Description
Denne oppfinnelse angår en fremgangsmåte for fremstilling av et farmasøytisk preparat for behandling av erektil funksjonssvikt gjennom administrasjon inn i urinrøret.
Normal ereksjon innebærer koordinering av en kompleks rekke av fysiologiske og psykologiske faktorer. Alt som påvirker én av disse kan forårsake impotens. Psykogen impotens kan forårsakes f.eks. av angst, depresjon, spenning og stress.
Fysisk impotens inntrer når sykdommer eller skader påvirker nerver, blodkar eller hormoner som styrer evnen til ereksjon. I U.S.A. er hovedårsakene til fysisk impotens sukkersyke, vaskulære sykdommer, impotens etter strålebehandling, ryggmargsskader og andre traumer, andre endokrine problemer samt multippel sklerose. Andre årsaker er infeksjoner i prostata, medikamentmisbruk, alkoholisme og bivirkninger av reseptpliktige medisiner. Selv røking kan forstyrre normal ereksjon.
Det er anslått at ca. 10 millioner menn i U.S.A. lider av impotens. I aldersgruppen over 60 er ca. 1 av 3 ute av stand til å oppnå brukbar ereksjon.
Det finnes for tiden flere medisinske behandlings-alternativer som avhenger av impotensproblemets natur og årsak, så som behandling med Yohimbine, et indisk trebark-ekstrakt som man tror gir en kjemisk stimulering av de nerver i penis som kontrollerer ereksjoner. Tidligere rapporter tydet på at normal ereksjon gjenoppnås hos 20-25% av pasientene, men effekten er diskutabel. Bivirkninger kan være svimmelhet, kvalme, nervøsitet og hodepine.
For enkelte menn med lave nivåer av mannlige kjønnshormoner (testosteron) kan testosteron-injeksjoner eller behandling med piller være til hjelp. De fleste pasienter har imidlertid ikke lave testosteron-nivåer og vil ikke være hjulpet av hormonsupplement. Det er dessuten adskillige bivirkninger ved testosteronbehandling.
I de senere år er pasienter med erektil funksjonssvikt av ulik opprinnelse, blitt behandlet ved intrakorporal injeksjon av forskjellige medikamenter. Et slikt medikament er papaverin, som i små mengder utvider de arterielle blodkar og svekker den venøse drenasje (Virag. R.( Intracavernous injection of papaverin for erectile failure. Letter to the editor. Lancet 1982; 2:938). Brindley, G.S. beskriver i Brit. J. Psychiat. (1983), 143, 332-337 en ny teknikk for undersøkelse og behandling av erektil impotens ved at pasienten selv injiserer intrakavernøst små doser av fenoksybenzamin eller fentolamin. De doser som herunder benyttes for intrakorporal injeksjon er ca. 2-10 mg fenoksybenzamin og ca. 0,5-1,5 mg fentolamin.
Bivirkningene ved selvadministrasjon består i risikoen for infeksjoner, oppskrapning, fibrose og arrdannelse med permanente forandringer i det indre av penis. Det foreligger også risiko for en smertefull, langvarig ereksjon (priapisme).
Det er dessuten kjent at intravenøs eller intramuskulær injeksjon av fentolamin i moderate doser kan forårsake et kraftig fall i blodtrykket som følge av perifer vasodilatering som fører pasienten inn i et sirkulasjonssjokk.
Eksperimentelt er det i enkelte tilfeller vist at påføring av nitroglycerolpasta på huden til en viss grad kan forbedre ereksjonskvaliteten.
Dessuten er kirurgisk implantering av penilt protesemateriale foretatt når enklere behandlinger har vært mislykket. All kirurgisk implantering av penilt protesemateriale medfører imidlertid høy risiko for infeksjon. Andre komplikasjoner ved kirurgiske inngrep innbefatter forbigående urinretensjon, smerte, blødning, sårdannelse, mekanisk svikt og ekstrusjon av implantatet.
Hensikten med foreliggende oppfinnelse har derfor vært å tilveiebringe en fremgangsmåte for fremstilling av et preparat for behandling av erektil funksjonssvikt som ikke medfører operativt inngrep, er lett å benytte, ikke har vesentlige bivirkninger (kortvarige eller langvarige), er rimelig billig, gi tilstrekkelig stivhet til vaginal penetrering gjennom et passende tidsrom (20-30 minutter), være selvbegrensende, dvs. at det ikke bør være mulig å overdrive bruken av medikamentet ved å ta mer enn én dose om gangen (for å unngå risikoen for priapisme).
Disse formål oppnås ved foreliggende fremgangsmåte som karakteriseres ved at ett eller flere lipofile vasodilaterende aktive midler valgt fra gruppen bestående av a-reseptorblokkere, vasoaktive intestinale polypeptider, prostaglandiner og nitroglycerol, blandes med et hydrofilt bæremiddel basert på makrogoler og eventuelt et antibakterielt middel.
Det har hittil vært vanlig antatt at mukosamembranen i urinrøret ikke tillater passasje av molekyler idet avfalls-produkter transporteres via urinrøret. Vi har mot alle teorier gjort den overraskende oppdagelse at det er mulig å administrere farmakologisk aktive substanser via urinrørets mukosa over i corpora cavernosa i penis.
Tidligere har man administrert medikamenter av annen natur enn de som det her er tale om, inn i urinrøret for å oppnå lokal desinfeksjon eller profylaktisk mot veneriske sykdommer eller for å indusere lokal anestesi. Administrasjon av medikamenter inn i urinrøret for å nå corpora cavernosa for å oppnå effekt, er ikke tidligere foretatt, da det faktum at dette kunne oppnås, ikke har vært kjent før foreliggende oppfinnelse.
Fysiologisk erektil funksjonssvikt er i det minste delvis forårsaket av en øket sympatisk tonus som forhindrer aktiveringen av ereksjonsmekanismen i corpora cavernosa. Som nevnt ovenfor har ett forsøk derfor bestått i å injisere en a-reseptorblokker, så som fentolamin og fenoksybenzamin intrakorporalt. Injeksjon av fentolamin gir imidlertid en kortvarig ereksjon, mens fenoksybenzamin har lang virkning men antas å ha alvorlige bivirkninger og medfører en rekke risikomomenter.
Eksperimentelt har vi funnet at når fentolamin administreres per urethra i en mengde på 10-200 mg, fortrinnsvis 50-60 mg, i et volum på 1-6 ml, i alminnelighet 2-3 ml, oppnås en fullstendig og tilfredsstillende ereksjon uten klare systemiske bivirkninger (dosen benyttet for intrakorporal injeksjon av fentolamin er 0,5-1,5 mg). De samme ønskelige resultater kan også oppnås ved bruk av fenoksybenzamin per urethra i høye doser, 50-300 mg, fortrinnsvis 100-150 mg, sammenlignet med dosen benyttet for intrakorporal injeksjon som er ca. 2-10 mg. En annen velegnet a-reseptorblokker for formålet, er prazosin i en mengde på ca. 20-200 mg, fortrinnsvis 30-70 mg. Andre of-reseptorblokkere kan også være egnet.
Ved hjelp av preparatet fremstilt ifølge oppfinnelsen administreres en stor dose av virkestoffet, fortrinnsvis instillert, i urinrøret, hvorved det langsomme opptak via urinrørets mukosa i corpora cavernosa gir en mer langvarig effekt enn de intrakorporale injeksjoner i henhold til tidligere metoder.
Virkestoffet administrert i henhold til foreliggende oppfinnelse, kan dessuten omfatte andre o^- og a2-blokkere og vasoaktive intestinale polypeptider, prostaglandiner, fortrinnsvis PGE17 PGE2 og PGF2 samt nitroglycerol. Virkestoffene må oppfylle de ovenfor angitte formål ved foreliggende oppfinnelse og også, hvilket er meget viktig, være fettoppløselige, for å kunne passere gjennom mukosamembranen i urinrøret.
Når nitroglycerol benyttes som virkestoff, bør det administreres i en dose per urethra som ikke overskrider 5 mg å grunn av risikoen for blodtrykksfall. Doseringsområdet er i henhold til foreliggende oppfinnelse 0,5-5 mg, fortrinnsvis 0,5-2,5 mg. Denne dose er imidlertid meget høy sammenlignet med den dose som normalt gis til angina pectoris pasienter, som er 0,25 mg til 0,5 mg.
Man kan også fremstille preparater som inneholder to eller flere virkestoffer, dvs. to eller flere ot-reseptorblokkere, vasoaktive intestinale polypeptider, prostaglandiner og nitroglycerol, som følge av deres ulike affinitet for a-reseptorblokkerne og at2. Slike preparater er f.eks. fentolamin og nitroglycerol, fenoksybenzamin og nitroglycerol, prazosin og nitroglycerol som alle gis i de ovenfor angitte doser. For eventuelle synergistiske effekter kan fentolamin pluss fenoksybenzamin og fentolamin pluss prazosin og fenoksybenzamin og prazosin administreres sammen med
nitroglycerol.
Hensikten med kombinasjonen mellom a-reseptor(er) og nitroglycerol, er at nitroglycerol gir en synergistisk effekt med a-reseptorblokkere ved at den stenger sympatikus som følge av en lokal sympatikolytisk effekt og forårsaker vasodilatering via en mekanisme som er forskjellig fra virkningen av a-reseptorblokkernes. Nitroglycerol medfører en hurtigere absorpsjon og gir derfor en tidligere innsettende effekt.
Som nevnt omfatter preparatet i tillegg til de(t) fettoppløselige virkestoff, også et hydrofilt bæremiddel og eventuelt et antibakterielt middel. Det hydrofile bæremiddel er basert på makrogoler, og hensikten med dette er ikke bare å kontrollere opptaket via urether-membranen over i corpora cavernosa og holde virkestoffet(ene) i urethra, men det øker antagelig også opptaket eller passasjen av urinrørets mukosa ved å tvinge de fettoppløselige stoffer inn i corpora cavernosa som følge av at bæremidlet er hydrofilt og at de fettoppløselige virkestoff(er) har tendens til å migrere mot og gjennom membranlipider fremfor å forbli i det hydrofile miljø. Formålet ved kombinasjonen av de lipofile virkestoffer og det hydrofile bæremidlet, er å holde reseptorene aktivert (blokkert) på kontrollert måte og under et lenger tidsrom.
Oppfinnelsen vil i det følgende bli beskrevet gjennom et eksempel under henvisning til den vedlagte tegning.
Eksempel
Figur 1 er en kurve som viser det prinsipielle forhold mellom mengde av fentolamin langs den vertikale akse og varigheten langs den horisontale akse. Pasienten ble initialt gitt 60 mg av a-reseptorblokkeren fentolamin administrert inn i urinrøret, hvoretter penis-stivheten ble kontrollert. Under kurvens maksimum, mellom 3 0 og 70 minutter fra administrasjon av virkestoffet, ble fullstendig ereksjon oppnådd. Det nødvendige nivå av fentolamin for å oppnå fullstendig ereksjon er vist ved den stiplede linje i Fig.l. Toppens areal over den stiplede linje er delt i to seksjoner som henholdsvis viser mengden av fentolamin som antas å metaboliseres i corpora cavernosa og mengden av fentolamin som antas å sive inn i det systemiske kretsløp. Den ereksjon som oppnås ved administrasjon av fentolamin i henhold til foreliggende oppfinnelse, er således betydelig lenger enn tidsrommet oppnådd ved injeksjoner i henhold til tidligere kjent teknikk.
Det antas at transportkapasiteten av urether-mukosa bare svakt overskrider corpora cavernosa's evne til å metabolisere fentolamin. Det overskytende fentolamin, dvs. over den stiplede linje i Fig. 1, metaboliseres hovedsakelig lokalt i corpora cavernosa, og antas bare i liten grad å sive inn i det systemiske kretsløp. Benyttes en lavere dose av fentolamin, vil hastigheten for transporten over urethermukosa være mindre enn metaboliserings-hastigheten i corpora cavernosa, og et tilstrekkelig antall a-reseptorer vil derfor ikke bli aktivert slik at fullstendig ereksjon oppnås. En så høy dosering som 200 mg er mulig, men det foretrukne området er i betraktning av risikoen for sirkulasjonssjokk, mellom 50 og 60 mg.
Foreliggende oppfinnelse tilveiebringer således en fremgangsmåte for fremstilling av et preparat for behandling av erektil funksjonssvikt, hvor tidligere kjente ulemper unngås, og som gir en mulighet for millioner av impotente menn til å ha et seksualliv som er så nær det normale som mulig.
Tabell 1 viser resultatet av forsøk på å behandle impotens hos cystektomiserte pasienter ved tilførsel av forskjellige substanser per urethra.
Claims (8)
1. Fremgangsmåte for fremstilling av et farmasøytisk preparat for behandling av erektil funksjonssvikt via urinrøret, karakterisert ved at ett eller flere lipofile vasodilaterende aktive midler valgt fra gruppen bestående av a-reseptorblokkere, vasoaktive intestinale polypeptider, prostaglandiner og nitroglycerol, blandes med et hydrofilt bæremiddel basert på makrogoler og eventuelt et antibakterielt middel.
2. Fremgangsmåte ifølge krav 1,
karakterisert ved at det som aktivt middel anvendes en a-reseptorblokker valgt fra fentolamin, fenoksybenzamin og prazosin.
3. Fremgangsmåte ifølge krav 1,
karakterisert ved at det aktive middel er valgt fra minst to representanter fra gruppen bestående av cn-reseptorblokkere, vasoaktive intestinale polypeptider, prostaglandiner og nitroglycerol.
4. Fremgangsmåte ifølge krav 1,
karakterisert ved at det aktive middel er valgt fra a-reseptorblokkere og nitroglycerol.
5. Fremgangsmåte ifølge krav 3 eller 4, karakterisert ved at det som a-reseptorblokker anvendes fentolamin.
6. Fremgangsmåte ifølge krav 1,
karakterisert ved at det som aktivt middel anvendes nitroglycerol i en mengde på 0,5 til 5 mg.
7. Fremgangsmåte ifølge krav 1,
karakterisert ved at det som aktivt middel anvendes et prostaglandin valgt fra prostaglandin E1 og prostaglandin E2.
8. Fremgangsmåte ifølge krav 1, karakterisert ved at prazosin blandes med det hydrofile bæremiddel og eventuelt et antibakterielt middel.
