Description
Foreliggende oppfinnelse angår vannoppløselige og -uopplø-selige polyaspartamider, polyaspartamid-derivater som er lastet med gallesyrer såvel som deres anvendelse.
Gallesyrer har en viktig fysiologisk funksjon ved fett-fordøyelsen, f.eks. som kofaktor for den pankreatiske lipasen. De blir syntetisert til leveren som sluttprodukt for kolesterolstoffskiftet, oppsamlet i galleblæren og fra denne avgitt til tynntarmen, hvor de har sin fysiologiske virkning ved kontraksjon.
Største delen av de utskilte gallesyrene blir tilbakevunnet over det enterohepatiske kretsløpet, de blir ført tilbake til leveren over tynntarmens mesenterialver og portnersystemet. Både aktive og passive transportsystemer spiller en rolle ved tilbakeresepsjonen i tarmen. I det enterohepatiske kretsløpet opptrer gallesyren såvel som frie syrer, som også i form av glycin- og taurinkonjugater.
Det er tidligere kjent å binde gallesyrer i ikke-resorber-bare, uoppløselige, basiske, tverrbundne polymerer (ionebyt-teharpiks = "harpikser"). Aktuelle mål for terapien er alle sykdommer som er ønskelig påvirket av en hemming av galle-syretilbakeresorpsjon i tarmen, spesielt i tynntarmen. Eksempelvis blir den kologene diaré etter fjerning av ileum, eller også forhøyet kolesterol-blodspeil, behandlet på denne måten. På det forhøyede kolesterol-blodspeilet kan det bli oppnådd en senking av dette speilet ved inngrepet i det enterohepatiske kretsløpet. Gjennom reduksjon av galle-syrelageret som befinner seg i det enterohepatiske kretslø-pet, blir den'tilsvarende nysyntesen av gallesyrer fra kolesterol i leveren øket. LDL-kolesterol (Low Density Lipoprotein), som befinner seg i blodkretsløpet, blir trukket inn i leveren for å dekke kolesterolbehovet, hvorved de hepatiske LDL-reseptorene kommer til anvendelse i øket antall. Den derav følgende økningen i LDL-katabolismen påvirker således senkingen av det aterogene kolesterol inn- holdet 1 blodet. Hittil har disse nevnte polymere, uopplø-selige ionebytteharpiksene vært den eneste muligheten til å ha innflytelse på det enterohepatiske kretsløpet ved forhøyet gallesyreutskillelse og den derav følgende reduksjonen i kolesterolspei let.
Det har nå vist seg at legemidlene som er basert på tverrbundne ionebytteharpikser, har forskjellige ulemper.
"Harpikser" for anvendelse som legemidler, må inneholde spesielt høy dagsdose. Den utgjør f.eks. for kolestyramin (inneholder kvartare ammoniumgrupper) 12-14 g, maksimaldose 32 g, for kolestipol (inneholder sekundære hhv. tertiære aminogrupper) 15-30 g.
En ytterligere ulempe er at smaken, lukten og den nevnte høye doseringen vanskeliggjør pasientens aksept.
Videre er det kjent at de nevnte "harpikser" viser bi-virkninger. Disse bivirkningen går tilbake på manglende selektivitet (f.eks. avitaminoser), som også må bli tatt hensyn til ved doseringen av samtidig gitte medikamenter, men også på gallesyreutarming som i forskjellig grad forårsaker forskjellige gastrointestinale forstyrrelser (obstipasjon, Steatorrhoe).
For begge preparatene ble det beskrevet en terapeutisk betydning ved kombinasjon med andre hypolipidemisk virkende farmaka som fibrater, HMG-CoA-reduktasehemmere, probukol (se f.eks. M.N. Cayen, Pharmac. Ther. 29, 187 (1985) og 8th International Symposium on Atherosclerosis, Rome, okt. 9-13, 1988, Abstracts S. 544, 608, 710), hvorved de ønskede effektene også blir muliggjort ved terapien av alvorlige hyperlipidemier. Derfor er det betydningsfullt å finne egnede stoffer med det angitte virkeprinsipp som er uten ulempene ved de idag anvendte preparatene. Følgende kjennetegn ved de nevnte preparatene, og spesielt for kolestipol, er ansett ønsket å forbedre: 1. De høye dagsdosene som er nødvendige når de kun gir et relativt lavt bindingsforhold ved nøytral pE i isotont medium, såvel som den (delvise) frigivingen av de adsorberte gallesyrene. 2. Den kvalitative forskyvningen av gallesyresammenset-ningen i gallen med tiltagende tendens for kenodesoksykolsyre og den derved forbundne faren for Cholelithiasis. 3. Mangelen av en dempende virkning på tarmbakterienes kolesterolstoffskifte. 4. Det for høye bindingsforholdet for vitaminer og farmaka gjør et substitusjonsbehov for disse stoffene og eventuell blodspeilskontroll nødvendig. 5. Levringsformen er hittil ansett som utilfredsstillen-de .
Målet ved foreliggende oppfinnelse var således å gi en mulighet for å binde gallesyrer på konsentrasjonsavhengig måte, hvorved den gallsyrebindende forbindelsen selv ikke blir resorbert, og derved ikke tatt inn i det enterohepatiske kretsløpet. Dessuten skal forbindelsene ha et høyt bindingsforhold for gallesyrer ved nøytral pH, og samtidig garantere at disse ikke igjen blir frigitt under fysiologiske be-tingelser, og derved ikke kan bli resorbert.
Dessuten skal disse forbindelsene ikke, hhv. ikke i den samme utstrekning, vise de eksisterende ulempene til de kjente "harpikser".
Løsningen av oppgaven består i fremstillingen av a- eller p- sammenbundne polyaspartamid-derivater med den generelle formel I

Akseptable, aromatiske karboksylsyrerester, som f.eks. kanelsyre eller di-hydrokanelsyre, er også anvendbare.
Som alkoholen, som esterkomponenten i dikarboksylsyren blir fremstilt av, blir de følgende benyttet: mettede og umettede lineære eller forgrenede alkoholer som heksanol, oktanol, dekanol, dodekanol, tetradekanol, heksadekanol, oktadekanol osv. Dessuten skal det under disse forståes de som kan bære ytterligere OH-grupper , som 1,2-heksandiol, 1,2-dodekandiol eller 1,2-heksadekandiol.
Forbindelsene ifølge oppfinnelsen har vanligvis en molekylvekt fra IO<3>til IO<6>, fortrinnsvis fra IO<4>til IO<5>.
Den ønskede substitusjonsgraden med restene R<1>, R<**>, R<*11>, R<*0>, sett i forhold til en aspartamidenhet, er avhengig av den benyttede resten og av de valgte reaksjonsbetingelsene. Til sist blir det innstilt slik at den ønskede farmakologiske virkningen av oppfinnelsens forbindelser blir optimalt nådd.
Normalt ligger substitusjonsgraden DS mellom 0,1 og 1,0, fortrinnsvis mellom 0,2 og 0,8 forR<1>ogR1<1>. For R<111>blir fortrinnsvis verdien bestemt mellom 0,1 og 0,5.
Polyaspartamidets derivatisering med forbindelsene med formlene II og III, fører til en lipofilisering av polymeren. Denne lipofiliseringen fører til at gallesyrene i forhøyet grad, og spesielt ved en fysiologisk pH-verdi, blir adsorpa-tivt fast bundet til polymeren. Her dreier det seg om en binding som består av en ionisk og en strukturell vekselvirkning mellom gallesyremolekylet og polyaspartamidmolekylet.
Gallesyrene foreligger ionisert eller protonisert ved en fysiologisk pH-verdi. I så måte er det gitt en ionisk vekselsvirkning. Den strukturelle vekselsvirkningen består i at gallesyrene blir romlig omgitt av lipofile polyaspartamid-molekyler. Det dreier seg således om en omslutningsfor-bindelse.
I prinsippet er det således mulig, å såvel ionisere poly-asparamid-derivatene (aminosyre, aminoalkoksyrest) som å lipofilisere (fettsyrer, alkoholer), hvorved den adsorbative virkningen kan forsterkes. Likeledes er det mulig å benyttet gallesyrer som derivatiseringsreagens for polyaspartamid, og å anvende derivatet som adsorbent for gallesyrer.
I dette tilfellet er substitusjonsgraden for den kovalent bundne gallesyren 0,01 til 1, 0, fortrinnsvis 0,3 til 0,8.
Som naturlige aminosyrer blir nevnt:
Glycin, L-alanin, L-valin, L-leucin, L-serin, L-treonin. L-lysin, L-arginin, L-sapragin, L-glutamin, L-fenylalanin, L-tyrosin, L-prolin, L-tryptofan.
Som syntetiske aminosyrer blir det foretrukket enantio-merblandinger av de naturlige amonosyrene, homoaminosyrene, isoaminosyrene, ■y-aminosmørsyre, a-aminosmørsyre osv.
Egnet som fettsyrer er mettede og umettede, rettkjedede eller forgrenede karboksylsyrer som kapron-, heptan-, oktan-, dekan-, dodekan-, tetradekan-, heksadekan-, oktadekansyrer, oljesyre, linolsyre, linolensyre, 2-metylheksansyre o.a.
Følgende alkoholer blir f.eks. tilsatt for lipofiliseringen: Mettede og umettede, lineære eller forgrenede alkoholer som heksanol, oktanol, dekanol, dodekanol, tetradekanol, heksadekanol, oktadekanol o.a. Dessuten skal det under disse også forståes de som bærer ytterligere OH-grupper som f.eks. 1,2-heksandiol, 1,2-dodekandiol eller 1,2-heksadekandiol.
Følgende aminoalkylrester med formelen A-N(R^)R2blir anvendt: 2-aminoetanol, 3-aminopropanol, N,N-dimetyl- eller dietyl-2-aminoetanol, N,N-dimetyl- eller -dietyl-3-aminopropanol, 4-aminobutanol, 6-aminoheksanol, 4-aminosmørsyre, 6-aminoka-pronsyre o.a.
Følgende gallesyrer kan bli bundet på polyaspartamidet ifølge oppfinnelsen; Kolansyre, desoksykolansyre, litokolansyre, ursodesoksykolansyre, kenodesoksykolansyre, såvel som deres tilsvarende taurin og glycinkonjugater.
Ved tilsetning av forbindelsene ifølge oppfinnelsen, kan den beskrevne mangelen i markedet på "harpiks" som inngriper i det enterohepatiske kretsløpet bli fullstendig fjernet. Gjennom hemming av gallesyretilbakeresorpsjonen i tynntarmen ved hjelp av oppfinnelsens forbindelser, blir gallesyrekon-sentrasjonen som befinner seg i det enterohepatiske kretslø-pet, redusert vesentlig på en effektiv måte, slik at det skjer en senking av kolesterolspeilet i serumet. Binding av vitaminer skjer i liten grad ved anvendelsen av foreliggende forbindelser, noe som også gjelder innvirkningen på resorp-sjonen av andre legemidler eller også den negative virkningen på tarmfloraen, da gallesyrebindingen til foreliggende forbindelser er meget stabil.
Ved anvendelse av foreliggende forbindelser, kan den idag vanlige doseringen av harpiksene bli kraftig senket, den anbefalte dosen utgjør 0,5-10 g/kg/dag. De kjente bivirk-ninger (obstipasjon, Steratorrhoe) blir derfor ikke obser-vert, dvs. fettfordøyelsen blir på den ene side ikke negativt påvirket av den nær naturlige strukturen til polyaspartamidet og på den annen side ikke negativt påvirket av den kjente positive innflytelsen av såkalte balaststoffer på fordøyel-sen .
På grunn av den høye affiniteten til oppfinnelsens forbindelser overfor gallesyre, eksisterer ikke lenger problemet ved doseringen, og derigjennom også aksepten sammenlignet med de høye dagsdosene til "harpiksene". Dessuten blir aksepten forbedret ved at foreliggende polyaspartamider er vann-oppløselige og derved kan bli benyttet for formuleringen uten tilsetningsstoffer som f.eks. smaksforbedrere, emulgatorer, søtningsstoffer osv.
Fremstillingen av polysuksinimid (=polyanhydroasparaginsyre) er kjent fra EP-Å-0.439.846 (tilsvarende US-PS 651.295).
Oppfinnelsen angår dessuten konjugatet av oppfinnelsens polyaspartamid-derivat og minst en gallesyre, som på grunn av en strukturell vekselvirkning er bundet adsorptivt til polyaspartamid-derivatet.
Oppfinnelsens konjugater kan benyttes eksempelvis i næringsmidler eller fruktsafter.
Oppfinnelsens polyaspartamider kan dessuten bli benyttet i analytiske fremgangsmåter for gruppeselektiv anrikning av gallesyrer fra biologiske væsker som f.eks. plasma, serum, urin, galle osv.
Bestemmelse av adsorbs. lonskapasitet
For in vitro-testing av absorbsjonskapasiteten til oppfinnelsens forbindelser, ble de følgende gallesyrene benyttet: 15,1 mg kolansyre, 13,8 mg desoksykolansyre, 13,8 mg kenodesoksykolansyre, 13,2 mg litokolansyre, 17,1 mg glykokolansyre, 15,8 mg glykodesoksykolansyre, 16,6 mg glykokenodesoksykolansyre og 16,0 mg glykol itokolansyre oppløst i 700 pl metanol, tilsatt 0,92 ml fosfatbuffret koksaltoppløsning (pH 7,2) og inkubert med 5 mg av oppfinnelsens forbindelser i 24 timer ved 37°C i et ristevann-bad. Deretter ble blandingen fylt i en dialyseslange av Visking-type, og dialysert i 72 timer ved romtemperatur mot fosfatbuffret koksaltoppløsning (pH 7,2). Bestemmelsen av gallesyrebindingen ble gjort ved analyse av yttermedier, f.eks. ved metodene beskrevet under.

autosampleren avkjølt til ca. 5°C.

1 et eppendorfkar ble i hvert tilsatt 900 pl av den følgende blanding:

20 ml millipor-vann
Til dette ble det pipettert 30 pl av prøven og 30 pl
enzymoppløsning.
Enzymoppløsning: 3-alfa-hydroksysteroid-dehydrogenase
0,5 enheter/ml
Prøvene ble blandet og inkubert ved romtemperatur i 2
timer.
Deretter ble prøvene fylt i 1 ml engangskuvetter og
målt i fotometeret ved 340 nm.
Kun egnet for gallesyreprøver med forbehold, da den
grønne farven forstyrret.

Forbindelsene ble fremstilt som følger:
Eksempel 1: Fremstilling av poly-a,p-(hydroksyetyl )-D,L-aspartamid-co-a,e.(dimetylaminoetyl)-D,L-aspartamid (40:60).
10 g (103 mmol) polyanhydroasparaginsyre (MVisc>=25000) ble oppløst i 80 ml DMF (vannfri ) eventuelt under svak oppvarming. Til denne oppløsningen ble det deretter dryppet til 2,5 g (41 mmol) nylig destillert dimetylaminoetylamin fortynnet i 20 ml DMF såvel som tilsatt 1 g vannfri 2-hydroksypyridin og omrørt 1 dag ved romtemperatur. 8,8 g (100 mmol) aminoetanol ble deretter tilsatt. Etter ytterligere en dags omrøring ble det oppvarmet i 2 timer til 50°C og DMF ble avdampet ved 40"C. Resten ble oppløst i vann og ultrafiltrert over en 10000 membran. Retentatet ble frysetørket og sammensetningen av den dannede kopolymeren ble undersøkt med NMR-spektroskopi.
Utbytte: 9,5 g.

Eksempel 2: Fremstilling av poly-a,e-(hydroksyetyl)-D,L-aspartamid-co-a,e-(dimetylaminoetyl)-D,L-aspartamid (75:25). 15 g (155 mmol) polyanhydroasparaginsyre (Mvisc# = 19000) ble oppløst i 80 ml DMF (vannfri) eventuelt under svak oppvarming. Til denne oppløsningen ble det deretter dråpevis tilsatt 3,40 g (39 mmol) nylig destillert dimetylaminoetylamin fortynnet i 20 ml DMF såvel som 1 g vannfri 2-hydroksypyridin og det ble omrørt i 1 dag ved romtemperatur. 10 g (164 mmol) aminoetanol ble deretter tilsatt. Etter ytterligere en dags omrøring, ble det oppvarmet i 2 timer til 50°C og gjentatte ganger utfelt i vannfri aceton.
Utbytte: 24 g.
Eksempel 3: Fremstilling av polyanhydroasparaginsyre-co-a,<p->
(hydroksyetyl )-D ,L-aspartamid-co-a ,e-(dimetylaminoetyl )-D,L-aspartamid (15 : 20 : 65).
10 g (103 mmol) polyanhydroasparaginsyre ble oppløst i 80 ml DMF (vannfri) eventuelt under svak oppvarming og tilsatt 1 g hydroksypyridin. Deretter ble det tildryppet 5,9 g (67 mmol) dimetylaminoetylamin, som var oppløst i 20 ml DMF, og dette ble omrørt i en dag ved romtemperatur. Deretter ble det tilsatt 1,25 g (20,6 mmol) aminoetanol, som var fortynnet i 10 ml DMF. Etter eventuelt en dag ved romtemperatur, ble det oppvarmet i 3 timer til 60°C for fullføring av reaksjonen, og det ble gjentatte ganger utfelt i vannfri aceton. Eksempel 4: Fremstilling av polyanhydroasparaginsyre-co-a,<p->(palmitoyloksyetyl )-D,L-aspartamid-co-a ,g-(dimetylaminoetyl)-D,L-aspartamid (15 : 20 : 65). 10 g polyanhydroasparaginsyre-co-a ,g-(hydroksyetyl)-D,L-aspartamid-co-a ,p-(dimetylaminoetyl-D,L-aspartamid (15 : 20 : 65) fra eksempel 3 ble oppløst i 100 ml vann- og aminfri DMF og langsomt tilsatt 10 ml palmitinsyreklorid. Deretter ble det tildryppet 10 ml pyridin som inneholdt en spatelspiss DMAP, og dette ble omrørt over natten. For fullførelse av reaksjonen ble det deretter oppvarmet i 2 timer til 60° C. Produktet ble flere ganger utfelt i vannfri aceton og tørket under vakuum.
Eksempel 5: Fremstilling av poly-a,<p->(palmitoyloksyetyl)-D,L-aspartamid-co-a,e-(dimetylaminoetyl)-D,L-aspartamid (40 :60).
10 g poly-a ,e-(palmitoyloksyetyl)-D,L-aspartamid-co-a,p-(dimetylaminoetyl)-D,L- aspartamid (40 : 60) fra eksempel 1, ble oppløst i 100 ml vann- og aminfri DMF og tilsatt 15 ml palmitinsyreklorid. Det ble dryppet til 15 ml pyridin og tilsatt en spatelspiss DMAP. Det ble omrørt over natten og oppvarmet i 2 timer til 60°C for fullførelse av reaksjonen. Den først heterogene reaksjonsblandingen, ble da klar. Deretter ble den utfelt i aceton, utfellingen tørket, oppløst i vann under oppvarming og etter filtrering, og ultrafiltrert over en 10000 membran hvoretter retenatet ble frysetørket.
Ubytte: 10,7 g
Eksempel 6: Fremstilling av poly-a,p-(palmitoyloksyetyl )-D,L-aspartamid-co-a,p-(3-butyloksykarbonyl )propionyloksy-etyl)-D,L-aspartamid-co-a,<p->(dimetylaminoetyl)-D,L-aspartamid (15 : 60 : 25). 5 g poly-a,e-(hydroksyetyl)-D,L-aspartamid-co-a,e-(dimetyl-aminoetyl )-D,L-aspartamid (75 : 25 ) ble oppløst i 60 ml tørr DMF. 1,25 g (1,4 ml) palmitinsyreklorid og 3,5 g 3-butyloksy-karbonyl-propansyreklorid ble innveid i en skilletrakt og deretter påfylt til 10 ml med tørr DMF. Oppløsningen ble langsomt dryppet til. Deretter ble 2 ml pyridin oppløst i 3 ml DMF og også sakt tildryppet reaksjonsblandingen. Etter 3 timer ble prosedyren med syreklorid- og pyridintilsatsen gjentatt, og blandingen ble videre omrørt i 5 dager. Blandingen ble deretter helt i diisopropyleter, resten tørket og deretter oppløst i vann. Oppløsningen ble ultrafiltrert over en 10000 membran hvoretter retenatet ble frysetørket. Sammensentingen må bli bestemt ved XME-spektroskopi. Der ble de følgende signaler oppnådd: 'H-NMR i D20: bred triplett ved 0,85 ppm, CH3fra
palmitoly- og butylgruppene,
bredt signal mellom 1,05 og 2 ppm, en CH2-gruppe fra butoksydelen og 13 CH2-grupper fra palmitoylresten, fra dette kan forholdet mellom butyloksykarbonyl-pro-
pionyloksyetylgrupper til palmitoylgrupper bli beregnet.
Singulett ved 3 ppm, (CH3)N-,
dette gir forholdet av ester- til aminogrupper.
Eksempel 7: Fremstilling av poly-a,p-(palmitoyloksyetyl )-D,L-aspartamid-co-a,6-(3-butyloksykarbonyl )propionyloksy-etyl )-D ,L-aspartamid-co-a ,P-( dimetylaminoetyl)-D,L-aspartamid-co-a,p-(hydroksyetyl )-D,L-aspartamid (15 50 : 25 : 10 ). 5 g poly-a,e-(hydroksyetyl)-D,L-aspartamid-co-a,g-(dimetyl-aminoetyl )-D,L-aspartamid (75 : 25 ) ble oppløst i 60 ml tørr DMF. 1,25 g (1,4 ml) palmitinsyreklorid og 3,5 g 3-butyloksy-karbonyl-propansyreklorid ble innveiet i en skilletrakt og påfylt til 10 ml med tørr DMF. Oppløsningen ble langsomt dryppet til. Deretter ble 2 ml pyridin blandet med 3 ml DMF og deretter langsomt dryppet til reaksjonsblandingen. Etter omrøring over natten, ble blandingen helt med diisopropyleter, resten tørket og deretter opppløst i vann. Oppløsningen ble ultrafiltrert over en 10000 membran og hvorpå retentatet ble frysetørket. Det ble analysert analogt med eksempel 6.
I motsetning til eksempel 6, forløp reaksjonen vesentlig mindre fullstendig, og førte til et produkt med omkring 10% ikke-forestrede hydroksylgrupper.
Eksempel 8: Fremstilling av a,<p->poly(2-dimetylaminoetyl)-co-(palmitoyloksyetyl)-D,L-aspartamid (80 : 20).
Fremstillinge ble utført analogt med eksempel 5.
