Description
Foreliggende oppfinnelse vedrører en anordning for fiksering av spinalosteosyntesestenger på pedikulærskruer som muliggjør anpasning og reetablering av de fysiologiske kurvene til ryggraden.
Generelt er de pedikulære skruene implantert ved stenger eller plater, hvor skruen i alle tilfeller er strengt rettvinklet til stangen eller platen.
Fra WO-A-89/00028 er det kjent et spinalfeste hvor skruen, for å kunne tillate en justering mellom stangen og skruen, har et ytre sfærisk hode og hver av mutterne har en komple-mentært utformet sfærisk konkav overflate som er festet på stangen på hver side av skruehodet. En ulempe med denne festeanordningen er at den krever en gjenget stang som er kostbar og gjengene danner spisse kanter som generelt er sterkt uønsket i implantater. Fra DE-A-2 834 891 er det kjent et festesystem som også innbefatter en gjenget stang. Stangen må være gjenget for å motta strammemuttere og de sfæriske delene av mutterne er i seg selv ikke istand til å utvise en strammeeffekt på stengene. Fra DE-A-3 504 495 er det kjent et festeorgan hvis hode har en gjennomgående åpning som strekker seg fra side til side. Denne publikasjonen hverken foreslår eller beskriver en gjenget plugg som virker som en sfærisk splittering.
DE-A-3 614 101 beskriver et festeorgan som utgjøres av tilsammen seks deler. Festeorganet er ikke utformet for å motta og låse på plass en sylindrisk stang. Innretningene som er nødvendig for å oppnå en justerbar vinkel mellom skruedelen og endedelen er ekstremt komplisert.
Fra US-A- 4 836 196 er det kjent et festeorgan som er ment å samvirk med en plate. Dette festeorganet vil kreve ytterst kompliserte modifikasjoner for å kunne tilpasses en ring med vinkeljustering og det er ikke foreslått noen slike modifikasjoner .
Fra EP-0-077 159 er det kjent en ryggmargsspreder, men det er 1 denne publikasjonen ikke foreslått noen slags sfærisk splittring som er festet på en glatt sylindrisk stang.
Fra EP-A-0 330 881 er det kjent et festeorgan med inn-retninger for binkeljustering mellom skruene og stangen. En ulempe med dette festeorganet er at før stanges festes i skruehodet, kan den løse strammedelen mistes. En annen ulempe med denne anordningen er at den krever tilskruing av to strammeskruer og finjusteringen av vinkelorienteringen av stangen krever etterfølgende stramming av begge skruene og dette er meget arbeidskrevende.
Fra EP-A-0 348 272 er det kjent et gaffelhode i henhold til kjent teknikk. Det er imidlertid ikke beskrevet at det kan oppnås en vinkeljustering av stangen ved bruk av en ytterli-gere splittring og passende utforming av den gjengede pluggenden og gaffelbunnen.
Oppfinnelsen prøver å unngå disse problemene og frembringer en stangfikseringsanordning som gjør det mulig å justere ribbene med hensyn til aksene til de pedikulære skruene.
Hensikten med oppfinnelsen er å frembringe en anordning for fiksering av spinal osteosyntesestenger på pedikulære stenger av den typen som inkluderer et gaffelformet hode som definerer to grener som mottar en stang som skal fikseres, hvilken stang låses i innkapslingen med en låseskrue, hvor den videre omfatter, plassert på stangen og innført mellom låseskruen og bunnen av innkapslingen, en splittring hvis ytre motstående flate er rettet mot låseskruen og bunnen er konveks og mottas i tilsvarende konkave overflater på enden av låseskruen og bunnen av den mottakende innkapslingen av stangen, hvilken ring tillater en viss vinklet klaring av stangen med hensyn til skruens akse før stramming av skruen. En slik anordning muliggjør en justering av vinkelen som dannes mellom stangen og aksen til pedikaerskruen, spesielt i saggitalplanet, og å låse stangen i den ønskede posisjonen ved hjelp av splittringen som deformeres og immobiliserer stangen under trykk av låseskruen.
Andre egenskaper og fordeler med oppfinnelsen vil fremgå fra den videre beskrivelsen av en utførelsesform, hvilken beskrivelse kun er gitt som et eksempel med henvisning til de medfølgende tegningene. Fig. 1 viser delene til en anordning i henhold til oppfinnelsen. Fig. 2 viser anordningen i fig. 1 sammensatt, bortsett fra kapselen. Fig. 3 viser en seksjon langs saggitalplanet til den pedikulære skruen i henhold til oppfinnelsen. Fig.4 er en perspektivskisse av en splittring i henhold til oppfinnelsen.

Fig. 6 viser et utsnitt av en låseskrue i henhold til oppfinnelsen. Fig. 7 viser en forstørrelse av enden av stangen i fig. 1. Figur 1 viser ved 1 en kjent type pedikulærskrue. Den består av en konisk skrue 2, en spiss 3, og et gaffelformet sylindrisk hode 4 gjennomskåret på begge sider. Mer spesielt omfatter hodet 4 to motsatt parallell grener 4a og 4b som sammen definerer et skall eller innkapsling 5 med en generell akse rettvinklet på skrueaksen 2 og som kan motta en generelt sylinderformet stang eller integraliseringselement 6. En slik konstruksjon er velkjent og er f.eks. beskrevet i GB-2.173.104 .
De motstående indre flatene til forgreningene 4a og 4b er sylindriske og gjenget på en slik måte at de kan motta en låseskrue 7 for låsing av stangen 6 i innkapslingen 5 på samme måte som skruelåseanordningen beskrevet i EP-0.010.527.
Hodet 4 mottar en kapsel 8 som dekker de øvre delene av forgreningene 4a og 4b.
I henhold til oppfinnelsen sikres låsing av stangen 6 ved hjelp av en splittring 9 som kan gli fritt over stangen 6 i innkapslingen 5 og som er låst av låseskruen 7.
Ringen 9, enden av låseskruen 7 og den delen av innkapslingen som er i kontakt med ringen 9 er tilpasset hverandre på en spesiell måte.
I den fremstilte utførelsesformen består ringen 9 (fig. 4) av en ytre sfærisk konveks flate 10 som for det første samarbeider med en sfærisk konveks flate 11 (fig. 6) i enden av den koniske delen av låseskruen 7, og for det andre med en sfærisk konkav flate 12 (fig. 3) i bunnen av den mottakende innkapslingen 5 mellom forgreningene 4a og 4b til stangen 6.
Ringen 9 er plassert på en slik måte at dens plan er omtrent rettvinklet (fig. 2) til saggitalplanet til skruen 1, som _er definert ved aksen 13 til skruen og den generelle aksen til innkapslingen 5 som mottar stangen 6.
Ringen 9 er forsynt med en rett slisse 14 (fig. 4 og 5) med en indre sylindrisk flate 15 såvidt større enn stangens 6 diameter.
Stangen 6 hvorpå ringen 9 er plassert er innført på innsiden av innkapslingen 5 til den pedikulære skruen 1 etter at skruen er plassert i en passende bendel. Ringen 9 inngripes av huset 5, 12 som vist på fig. 2, deretter skrus låseskruen 7 inn i mellomrommet mellom forgreningene 4a og 4b. Før ringen låses plasseres kapselen 8 ved ganske enkelt å skreve over de to forgreningene 4a og 4b, deretter orienteres stangen 6 passende på innsiden av saggitalplanet (fig. 2) og låses med den ønskede orienteringen ved å komprimere splittringen 9 med låseskruen 7. Klaringen til stangen 6 er f.eks. 15 ° rundt posisjonene rettvinklet til aksen 13 til skruen 1 som vist i fig. 2.
Ved justering av stangen 6 glir ringen 9 over de sfæriske overflatene 11 og 12 som omgir stangen.
For å erholde en fri vinkel mellom stangen 6 er bunnen av innkapslingen 5 til høyre for de to motstående innløpene såvidt utspart ved 15 (fig. 2 og 3), vil den koniske seksjonen 16 til punktet av låseskruen 7 (fig. 6) sikrer stangen en fri klaring fra låseskruesiden.
Kapselen 8 omfatter (fig. 2) to motstående skjellformer 17 for den frie klaringen av stangen 6 og en midtre gjennombo-ring 18 for innføring av en skrunøkkel, f. eks. en sek-skantnøkkel, for tilstramming av låseskruen 7 ved hjelp av en hul seks-kantet utsparing 19 på den øvre delen av skruen 7.
Hensikten med kapselen 8 er i hovedsak å unngå avstand mellom de to forgreningene 4a og 4b når låseskruen tilstrammes. Kapselen 8 kan presses sammen dersom materialet i skruen 1 eller størrelse av forgreningene 4a, 4b, spesielt med hensyn til tykkelsen, tillater det.
En skikkelig tilstramming av skruen 7 medfører at stangen 6 låses av ringen 9 i den valgte posisjonen, hvor åpningen 14 fortrinnsvis er plassert på siden, som vist i fig. 2. Stangen er dermed fullstendig låst, både med hensyn til orientering og rotasjon om dens akse.
Stangen 6 kan f.eks. være en glatt sperreelement med seks-kantede utsparinger i hver ende (fig. 7) eller et jevnt element sammensatt av fleksible flettede sylindriske fibre.
Ved å frembringe hull eller utsparinger 20, gjør dette at en tilsvarende formet skrunøkkel kan innføres for å bevirke dreining av ryggsøylen.
Ved å bevirke rotasjonen med skrunøkkelen i samme retning som stangens 6 akse, vil denne stangen rotere og dermed vil ryggsøylen dreie langs sin vertikale akse.
Oppfinnelsen er ikke kun begrenset til den beskrevne utførelsesformen, men dekker også alle mulige varianter, spesielt med hensyn til form og plassering av de konvekse og konkave overflater til ringen 9 og overflatene 11 og 12 mellom hvilke den skrues og med hensyn til låseskruen 7 som kan skrues i de motstående forgreningene 4a og 4b eller i et stykke som er montert på hodet 4 til skruen. På samme måte kan form og dimensjoner til ringen 9 og slissen 14 også varieres uten å avvike fra oppfinnelsen i den grad at slissen spiller samme rolle for elastisk festing av ringen ved å klemme den på stangen som skal låses.
I tillegg kan endene av stangen 6 generelt ha indre eller ytre vinklede former som vist på fig. 7 og konturen av denne formen kan f.eks. være polygon med n sider og passende anpasset konturen til skrunøkkelen som gjør at stangen 6 kan rotere om sin akse.
Claims (5)
1.
Anordning for fiksering av spinal osteosyntesestenger på pedikulærskruer (1) av den typen som innbefatter et gaffelformet hode (4) som definerer to mot hverandre vendende parallelle grener (4a, 4b) som sammen definerer et hus (5) med en akse generelt rettvinklet til skrueaksen for å motta en stang (6) som skal festes som er blokkert i huset (5) av en strammekrue (7) innført mellom grenene (4a, 4b), karakterisert ved at anordningen ytterli-gere innbefatter en splittring (9) anordnet på stangen (6) og plassert mellom låseskruen og bunnen av huset (5), hvis motstående ytterflater (10) rettet mot strammeskruen (7) og bunnen er konveks og mottas i komplementære konkave overflater (11, 12) som enden av strammeskruen (7) og bunnen av huset (5) som mottar stangen (6) er utstyrt med, hvilken ring (9) tillater en viss vinklet klaring av stangen (6) med hensyn til skruens (1) akse før stramming av skruen (7).

Anordning i henhold til krav 1, karakterisert ved at de konvekse (10) og konkave (11, 12) overflatene til strammeskruen (7) og bunnen av huset (5) som mottar stangen (6) er sfæriske.

Anordning i henhold til et eller flere av kravene 1-2, karakterisert ved at bunnen av huset (5) som mottar stangen (6) på nivå med inngangene til huset og på begge sider av den konkave overflaten (12) er utstyrt med klaringsutsparinger (15).

Anordning i henhold til et eller flere av de foregående krav, karakterisert ved at grenene (4a, 4b) til hodet (4) til skruen og strammekruen (7) er dekket av en
kapsel (8) som er utstyrt med et midtre hull (18) for tilgang til utsparingen for kontroll av strammeskruen.

Anordning i henhold til krav 4, karakterisert ved at kapslen (8) på sin kant er utstyrt med ved motstående utsparinger (17) som gir fri klaring til stangen (6).
