spesielt vedrører oppfinnelsen en sammensetning egnet som elementær medisinsk mat hvor karbohydratkomponenten omfatter 38-56% av det totale kaloriinnhold i maten, og hvor lipidkomponenten utgjør 38-50% av det totale kaloriinnhold av maten, og hvori aminosyren eller proteinekvivalentkomponenten omfatter 10-20% av det totale kaloriinnhold av maten. I en mer foretrukket utførelsesform omfatter lipidkomponenten 38-46% av det totale kaloriinnhold i blandingen. Lipidkomponenten i foreliggende oppfinnelse tilfører 2-8 vekt % av pulverproduktet som linolsyre (5-15 % kalorier) og 0,4-1,0 vekt % av pulverproduktet som a-linolensyre (0,8-2,0 totale kalorier).
Fortrinnsvis består karbohydratkomponenten ifølge oppfinnelsen av maissirupfaststoffer med en dekstroseekvivalent (DE) på 23 eller mindre.
Et stabilisatorsystem anvendes i foreliggende sammensetning og kan være en enkeltkomponent eller en blanding av kjente stabilisatorer. Som tidligere nevnt er en hovedutfordring ved utvikling av et elementært næringsmiddel dets fysikalske stabilitet. Fysikalsk stabilitet vedrører olje-og vannemulsjonen i blandingen. Fagmannen vil forstå at frie aminosyrer og hydrolyserte proteiner ikke danner emulsjoner som er stabile over tid. Det er viktig at den elementære medisinske mat ifølge oppfinnelsen utviser en akseptabel fysikalsk stabilitet, da separasjon av emul-sjonen i vann- og oljefaser i vesentlig grad vil nedsette blandingens smak og kan resultere i utilstrekkelig næring.
Et stort antall emulsjonsstabilisatorer er blitt evaluert og det er funnet at forestrede glyserolemulgatorer, så som diacetylvinsyreestere av monoglyserider er meget nyttige i foreliggende oppfinnelse. Diacetylvinsyreestere av monoglyserider - (i det etterfølgende<n>DATEM")"er kommersielt tilgjengelige og er generelt betraktet som sikre for human konsumpsjon av USFDA. Representative DATEM-emulsjonsstabilisatorer nyttige i oppfinnelsen innbefatter"Panodan<®>linjen av emulsjonsstabilisatorer solgt av Grinsted Products, Inc. of Kansas, U.S.A.. Panodan<®>er fremstilt av spisbart raffinert vegetabilsk fett.
I én utførelsesform av foreliggende oppfinnelse er DATEM-emulsjonsstabilisatorene den eneste emulsjonsstabilisator og er til stede i den elementære medisinske mat ifølge oppfinnelsen i et nivå opp til 8 vekt % av lipidet. DATEM er gruppert i lipidkomponenten, da esteraser ved fordøyelse spalter fettsyregruppen fra vinsyregruppen. Fettsyren blir deretter absorbert og metabolisert, mens vinsyren forlater kroppen u-metabolisert. Profilen for Panodan<®>emulsjons-stabilisatoren solgt av Grinsted er som følger:
Et annen viktig aspekt vedrører anvendelsen av et antioksy-dantsystem for å nedsette oksidasjonen av lipidkomponenten. Fagmannen vil forstå at anvendelse av umettede oljer i et pulverisert elementært produkt byr på spesielle problemer. For å overkomme oksidasjonen (ødeleggelse) av oljene i det pulveriserte produktet ifølge oppfinnelsen, anvendes i henhold til én foretrukket utførelsesform et antioksidantsystem omfattende askorbylpalmitat, betakaroten og citrat. I denne forbindelse henvises det til U.S. patent nr. 5.234.702 til Katz, et al., med tittelen ANTIOXIDANT SYSTEM
FOR POWDERED NUTRITIONAL PRODUCTS.
Detaljert beskrivelse av oppfinnelse
Alle komponentene i den nye sammensetningen er kommersielt tilgjengelige fra et antall kilder. Eksempelvis er den foretrukne karbohydratkilde fortrinnsvis maissirupfaststoffer med en DE mindre enn 23 og er tilgjengelig fra Grain Processing Coroporation, Muscatine, IA. Aminosyrene tilgjengelige fra Kyowa Hakko, Ajinomoto and Tanabe of Japan, og Degussa of Germany. Lipidene er tilgjengelige fra Stepan Co., Maywood, N.J. og Califonia Oils, Richmond, CA. L-glutaminkomponentene er tilgjengelige fra Kyowa Hakko i Japan og L-asparaginmonohydratet er tilgjengelig fra Degussa.
I henhold til oppfinnelsen kan sammensetningens osmolalitet ligge i området 300 - 400 mOsm/kg H20, og mer foretrukket 350-375 mOsm/kg H20 når 4,2 g av den pulveriserte sammensetning tilsettes til vann for å gi 29,57 ml av blandingen. Fagmannen vil forstå at sammensetningens osmolalitet vil variere med konsentrasjonen eller kaloridensiteten for blandingen. F.eks. ved en kaloridensitet på 68 kcal/100 ml, vil osmolaliteten for RTF ("ready-to-feed") blandingen ligge i området 350-375 mOsm/kg H20. Ved en kaloridensitet på 135 kcal/l00 ml, vil osmolaliteten for blandingen ligge i området 850-900 mOsm/kg H20.
Ett trekk ved foreliggende oppfinnelse ligger i anvendelsen av maissirupfaststoffer som karbohydratkilde. Det er viktig å anvende en kilde for karbohydrat som ikke uheldig vil påvirke (øke til en høy verdi) osmolaliteten for slutt-produkt, spesielt når L-aminosyrer tilfører proteinekvi-valenten, da disse er små molekyler som bidrar vesentlige til osmolaliteten. Fagmannen vil forstå at høye osmolali-teter (dvs. over 450 mOsm/kg H20 for småbarn, og ca. 750 mOsm/kg H20 for barn) i et flytende næringsmiddelprodukt kan forårsake mageforstyrrelser og diaré. Derfor frarådes det å anvende karbohydrattyper, så som dekstrose og sukrose, som ville forøke osmolaliteten til over 400. mOsm/kg H20 for spedbarn og 750 mOsm/kg H30 for barn.
I en foretrukket utførelsesform har karbohydratet som anvendes i foreliggende oppfinnelse en DE (dekstrose ekvivalent) på ikke mer enn 23 og kan være maissirupfaststoffer. Anvendelse av et karbohydrat med en DE større enn 23 i kombinasjon med L-aminosyrer vil gi et osmotisk trykk i tarmene som kan føre til oppsvulming, diaré og dehy-dratisering. Disse er tilstander som bør unngås hos pasienter som allerede lider av fordøyelsesproblemer. Den medisinske mat i henhold til oppfinnelsen tilveiebringer en god balanse mellom kaloridensitet og akseptabel osmolalitet. Fagmannen vil forstå at når DE for karbohydratet øker vil osmolaliteten for blanding også øke. Den foretrukne kilde for karbohydrater er maissirupfaststoffer med en DE på 23 eller mindre.
Det er også påtenkt at i henhold til oppfinnelsen er sammensetningen egnet som elementær medisinsk mat smakssatt. Smaksstoffer og søtningsmidler (fortrinnsvis ikke sukkrose eller glukose) kjent innen matvareindustrien er akseptable, forutsatt at de ikke påvirker pasientene allergisk og at osmolaliteten for det ferdige produkt er akseptabel. Et vaniljesmaksprodukt søtet med det kunstige søtningsmiddel aspartam (NutraSweet<®>) ble produsert i henhold til eksempel 1. Osmolaliteten for det smakssatte produkt ved 68 kcal/100 ml var 370 mOsm/kg H20 og 895 mOsm/kg ved 135 kcal/100 ml.
Det er også vist en lipid komponent for anvendelse i foreliggende oppfinnelsen som omfatter, regnet som vektprosent av lipidkomponenten, 35-43% høyoleinsaflorolje, 28-35% fraksjonert kokosnøttolje (MCT), 24-30% soyaolje og 0,5-8% forestrede glyseroler. I lipidkomponenten i denne oppfinnelsen kan nivået av linolsyre ligge i området 15-22% og nivået av a-linolensyre kan ligge i området 2-5 vekt % av den totale lipidkomponent.
En representativ blanding av 6,4 g soyaolje, 9,0 g høy-oleinsaflorolje, 7,6 g fraksjonert kokosnøttolje og 1,4 g DATEM ble innarbeidet i den elementære medisinske mat i henhold til oppfinnelsen og deretter analysert for fettsyreinnhold. I tabell I er vist navnet på fettsyrer, g fettsyre pr. 100 g pulver, % energi og vekt % fett.
Ved analyse kan man ikke differensiere fettsyrene fra
Panodan, soyaolje og HO saflorolje.
I en ytterligere utførelsesform av foreliggende oppfinnelse
er det vist en pulverformig, elementær medisinsk mat som omfatter pr. 100 g pulver som ytterligere beskrevet i tabell II.
Enda mer spesielt omfatter sammensetningen egnet som elementær medisinsk mat i henhold til oppfinnelsen basert på 100 kcal av blandingen, 2,8 - 3,8 g proteinekvivalent, 4,0 - 5,6 g fett, 9,0 - 11,8 g karbohydrater, 0,4 - 0,8 g linolsyre, 262 - 475 IU vitamin A, 40 - 80 IU vitamin D, 2,0 - 3,5 IU vitamin E, 5-20 ug vitamin K, 0,11 - 0,42 mg tiamin, 0,1 - 0,21 mg riboflavin, 0,09 - 0,17 mg vitamin B-6, 0,40 - 1,36 ug vitamin B-12, 1,6 - 3,2 mg niacin, 28 - 60 ug folinsyre, 0,04 - 1,70 mg pantotensyre, 4,0 - 18 ug biotin, 8,6 - 85 mg vitamin C, 7,5 - 14,6 mg kolin, 4,6 - 9,9 mg inositol, 4 - 6,5 mg L-carnitin, 103 - 127 mg kalsium, 76 - 103 mg fosfor, 8,0 - 12,7 mg magnesium, 1,6 - 2,4mg jern, 1,0 - 1,7 mg sink, 0,10 - 0,15 mg mangan, 0,11 - 0,19 mg kobber, 6,7 - 11,8 ug jod, 2,2- 4,7 ug selen, 2,2 -4,7ug krom, 2,4 - 5,5 ug molybden, 40 - 55 mg natrium,
144-174 mg kalium og 55 - 85 mg klorid.
For å omdanne næringsverdier pr. 100 kcal til verdier av næringsmiddel pr. 100 g pulverformig produkt, så multi-pliseres 100 kcal verdien med 4,75.
Inkludering av nukleotider og/eller nukleosider i den elementære mat i henhold til oppfinnelsen er også påtenkt. Med hensyn til innbefattelse av nukleotider i næringsrike blandingen henvises det til U.S. patentene 5.492.899 til Maser, et al., 3.231.385 til Ziro, et al., 4.544.559 til Gil og 4.994.442 til Gil, et al.
I henhold til en annen utførelsesform av foreliggende oppfinnelse har proteinekvivalentkomponenten av den elementære medisinske matvare en aminosyreprofil som gjengitt i tabell
III.
Kaloridensiteten for den oppfinneriske blanding er fortrinnsvis 450 - 500 kcal/100 g pulver. Kaloridensiteten for RTF produktet kan naturligvis justeres ved graden av fortynning med vann.
Oppfinnelsen vedrører forbedrede enterale nærende hypoallergene sammensetninger og mer spesielt sammensetning egnet som hypoallergen medisinsk mat omfattende: a) en karbohydratkomponent som omfatter 38 - 56 % av det totale kaloriinnhold i maten: b) en lipid komponent som omfatter 38 - 50 % av det totale kaloriinnhold i maten og hvor lipidkomponenten er en blanding av høyoleinsaflorolje, fraksjonert kokosnøttolje, forestrede glyseroler og soyaolje; og c) en aminosyrekomponent som omfatter 10 - 20 % av det totale kaloriinnholdet i maten, og hvor aminosyrekomponenten utgjør mindre enn 5 mg av L-glutaminsyre og L-aspartinsyre per 100 g av maten.
Denne fullverdige komplette sammensetning er egnet som hypoallergen medisinsk mat for individer med multiple allergier, avkortet tarmsyndrom, cystisk fibrose, pankreassykdom, gastroenteritt, tarminfeksjonssykdom, intraktabel diaré, feilernæring, proteinfeilfordøyelse, infeksjonssykdommer eller for pasienter med hypermetabolisme, så som brann- og traumaofre eller kreftpasienter.
Sammensetningen av oppfinnelsen kan benyttes som næringsmiddel til skudd til mennesker som lider av fordøyelses-tilstander forårsaket av matproteinallergener eller alvorlige matallergier.
En sammensetning i henhold til oppfinnelsen er for enteral administrasjon og er sammensatt for å gi komplett næring (tilfredsstiller alle RDA'er) i et lite volum. F.eks. når RDA-energien for alder tilføres av foreliggende elementær-mat vil alle næringsbehov tilfredsstille både "USFDA Infant Formula Act"-spesifikasjonene og RDA for alder i volum gjengitt i tabell IV.
Den følgende tabell V er en representativ opplistning av bestanddeler og et område for vekt % av hver komponent som kan anvendes for å fremstille den velsmakende elementære medisinske mat ifølge oppfinnelsen.
En spesifikk utførelsesform av en elementærblanding i henhold til oppfinnelsen er vist i tabell VI. Foreliggende oppfinnelse skal forklares på basis av visse spesifikke utførelseseksempler. Hvis intet annet er angitt så er konsentrasjonen pr. vektdeler.
EKSEMPEL 1
Generell Prosedyre
For fremstilling av aminosyreblandingene og fremgangsmåten ved fremstilling av elementærblandingen ble fremgangsmåten vist i US patent nr. 5.236.569 til Acosta, et al., anvendt. En enteral sammensetning i henhold til foreliggende oppfinnelse, blir generelt oppnådd ved tørrblanding av en pulverformig forblandingsbase med en aminosyreforblanding. Fremgangsmåten ved fremstilling av den forhåndsblandede base erholdes ved de følgende summariske etterfølgende trekk:
1. Fremstilling av forrådsoppløsninger:
a. Fremstilling av et vannoppløselig vitamin og spor
mineralblanding:
b. Fremstilling av en askorbinsyreblanding; c. Fremstilling av en oljeblanding inneholdende oljeoppløselige vitaminer, og d. Fremstilling av en karbohyrat/mineraloppslemning: 2. Kombinasjon i spesifikk rekkefølge av forrådsoppløs-ningene: a. Blande oljeblandingen og karbohydrat/mineralopp-slemningen; b. Tilsetning av vannoppløselige vitaminer til oppslemningen, og c. Tilsetning av askorbinsyre til oppslemningen; 3. Tørke de kombinerte forrådsoppløsninger til å gi den pulverformige forblandingsbase. 4. Tørrblande den pulverformige forblandingsbase med aminosyreforblandingen til å gi den elementære blanding i henhold til foreliggende oppfinnelse.
Kommersiell skala
Det kliniske produkt anvendt i eksempel 2 ble fremstilt i kommersiell skala. En sats på 1,000 kg ble fremstilt under anvendelse av fremgangsmåten beskrevet ovenfor til å gi den angitte sammensetning i lagringstiden som angitt i tabell
VII.
EKSEMPEL 2
Et klinisk studium av den oppfinneriske sammensetning ble utført ved The Johns Hopkins Hospital {Baltimore, Maryland) for å bestemme vekst, toleranse og biokjemisk respons hos barn matet med blandingen i henhold til oppfinnelsen som hovednæringsinntak i fire (4) måneder. Barna som inntok den oppfinneriske sammensetning var på dietter fri for kjente matallergener som de var følsomme for. De primære indika-sjoner anvendt for å evaluere sammensetningen i henhold til oppfinnelsen var vekt, høyde, inntak, avføringsmønster, blodbiokjemi (fullstendig blodtelling, serumalbumin, ferritin, transtyretin, retinolbindende protein, retinol og ureanitrogen, samt plasma aminosyrer), og bestemmelse av innesluttet blod i avføring. Sekundære variable innbefattet gastrointestinale symptomer.
Den ikke-randomiserte matundersøkelse ble utført med fjorten (14) barn med proteinfølsom eosinofil gastroenteri-tis eller multiple matproteinfølsomhet. Hvert barn forsynte seg i henhold til hans/hennes egen kontroll. Alle personer ble før igangsetting av undersøkelsen underkastet en dobbeltblind, placebokontrollert matutfordring inneholdende barnets vanlige blanding og den nye sammensetning for å bestemme om den fremkalte noen allergiske symptomer. Tidligere medisinsk historie, veksthistorie opp til de siste tolv (12) måneder, detaljerte medisinske og dietthistorier og nedtegninger av orale utfordringsresultater for hver pasient ble erholdt.
Det elementære produkt fremstilt ifølge eksempel 1 ble tilveiebrakt i et pulver i 350 g bokser og ble rekonstituert for å tilfredsstille de individuelle behov for hvert barn bestemt av en forskningsdiettiker. Den nye sammensetning ble smaksatt med godkjente smaksmidler bestemt av vedkommende som undersøkte. Barna fikk kun fordøye de matvarer som kunne tolereres, den undersøkte blanding og smaksstoffer var godkjent av forskeren.
På dag l av undersøkelsen ble blod- og avføringsprøver innsamlet og forsøkspersonene ble igjen bestemt ved én(l) og fire (4) måneder etter begynnelsesdatoen med hensyn til vekst, inntak av blandingen, toleranse og blod- og avføringsparametere (kun 4 måneder).
Forsøkspersonene ble fjernet fra undersøkelsen på basis av en protokollfeil eller mislykket behandling. Et barn ble betraktet som en protokollfeil hvis han eller hun ikke ville følge den planlagte undersøkelse, ble frivillig fjernet av foresatte, ikke var i stand til å innta de nødvendige mengder av den undersøkte blanding eller fjernet av legen på grunn av at han eller hun antok at det var til barnets beste. Barna ble antatt å være feilbehandlet hvis de ikke var i stand til å forbruke den undersøkte blanding som følge av intoleranse eller allergisk reaksjon mot den undersøkte blanding.
Omhyggelige bestemmelser av vekt, høyde og hodeomkrets ble utført ved påmelding til undersøkelsen, ved hvert besøk og ved undersøkelsens avslutning under anvendelse av standard teknikker og utstyr. Tilgjengelige vekt- og høydedata oppsamlet under en ettårsperiode før begynnelsen av undersøkelsen ble anvendt for historiske sammenligninger.
Ved 4-måneders-tidspunktet ble elemtærproduktet fremstilt i eksempel 1 tilført som gjennomsnittlig 58 % av de totale kalorier pr. døgn for barn, mens resten ble tilført i form av autorisert (dvs. tolererte matvarer) matvarer. Generelt bibeholdt barna veksten mens de gikk på den undersøkte blanding. Kun én person erfarte en liten nedsettelse av veksthastigheten mens personen gikk på den undersøkte blanding, og dette kan muligvis være forårsaket av problemer i hjemmet (foreldrenes separasjon).
Barna i undersøkelsen hadde ofte liten appetitt for faste matvarer og vektforøkelsen kunne ha vært forbedret hvis energiinntakene ble øket for å tilfredsstille en anbefalt RDA. Hemoglobin- og hematokrittkonsentrasjonene for forsøkspersonene ble signifikant forbedret under prøve-perioden. Andre blodbestemmelser ble generelt bibeholdt ved nivåer tilsvarende de ved begynnelsen av forsøket. Util-strekkelige data var tilgjengelige ved tidspunktet for innlevering av foreliggende søknad for å bestemme effekten av den undersøkte blanding på serumvitaminer. Data skal kontinuerlig innsamles under dette forsøk og vil bli statisk analysert.
Avføringsmønsteret ble noe forbedret under forsøksperioden; i realiteten ble to av barna i stand til å gå på toalettet etter å ha fått den undersøkte bestanddel, dette som følge av forbedring i deres avføringer i forhold til deres tidligere matinntak.
Som konklusjon så underbygger de preliminære resultater at sammensetningen i henhold til oppfinnelsen er en hypoallergen sammensetning egnet for behandling av barn med multiple matallergier eller allergisk eosinofil gastroen-teritis. Barnas vekst på det oppfinneriske produkt var akseptabelt når det tilveiebrakte hoveddelen av den nød-vendige energi. Barna tolererte godt det undersøkte produkt og fant at smaken var akseptabel. Tilfeldige bemerkninger fra foreldre og tilstedeværende personell har vært posi-tive. Data vil bli oppsamlet og statistisk analyse vil bli utført når datainnsamlingen er avsluttet.
EKSEMPEL 3
Den hypoallergene sammensetning i henhold til oppfinnelsen ble analysert for proteineffektivitetsforhold (PER) som er et mål for proteinkvaliteten bestemt under anvendelse av laboratorierotter. PER bestemmes ved å dividere dyrenes vektøkning under proteininntaket. To grupper med ti rotter i hver ble foret med en diet i 28 døgn med den hypoallergene sammensetning og en kaseinbasert blanding under anvendelse av AOAC metode-960.48, AOAC Official Met nods of Analysis, 15. utgave, 1990. Hver syvende dag ble rottene veid og deres matforbruk bestemt. Ved slutten av de 28 døgn ble total vektøkning og proteinsammensetning i de to grupper beregnet. Disse verdier ble anvendt for å bestemme
PER.
Resultatene av denne rottebioundersøkelse er gjengitt i tabell VIII, og indikerer at den pediatriske elementære diett, i henhold til foreliggende oppfinnelse, har et vesentlig høyere proteineffektivitetsforhold enn den konvensjonelle kaseinbaserte blanding.
EKSEMPEL 4
Immunogen i s i t e t
Dette forsøk ble utført for å evaluere det allergene potensiale for den elementære medisinske mat i henhold til oppfinnelsen (fremstilt i eksempel 1) sammenlignet med tre kommersielt tilgjengelige hypoallergene næringsmidler. Lav immunogenisitet vist i dette forsøk er meget predikativ for meget lavt allergenaktivitet for et næringsprodukt.
Tre kommersielt tilgjengelige produkter var Neocate<®>, Neocate<®>One og Vivonex®Pediatrie. I dette forsøk ble kaniner hyperimmunisert med næringsmidlet i henhold til oppfinnelsen eller de ovenfornevnte kommersielle produkter under anvendelse av en meget kraftig immuniseringsprotokoll under anvendelse av "Complete Freunds' Adjuvant "(CFA) med multiple injeksjoner. Enzymbundne immunosorbentbestemmelse (ELISA) ble anvendt for å bestemme den systemiske (lgG) antigene spesifikke immunrespons hos kaniner for å definere den relative immunogenisitet for hvert produkt. Prøver av hvert produkt ble blandet med steril fosfatbufret saltlake (PBS) for å gi en konsentrasjon på 6 mg protein (aminosyrer) pr. 1,5 ml (PBS oppløsning). Den primære immunisering ble fremstilt ved å emulgere 1,5 ml PBS-produktoppløsningen med 3,0 ml CFA. "Booster"-immmunisering ble fremstilt ved å blande 1,5 ml av hver PBS-oppløsning med 1,5 ml CFA. Disse prøver ble deretter administrert i en dose på 5 mg proteinekvivalent pr. hver av to immuniseringer.
Dyrene ble deretter satt på en vegetabilsk diett (fri for melk, soya og ris) i 14 døgn før den primære immunisering og forble på denne diett i resten av undersøkelsen. Blod-prøver før immunisering ble tatt fra hver kanin på dag 0, og deretter ble hver kanin immunisert. På dag 21 ble den andre immmunisering utført. Dyrene ble årelatt på dag 35. Serum ble oppsamlet og lagret ved 20°C inntil nærmere undersøkelse.
Antistoffresponser ble kvantifisert ved et ELISA utviklet for hvert antigen som følger: ELISA-plater ble belagt med den immuniserende blanding ved en optimal (metning) konsentrasjon. Platene ble blokkert med PBS inneholdende 0,05 % eggalbumin og 0,1 % "Tween 20" for å eliminere ikke-spesifikk binding. Forsøksantisera ble fortynnet i serie og tilsatt til de belagte plater. Deretter ble geit-IgG-pepperrotperoksydasekonjugat tilsatt for å reagere med eventuelt kaninantistoff bundet til immunogenet. Tilsetning av enzymsubstratet (tetrametylbenzidin) forårsaket en fargeendring som var direkte proporsjonal med mengden av funnet antistoff. Antistofftiter ble definert som den resiproke verdi av fortynningen av serumet som ga en ELISA optisk densitet på 0,3 ved 450 nm i 10 minutter etter tilsetningen av enzymsubstrattilsetningen.
Fem (5) kaniner ble immunisert med hvert næringsprodukt. De data som er gitt i -tabell IX gir den midlere titer for hver gruppe av fem kaniner og standard avvik ved dag 0 og 35. Selv om titrene for blandingen i henhold til oppfinnelsen i realiteten var lavere enn for alle de kommersielle produkter så kan det kun konkluderes med at elementærsammen-setningen fremstilt i eksempel 1 var overlegent meget bedre i forhold til "Neocat One+". Det kan også konkluderes med at sammensetningen ifølge oppfinnelsen utviste en meget lav immunogenitet, hvilket indikerte at produktet i henhold til oppfinnelsen vil demonstrere klinisk hypoallergen virkning. Disse data understøtter bruken av foreliggende oppfinnelse for folk som er høyfølsomme for melkeproteiner eller som reagerer omfattende på hydrolyserte proteinblandinger.
EKSEMPEL 5
Organoleptisk forsøk
Et viktig trekk ved oppfinnelsen ligger i smaken av sammensetningen fremstilt i henhold til oppfinnelsen. De fleste elementære medisinske matvarer som for tiden er tilgjengelige har en meget ubehagelig smak, og som sådanne vil pasienter som anvender disse blandinger neppe innta den nødvendige kalorimengde og kan til og med være uforenelig med terapi.
Den elementære medisinske mat fremstilt i henhold til eksempel 1 ble undersøkt med hensyn til smak mot fire kom mersielt tilgjengelige blandinger. Produktnavnet og prøve-nummer for hvert produkt er gjengitt i den etterfølgende tabell X.
Alle prøver ble gitt som ikke-smakssatte pulvere og ble rekonstituert med vann som vist i tabell XI.
Femtien (51) frivillige ble rekruttert for å evaluere den relative aksepterbarhet av sammensetningen i henhold til oppfinnelsen sammenlignet med de fire kommersielt tilgjengelige elementære medisinske matvarer. Prosedyren for denne organoleptiske evaluering besto av å tilveiebringe en prøve av den oppfinneriske sammensetning til hver evaluator som deretter prøvesmakte og sammenlignet smaken med de fire kommersielle blandinger med smaken av den oppfinneriske
sammensetning. Hver prøve ble evaluert ved 24 °C og evaluert etter den følgende skala:
9 = ekstremt bedre enn eks. l
8 = meget bedre enn-eks. 1
7 = moderat bedre enn eks. 1
6 = noe bedre enn eks. l
5 = verken bedre eller verre enn eks. 1
4 = noe verre enn eks. l
3 = moderat verre enn eks. 1
2 = meget verre enn eks. 1
1 = ekstremt verre enn eks. 1.
Rådata fra hver evaluator ble innsamlet. Middeltall, standard avvik og p-verdier ble beregnet. Signifikans ble bestemt ved 95 % konfidensintervallet, Turkey Criteria. Sammenligningen av eksempel 1 ble betraktet som signifikant hvis p-verdier var <0,0125 under anvendelse av Bonferroni Criteria. Resultatene av denne organoleptiske undersøkelse er vist i tabell XII.
Prøvene som deler et bokstav- under overskriften Statistisk Gruppe er ikke signifikant forskjellige. Fra de data som er gjengitt i tabell XII er det åpenbart at de kommersielle produkter A og B var signifikant dårligere enn eks. 1. Derfor vil sammensetningen i henhold til oppfinnelsen gi elementær medisinsk mat som utviser en meget akseptabel smak.
Industriell anvendelighet
Foreliggende oppfinnelse tilveiebringer et velsmakende hypoallergen/elementært produkt for næringstilskudd for mennesker som lider av proteinallergier. Den medisinske profesjon er i konstant søken etter forbedrede blandinger for sine pasienter for en tilførsel av en fullstendig diet i en velsmakende matriks. Som.vist i eksemplene så er sammensetningen i henhold til foreliggende oppfinnelse lett å fremstille og tilveiebringer akseptabel vekst og toleranse hos pasienter som inntar produktet.
Claims (12)
1. Sammensetning egnet som elementær medisinsk mat for administrasjon til mennesker
karakterisert vedat den omfatter: a) en karbohydratkomponent som omfatter 38 - 56 % av det totale kaloriinnhold i maten; b) en lipid komponent som omfatter 38 - 50 % av det totale kaloriinnhold i maten og hvor lipidkomponenten er en blanding av høy-oleinsaflorolje, fraksjonert kokosnøttolje, forestrede glyseroler og soyaolje,- og c) en aminosyrekomponent som omfatter 10 - 20 % av det totale kaloriinnholdet i maten, og hvor aminosyrekomponenten utgjør mindre enn 5 mg av L-glutaminsyre og L-aspartinsyre per 100 g av maten.
2. Sammensetning i henhold til krav l, hvori aminosyrekomponenten omfatter: 2,3-3,2 vekt % L-histidin; 5,9 - 8,5 vekt % L-isoleucin; 9,5-14,0 vekt % L-leucin; 6,6-9,7 vekt % L-valin; 5,2 - 7,6 vekt % L-lysin; 2,2-3,2 vekt % L-metionin; 4,9-7,1 vekt % L-fenylalanin; 3,8-5,7 vekt % L-treonin; 1,4-2,2 vekt % L-tryptofan; 3,0-4,2 vekt % L-alanin; 5,3-7,7 vekt % L-arginin; 8,6-12,4 vekt % L-asparaginmono-hydrat; 1,1-1,9 vekt % L-cystindihydroklorid;
11,0-15,8 vekt % L-gluramin; 2,3-3,2 vekt % glycin; 3,0-4,1 vekt % L-prolin; 3,0-4,2 vekt % L-serin; og 5,0-7,1 vekt % L-tyrosin.
3. Sammensetning ifølge krav 1, hvori karbohydratkomponenten består i det vesentlige av maissirupsfaststoffer med en DE på 23 eller mindre.
4. Sammensetning ifølge krav 1 i pulverform.
5. Sammensetning i henhold til krav 1, hvor ca. 4 g av blandingen tilsettes til vann til å gi 3 0 ml som har en osmolalitet på 300-400 mOsm/kg H20.
6. Sammensetning ifølge krav l, hvori lipidkomponenten omfatter 37-41 % høyoleinsaflorolje, 28-35% fraksjonert kokosnøttolje, 24 - 30 % soyaolje og 0,5 - 8 % forestrede glyseroler basert på den totale vekt av lipidkomponenten.
7. Sammensetning ifølge krav 1, ytterligere omfattende minst ett element valgt fra gruppen bestående av nukleosider, nukleotider, antioksidantsystem, naturlig smak, kunstig smak, kunstige søtningsstoffer, hovedspor- og ultraspormineraler, vitaminer og m-inositol.
8. Sammensetning ifølge krav 5, hvori blandingen har en kaloridensitet på 0,68 kcal/ml og en osmolalitet på 350-375 mOsm/kg H20.
9. Sammensetning ifølge krav l omfattende, basert på 100 kilokalorier av blandingen: 2,8-3,8 proteinekvivalent; 4,0-5.6 g fett, 9,0-11,8 g karbohydrat; 0,4-0,8 linolsyre; 262-475 IU vitamin A; 40-80 IU vitamin D; 2,0-3,5 IU vitamin E, 5-20 ug vitamin K, 0,11-0,42 mg tiamin, 0,1-0,21 mg riboflavin; 0,09-0,17 mg vitamin B-6; 0,40-1,36 ug vitamin B-12; 1,6-3,2 mg niacin; 28-60 ug folinsyre; 0,40- 1,70 mg pantotensyre; 4,0-18,0 ug biotin, 8,6-85 mg vitamin C,7,5-14,6 mg kolin, 4,6-9,9 mg inositol, 4,0-6,5 mg L-carnitin;
103-127 mg kalsium; 76-103 mg fosfor; 8,0-12,7 mg magnesium; 1,6-2,4 mg jern; 1,0-1,7 mg sink; 0,10-0,15 mg mangan; 0,11-0,19 mg kobber, 6,7-11,8 ug jod; 2,2-4,7 ug selen; 2,2-4,7 ug krom, 2,4-55 ug molybden; 40-55 mg natrium; 144-174 mg kalium og 55-85 mg klorid.
10. Sammensetning ifølge krav 4, omfattende basert på 100 g pulver: 14,3 proteinekvivalent; 22,6 g lipid; 46,7 g karbohydrat; 2,85 g linolsyre; 0,48 g a-linolensyre; 515 mg kalsium, 385 mg fosfor; 390 ug vitamin A; 5 ug vitamin D;
6.7 mg vitamin E; 30 ug vitamin K; 1,0 mg tiamin, 0,50 mg riboflavin; 0,48 mg vitamin B-6; 2,0 ug vitamin B-12; 8,0 mg niacin, 140 ug folinsyre, 2,0 mg pantotensyre, 20 ug biotin, 43 mg vitamin C, 38 mg kolin; 24 mg inositol; 23 mg L-canitin; 40 mg magnesium; 8,4 mg jern, 5,3 mg sink, 0,50 mg mangan; 0,60 mg kobber; 34 ug jod; 11 ug selen; 11 ug krom, 12 ug molybden; 9,35 mEq natrium;18,29 mEq kalium og 8,04 mEq klorid.
11. Sammensetning ifølge krav 1, hvori karbohydratkom-ponentene er maissirupsfaststoffer med en DE på 23 eller mindre; lipidkomponenten utgjør 35-41 vekt % høyoleinsa-florolje; 28-35 vekt % fraksjonert kokosnøttolje, 24-30 vekt % soyaolje; og 0,5-8 vekt % diacetylvinsyreestere av monoglyserider; og aminosyrekomponenten omfatter L-asparaginmonohydrat og L-glutamin.
12. Sammensetning egnet som hypoallergen, medisinsk matkarakterisert vedat den omfatter: a) en karbohydratkomponent som omfatter 3 8 - 56 % av det totale kaloriinnhold i maten; b) en lipid komponent som omfatter 38 - 50 % av det totale kaloriinnhold i maten og hvor lipidkomponenten er en blanding av høyoleinsaflorolje, fraksjonert kokosnøttolje, forestrede glyseroler og soyaolje; og c) en aminosyrekomponent som omfatter 10 - 20 % av det totale kaloriinnholdet i maten, og hvor aminosyrekomponenten utgjør mindre enn 5 mg av L-glutaminsyre og L-aspartinsyre per 100 g av maten.
